1. Comparison of tear film stability and corneal sensitivity between laser-assisted subepithelial keratectomy with laser in situ keratomileusis

Objective:To compare the changes in tear film stability and corneal sensitivity after laser-assisted subepithelial keratectomy (LASEK) and laser in situ keratomileusis (LASIK). Methods: Forty eyes of 20 patients underwent LASIK and 40 eyes of 20 patients underwent LASEK to correct mypia. The patients were observed for corneal sensitivity, tear break-up time (BUT) and Schirmer test under local anesthesia before and 1, 3 and 6 months after operation. The results were analyzed statistically. Results: The tear film stability and corneal sensitivity were decreased to some extent in both groups after operation. Compared with before operation, there were significant differences in corneal sensitivity 1, 3 and 6 months after operation and in BUT 1, 3 months after operation in LASIK group (P<0.05); there were also significant differences in corneal sensitivity 1, 3 months after operation and in BUT 1 month after operation in LASEK group (P<0.05). The corneal sensitivity and BUT values were obviously different between the 2 groups 1, 3 months after operation (P<0.05), with the decrease of corneal sensitivity and BUT values more significant in LASIK group. There was no significant changes in Schirmer test results before and after operation in both groups (P>0.05). Conclusion: Both LASIK and LASEK influence the corneal sensitivity and tear film stability after operation; the influence diminishes with the recovery of the cornea. The influence in LASIK group is more serious and more persistent than that in LASEK group.
2. Construction of luciferase reporter gene cell model and its application in screening rhein derivatives targeting RAC1

Aim To establish luciferase reporter gene expression cell models of CNEi-RACI-Luc2 based on the target of RAC1 promoter, and explore the application of screening anti-tumor active of rhein derivatives targeted regulating RAC1 at transcriptional level. Methods The lentiviral carrying luciferase reporter vector was designed and synthesized using RAC1 promoter se-quence, and CNE1 cells were infected with recombinant plasmid lentiviral to obtain cell lines that stably expressed firefly luciferase. Luciferase reporter assay was used to detect the luciferase luminescence value after stimulating with RAC1 activator PMA and inhibitor NSC23766 that targeted regulating the RAC1 promoter activities in cells, and RAC 1 expression was verified by Western blot. The effect of series of rhein derivatives on the lu-cifcrase activity of RAC1 promoter was observed, and RAC1 expression was determined by Western blot. Results The identification result of double enzyme digestion showed that a lentiviral expression vector carrying luciferase reporter vector rc-combined with RAC1 promoter was successfully constructed. Lcntivirus-infcctcd CNE1 cells were screened by puromycin, the CNE1-RAC1-Luc2 cells stably expressing firefly luciferase were obtained, and the Iransfection efficiency was over 90%. The RAC1 luciferase reporter assay system was sensitive to FMA and NSC23766 and consistent with the result of RAC1 protein expression by Western blot. The regulation of series of rhein derivatives to RAC1 luciferase activity of CNE1-RAC1-Luc2 cells was consistent with the results of Western blot. Conclusions The cell model of luciferase reporting system containing RAC1 promoter can be successfully constructed, which provides a practical platform for high throughput screening of RAC1-targeted drugs.

4.Progress in molecule-targeted therapy for pancreatic cancer

Pancreatic cancer is one of the most malignant cancer of all neoplasm.85% cases are not suitable for surgery when at first counsultation to doctors.The effects of chemotherapy and radiotherapy are not satisfactory either.With the advancement of the study of molecular biology,the research of the molecular targeted drugs such as EGFR inhibitor and VEGF inhibitor,become one of the hot spots.This article reviews progress in the research of molecule-targeted therapy.
5.Application of frontal sinus optical probe in endoscopic frontal sinus surgery.

OBJECTIVE:
To discuss the application of frontal sinus optical probe in endoscopic frontal sinus surgery.
METHOD:
The clinical data of 58 adult patients (86 sides) suffering from chronic frontal sinusitis were analyzed retrospectively. The patients received endoscopic frontal sinus surgery with frontal sinus optical probe because of difficulty in confirming ostia under endoscope. All cases were followed up post operation to evaluate the efficacy.
RESULT:
The ostia of frontal sinus were successfully opened with this method in all cases, and no serious intracranial or orbital complication occurred. After 6 to 18 months follow-up postoperatively, the symptoms vanished in 51 cases and improved in 7 cases and none of all recurred. The endoscopic examinations showed well opening of frontal sinus ostia.
CONCLUSION
Localization of frontal ostium with frontal sinus optical probe is a simple, convenient, safe and effective accessorial method for revision endoscopic frontal sinus surgery.

7.Preoperative predictors for nature of duodenal papillary tumors

Objective:To establish a preoperative forecasting model for the nature of duodenal papillary tumors and to discuss its main predictors.Methods:A case-control study was conducted;the case group included patients with malignant duodenal papillary tumors and the control group included patients with benign duodenal papillary tumors.All the patients were from Changhai Hospital. Data of patients,including the demographic characteristics,clinical symptoms during onset,laboratory findings,and radiological data, were collected by face-to-face interviews or by reviewing the medical history.Chi-square,t-test or ANOVA were employed to performed univariate analysis.All factors with P values less than or equal to 0.25 in the univariate analysis were used as independent variables for multivariate analysis,and a Logistic regression forecasting model for the nature of duodenal papillary tumors was established.Results: Totally 199 patients with pathologically-confirmed duodenal papillary tumors were included in the present study,with 166 in the case group and 33 in the control group.Multivariate analysis showed that hemoglobin(Hb),total bilirubin(Tbil),direct bilirubin(Dbil), aspartate transferase(AST),alkaline phosphatase(AKP),gamma glutamyl transpeptidase(GGT),and carcinoembryonic antigen(CEA) were independent predictors for nature of duodenal papillary tumors,with their odds ratios(95% confidence interval)being 0.981 (0.959,1.003),0.867(0.794,0.948),1.207(1.075,1.355),1.028(1.008,1.048),0.996(0.992,1.000),1.002(1.000,1.004),and 0.974(0.953,0.994),respectively.Conclusion:The Logistic regression model,which takes into consideration of Hb,Tbil,Dbil,AST, AKP,GGT,and CEA,can be used to predict the nature of duodenal papillary tumors,and its clinical value need to be further studied.
8.Combined use of RGD-peptide modified PLGA and TGF-beta1 gene transfected MSCs to improve cell biobehaviors in vitro.

In order to improve the surface properties of PLGA polymer for a better material/cell interface to modulate the cells behaviors, we prepared a novel three-block copolymer, PLGA-[ASP-PEG], and immobilized an RGD-containing peptide, Gly-Arg-Gly-Asp-Ser-Pro-Cys (GRGDSPC) on the surface of it. Transforming growth factor-beta1 (TGF-beta1) was transfected into bone marrow stromal cells (MSCs) employed as seeded cells. Cell adhesion, spreading, proliferation and differentiation on this material were investigated. The results showed that the cell adhesive ratio on RGD-modified materials was higher than on un-modified materials (P<0.05). The extent of cell spreading was also wider on RGD-modified materials than on un-modified materials. Cell proliferation indices of transfected MSCs were increased as compared with the un-transfected MSCs (P<0.05). The ALP activities in the MSCs cultured with RGD-modified materials were higher than on un-modified materials after 14 days (P<0.05), and those in transfected MSCs were higher than in un-transfected MSCs (P<0.05). It was suggested that the combined use of RGD-modification and TGF-beta gene transfection could improve the interaction of biomaterial and cells.
9.The value of assessment of area of ground glass opacity in lungs cast by high-resolution computed tomography on the prognosis of patients with acute paraquat intoxication

ObjectiveTo assess the value of the area of ground glass opacities (GGOs) in lungs displayed by high-resolution computed tomography (HRCT) in paraquat (PQ) poisoned patients in evaluating prognosis. Methods Clinical and imaging data of 137 patients with acute PQ poisoning admitted to Affiliated Hospital of the Medical College of Chinese People's Armed Police Forces from January 2012 to August 2014 were analyzed retrospectively. The plasma concentration of PQ on admission and the area of GGOs were compared between two groups. The lung HRCT within 10 days of poisoning was performed every 3 days, and the areas of GGOs were evaluated on five levels, including aortic arch, aortic pulmonary window, left upper lobe bronchial, right inferior pulmonary vein, and left diaphragmatic dome. Receiver operating characteristic curve (ROC) was plotted to evaluate the value of all the parameters for prognosis.Results Among 137 patients, 45 died within 28 days after poisoning, with the mortality rate of 32.85%. The plasma PQ level in the non-survivors was significantly higher than that in the survivors (mg/L:7.06±0.67 vs. 3.51±0.34,t = 5.280,P = 0.000). The areas of GGOs at three time points in the non-survivors were significantly higher than those in the survivors [1-3 days: (32.0±5.0)% vs. (2.5±0.4)%,t = 7.860,P = 0.000;4-6 days: (45.5±5.7)% vs. (2.8±0.5)%,t = 12.420,P = 0.000; 7-10 days: (68.0±4.8)% vs. (3.0±0.6)%, t = 23.950,P = 0.000]. ROC analysis demonstrated that the area under the ROC curve (AUC) of GGOs in 7-10 days was 1.000, which could be used to determine the prognosis, but it was too late for the treatment. The AUC of GGOs in 4-6 days was 0.979, with the threshold of> 12.0%, the specificity of 96.15%, the sensitivity of 85.19%, the positive predictive value of 88.46%, and the negative predictive value of 94.94%, which presented good effect in predicting prognosis in the early stage of acute PQ intoxication. But plasma PQ concentration was relatively poor for determining prognosis, AUC was 0.821, with the threshold of> 1.95 mg/L, the specificity of 34.52%, the sensitivity of 88.64%, the positive predictive value of 41.49%, and the negative predictive value of 85.29%.Conclusions The area of GGOs displayed by HRCT can be used to evaluate the fully developed acute PQ lung injury, and it is superior to plasma PQ concentration. The area of GGOs displayed by HRCT 4-6 days after intoxication can be used for the evaluation of PQ induced pulmonary injury in the early stage and the evaluation of clinical prognosis.
10.Value of bronchofibroscopy in etiological diagnosis and treatment of hemoptysis in children

Objective To evaluate the value and safety of flexible bronchoscopy in etiological diagnosis and treatment of hemoptysis in children. Methods The flexible bronchoscopy findings and clinical data of 22 children with hemoptysis were ana-lyzed retrospectively. Results The causes of hemoptysis were as follows, bronchiolitis and pneumonia in 8 cases (2 cases with bronchiectasis), foreign body in bronchus in 3 cases , idiopathic pulmonary hemosiderosis in 3 cases, neoplasms in airway in 2 cases, suspi-ciion of bronchial pulmonary vessel dysplasia in 4 cases (requiring angiogram) and unknown causes in 2 cases. Two cases suffered hypoxemia. Active bleeding was found in 6 cases and topical hemostasis treatment was effective. The bronchial foreign body was removed from right lower lobe bronchus in 3 cases. One case with atelectasis caused by granulation was treat-ed with bronchoscopic cryotherapy 2 times, no granulation was found during the third bronchoscopy and chest CT showed the atelectasis was cured 2 weeks later. Conclusions Flexible bronchoscopy is one of the necessary and safe examinations in chil-dren with hemoptysis and is of important value in clinical diagnosis and treatment.
