2.The cardioprotection of intermittent hypoxic adaptation.

Intermittent hypoxia (IH), or periodic hypoxia is referred as exposure to hypoxia interrupted by normoxia that occurs under many physiological and pathophysiological conditions. A lot of researches showed that IH adaptation, like ischemic preconditioning (IPC) and long-term high-altitude hypoxic adaptation (LHA), had significant cardioprotective effects including increasing the tolerance of myocardium to ischemia/reperfusion injury, limiting infarction size and morphologic damage, inhibiting apoptosis of myocardial cells, enhancing recovery of cardiac function in ischemia/reperfusion, and antiarrhythmia. However, the precise mechanisms underlying the protective effects of IH against ischemia/reperfusion injury are far from clear. The potential candidates participating in the protective effects of IH include oxygen transport, energy metabolism, neurohumoral regulation, antioxidase, stress protein, adenosine, ATP-sensitive potassium channel, mitochondrion, calcium control, nitric oxide and protein kinase. The effects of IH are affected by the protocol of hypoxic exposure, age and sex of experimental animals. IH adaptation, for longer lasting time than IPC and lesser side effect than LHA, might have a practical value for using.

3.Optimization of Ginger Juice Baking for Rhizoma Coptis by Orthogonal Test

Objective:To research the best processing method of ginger juice baking for Rhizoma coptis. Methods:The total con-tent of four alkaloids including berberine hydrochloride determined by HPLC was used as the evaluation index, and an L9(34)orthogo-nal design with three factors including the amount of ginger juice, baking temperature and baking time and variance analysis were ap-plied to study the ginger juice baking technology for Rhizoma coptis. Results:The best processing conditions were as follows:Rhizoma coptis was soaked with 15% ginger juice, baked at 150℃ for 40 min, and withdrawn to be cool. Conclusion:The optimal ginger juice baking technology for Rhizoma coptis is reasonable, which can be used to guide the standardized production of Rhizoma coptis with gin-ger juice baking.
4.Efficacy of European Organization for Research and Treatment of Cancer (EORTC) risk tables for the prediction of recurrence and progression of non-muscle invasive bladder cancer after intravesical pirarubicin instillation.

OBJECTIVETo analyze the influence of intravesical Pirarubicin (THP) instillation on the prediction results of European Organization for Research and Treatment of Cancer (EORTC) risk tables and to discuss the efficacy of EORTC risk tables in clinical application.
METHODSWe retrospectively reviewed the clinical data of 389 patients with non-muscle invasive bladder cancer after TURBT treated with intravesical pirarubicin instillation. According to the EORTC Scoring System, all the cases were divided into low risk group, intermediate risk group and high risk group. The 1-year and 5-year recurrence and progression rates of each group were calculated and compared with the prediction results of the EORTC risk tables.
RESULTSThe 1-year recurrence and progression rates of the low risk group were 8.0% and 0, those of the intermediate risk group were 31.0% and 2.8%, and those of the high risk group were 52.5% and 18.6%, respectively. The 5-year recurrence and progression rates of low risk group were 16.0% and 5.3%, those of the intermediate risk group were 42.6% and 10.7%, and those of the high risk group were 63.9% and 41.9%, respectively. The prediction results of progression rate were similar to that of the EORTC risk tables while the overall recurrence rate was lower.
CONCLUSIONSThe EORTC risk tables can be effectively used to predict the recurrence rate and progression rate of non-muscle invasive bladder cancer. However, the EORTC risk tables have a tendency to overestimate the recurrence rate. Intravesical pirarubicin instillation is helpful to reduce the recurrence rate, yet has no obvious influence on the tumor progression.

5.Therapeutic effects and long term follow-ups in 42 cases of lipid storage myopathy

Objective To clarify the clinical features,therapeutic strategy and prognosis of lipid storage myopathy (LSM).Methods The clinical data and therapeutic effects of 42 LSM patients were summarized retrospectively.All patients were followed up to evaluate their prognosis.Results Data of short-term therapeutic results of all the 42 patients were available.Thirty-three cases were placed in low- doses prednisone and 9 cases in riboflavin.All patients showed marked and quick improvement of symptoms within one month.Among thirty-two patients followed up for more than one year,26 cases had a full recovery and 6 remained to have intolerance to heavy exercise.Thirteen patients had relapses of muscle weakness in various degrees and most of which were induced by exertion,exposure to coldness and upper respiratory tract infection.In 5 patients the symptoms were recurred for more than one time.Among 13 cases with relapses, 7 had family history.Conclusions Our data suggest that LSM is a treatable disease and well responsive to low-doses prednisone.The disease tends to recur,especially in patients with family history.Glutaric aciduria type Ⅱ should be considered in LSM patients who are responsive well to riboflavin,indicating drug therapeutic strategy for LSM should be based on the etiology of the disease.
6.Preliminary observation on the efficiency and safety of Lamivudine used in decompensative hepatic cirrhosis (B) accompanied by hypersplenism

OBJECTIVE: To study the efficacy and safety in patients with decompensative hepatic cirrhosis treated with Lamivudine. METHODS: Eighteen decompensative hepatic cirrhosis (B) (active phage) patients accompanied with hypeersplenism were treated with Lamivudine 100mg po. per day. The total course of treatment was 3 months to 6 months when HBVDNA became negative and HBeAg seroconversion occurred in these patients after Lamivudine treatment. The efficacy and safety in patients were evaluated as follows: HBVDNA were negative, HBeAg seroconversion occurred and hepatic cirrhosis child-stageing changed. The efficacy and safety between treated group and contrast group were compared during treatment with Lamifudine for 1 year and follow-up foe 1 year after completing treatment. RESULTS: The total efficacy of treated group was 27.7% and 71.43% respectively during the phase II trial and the safety was good in these patients. CONCLUSION: The efficacy and safety of Lamivudine are good while it is used in non-registered adaptation of decompensative hepatic cirrhosis with hypersplenism.
7.Beneficial effects of intermittent hypobaric hypoxia on the body.

Myocardial ischemia and reperfusion (I/R) is a common problem in clinic and there is no satisfactory method for prevention or treatment of I/R injury so far. Chronic intermittent hypobaric hypoxia (CIHH), similar to the concept of ischemia preconditioning (IPC)or altitude hypoxia adaptation (AHA), has been recognized to confer a protective effect on heart against I/R injury with a longer protective effect than IPC and a less adverse effect than AHA. It has been proved that CIHH increases myocardial tolerance to ischemia or hypoxia, reserving cardiac function and preventing arrhythmia during I/R. Multiple mechanisms or pathway underlying the cardiac protection of CIHH have been proposed, such as induction of heat-shock protein, enhancement of myocardial antioxidation capacity, increase of coronary flow and myocardial capillary angiogenesis, activation of adenosine triphosphate (ATP)-sensitive potassium channels, inhibition of mitochondrial permeability transition pores, and activation of protein kinase C (PKC) and induced nitric oxide synthase (iNOS). In addition, CIHH has been found having many beneficial effects on the body, such as promotion of health, increase of oxygen utilization, and prevention or treatment for some diseases. The beneficial effects of CIHH and potential mechanisms are reviewed mainly based on the researches performed by our group.

9.Significance of human papillomavirus test in triage of patients with atypical squamous cell of undetermined significance

Objective To investigate the significance of human papillomavirus teat in triage of patients with atypical squamoas cell of undetermined significance (ASCUS) diagnosed by cervical cytology. Methods Human papillomavirus teat,colposcope and cervical biopsy were performed in 184 patients with a referral diagnosis of ASCUS by cervical cytology. Results Confirmed by pathological diagnosis of cervical biopsy, 112 cases were chronic inflammation(60.87%),33 CIN Ⅰ (17.93%),17 CIN Ⅱ (9.24%),8 CIN Ⅲ (4.35%),4 cervical squamous carcinoma (2.17%) and 10 condyloma (5.43%). Of the 184 women with cytological ASCUS, 124(67.39%) cases were positive in high-risk HPV test among which 66 cases were histologically confirmed as chronic inflammation(53.23%),22 as GIN Ⅰ(17.74%), 16 as CIN Ⅱ(12.90% ), 8 as CIN Ⅲ(6.45%) ,4 as cervical squamoas carcinoma (3.23%) and 8 as condyloma (6.45%). The positive rate of HPV in groups of ASCUS were higher than those with negative HPV (P<0.003). Conclusion Women with ASCUS should be tested for HPV. Cervical biopsy under colposcopy is recommended for women with HR-HPV infection.
10.Clinic value of combination of high-risk human papillomavirus test and cervical cytology test in diagnosing cervical lesions

Objective To investigate the clinic value of combination of high-risk human papillomavirus test and cervical cytology test in diagnosis of cervical lesions. Methods Patients underwent physical examination at our department were checked by high-risk human papillomavirus test,cervical cytology test and colposcope from October 2004 to December 2006. Abnormal patients with cervical abnormalities were asked for pathological test. The diagnostic value of cervical lesions among these different methods were compared. Results Based on the criteria of histopathology,the sensitivity,specificity,positive-predictive value and negative-predictive value of high-risk human papillomavirus test for detecting all cases of CIN Ⅱ and CIN Ⅲ were 94.83% ,31.06% ,55.22% and 87.02% respectively,and those of the cervical cytology were 92.10% ,31.06% ,54.50% and 81.43% respectively.Those values changed to 99.65%,18.55%,61.46% and 97.62% respectively if two methods were combined. Conclusions Human papillomavims test and cervical cytology test combined with pathological test can improve the detective rate of cervical lesions and facilitate the treatment.
