1.Safety and effectiveness of finasteride combined with 1470 nm Semiconductor laser vaporization and resection for Moderate to severe benign prostatic hyperplasia

Objective To study the safety and effectiveness finasteride combined with 1470 nm semiconductor laser vaporization and resection for moderate to severe benign prostatic hyperplasia (BPH). Methods A total of 110 consecutives from January to December 2015 were divided randomly into control and observation groups and each of 55 cases; the patients in control group received finasteride combined with transurethral prostatectomy (TURP) and they in observation group adopted finasteride combined with 1470 nm semiconductor laser vaporization and resection, then to compare the differences of surgical success rate,complications rate,mean operation time,blood bloss during and after operation, indwelling catheter time,prostate volume before and after operation; the follow-up time was 12.0 months, the differences of international prostate symptom score (I-PSS), Quality of life score (QOL), peak flow rate (Qmax) peak flow rate (Qmax) and post-void residual (PVR). Results The surgical success rate in the two groups were no statistical difference, the complications rate in observation group was significantly lower (P<0.05). The mean operation time and indwelling catheter time in the two groups were no statistical difference, while the total blood bloss and prostate volume after operation in observation group were both less, and the difference of prostate volume was more (P<0.05). The I-PSS and PVR were lower, QOL and Qmax higher in the two groups after operation, what'more, there were more improvements in observation group (P<0.05). Conclusion It is more prior to finasteride combined with 1470 nm semiconductor laser vaporization and resection for moderate to severe BPH on the Safety and effectiveness than TURP.
2.The effect of mitoxantrone on prostate cancer and analysis of pain relief

Objective To investigate the therapeutic effect of mitoxantrone on prostate cancer and the effect of relieving pain of patients.MethodsRetrospective analysis of clinical data of patients with bone pain in April 2010 to May 2016 year in our hospital for treatment of prostate cancer, according to their treatment divided into control group and observation group, the control group was 40 cases given docetaxel treatment, the observation group was 30 cases treated with mitoxantrone.Compared the pain relief, sleep quality, tumor markers and the incidence of adverse reactions of the two groups.ResultsAfter treatment, the observation group pain control rate was 96.67%, significantly higher than the control group, the difference was statistically significant (P<0.05);the observation group sleep quality was significantly better than the control group, the difference was statistically significant (P<0.05);the observation group cPSA and CEA were lower than those of control group, the difference was statistically significant (P<0.05).Two groups of patients with bone marrow suppression, nausea and vomiting, rash and liquid retention rate had no significant difference (χ2=0.648, P=0.421).ConclusionMitoxantrone has better therapeutic effect on prostate cancer, can significantly relieve the situation and quality of sleep in patients with bone pain.
3.Analysis of NIH-CPSI score, clinical efficacy and safety of prostate cancer patients treated with prostaglandin E and alpha blockers

Objective To investigate the effect of the treatment of prostatitis and NIH-CPSI score and clinical therapeutic effect of prostate cancer treated with prostaglandin E and alpha blockers.MethodsThe clinical data of patients with prostatitis treated in The First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine from March 2015 to August 2016 in were retrospectively analyzed.The patients were divided into control group and observation group.The control group was treated with the treatment of the first aid, while the observation group was treated with the combination of the first and the second.The therapeutic effects of the two groups were observed and compared between the two groups before and after treatment NIH-CPSI score, prostate fluid routine, cytokine levels and adverse reactions.ResultsIn the observation group, the treatment efficiency was 97.78%, significantly higher than the control group (85%), the difference was statistically significant (χ2=4.575, P=0.032);Two groups of patients before treatment NIH-CPSI score no difference, after treatment, the observation group of pain or discomfort, voiding symptoms, quality of life and total score were lower than the control group(P<0.05);Two groups of patients before treatment, no difference in the prostatic fluid, after treatment, the observation group was higher than the control group, the number of white blood cells was lower than the control group(P<0.05);There was no difference in the levels of cytokines before and after treatment in the two groups.After treatment, the levels of TNF-α, IL-6 in the observation group were lower than those in the control group(P<0.05).There were no obvious adverse reactions such as liver and kidney injury during the treatment of two groups of patients.ConclusionThe treatment of prostatitis combined with alpha blocker has good therapeutic effect, can significantly improve the clinical symptoms, and has good application value.
4.Effects of Tamsulosin on Proliferation and Apoptosis in Prostatic Cancer PC-3 Cells

OBJECTIVE:To explore the effects of tamsulosin on proliferation and apoptosis in prostatic cancer PC-3 cells. METHODS:After treated with 0 (blank control group),12.5,25 and 50 μmol/L tamsulosin (tamsulosin low,medium and high-concentration groups)for 48 h,the viability of PC-3 cells was detected by MTT method. Hoechst 33258 staining was used to detect cell apoptosis rate. Western blot was used to determine the expression level of Bax and Bcl-2 protein,and the phosphoryla-tion level of protein kinase B(Akt),mammalian target rapamycin(mTOR),ribosomal S6 protein kinase(p70S6K)and 4E bind-ing protein 1(4E-BP1). RESULTS:Compared with blank control group,PC-3 cells viability and the phosphorylation level of Akt, p70S6K and 4E-BP1 decreased in tamsulosin low,medium and high-concentration groups,while expression level of Bax protein in-creased (P<0.05 or P<0.01);the apoptosis rate of PC-3 cells was increased in tamsulosin medium and high-concentration groups,while the expression level of Bcl-2 and phosphorylation level of mTOR were decreased(P<0.01),in concentration-depen-dent manner. CONCLUSIONS:Tamsulosin can inhibit PC-3 cells proliferation and induce cell apoptosis via blocking Akt/mTOR signal pathway.
5.CT findings in Crohn′s disease

Objective To evaluate the CT features and the role of CT in the diagnosis of Crohn′s disease. Methods CT manifestations of 18 patients with pathology confirmed Crohn′s disease were retrospectively analyzed. The majority of the patients were scanned with spiral CT or multidetector row CT.Results All 18 patients, segmental thickening of the bowel wall was detected. There were two different appearances in contrast-enhanced CT scans: (1) mural thickening without enhancement or mural stratification, and homogeneous attenuation in the thickened wall were observed in 7 patients; (2) mural thickening and stratification, resulting in the target or double-halo appearance, were detected in 11 patients, and the bowel wall thickening ranged from 5 to 10 mm. Nine patients had perienteric abnormalities including fibrofatty proliferation, phlegmon and mesenteric hypervascularity. According to the CT manifestations, 5 patients were initially diagnosed as other diseases including intestinal obstruction (1 case), tuberculosis (1 case), cancer (2 cases), and abscess (1 case), respectively. Conclusion CT has the unparalleled ability to depict the abnormalities in the bowel wall, mesentery, abdominal and pelvic viscera, and contrast-enhanced CT can distinguish active Crohn′s disease from quiescent Crohn′s disease. These features make CT particularly valuable in the management of Crohn′s disease.
6.ORIGINS OF THE AFFERENTS TO THE HIPPOCAMPUS IN THE RAT——HRP METHOD

The afferent projections to the hippocampus were studied with HRP. Thirty three per cent solution of HRP was slowly injected into the dorsal or ventral hippocampus on its right side in 25 albino rats and HRP-labelled cells were found in the following nuclei:1. On the ipsilateral side: entorhinal cortex, medial septal nucleus, supramammillary nucleus, submammillothalamic nucleus, dorsal raphe nucleus, median raphe nucleus, locus coeruleus, diagonal band and nucleus of superior colliculus.2. On the contralateral side: areas CA_3 and CA_4 of hippocampus and locus coeruleus.
7.THE SEGMENTAL DISTRIBUTION OF THE AFFERENT NERVES OF THE CAECUM AND TSUSANLI REGION IN RABBITS——A STUDY WITH THE HORSERADISH PEROXIDASE METHOD

Ten adult rabbits (7 and ♀3; body weights 1.5~2 kg) were selected for thee present study. A solution of 2~10% HRP (RZ=2.9) was injected into the subserosa of the caecum in seven rabbits and a solution of 5~10% HRP into the deep peroneal nerve of Tsusanli (足三里) region in the other three. The uptake and retrograds transmission of HRP in the afferent fibres of both the somatic and visceral nerves were traced to the spinal ganglia. The range of segments where the neurons from which these two afferent fibres originate overlap each other. The results are a follows:1. Labelled sensory neurons from the region of the caecum where HRP was injected are observed in the spinal ganglia C_8~S_3 with a higher concentration in T_(11)~L_2.2. Labelled neurons from the region of Tsusanli are found in the spinal ganglia L_1~S_3 with a higher concentration in L_6~S_2.3. The ranges of distribution of labelled neurons from the two groups of afferent fibres overlap in the segments L_1~S_3.4. Most of the labelled cells are small and medium in size and the Iabelled cells are found more concentrated in the lumbosacral segments.
9.Poisoning by exhaust gas of the imperfect combustion of natural gas: 22 cases study.

OBJECTIVE:
To analyze the case characteristics of poisoning by exhaust gas of the imperfect combustion of natural gas and provide references for forensic identification and prevention of such accidents.
METHODS:
Twenty-two cases of poisoning by exhaust gas of the imperfect combustion of natural gas in Minhang District during 2004 to 2013 were collected. Some aspects such as general conditions of deaths, incidence time, weather, field investigation, and autopsy were retrospectively analyzed.
RESULTS:
In the 22 cases, there were 15 males and 16 females. The age range was between 2 and 82 years old. The major occurring time was in January or February (8 cases in each) and the cases almost occurred in small area room (21 cases). There was wide crack next to the exhaust port when the gas water heater was been used in all cases.
CONCLUSION
There are more prone to occurrence of exhaust gas poisoning of imperfect combustion of natural gas in small area room with a ventilation window near the exhaust port of gas water heated. It shows that the scene of combustion exhaust gas poisoning should be more concerned in the cold season.

10.Self-made Kirschner wire hook for the treatment of severe displaced proximal humeral fractures in children.

OBJECTIVETo explore the clinical efficacy of self-made Kirschner wire hook in the treatment of severely displaced proximal humerus fractures in children.
METHODSFrom January 2007 to February 2012,35 children with severely displaced proximal humerus fractures were treated with self-made Kirschner wire hook fixation,including 25 boys and 10 girls with an average age of 13.2 years old ranging from 5 to 17. The duration from injury to operation was 1 to 10 days with an average of 4.5 days. Preoperative diagnoses were confirmed by X-ray films as type III in 26 cases and type IV in 9 according to the Neer-Horwitz classification of the proximal humerus fractures. All fractures were close fracture without nerve or vascular injuries. Anatomical reduction was achieved by open reduction and the fractures were fixed by self-made Kirschner wire hook. Intra-operative and postoperative complications, postoperative radiographic examination, upper extremity length and range of shoulder motion were observed and recorded. Neer score system was used to evaluate shoulder function.
RESULTSAll patients were followed up from 6 to 18 months with an average of 12.1 months. The mean Neer score of the injured side was 94.2 +/- 4.8 (ranged, 84 to 99), 30 patients got an excellent result and 5 good according to the Neer scores. The X-ray showed all the fractures healed without shortening deformity or epiphyseal arrest at early stage. There were no complications related to the implant including loosening or breakage of the self-made Kirschner wire hook. The range of shoulder motion in the injuried side was similar to the normal side, except in abductor lift. All the patients could participate in normal physical activities.
CONCLUSIONThe method of open reduction and fixed with self-made Kirschner wire hook is a safe,effective,convenient treatment for severely displaced proximal humeral fractures in children.
