1.Treatment of the fracture of neck of femur among young people by using the DHSN and the sartorial-pedicled ilium bone graft

[Objective]To discuss the curative effect of using the DHSN and the sartorial-pedicled ilium bone graft in treating the fracture of neck of femur among young people.[Method]A total of 46 cases of the fracture of neck of femur treated with the DHSN and the sartorial-pedicled ilium bone graft were followed up for 1~8 years.The position of fracture or the area of bony defect was filled with the sartorial-pedicled ilium bone.[Result]The clinical results were assessed according to the functional rehabilitation.So,there were excellent results of 23 hips,good of 19 hips and poor of 4 hips in this study and the satisfactory rate was 91%.There were 44 cases reached osteal healing.The average healing time was 8 months and the average time of taken out was 14 moaths.[Conclusion]In treating the fracture of neck of femur among young people,the DHSN and the sartorial-pedicled ilium bone graft have the advantage of firm fixation and highly healing rate.

Using questionnaire of self-concept to 144 senior students,the study tries to understand the character of university students'appraisals of their social comparison,appraisals of their earlier and present selves.The result shows:For all social comparison in the past and present,university students tend to rank themselves as superior to a majority of their peers;in comparison with their earlier and present selves,there is significant difference in feature,school achievement,maturation,ambition,and self-acceptance.And present self is superior to earlier self on feature and self-acceptance,inferior to earlier self on school achievement,ambition and maturation.

According to years of treatment experience on herpes zoster,combined with its current onset characteristics as well as modem medicine,the authors probed into the relation between liver depression and herpes zoster on the following two aspects:one was liver depression and healthy qi (immunity) ; the other was liver depression and the incidence of herpes zoster.We considered that long-term liver depression impairs healthy qi.Then herpes zoster appears because detriment of healthy qi leading to decreased immunity,reactivation and reproduction of virus.Liver depression is a significant factor for the onset of herpes zoster.
5.Surgical treatment of liver adenoma secondary to hepato-glycogenosis

Glycogen storage disease (GSD) is a rela-tively rare inherited metabolic disease. However, its relative rarity implies that no metabolic centre has experience of large numbers of patients and experience with long-term manage-ment is limited. In addition, there is wide variation in meth-ods of medical treatment. With the development of medical care, life-expectancy in glycogen storage disease (GSD) has improved considerably. With ageing liver adenoma may de-velop which will bring about several complications-compres-sion, hemorrhage, or even transformation into carcinomas-and needs immediate intervention. In this paper, reviews for the mechanism and surgical intervention of GSD are presented.
6.Gastrofiberscope-assisted balloon catheter dilatation of esophageal stricture in 30 children

Objective To explore the efficacy of gastrofiberscope-assisted balloon catheter dilatation of esophageal stricture. Methods A total of 30 children with esophageal stricture underwent gastrofiberscope-assisted balloon dilatation between October 1996 and November 2005 in this hospital.The procedure was conducted using the type JIF-SP gastrofiberscope(Olympus Company,Japan) and a balloon catheter with pressure monitor(Boston Company,USA).The stricture was 0.2~0.6 cm in diameter and 0.6~2.5 cm in length(mean,1.7 cm).Under the direct vision of gastrofiberscopy,the balloon catheter was perorally inserted to the stricture.Then the balloon was insufflated with air while monitoring the pressure.The balloon dilatation lasted for 5 min. Results A total of 113 times of balloon dilation were performed in 29 children,while a conversion to traditional surgery was required in 1 child with serious esophageal stricture due to difficulty to dilatation.In the 29 children,the dilatation was conducted for 2~3 times every month until the strictured esophagus reached 0.6~0.7 cm in diameter after 4~5 times of balloon dilatation.Follow-up surveys for 3 months ~ 5 years in the 29 children showed no dysphagia or recurrence of stricture.Conclusions Gastrofiberscope-assisted balloon catheter dilatation of esophageal stricture is a feasible and safe technique.
7.Laparoscopic repair of congenital diaphragmatic hernia in children

Objective To explore the curative effect of laparoscopic repair of congenital diaphragmatic hernia in children. Methods Three newborns(age,3~24 hours) and 8 infants(age,2~24 months) with congenital diaphragmatic hernia underwent 3-port or 4-port laparoscopic repair from June 2002 to December 2005 in this hospital.The hernial content was reduced and the defect was repaired by using intermittent sutures.Results The laparoscopic operation was successfully completed in 10 cases,while a recurrence was encountered in 1 newborn 3 days after the laparoscopic operation and then was cured by a re-operation of laparoscopic repair.The operative time ranged 55~180 min(mean,100 min),and the intraoperative blood loss was 1~2 ml.All the patients began to take food at 1~2 days after operation.Follow-up examinations in the 11 patients for 9~24 months(mean,16 months) showed a normal position of the diaphragm under fluoroscopy. Conclusions Laparoscopic repair of pediatric congenital diaphragmatic hernia is a feasible and safe technique,with advantages of clear visualization and thorough abdominal exploration.
8.Two-port laparoscopy versus open high ligation for indirect inguinal hernia in children

Objective To compare curative effects between laparoscopic and open surgery in the treatment of pediatric indirect inguinal hernia.Methods A group of 410 children with indirect inguinal hernia underwent micro-laparoscopic operation(Laparoscopic Group) from April 2001 to January 2006.This group was compared with another group of 408 children with indirect inguinal hernia,treated with open high ligation of the hernial sac(Open Group) from September 2004 to January 2006.In the Laparoscopic Group,a 5-mm trocar was inserted at the umbilicus for the exploration of bilateral internal rings.Another 5-mm trocar was inserted at the site slightly superior to the McBurney's point for introducing a needle holder.At the level of the neck of the hernial sac,the orifice of the vaginal process was circumferentially sutured and ligated.In the Open Group,high ligation of the hernial sac was conducted through a transversal incision at the external ring.Results The operations in the Laparoscopic Group were smoothly completed.The operative time for one side of hernia was significantly shorter in the Laparoscopic Group(33.4?9.6 min) than in the Open Group(38.0?7.6 min)(t=-7.596,P=0.000).In the Laparoscopic Group,no edema of the scrotum,subcutaneous hydrops,or infection occurred.The patients were followed for 1~55 months(mean,23.2 months).Short-term recurrence was observed in 3 children,who required a re-operation of laparoscopic repair.No testicular atrophy,iatrogenic retained testis,intestinal adhesion,or other long-term complications happened.In the Open Group,follow-up observations for 1~18 months(mean,10 months) revealed recurrence in 18 children,8 of whom were then cured with laparoscopic surgery and 10 of whom with open surgery.Iatrogenic retained testis happened in 9 children and a re-operation of open surgery was carried out.Conclusions Two-port laparoscopic high ligation of hernial sac for pediatric indirect inguinal hernia is minimally invasive,safe,and effective,but not suitable for children with cardiac insufficiency or newborn incarcerated hernia.
9.Analysis of factors related to psychological status for adolescent orthodontic patients in stomatology department and intervention strategies

Objective To explore and analyze the risk factors affecting psychological status of adolescent orthodontic patients in stomatology department, and to develop intervention strategies according to the results. Methods A total of 186 cases of adolescent orthodontic patients in stomatology department from September 2012 to September 2015 were selected, and psychological status was investigated by Chinese Middle School Students Mental Health Scale (MMHI-60 scale) and Symptom Self-rating Scale (SCL-90).The patients were divided into observation group (with psychological problems) and control group (without psychological problems) according to the psychological status scale scores. Univariate descriptive analysis and multivariate Logistic regression analysis were used to determine related factors of the possible influencing factors, and the intervention strategies were developed. Results The factors such as social roles, living area, single parent, only child, internal and external tropism, neuroticism, being given health education, malocclusion severities of the deformities were closely related to psychological status of adolescent orthodontic patients in stomatology department (χ2=2.534-44.297, P<0.05 or 0.01). The independent risk factors were classified as high to low according to the degrees of danger: severe malocclusion (OR=3.578, P=0.000), instability emotional (OR=2.934,P=0.000), workers (OR=1.857, P=0.011) and rural life (OR=1.632, P=0.014), while the factors such as outgoing personality (OR=0.628, P=0.008) and being given health education (OR=0.746, P=0.010) were protective factors. Conclusions The factors such as severe malocclusion, instability emotional, workers,rural life,outgoing personality and health education are the related risk factors affecting psychological status of adolescent orthodontic patients in stomatology department, and psychological health intervention should be strengthened in the clinical practice to improve the psychological status and compliance.
10.Papaverine treats vasospasm caused by PICC intubation with B-mode ultrasonography

Objective To study the effect of papaverine on vasospasm caused by PICC intubation with B-mode ultrasonography. Method Three mg papaverine were injected into the median cubital vein for at least 2 minutes in 15 patients with vasospasm. Results The vasospasm in the 15 patients was relieved 36~270 s seconds after injection. The followed intubation was all successful. There was no abnormality in their liver function and heart rate, or abnormal bleeding, or other serious complications. Conclusion Papaverine can relieve vasospasm caused by PICC intubation, so it can allow another intubation. It also can avoid delayed intubation reduce patients′pain and cost and reduce psychological pressure of the nursing practitioner.
