1.Investigation on the status of human resources for AIDS control at city and county level in Hebei

Objective To understand the status of human resources for AIDS control at city and county level in Hebei Province,and to improve the outfit of human resources.Methods General investigation and self-designed questionnaire were used by the trained investigators.Epi Data 3.0 was used to establish a database,and SPSS 13.0 was used to analyze the data.Results There were 1 347 missionaries at the city and county level,547 of whom were full-time staff.Significant differences existed in academic qualifications and titles between city and county level.A lack in technical staff in county level appeared.Conclusions The number of staff for HIV control in Hebei is not sufficient.Therefore human resources in these places should be strengthened and rationally allocated for AIDS control.
2.Core needle biopsy in diagonsis of palpable breast cancer

Objective To study core needle biopsy in diagnosis of palpable breast cancer.Methods Retrospectively analyze core needle biopsy in 200 cases of palpable breast cancer.We recorded the clinical and pathology data and used crosstable test to analyze the effect of maximal and minimal tumor size.and TNM stage on the results. Results The ratio of success was 97%. No significant difference was found among the groups of different maximal tumor size, minimal tumor size and TNM stage (P > 0.05 ). Conclusion ( 1 )For the diagnosis of the suspicious malignant palpable mass, core needle biopsy is effective, safe, microtraumatic and suitable to be used in different-class hospitals ; (2) The success of core needle biopsy for palpable breast cancer is correlated with dexterity of operator; (3)Biopsy channel should be in the range of operation and avoid of affecting sentinel lymphatic channels.
3.Biomechanical properties and biocompatibility of the L4/5lumbar spine fusion cage

BACKGROUND:Lumbar spinal stenosis can result in lumbar nerve root pain and other symptoms or even disability, which seriously impacts people’s quality of life. Interbody fusion is the main clinical treatment. Studies have shown that an interspinous fusion cage is prone to have hairline fractures and loss of implant; therefore, biomechanical properties and biocompatibility of the interbody fusion cage at L4/5need to be studied.
OBJECTIVE:To analyze the stability of the interspinous fusion cage at L4/5, and to explore the biomechanical properties and biocompatibility of adjacent segments.
METHODS: Ten adult fresh frozen spine specimens were selected and divided into normal group and model group, with five specimens in each group. The specimens were numbered and placed in a special fixture. The interspinous fusion cage, made in Zhengzhou Cast Medical Instrument Co., Ltd., China, was implantedin vivo and fixed using a cage fixator made of Ti6AL-4V ELI.
RESULTS AND CONCLUSION:Artificial vertebral bodies had less strain changes than the normal vertebrae in different implantation positions in terms of central compression, anteflexion, rear protraction, lateroflexion (P < 0.05). During vertebral displacement under the maximum load of 500 N, the linear displacement and angular displacement in anteflexion, rear protraction, left lateral flexion, levorotation were decreased significantly in the model group than the normal group (P < 0.05). These findings indicate that the interspinous fusion cage can maximaly preserve the range of motion of the injured vertebral body, maintain the stability of the segment, and reduce the stress of the intervertebral disc.

Objective:To explore the influence of liquid for post-operative irrigation on the effect of ESS.Method:192 cases with chronic sinusitis and/or nasal polyps of groupIIstageII who had undergone the ESS from September,1998 to June,1999 and completed a3-month follow-up were divided randomly into 3 groups to accept 3 different kinds of irrigation liquid.The evaluation of curative effect was according to the index of HaiKou ESS-97.Result:The curative effect of Herba Houttuyniae group was better than the other two.Conclusion:The post-operative care is important and the irrigation liquid can affect the effect of ESS.
6.Progress in diagnosis and treatment of local recurrence after radical radiotherapy for esophageal cancer

The diagnosis of local recurrence after radiochemotherapy for esophageal cancer requires the combination of multiple imaging methods (esophageal X-ray,computed tomography,endoscopic ultrasonography,and positron emission tomography-computed tomography),dynamic changes in esophagoscopy,and biopsy.The main treatment of recurrence includes salvage surgery,radiochemotherapy,endoscopic mucosal resection,and chemotherapy.The overall survival rate after the above treatment is improved compared with palliative care but still unsatisfactory.Early diagnosis of recurrence is fundamental for improved treatment outcomes.
7.Clinical study on treatment of acute cardiogenic pulmonary edema by noninvasive mechanical ventilation

Objective To evaluate the clinical applications of noninvasive mechanical ventilation in the treatment of acute cardiogenic pulmonary edema. 9.Effect of propofol on vascular reactivity in rat septic shock

Objective To study the effect of propofol on vascular reactivity and plasma malondialdehyde (MDA) and nitric oxide (NO) in septic shock.Methods Forty male SD rats weighing 200-250 g were randomly divided into 4 groups (n = 10 in each group) : group Ⅰ control; group Ⅱ septic shock; group Ⅲ septic shock + propofol and group Ⅳ septic shock + melatonin. The animals were anesthetized with intraperitoneal pentobarbital 40 mg?kg-1. The femoral artery and vein were connulated for MAP monitoring and drug administration. The animals were breathing spontaneously. Septic shock was induced by intravenous LPS 15 mg?kg-1 . In group Ⅲ a bolus of propofol 10 mg?kg-1 was given i.v. at 1 h after intravenous LPS followed by intravenous propofol infusion at 10 mg?kg-1?h-1. In group Ⅳ melatonin 10 mg was given intraperitoneally at 1h after LPS i.v. . Six hours after LPS administration 4 doses of phenylephrine (PE) 0.5, 1, 2, 2.5 ?g?kg-1 were given i.v. in succession. The next dose was given when MAP returned to the baseline level after previous PE. The percent change in MAP after each dose was recorded. Blood samples were taken at 6 h after LPS administration for determination of plasma MDA and NO concentrations. After the in vivo experiment the animals were sacrificed and thoracic aortas were removed and cut into segments of 3 mm in length which were bathed in Krebs buffer aerated with 95% O2 and 5% CO2 at 37℃. The aortic rings were stretched to a resting tension of 2.0 g. The segments were then exposed to increasing concentrations of PE (from 1 nmol?L-1 to 30 ?mol?L-1). The dose-response curves were obtained. Emax and EC50 were calculated. Results The percent increase in MAP induced by PE was significantly reduced by septic shock (groupⅡ,Ⅲ,Ⅳ) as compared with control group (Ⅰ), but was significantly larger in propofol and melatonin groups (Ⅲ,Ⅳ) than in group Ⅱ. 10.Clinical observation of unfractioned heparin for preventive and therapeutic effects on severe sepsis

Objective To evaluate preventive and therapeutic effects of low dose of unfractioned heparin on severe sepsis. Methods 52 Patients with severe sepsis hospitalized in Intensive Care Units were randomly divided into control group and heparin group. On the basis of conventional therapy, patients with sepsis were given low dose of unfractioned heparin or isovolume physiologic saline by continuous intravenous drip infusion over 5 days. Acute Physiologic and Chronic Healthy Evaluation (APACHE-Ⅱ) score, blood platelet count(BPC), prothrombin time(PT), activated partial thromboplastin time(aPTT), thrombin time(TT), plasma fibrinogen(FDP) and D-dimer levels were assessed on admission, 3rd, 5th day after intervention.
