Objective To detect the changes of gastric emptying function by gastroenteric contrastenhanced ultrasonography and to offer the evidence for the diagnosis and treatment of diabetic gastroparesis.Methods The gastric emptying function was measured by gastroenteric contrast-enhanced ultrasonography in 35 cases of diabetic gastroparesis (DGP,DGP group) and 35 volunteers (control group).The gastric emptying rate(GER) and emptying time(ET) after taking orally gastroenteric ultrasound developer in DGP group were observed and compared with control group.Results The GER in DGP group was lower than that in control group [(27.05 ± 11.32)％ vs.(45.38 ± 15.08) ％] (P ＜ 0.05).The ET in DGP group was longer than that in control group[(102.43 ± 15.76) min vs.(75.31 ± 11.52) min] (P ＜ 0.05).Conclusions The method of gastroenteric contrast-enhanced ultrasonography is a simple,and repeatable imaging method,which can be used to make a real-time assessment on the gastric dynamic changes,but don't influence gastroenteric function and can provide a dependent and noninvasive examine means for clinical physician to test the gastric emptying function in DGP.

Objective To determine whether the supplementation of thyroid hormone(TH)improves β-adrenoceptors(β-AR)density and expression of peripheral blood lymphocyte in elderly patients with chronic heart failure(CHF). Methods A total of 42 elderly patients with advanced CHF(NYHA classⅢorⅣ)due to dilated cardiomyopathy(DCM)or ischemic cardiomyopathy (ICM)were randomly divided into a treatment group and a control group.Low-dose L-thyroxine(LT50)was added to the treatment group,both groups had routine anti-heart-failure treatment.Before and after 1 month of treatment,125I-Pindolol radioligand binding assay was used to measure β-AR in peripheral lymphocytes.Reverse-transcriptase polymerase chain reaction(RT-PCR)was used to measure β1-AR expression in peripheral lymphocytes. Results The density and expression of β-AR of lymphocytes in patients with CHF due to ICM or DCM were lower than those in normal health people.and the serum TH level in patients with CHF was lower than that in normal health people.The L-T4 administration induced an up-regulation of β-AR density and expression in peripheral lymphocytes. Conclusions Thyroxine administration induces an up-regulation of β-AR density and expression in peripharal lymphocytes.

Objective To evaluate the clinical effect of surgical operation for recovering the function of ulnar nerve with large segment defect. Methods Twentycases with the large segment defect of ulnar nerve were retrospectively analyzed from September 1999 to December 2006 in the hospital.All the patients were treated by the operation of nerve end-to-side neurorrhaphy . The broken end of ulnar nerve was anastomosed with the median nerve. And observed the recovery of the sensation and motion function of the little finger, interosseous muscles and claw hand, then compared with before. Results All cases were followed up for 8 to 36 months. The mean was 16 months. The sensation and motion function of the little fingers had better restoration after operation. No incision infection, anchylosis or myatrophy was occurred.Excellent(M4 + S4 +) ,Good (M3S3), moderate (M2S2), and poor effects (M1S1) were achieved respectively in 6, 4,2 and 0 cases based on the scale of XU' s grading standard.The excellent and good rate was 83.3%. Conclusion The nerve end-to-side neurorrhaphy was a effective treatment for the patients with large segment defect of ulnar nerve.

Objective To explore the effect of nurse-in-charge-led ward rounds on the multi-level training of nurses. Methods The traditional nursing ward round was adopted from Juanuary to December 2013 and the nurse-in-charge-led ward round was adapted from January to December 2014. The nurse-in-charge-led round mode and flowchart was developed by designating one nurse-in-charge to carry out the process of nursing round and other nurses in the group to carry out nursing interrogation, check-up, physical examination, nursing diagnosis, making nursing strategy and conclusion. Additionally, the two links of nurse-in-charge-led seminar and nursing feedback were added in the nursing round. The effect of nursing ward round and the results of nursing examination were compared between before and after the improvement. Results A year after implementing new nursing round mode, the effect of nursing round was significantly better than that before the implementation of new mode and the nursing examination score was significantly higher than that before its implementation (all P < 0.05). Conclusion The nurse-in-charge-led round mode can improve the effect of nursing round, improve clinical and professional ability to discover, analyze and solve problems.

Objective This study evaluates the effectiveness for using acute plateletpheresis (APP) as a blood conservation method to reduce the need of blood transfusion and increase coagulation function in aortic arch surgery with deep hypothermic circulatory arrest (DHCA ). Methods Thirty-six type-A aortic dissections patients (male 31,female 5,age 23-65 years,ASA physical status II-IV)undergoing frozen elephant trunk with total arch replacement (Bentall plus Sun's surgery)were enrolled in the prospective randomized trial.The patients were randomized into two groups:regular blood conservation group (group control,n = 18)and group APP (n = 18).Blood sample was collected respectively after anesthesia induction (T1 ),before heparinization (T2 ),by the end of surgery (T3 )and 24 hours after surgery (T4 ).Data was collected and reviewed in terms of perioperative transfusion needs,normal laboratory examination,clinical outcomes including blood routine analysis (Hb,Plt,MPV,P-LCR)and thrombelastography (TEG-R,TEG-K,TEG-α,TEG-A,TEG-MA,TEG-EPL).Kaolin and heparinase detections were performed for TEG.Results Com-pared with T1 ,TEG-R,TEG-K,TEG-A,TEG-MA,TEG-EPL and Plt were significantly decreased while TEG-CI,MPV,P-LCR significantly increased in T4 in group APP (P <0.05 ).TEG-A,TEG-MA and Plt were significantly greater(P <0.05)in group APP than in group control at T2 ,and TEG-K,TEG-ELP and HBG were significantly less (P <0.05)in group APP than in group control at T3 . Conclusion The utilization of APP technique was associated with the improved coagulation function in aortic arch surgery with DHCA.

The effect of phenol concentration on the structure and function of microbial communities,which were cultured in different conditions using coking wastewater biofilm as seeding,was investigated by Biolog and denaturing gradient gel electrophoresis(DGGE)methods.The less number of bands of cultivated sam-ples on the denaturing gradient gel electrophoresis fingerprint of 16S rRNA gene indicated reduction of di-versity after enrichment and cultivation.Some bands on the DGGE gel were significantly influenced by the phenol concentration in medium.The results of Biolog showed that the original biofilm sample had the highest substrate utility capacity as measured by average well color development(AWCD).But low concen-tration of phenol enriched sample S7 showed more diverse activity on the utility of Carboxylic acids.The principal component analysis(PCA)of Biolog data revealed that the metabolic patterns were similar when using the same phenol concentration,although the sample S7 much less similar to other cultivated samples.These results suggested that the enrichment and cultivation with phenol supplemented decreased the diver-sity and also changed the metabolic function of the microbial community.Lower phenol concentration in-creased the microbial community metabolic activity.The phenol degrading capacity of isolates from each samples indicated that the enrichment and cultivation condition had changed the type and property of cul-truable bacteria.Based on these results,we concluded that the different microorganisms will be isolated un-der different cultivation condition.

Objective To analysis the result of tumor screening for health examination participants.Methods Tumor screening was applied for 15 863 health examination participants without malignant tumor disease history.Clinical examination,laboratory tests and imaging studies were comprehensively analysed combining the results of other medical items.The cancer detection rate and asymptomatic rate were compared among groups of different age,gender and existence of relevant clinical symptoms,respectively.χ2 test and Fisher exact test were adopted for statistical analysis.Results Among 475 tumor patients with a definite diagnosis,116 were malignant tumor;The total detection rate of the group who were younger than 35 year-old(0.617%)was higher than other groups,the detection rate of intracranial neoplasms of the two groups who were younger than 45 year-old (0.206% and 0.132%,respectively)was higher than other groups,the total detection rate of 75-85 group (0.248%)was lower than other groups; The detection rate of lung cancer,liver cancer of the male(0.228% and 0.080%,respectively)was higher than the female,The detection rate of breast cancer and intracranial neoplasms of the female(0.366% and 0.100%,respectively)was higher than the male; There are 67 patients without correlated clinical symptoms,the asymptomatic rate was 57.8%.Conclusions Tumor screening was of importance in health management,which should be carried out actively in health examination.

Objective To investigate the inflammation levels of 2-diabetes patients before and after 3 months of improving glycemic control.Methods A longitudinal study was performed in a subgroup of 48 subjects with T2D and poor glycemic control.Forty-four healthy individuals were taken as control group.The serum concentration of C-reactionprotein (CRP),interleukin-6 (IL-6),interleukin-6 (IL-8),transforming growth factor-β1 (TGF-β1) and transforming growth factor-β1 (MCP1) in all participants were measured simultaneously by multiplexed Luminex assay.Results The serum levels of CRP,MCP-1 of 2-diabetes patients were 3.96 (3.45,5.58) mg/L and (195.0± 129.8) ng/L,significant higher than those in control group (2.25 (1.24,3.22) mg/L,(148.5±85.7) ng/L),and the differences were significant(t=-2.580,P=0.010;t=-2.118,P =0.047).No significant difference was found in the serum levels of IL-6,IL-8,TGF-β lbetween the two groups (P＞0.05).TGF-β1 level in patients with good glycemic control decreased to 26.85 (23.17-31.12) ng/l,significant lower than that before glycemic control (43.5(26.5-62.25) g/L;Z=-2.191,P=0.028),and there were no significant differences among the other 4 kinds of inflammatory factors before and after blood glucose control(CRP:Z =-0.937P =0.372;IL-6:Z =-0.875,P =0.396;IL-8:Z =-1.215,P =0.286;MCP-1:t =-1.846,P=0.065).Conclusion Low grade systemic inflammation status in T2D patients.Improvement of glycemic control reduces TGF-β1 levels and plays a key role in delaying the development of diabetic nephropathy.

Objective To investigate the relationship between magnetic resonance imaging(MRI) and the type of cerebral palsy (CP) and gestational age at birth.MethodsThe MRI and clinical data of 224 CP children with spastic type were analyzed retrospectively.ResultsAmong these children, 27 cases had spastic hemiplegia, 149 had spastic diplegia, and 48 had spastic tetraplegia. 201 cases (89.7%) had abnormal MRI result. The abnormal rate of MRI in spastic hemiplegia, diplegia and tetraplegia were 100%, 87.2% and 91.7%, respectively. Periventricular leukomalacia (PVL) was observed mostly in children with spastic diplegia, other types of brain lesions were uncommon. In spastic tetraplegia the lesions were more varied. Three predominated types of MRI abnormalities were: PVL (45.8%), term-type brain injuries (20.8%) and congenital brain abnormalies (18.8%). Unilateral lesions were observed mostly in children with hemiplegia spastic diplegia born at term who showed unilateral motor disorder and upper extremity were heavier than lower extremity. Unilateral and bilateral PVL were observed in children with hemiplegia spastic diplegia born at preterm who showed lower extremity were heavier than upper extremity. Of 152 cases with PVL, 108 cases (71.1%) were preterm infants and 44 cases (28.9%) were term infants. Of 22 cases with term-type brain injuries, 20 cases (90.9%) were term infants. PVL was most observed in preterm brain injuries and was observed most in preterm children but was also in term children. Term-type brain injuries (border-zone infarct, basal ganglia-thalamic lesion, subcortical leukomalacia, and multicystic encephalomalacia) were observed most in term and rare in preterm children.ConclusionThe MRI findings in CP show a good correlation with type of CP and gestational age.
