1.Diagnostic Value of Spiral CT Scanning and CT Virtual Endoscopy for Gastric Carcinoma

Objective To evaluate spiral CT scanning and CT virtual endoscopy(CTVE) in the clinical diagnosis of gastric carcinoma.Methods Spiral CT scan was performed in 28 patients with pathologically-proved gastric cancer . The CT scan parameters were : collimating width 5 mm , Pitch 1 . The volumetric data were acquired using a spiral CT scanner and transferred to computer workstation . CTVE was performed by navigator smooth function . The results were compared with the findings of fiberoptic endoscopy and/or surgery .Results Spiral CT scan could demonstrate the location ,size,extension of lesions and the relation with organs around .In 28 patients,CT findings included omentum involvement(n=7),pancreatic ivolvement (n=4),hepatic involvement (n=3),lymph node metastasis(n=3). CTVE could clearly demonstrate the three-dimensional image of normal wall stomach and the lesions , size and extension of lesions . The appearances of the inner lesions and normal wall of stomach on CTVE were similar to those on fiberoptic endoscopy and/or surgery . The accuracy in detecting lesion by CTVE was 92.8% in 28 patients . Conclusion CTVE combination with spiral CT scan is of great value in clinical diagnosis of gastric carcinoma .
2.Effect of Estradiol and its Metabolite on Hypoxic Induced Factor-1αand Alkane Hydroxylase in Experimental Rats With Ovariectomy and Hypoxic Pulmonary Hypertension

Objective: To explore the effects of 17 β-estrogen (E2) and 2-methoxyestradiol (2ME) on hypoxic induced factor-1α (HIF-1α) and alkane hydroxylase (AlkB) in experimental rats with ovariectomy and hypoxic pulmonary hypertension. Methods: A total of 60 healthy female SD rats with castrated surgery were randomly divided into 6 groups:①Routine oxygen group,②Routine oxygen + E2 group, the rats received subcutaneous injection of E2 (20 μg/kg?d),③Routine oxygen + 2ME group, the rats received 2ME (240 μg/kg?d) and④Hypoxia group,⑤Hypoxia + E2 group,⑥Hypoxia + 2ME group.n=10 in each group and all animals were treated for 8 weeks to establish the hypoxic pulmonary hypertension model. The mean pulmonary artery pressure (mPAP) was measured after bloodletting, right ventricle hypertrophy index (RVHI) was calculated and small pulmonary artery remodeling was observed by HE staining. The expression level of HIF-1α and AlkB were examined by RT-PCR and Western blot analysis. Results: Compared with Routine oxygen group, the rats in Hypoxia group had obviously thickened small pulmonary artery wall with narrowed lumen, increased mPAP and RVHI; the above changes in Hypoxia + E2 and Hypoxia + 2ME groups were relatively smaller, their mPAP and RVHI were higher than Routine oxygen group, while mPAP and RVHI were similar between Hypoxia + E2 and Hypoxia + 2ME groups. There were no real morphological changes in small pulmonary vessels in Routine oxygen + E2 and Routine oxygen + 2ME groups. The HIF-1α expression was obviously elevated in Hypoxia group than Routine oxygen group, while the elevation was less in Hypoxia + E2 and Hypoxia + 2ME groups. HIF-1α expression had no real changes in Routine oxygen+E2 and Routine oxygen + 2ME groups. The AlkB expression was obviously reduced in Hypoxia group than Routine oxygen group, while the reduction was less in Hypoxia + E2 and Hypoxia + 2ME groups. AlkB expression had no real changes in Routine oxygen + E2 and Routine oxygen + 2ME groups. Conclusion: Estradiol E2 and 2ME could remit pulmonary hypertension which might be via up-regulating AlkB expression and down-regulating HIF-1α expression in experimental rats with hypoxic pulmonary hypertension.
3.Diagnostic Value of Spiral CT and 3D Reconstruction in Pulmonary Tuberculosis

Objective To evaluate the diagnostic value of spiral CT and all kinds of 3D reconstruction in pulmonary tuberculosis .Methods 145 cases of pulmonary tuberculosis confirmed by clinical study and histopathology were analyzed retrospectively. All cases weregiven to helical scan and different cases were given to different 3D reconstruction (CTVE, MPR and CPR) .Results Spiral CT (HRCTespecially) could find many signs of pulmonary tuberculosis, such as centrilobular distribution nodules, tree-in-bud, cavitations,tuberculous pneumonia, miliary nodules, branchogenic dissemination, endobranchial tuberculosis, lymph node enlargement and calcification. The accuracy of spiral CT in the diagnosis of pulmonary tuberculosis was 96.6%, The accuracy of spiral CT with 3Dreconstruction in the diagnosis of pulmonary tuberculosis was 99.31%.Conclusion Pulmonary tuberculosis can be diagnosed accurately by spiral CT with all kinds of 3D reconstruction.
4.Research progress of quantitative perfusion parameters of dynamic contrast-enhanced MRI in diagnosis and efficacy evaluation of uterine tumors

The quantitative parameters based on dynamic contrast-enhanced MRI (DCE-MRI)scan,simulate the distribution of contrast inside and outside of the blood vessels through a variety of tracer kinetic models.The DCE-MRI perfusion parameters now are widely applied in clinical management of uterine tumors,to analyze the microcirculation characteristics in tumor and to guide the diagnosis,tumor grading and efficacy evaluation.
5.Efficacy of combined pranoprofen eye drops and artificial tears on the treatment of mild to moderate dry eye syndrome after trabbeculectomy

AlM:To evaluate the efficacy of combined pranoprofen eye drops and artificial tears on the treatment of mild to moderate dry eye syndrome after trabbeculectomy.METHODS:This prospective case control study included 63 cases (63 eyes) of patients with mild to moderate dry eye syndrome after trabbeculectomy in our hospital from November 2013 to June 2013. All subjects were randomly divided into two groups. Observation group was treated with combined pranoprofen eye drops and artificial tears and control group received simple artificial tears marking the eyes at 1, 2, 4wk. The patient's symptoms, signs, BUT, S▏t, and FL were observed before treatment and 1, 2, 4wk after treatment.RESULTS: After 2wk, the symptoms of observation group were improved, there was statistically significant difference (P<0. 05). FL difference of each group was statistically significant ( P<0. 05 ); After 4wk, symptoms and signs were improved. There was statistically significant difference ( P < 0. 05 ). The BUT of the observation group and corneal FL scores of two groups showed significant differences (P<0. 05). CONCLUSlON: Artificial tears joint pranoprofen eye drops has good curative effect in the treatment of mild to moderate dry eye syndrome after trabbeculectomy.
6.Effect of β-adrenergic receptor stimulation on vascular smooth muscle cell apoptosis

ObjectiveTo investigate the effect of β-adrenergic receptor(β2AR) stimulation on vascular smooth muscle cell apoptosis in physiological state and receptor overexpression model.Methodsβ2AR overexpression model was established by transgenic techniques. Hoechst 33342 staining as well as flow cytometer(FCM) detected were chosen to measure the incidence of vascular smooth muscle cell apoptosis.ResultsVascular smooth muscle cell exhibited significantly fewer viable cell rate when stimulated with β2AR agonist isoproteronol for 48 hours compared with control (P<0.01),while no difference at the time point of 24 hours. Much fewer viable rate detected by FCM and high apoptotic rate by Hoechst staining were observed when VSMCs overexpressing β2AR were stimulated with isoproteronol for 24 hours (P<0.01).ConclusionStimulation of physiological and overexpressing β2AR could induce apoptosis of vascular smooth muscle cell.
7.Direct synthesis of biodegradable ploy L-lactic acid by melt polycondensation

AIM: To synthesize biodegradable poly lactic acid (PLA) through the optimization and selection of process and catalyst.METHODS: This experiment was performed at the Research Center for Green Chemistry and Technology in the School of Municipal and Environmental Engineering of Harbin Institute of Technology from February 2003 to October 2004. Biodegradable poly L-lactic acid (PLLA) was synthesis by melt polycondensation using L-lactic acid (LA) as material. Firstly, oligo L-lactic acid (OLLA) was prepared by dehydrating aqueous solution of LA without catalyst. And then, the mixture of OLLA and catalyst was heated at a certain temperature and pressure for a period of time to get the product of PLLA. The structure of PLLA was characterized by Fourier transform infrared (FTIR) spectra and 1H- nuclear magnetic resonance (1H-NMR) spectra. The polydispersity was determined by gel permeation chromatography (GPC).RESULTS: OLLA with the viscosity average molecular weight (Mη) of 6 500 g/mol was prepared by the following steps: the solution of LA without catalyst was dehydrated at 140 ℃, first at reduced pressure of 30 kPa for 2 hours, and then at 5 kPa for 4 hours. SnCl2-p-toluenesulfonic acid (TSA) system was the effective catalyst for the polycondensation of OLLA. After the mixture of OLLA and catalysts polycondensed at 165 ℃ and 5 kPA for about 8 hours, PLLA with the Mη of 65 000 g/mol was obtained by using SnCl2-TSA system as catalyst with the SnCl2 amount of 0.4wt% to OLLA and equal mol ratio of SnCl2 and TSA.CONCLUSION:PLLA with a certain practicability was obtained under the optimal process and catalyst. Oligomerization of LA played an important role on improving the molecular weight of PLLA.
8.Effects of Hydrophilic Polymers on the Stability of Self-microemulsifying Drug Delivery Systems

OBJECTIVE:To investigate the effects of hydrophilic polymers on the stability of self-microemulsifying drug deliv-ery systems (SMEDDS). METHODS:Taking felodipine (FDP) as model drug,the content of FDP was determined by HPLC method. The effects of pure water,0.5% Kollidon VA64,HPMC E5,HPMC K100LV,HPMC K4M,PVP K30 solution,while 0.1%,0.5% and 1.0% HPMC E5 and Kollidon VA64 on residual content of dissolved FDP were determined in SMEDDS. RE-SULTS:The residual contents of dissolved FDP in SMEDDS placed in Kollidon VA64,HPMC E5,HPMC K100LV,PVP K30, HPMC K4M and pure water for 1 h were 92.7%,63.6%,50.2%,46.2%,36.0%and 24.0%,respectively. The order of maintain-ing the supersaturation state was Kollidon VA64>HPMC E5>HPMC K100LV>PVP K30>HPMC K4M>pure water. The residu-al contents of dissolved FDP in SMEDDS placed in 0.1%,0.5%,1% Kollidon VA64 and HPMC E5 and pure water for 1 h were 93.2%,95.1%,96.0% and 48.4%,62.1%,75.1%. CONCLUSIONS:Kollidon VA64 and HPMC E5 can significantly inhibit drug release in SMEDDS and be used as stabilizer of SMEDDS,wherein Kollidon VA64 was better.
9.Effects of Propafenone on Action Potential of Rabbit Ventricular Myocytes and the Use-dependent Block of Transient Sodium Current

Objective: To study the effects of propafenone on action potential (AP) of rabbit ventricular myocytes with the tonic block and use-dependent block of transient sodium current (INa-T). Methods: A total of 10 adult New Zealand white rabbits were sacriifced and 10 individual ventricular myocytes were isolated by enzyme digestion method. Microelectrode technologies were used to record AP-related parameters: maximum diastolic potential (MDP), maximum rate of rise of the action potential upstroke (Vmax), action potential amplitude (APA) and action potential duration at 20%, 50% and 90% (APD20, APD50 and APD90).INa-T was measured, I-V curves and peak currents at different frequencies were detected by whole cell patch clamp before and after propafenone perfusion at 10 μmol/L. Results: There was no statistical difference in MDP at before and after propafenone perfusion as (-80 ± 6) mV vs (-82 ± 5) mV,P>0.05. After perfusion, APA was signiifcantly decreased as (95 ± 12) mV vs ( 125 ± 10) mV,P<0.05, the Vmax slowed down as (330 ± 43) V/s vs (420 ± 54) V/s,P<0.05, while APD20, APD50 and APD90 were unchanged as (8 ± 2) ms vs (6 ± 2) ms,P>0.05, (16 ± 3) ms vs (12 ± 3) ms,P>0.05 and (86 ± 14) ms vs (85 ± 12) ms,P>0.05. After propafenone perfusion, I-V curve ofINa-T was shifted upward and the peak current was decreased as (3001 ± 383) pA vs (4193 ± 378) pA, P<0.05. Before perfusion, when stimulated at 0.06 Hz, 1 Hz, 2 Hz, 5 Hz and 10 Hz, there were no signiifcant use-dependent block inINa-T , and no real difference inINa-T between the 10th and 1st pulse,P>0.05. After perfusion, no significant use-dependent block was observed when stimulated at 0.06 Hz and 1 Hz,P>0.05, while at 2 Hz, 5 Hz and 10 Hz, propafenone perfusion demonstrated signiifcant use-dependent block uponINa-T with the inhibition fractions of (22 ± 11)%, (38 ± 14)% and (52 ± 17)% respectively, those were signiifcantly different from the inhibition fractions at either 0.06 Hz or 1Hz,P<0.05. When the inhibition fractions were compared by each 2 conditions, allP<0.05. Conclusion: Propafenone may slow down the Vmax of AP, reduce APA and without the impact on APD; the effects onINa-T is not only in tonic block, but also more obviously in use-dependent block in isolated ventricular myocytes of New Zealand rabbit. Such inlfuences minimized the impact on QT interval and meanwhile, decreased the incidence of brad arrhythmia.
10.Risk factors for healthcare-associated infection in senile dementia patients

Objective To evaluate the risk factors for healthcare-associated infection(HAI)in senile dementia pa-tients,so as to adopt effective nursing measures to reduce the incidence of HAI.Methods Clinical data of 82 senile dementia patients aged≥60 years and hospitalized between January 2011 and June 2013 were analyzed retrospective-ly.Results Of 82 patients,28 (34.15%)developed HAI.The main infection site was lower respiratory tract(n=15,53.57%),followed by urinary tract(n=6,21 .43%).Univariate analysis revealed that risk factors for HAI in senile dementia patients were bedridden,long length of hospital stay ,dysphagia,indwelling urinary catheter,irra-tional use of antimicrobial agents,combined with tumor,and hypoproteinemia (all P <0.05 ).The main isolated bacteria were gram-negative bacilli(n=40,62.50%),the top three pathogens were Klebsiella pneumoniae (n=12, 18.75%),Escherichia coli (n =10,15.63%),and Pseudomonas aeruginosa (n =8,12.50%).Conclusion Reali-zing the risk factors and common pathogens of HAI in senile dementia patients is helpful for taking effective meas-ures to prevent and control the incidence of HAI .
