2.Effect of curcumin on expression of synovial osteoprotegerin and receptor activator of nuclear factor-κB ligand protein in adjuvant arthritis rats

Objective:By observing the effects of curcumin on synovial pathology, synovial osteoprotegerin and receptor activator of nuclear factor κB ligand expression factor of adjuvant arthritis rat models, we try to explore the possible mechanism of prevention and treatment of rheumatoid arthritis.Methods: There were three groups, the normal group, the model group and the curcumin group.We made adjuvant arthritis rat models with Freund′s complete adjuvant,and they were divided into the model group and the curcumin group.On d 28 after modeling,all rats were killed.All synovial membranes were sent for routine histological evaluation with HE staining.And synovial RANKL and OPG protein expression were measured by Western blot.Results: Curcumin could influence synovium pathology change of adjuvant arthritis rats,the pathological picture showed that infiltration of joint synovial cell,pro-liferation of synovial fibroblasts in model group rats were significantly increased,compared with normal group( P<0.01).Pathology of rats synovial tissue in rats of curcumin group were significantly improved, compared with that in rats of model group.Expression of RANKL,OPG protein in synovial tissue of curcumin group,based on pairwise comparison between with model group,the results were statistically significant( P<0.01 ).Synovial RANKL/OPG ratio were significantly lower than model group ( P<0.01 ).Conclusion:Curcumin can improve synovial pathology damage of adjuvant arthritis rats,curcumin may exert a protective effect on arthritis rats by regulating OPG/RANKL signaling pathway.

4.Primary non-Hodgkin′s Lymphoma of the right humerus bone:A case report

Objective To investigate the clinical characteristics and surgical treatment of primary bone non-Hodgkin′s lymphoma.Methods Recalling a case of primary bone non -Hodgkin′s lymphoma patient data in Affiliated Maojian Hospital of Hubei University of Medicine .This patient was hospitalized because of pain in the right upper arm.By improving the relevant examination and diagnosis of right humeral bone tumors ,We had successfully completed a bone tumor resection and graft as well as dual plate fixation .Results Pathological diag-nosis after surgery showed the primary non -Hodgkin′s lymphoma of bone ,9 months follow-up after surgery ,the treatment effecincy was good .There were no tumor recurrence and distant metastasis .Conclusion Primary non-Hodgkin′s lymphoma of bone is uncommon in clinical practice .If there is primary bone tumor with no obvious sys-temic symptoms in elderly patients , we should considered it as a possible diagnosis of primary bone non -Hodgkin′s lymphoma,in order to reduce the possibility of misdiagnosis and mistreatment .Short-term effects of surgical treatment of this disease is excellent .

Objective: To observe the clinical therapeutic effect of Tang Shen Ning(TSN,a prescription of traditional Chinese medicine)for curing the early stage of diabetic nephropathy(DN).Methods: Adopting random and double-blind controlled trial methods,several indexes were observed in terms of uric micro-amount albumin excretion rate,creatinine clearance,hemorheology and platelet function et al.Results: TSN could significantly reduce urinary albumin,glomerulus's hyper-filtration,improve blood stream transform and inhibit platelet aggregation.Conclusion: TSN could protect renal functions during the early stage of DN.
6.Remodeling of cross electro-nape-acupuncture on cough reflex in patients with tracheotomy after cerebral hemorrhage: a randomized controlled trial.

OBJECTIVETo observe the effects of cross electro-nape-acupuncture on reflex remodeling of airway protective reflex cough in patients with tracheotomy after cerebral hemorrhage.
METHODSWith the method of completely random design, according to treatment order, 60 patients who received tracheotomy after cerebral hemorrhage accompanied with cough reflex difficulty were randomly divided into a cross electro-nape-acupuncture group and an acupuncture group, 30 cases in each group. Both groups were treated with basic treatment, including anti-inflammation, eliminating phlegm, improving cerebral metabolism and so on. The acupuncture group was treated with acupuncture at Yifeng (TE 17), Fengchi (GB 20), Lianquan (CV 23), Baihui (GV 20), Touwei (ST 8), Dicang (ST 4) through Jiache (ST 6), Hegu (LI 4), Quchi (LI 11), and motor area on the affected side, and the needles were retained for 30 min. Based on the treatment of acupuncture group, the cross electro-nape-acupuncture group was additionally treated with cross electro-nape-acupuncture (continuous wave) for 30 min per treatment. The treatment was both given twice a day from Monday to Friday and once a day on Saturday and Sun day for 4 weeks. Tracheostomy cough reflex grading score (TCRGS) and clinical pulmonary infection score (CPIS) were observed before and after treatment in the two groups, and the clinical efficacy of two groups was evaluated.
RESULTSCompared before the treatment, TCRGS and CPIS were both reduced in two groups (both P < 0.01); after treatment, there were significant differences of TCRGS and CPIS between two groups (both P < 0.01), indicating cross electro-nape-acupuncture group was superior to acupuncture group. Regarding the effects of cough reflex remodeling, the cured and markedly effective rate was 96.7% (29/30) in the cross electro-nape-acupuncture group, which was significantly different from 55.2% (16/29) in the acupuncture group (P < 0.01).
CONCLUSIONCross electro-nape-acupuncture could effectively improve the remodeling of cough reflex and promote the recovery of lung infection in patients with tracheotomy after cerebral hemorrhage, leading to an increased quality of life.

7.The safety of the mobilization of peripheral blood stem cells by granulocyte colony-stimulating factor in elderly donors

Objective To explore the safety of the mobilization of peripheral blood stem cells(PBSC)by granulocyte colony-stimulating factor(G-CSF)in elderly donors.Methods 28 peripheral arteriosclerotic occlusive disease(PAOD)elderly patients(aged≥60 years),and 29 healthy sibling young/adult donors(aged<60 years)for peripheral allogenic stem cell transplantation were included.Blood samples were collected immediately before starting G-CSF and prior to PBSC collection to analyze the following parameters:the WBC counts,fibrinogen(FIB),D-dimer (D-D),thrombin antithrombin complex(TAT),antithrombin(AT)and yon Willebrand factor antigen(vWF:Ag). Results It had a very significant increase in D-D and vWF:Ag and a very significant decrease of AT(P<0.01),af- ter mobilization by G-CSF,and a increase in FIB and TAT were also observed(P<0.05,P<0.01)in elderly group.In the young/aduh group,the increase in FIB was significant(P<0.05).The elevating extent of D-D and TAT after G-CSF administration was significantly higher in elderly group than that in young/adult group(P<0.05).Compared to young/adult group,there was a significant increase in thrombotic events and cerebrovascular ac- cident(P<0.05).Conclusion In PBCS donorsreceiving G-CSF it reveals activation of both coagulation and en- dothelial cells and inhibition of anticoagulant system that can favor the developing of thrombotic events,which is more remarkable in elderly donors.Therefore a careful monitoring for coagulation system should be considered in those elderly cases.
8.Determination the concentration of losartan and hydrochlorothiazide in healthy volunteers and study on its relative bioavailability by LC-MS/MS

Objective To establish sensitive and selective method and study on its relative bioavailability in healthy volunteers by LC - MS/ MS. Method A single oral dose of two different dosage losartan/hydrochlorothiazide (50 mg/12.5 mg) tablets was administrated to 20 healthy volunteers in a randomized cross - over design. Plasma concen-tration of losartan and hydrochlorothiazide were determined by HPLC -MS/MS method, and relative bioavailability was evaluated by DAS 1.0.Results Main pharmacokinetic parameters for losartan were as follows: tmaxwere (1.09±0.56) hand (1.12±0.55)h, Cmax were (118.9±68.9) and (110.2±51.0)ng- mL-1, t1/2were(3.15±0.76) and(2.96±0.71)h, AUC0-twere (226.2±65.0) and (241.9±77.4)ng· h · mL-1, AUC0-∞were (250.5±68.7 ) and (265.7±81.5 )ng · h · mL-1 for the test and reference drugs. Main pharmacokinetic parameters for hydrochlorothiazide were as follows: tmax were (1.93 ±0.54) hand (2.25±0.60) h, Cmaxwere (73.2±11.0) and (74.5±17.4) ng·mL-1, t1/2were (9.45±3.57) and (8.33±2.58) h, AUC0-t, were (401.8±138.3)and (390.6±149.3) ng · h · mL-1, AUC0-∞ were (438.2±146.8) and (415.5 ± 156.1) ng · h · mL-1 for the test and refer-enee drugs. The relative bioavailabilities were (96.5 ± 21.2 ) % and ( 106.8 ± 22.9 ) % for losartan and hydrochloroth-iazide, respectively. Conclusion The test and reference drugs of losartan/hydroehlorothiazide are bioequivalent.
9.Stromal thrombospondin 1 suppresses angiogenesis in oral submucous fibrosis

A decline in mucosal vascularity is a histological hallmark of oral submucous fibrosis(OSF),a premalignant disease that is largely induced by betel quid chewing.However,the lack of available models has challenged studies of angiogenesis in OSF.Here,we found that the expression of thrombospondin 1(THBS1),an endogenous angiostatic protein,was elevated in the stroma of tissues with OSF.Using a fibroblast-attached organoid(FAO)model,the overexpression of THBS1 in OSF was stably recapitulated in vitro.In the FAO model,treatment with arecoline,a major pathogenic component in areca nuts,enhanced the secretion of transforming growth factor(TGF)-β1 by epithelial cells,which then promoted the expression of THBS1 in fibroblasts.Furthermore,human umbilical vein endothelial cells(HUVECs)were incorporated into the FAO to mimic the vascularized component.Overexpression of THBS1 in fibroblasts drastically suppressed the sprouting ability of endothelial cells in vascularized FAOs(vFAOs).Consistently,treatment with arecoline reduced the expression of CD31 in vFAOs,and this effect was attenuated when the endothelial cells were preincubated with neutralizing antibody of CD36,a receptor of THBS1.Finally,in an arecoline-induced rat OSF model,THBS1 inhibition alleviated collagen deposition and the decline in vascularity in vivo.Overall,we exploited an assembled organoid model to study OSF pathogenesis and provide a rationale for targeting THBS1.
10.Preparation of Scutellariae Radix total flavonoids-loaded liposomes and their in vitro anti-tumor activity

AIM To prepare Scutellariae Radix total flavonoids-loaded liposomes and to evaluate their in vitro anti-tumor activity.METHODS Liposomes were prepared by reverse evaporating method.Taking phospholipid concentration,ratio of phospholipid to cholesterol,ratio of organic phase to aqueous phase and total flavonoids concentration as influencing factors,together with encapsulation efficiency as an evaluation index,Box-Behnken design was applied to optimizing the preparation,after which MTT was employed to detect the obtained liposomes' inhibitory effect on human hepatocellular carcinoma cells HepG2.RESULTS The optimal conditions were determined to be 30 mg/mL for phospholipid concentration,3 ∶ 1 for ratio of phospholipid to cholesterol,2 ∶ 1 for ratio of organic phase to aqueous phase,and 5 mg/mL for total flavonoids concentration.The obtained round and uniform liposomes demonstrated an average particle size of (160.7 ± 12.0) nm,Zeta potential of (-41.4 ±2.3) mV,encapsulation efficiency of (86.19 ±0.44)%,drug loading of (5.32 ± 0.04)%,and accumulative release rate of (80.77 ± 2.53) % at 24 h.Compared with total flavonoids,liposomes exhibited stronger inhibitory effect on HepG2 cells with the IC50 of 48.853 μg/mL at 48 h in a time-and dose-dependent manner.CONCLUSION Scutellariae Radix total flavonoids-loaded liposomes stored at low temperature (4 ℃) display sustained-release effect and in vitro anti-tumor activity.
