1.A cohort study on the association between fasting plasma glucose level over 5.3 mmol/L and risks of abnormal glucose metabolism and cardiovascular diseases in the elderly

Objective The aim of the study was to evaluate the association of fasting plasma glucose (FPG) level over 5.3 mmol/L to the development of abnormal glucose metabolism and cardiovascular diseases (CVD).Methods This was a retrospective cohort study with 1 064 non-diabetic subjects(980 males;84 females) aged 60 or over, who carried out annual health check-up in Chinese PLA General Hospital from May, 1996 to May, 2015.Based on the average FPG level of 3 years before enrollment, the subjects were divided into four groups : < 5.3 mmol/L, 5.3-< 5.6 mmol/L, 5.6-< 6.1 mmol/L and 6.1-< 7.0 mmol/L.Glucose metabolic changes, complications and mortality were follow-up until May, 2015.Results (1)The initial 3-year average FPG levels were (4.9 ±0.4) mmol/L in the total 1 064 subjects.Among them, 126 subjects developed diabetes mellitus (DM) and 144 subjects developed impaired glucose regulation (IGR) during the follow-up visits.The proportions of IGR and diabetes increased with the FPG levels (P < 0.05).The risk for developing IGR was significantly higher in subjects with FPG≥5.3 mmol/L than in those with FPG < 5.3 mmol/L (RR =3.08, 95% CI 2.02-4.81, P <0.01).The risk for incident DM was markedly increased in subjects with FPG ≥ 5.6 mmol/L than in those with FPG <5.6 mmol/L (RR =6.73, 95% CI 3.90-11.52, P <0.01);(2)The risk for CVD was eight folds higher in subjects with FPG ≥5.3 mmol/L than in subjects with FPG < 5.3 mmol/L (RR =8.42,95% CI 5.11-13.82, P < 0.05);(3) Survival analysis showed that the risk of death was 1.47 times higher in subjects with FPG ≥ 5.3 mmol/L than in subjects with FPG < 5.3 mmol/L after years of followed-up (RR=l.47, 95%CI 1.09-1.98, P=0.0127).Conclusion The risks for IGR, CVD and mortality are higher in the elderly with FPG ≥5.3 mmol/L, which highlights the importance for the disease prevention in elder people with FPG 5.3 mmol/L or more.

Objective To investigate the safety and effectiveness of left internal mammary artery (LIMA) angiography via right transradial approach with 5F universal coronary angiography catheter. Methods Left internal mammary artery (LIMA) angiography via right transradial approach with 5F universal coronary angiography catheter was performed following coronary angiography in 86 patients.The success rate of the procedure, the result of LIMA angiography, the time to LIMA, the volume of contrast, the incidence of complications and bypass vessels in patients who received coronary artery bypass grafting (CABG) were recorded and analysed. Results The success rate was 97.6%(84/86), and the result of LIMA angiography was satisfied; the time to LIMA was (6.5 ± 2.8) min, and the volume of contrast was (17.6 ± 11.1) ml;the incident of complications was 4.7%(4/86);56 patients received CABG. LIMA was harvested in all cases and left radial artery in 53 cases. Conclusions LIMA angiography via right transradial approach with 5F universal coronary angiography catheter is safe and effective, and it has significant sense to patients who will receive CABG.
3. Application of hydrosurgery system in debridement of burn wound: An overview

Debridement is an indispensable step in repairing of burn wounds. Hydrosurgery system is an advanced instrument for debridement. Compared with the conventional debridement instruments, the hand-piece of hydrosurgery system is more facile and easy to be controlled, which is helpful for the debridement of wounds with irregular shape and uneven surface; besides, hydrosurgery system combines necrotic tissue resection and wound perfusion simultaneously, and its debridement process is highly tissue-selective, which is helpful to improve the wound-bed and to create favorable conditions for skin grafting and wound healing. Hydrosurgery system has been used in burn wounds debridement for more than 10 years. It is widely used abroad and has been introduced to China in recent years. Due to the complexity of burn wounds, there are also many different opinions about the clinical use of hydrosurgery system for different wounds, including debridement efficiency, effect, cost and the amount of bleeding. In this paper, we systematically summarized the effect of hydrosurgery system on the debridement of burn wounds, so as to provide reference for clinicians.
4.Interaction between ambient particles and ozone and its effect on daily mortality.

OBJECTIVETo examine the effect of particulate matter (PM) less than 10 microns in diameter (PM10) and ozone (O3) on daily mortality in Shanghai, China.
METHODSA generalized additive model with penalized spline function was used to observe the acute effect of PM10 and O3 on daily mortality.
RESULTSHigher PM10 significantly increased the effect of O3 on total mortality, and O3 also increased the effect of PM10 although the estimated increment was statistically insignificant.
CONCLUSIONOur findings provide further evidence for the effect of PM10 and O3 on daily mortality.

5.Exogenous carbon monoxide protects against liver injury induced by ischemia/reperfusion of hind limbs in rats.

AIMTo investigate the protective effect of exogenous carbon monoxide (CO) on the liver injury induced by ischemia/reperfusion (I/R) of hind limbs in rats.
METHODS100 SD rats were divided randomly into sham operated group (S), S+ CO group (SC), I/R group (I/R), I/ R+ CO group (RC). A rat model of ischemia in hind limbs and the reperfusion liver injury was established with the occlusion of the femoral arteries for 4 h and re-opening for 6 - 72 h, 10 d. The rats in SC and RC groups were exposed to air containing CO (the volume traction of CO: 0.05%) for 2 h before and after reperfusion or the corresponding control time point, while the other two groups were exposed to the routine air. The pathologic changes of liver tissue were morphologically observed by HE stain. Serum GPT activity was tested by Automatic Biochemical Analyzer. The percentage of apoptosis, expression levels of bax and bcl-2 protein in liver tissue were detected by Flow Cytometry.
RESULTSThere was no difference between S and SC groups. Compared with SC group: (1) Pathological changes in liver tissue were significant in I/R and RC groups. (2) The serum GPT activity of I/R and RC groups was obviously increased. (3) In IR and RC groups, the percentage of apoptosis in liver tissue was all significantly increased. (4) The bax expression level was significantly increased. Compared RC group with I/R group: (1) Pathological change was slight. (2) The serum GPT activity was depressed. (3) The percentage of apoptosis and expression level of bax protein in liver tissue were depressed. (4) The expression level of bcl-2 protein in liver tissue was increased.
CONCLUSIONExogenous CO could attenuate liver tissue injury induced by limbs I/R in rats.

6.Clinical application of bone marrow mesenchymal stem cell transplantation in children with severe mental retardation

Objective To observe the clinical efficacy of transplantation with autologous bone marrow mesenchymal stem cells (MSCs) in the treatment of children with severe mental retardation (MR). Methods Fifty children with severe MR (2-6 years old), admitted to our hospital from May 2008 to December 2009, were chosen; self-control observation on the mental and motor developments of these children in the same length of time periods was performed 6 months before and 6 months after the autologous bone marrow MSCs transplantation. Meanwhile, the improvements of each patient were followed up and analyzed 1, 3 and 6 months after the transplantation. Results Two patients failed in the culture of bone marrow MSCs; 6 patients failed in follow-up; the other 42 were observed completely.Significant improvement was noted in 25 patients after the transplantation with an effective rate reaching 59.5%; gradual mental and motor improvements appeared in these 25 patients 1 month after the transplantation, and lasted till the 6th month of transplantation. The average scores of mental improvement were (4.67±4.20) in 6 months before transplantation, and (17.12±13.05) in 6 months after transplantation (t=6.816, P=0.000). The average times having gross motor improvement 6 months before and 6 months after transplantation were (1.019±0.78) and (1.15±0.91), respectively (t=1.610, P>0.05); the average times having fine motor improvement were (0.89±0.79) and (3.40±3.16) months in 6 months before and 6months after transplantation (t=5.030, P<0.05); the average times having language improvement were (0.99±0.95) and (3.23±3.30) months in 6 months before and 6 months after transplantation (t=4.000, P<0.05); the average times having personal-social behavior progress were (1.03±0.81) and (2.95±3.68)months in 6 months before and 6 months after transplantation (t=3.509, P<0.05); the average times having adaptive behavior progress were (0.88±1.22) and (1.19±1.48) months in 6 months before and 6 months after transplantation (t=1.322, P>0.05). Conclusion Autologous bone marrow MSCs transplantation can improve the intellectual development in a short time, mainly in the fine motor, language and personal-social behavior, in children with severe MR. Bone marrow MSCs transplantation for severe MR is safe and effective in a short-term.
7.Clinical effect of stem cell transplantation via hepatic artery in the treatment of type II hyperammonemia: a report on 6 cases.

This study aimed to investigate the clinical effect of transplantation of CD133⁺ peripheral blood stem cells or umbilical cord mesenchymal stem cells via the hepatic artery in children with type II hyperammonemia and its possible action mechanism. Umbilical cord mesenchymal stem cells were obtained by collecting cord blood (100-150 mL) from healthy fetuses and separating stem cell suspension (5 mL) from the cord blood by hydroxyethyl starch sedimentation. CD133⁺ peripheral blood stem cells were obtained by mobilizing peripheral blood from the fathers of sick children using recombinant human granulocyte colony-stimulating factor for 5 days, collecting mononuclear cells (120 mL), and separating out CD133⁺ cells by sorting. With catheterization and percutaneous puncture, the obtained stem cells were slowly injected into the liver of sick children via the hepatic artery. The changes in clinical symptoms and laboratory indices such as blood ammonia, liver function, and arginine and citrulline concentrations were observed. After stem cell transplantation via the hepatic artery, the 6 children showed significantly decreased blood ammonia levels, and their blood ammonia levels slowly increased 1 to 2 weeks later, but remained below 100 μmol/L, and changes in glutamic-pyruvic transaminase levels were similar to blood ammonia. Plasma citrulline and arginine concentrations increased significantly after transplantation and the increase in citrulline level exceeded the increase in arginine level. An 8 months follow-up visit for one typical patient showed that the weight and height increased after transplantation and sleep was improved without night crying. The child could actively gaze at interesting objects instead of responding indifferently and started to say simple words. With regard to fine motor skills, the child could pinch things with the thumb and middle finger instead of displaying a lack of hand-eye coordination and progress was also made in gross motor skills. Gesell test showed that the child made progress for an average of 3.82 months in all areas. It was concluded that after stem cell transplantation, children with type II hyperammonemia have decreased blood ammonia levels, stable and improved liver function and steadily increased plasma citrulline and arginine concentrations. They display a progressive trend in such aspects as movement, language and environmental adaptability. It is hypothesized that stem cell transplantation via the hepatic artery partially or totally activates, or provides supplementary ornithine carbamoyl transferase, so that plasma citrulline and arginine concentrations increase and urea cycle disorder can be corrected to some extent.

8.Association between diurnal temperature range and respiratory tract infections.

OBJECTIVEThis study aimed to assess the association between emergency-room visits for respiratory tract infection (RTI) with diurnal temperature range (DTR), a weather parameter closely associated with urbanization and global climate change.
METHODSWe conducted a semiparametric time-series analysis to estimate the percentage increase in emergency-room visits for RTI associated with changes in DTR after adjustment for daily weather conditions (temperature and relative humidity) and outdoor air pollution.
RESULTSDTR was significantly associated with daily emergency-room visits for RTI. An increase of 1 °C in the current-day (L0) and in the 2-day moving average (L01) DTR corresponded to a 0.94% [95% confidence interval (CI), 0.34%-1.55%] and 2.08% (95% CI, 1.24%-2.93%) increase in emergency-room visits for RTI, respectively.
CONCLUSIONDTR was associated with increased risk of RTI. More studies are needed to understand the impact of DTR on respiratory health.

9.Application of global positioning systems and geographic information systems in drinking water defluoridation project in Liaoning province

Objective To establish information management systems of drinking water defluoridation project in water-related endemic fluorosis areas and investigate the status of drinking water defluoridation project in Liaoning, provide the basis for the development of control measures. Methods Global positioning systems (GPS)and geographic information systems(GIS) were used in the study in August 2006 - July 2008. Water defluoridation projects of 1234 in 48 counties(cities, districts) in drinking water type of fluorosis areas were positioned. Latitude and longitude, water samples, water fluoride content were collected or tested. GIS was used to establish information management system of water defluoridation projects. Results We have established information management system for the facilities of decreasing water fluorine in drinking water type of endemic fluorosis regions in Liaoning. One thousand two hundred and thirty four defluoridation facilities distributed in east longitude between 39.39° - 43.37°,north latitude between 119.25° - 125.50°, and altitude between - 6.60 and 801.14 meter in 48 endemic fluorosis counties in 13 cities. Nine hundred and twenty seven facilities for decreasing fluorine were able to supply water regularly, accounting for 75.1% of investigated projects;29 facilities was .not yet completely rebuilt, accounting for 2.4%;278 facilities(supply water for 344 villages) were out of order or discarded for 22.5% of investigated projects.Water fluorine contents of 63 facilities were greater than 1.2 mg/L, accounting for 6.8% of investigation project.Facilities working regularly and water fluorine was in accord with hygienic standard for drinking water facilities were 70.0%. Conclusions The establishment of Liaoning province defluoridation project information management system in the whole province of drinking water type of fluorosis areas provides scientific basis for accurate decision-making on prevention and control of the disease.
10.The G990T mutation of the FRMD7 gene in a Chinese family with congenital idiopathic nystagmus.

OBJECTIVETo study the mutation of FRMD7 gene in a Chinese family with congenital idiopathic nystagmus.
METHODSForty-six individuals in the Chinese family with congenital idiopathic nystagmus, including 16 patients, 19 normal siblings and 11 spouses, were investigated under informed consent. Genomic DNA of all 46 members was isolated by standard protocol. The X-linked inherited pattern was ascertained by investigating the history of the family members and the clinical feature of each individual. The genome scan on X chromosome was performed after PCR amplification for microsatellite markers. LOD scores were calculated with Linkage 5.1. Direct DNA sequence analysis was carried out to find the gene mutation responsible for the disease.
RESULTSA maximum LOD score of 8.55 (theta=0) was obtained with polymorphic marker DXS1047. Haplotype construction of the family defined the disease interval between DXS8059 and DXS8033. Direct DNA sequence analysis revealed a heterozygous mutation of G990T in exon 9 of the FRMD7 gene in all patients, which was not present in unaffected family members.
CONCLUSIONCongenital nystagmus is a clinically and genetically heterogeneous ocular movement disease. The mutation of G990T of the FRMD7 gene is the underlying molecular pathogenesis for this family with congenital nystagmus.
