3.Bacteria Contens in Air of Tuberculosis Wads Before and After Use:A Comparative Study

OBJECTIVE To detect the condition of bacteria of the air in wards of our hospital in order to reduce the possibility of air spread. METHODS Ten tuberculosis wards were chosen at random for study.Detected the bacteria content of the air in wards used before and after respectively and analysed the results. RESULTS The average of backgroud bacteria was 164 CFU/m 3 before the wards were used.Two years later,the average of bacteria increased to 682 CFU/m 3.The ratio was 1∶3:2 on average.There were significant differences between them. CONCLUSIONS Humen are the main facters that make the air polluted in these wards.The bacteria content will decline by good environment cleaning sanitation,ventilation and strengthening steriling management.Infection will decline in the hospital.

Objective To explore the correlation between hepatitis B virus large surface protein(HBV-LP)and hepatitis B virus deoxyribonucleic acid(HBV-DNA),analyze the clinical significance of detecting the two index dynamically for diagnosis and treatment of hepatitis B.Methods Enzyme linked immunosorbent assay(ELISA)and fluorescent quantitation polymerase chain reaction(FQ-PCR)were used to detect the levels of HBV-LP and HBV-DNA in serum specimen of 230 hepatitis B patients.Results There was a positive correlation between the content of HBV-LP and copy numbers of HBV-DNA in serum of hepatitis B positive cases(r=0.84,P<0.01).The positive rate was 84.78% in HBV-LP and 84.35% in HBV-DNA of 230 HBV positive cases.As a result there was no significant difference(P>0.05)in this study.Therefore,the positive rate was 63.63 %(35/55)in HBV-LP and 58.18%(32/55)in HBV-DNA of 55 HBV negative cases.There was also no significant difference(P>0.05)in this study.However,the positive rate of HBV-LP(82.47%)was higher than HBE(52.06%)in the 194 HBV-DNA positive cases.There was significantly different(P<0.01).Conclusion HBV-LP and HBV-DNA are the significant index of the degree of hepatitis B virus replication in hepatitis B positive cases,especially in determining the virus replication and prognosis of treatment in HBe Ag negative,providing the reliable laboratory data for the clinical diagnosis and treatment.

7.The mechanism of Glutathione in the prevention of ventilator -associated lung injury

Objective To investigate the mechanism of Glutathione in the prevention of ventilator-associated lung injury.Methods 30 mechanical ventilation patients who were healthy before were selected and randomly divided into the two groups.Group A was the high tidal volume group:10mL/kg;group B was used the tidal volume plus the injection of Glutathione(2.4g/d).After 48 hours,we collected lavage fluid and use Coomassie brilliant blue to detect the protein content of alveolar lavage fluid of the two groups.Results There was a significant difference (P<0.01)between the two groups.The protein content in group A[(9.90 ±2.25)g/L]was higher than that of group B [(4.17 ±0.39)g/L],and there was statistically significant (P<0.01).Conclusion Glutathione can reduce the incidence of ventilator-associated lung injury and prevent the occurrence of ventilator-associated lung injury.
8.Practice and exploration on clinical evaluations of hospital-made Chinese herbal formulas in the Pudong new area of Shanghai.

Chinese herbal formulas can well present the characteristics of traditional Chinese medicine (TCM) with their simple, convenient, inexpensive and effective uses. However, due to the high cost of production, manufacturing pharmacies inside the hospital closed down one after another, which rendered the difficult situation of developing preparation of Chinese herbal formulas. The Pudong New Area of Shanghai, as a pilot region for comprehensive reforms on national development of TCM, vigorously explores the standardized research on and application of hospital-made Chinese herbal formulas. The Health Bureau of the Pudong New Area, based on the Shuguang Hospital, has established a clinical evaluation center for hospital-made Chinese herbal formulas. Through screening, manufacturing, quality control, unified allocation, and standardized clinical evaluation, the clinical evaluation center has summarized its experience on these processes.
9.Infectivity of different human immunodeficiency virus strains for mucosal epithelial cell lines

Objective To compare the infectivity between laboratory adapted human inununodefi- ciency virus(HIV-1) and primary HIV-1 isolates for different mucosal epithelial cell lines. Methods Mu-cosal epithelial cells Caco-2, T-84, HeLa and lymphocyte MT-4 were infected with laboratory adapted HIV-1 SF33 and 2 primary HIV-1 isolates (02010561, 02010141). Culture supernatant and cells were collected respectively on 3-4 days interval after virus inoculation. The former was tested for HIV-1 antigen P24 level and viral load, and the latter was tested for total viral DNA and integrated viral DNA. Results All 3 virus strains could infect MT-4 cells and integrate into their genome. Only HIV-1 SF33 could infect Caco-2 cells but could not integrate into their genomic DNA. Both HIV-1 SF33 and 02010561 infected HeLa cells but only integration of HIV-1 SF33 was detected. All the 3 HIV-1 strains infected T-84 cells but only the integra-tion of HIV-1 SF33 and 02010141 was observed. Conclusion Although laboratory adapted and primary HIV-1 strains are able to infect human mucosal epithelial cell lines, transient or productive infection estab-lished in different mucosal epithelial cells is dependent on the character of cells and virus strains.
10.Analysis of clinical features and prognostic factors in middle and old age patients with angioimmunoblastic T-cell lymphoma

Objective:To investigate clinical features, diagnosis and treatment of angioimmunoblastic T-cell lymphoma(AITL)in middle and old age patients.Methods:This was a retrospective study.A total of 33 middle-aged and elderly patients(a median age of 64 years, range 47~85 years)with AITL admitted to our hospital from May 2008 to March 2017, including 54.5% male(18 cases), were enrolled in this study.Clinical manifestations, pathology, imaging and survival data of patients were collected.The objective response rate(ORR)of patients with different therapeutic regimens was analyzed.The survival analysis was conducted by using the Kaplan-Meier method, the survival rate was analyzed by using the Log-rank method, and multivariate analysis was conducted by using the proportional hazards regression model.Results:The median overall survival(OS)was 26.0 months(8.5-43.5 months). The 1-year, 3-year and 5-year OS rate was 66.7%(22 cases), 45.5%(15 cases)and 24.2%(8 cases), respectively.The ORR of first-line chemotherapy with CHOP-like regimens(cyclophosphamide, doxorubicin, vincristine, prednisone)was 65.5%(19/29)and the incidence of serious adverse reactions was 64.5%(20/31). Single-factor chi-square testing showed that age ≥60 years, Barthel score ≥90, Eastern Cooperative Oncology Group performance status score(ECOG-PS)≥2, anemia, International prognostic index(IPI)score of 4~5, receiving chidamide treatment were influncing factors for the prognosis in middle-aged and elderly patients with AITL( χ2=5.103, 4.306, 6.004, 4.030, 6.348 and 4.080, P<0.05). Cox multivariate analysis showed that age ≥60 years and receiving chidamide treatment were independent prognostic factors affecting the 5-year survival rate of middle-aged and elderly AITL patients( OR=0.313 and 4.964, P<0.05). That the OS was better in the group receiving chidamide treatment than in the group without chidamide treatment( P<0.05). Conclusions:Clinical features of AITL are diverse and lack of specificity.Most patients present with advanced stage AITL at the initial diagnosis.The 5-year OS rate is low.AITL patients aged over 60 years have a poor prognosis.Chidamide can improve the OS rate.
