Objective Pericardiocentesis and drainage are the common measures for the treatment of cardiac temponade and massive pericardial effusion. In this issue, the complications of pericardiocentesis and drainage were discussed. Methods 5 in-hospital cases of cardiac temponade or massive pericardial efiusion who had complications of pericardiocentesis and drainage were reported and analyzed. Results The complications were 2 cases with neural mediated syncope, 1 case with pleural efiusion, 1 case dead of acute right ventricular dilation, and 1 case with acute pulmonary edema and effusion leaking into the subcutaneous tissue. Conclusion The importance of hemodynamic change during pericardiocentesis and drainage could not be over-emphasized, and the patients whose hemodynamic status were unstable should be monitored by bedside hemodynamic monitoring or echocardiography. Also we should pay more attention to the drainage catheter per se, which may cause the complications.

An analysis of the fundamental concepts for public hospital reform further defined public hospitals and other hospitals,business and non-business in nature,public benefits and welfare,as well as public benefits and profit-oriented hospitals.These distinctions can help reformers of public hospitals clarify their concepts,make clear the purpose and direction of the public hospitals reform,so as to speed up the reform with theory support.

This paper studied the development stages in terms of the public nature of public hospitals in China and analyzed the causes for their weakened public benefits.It proposed the principle of government guidance,achieving government purchasing of healthcare services as the roadmap,as well as building and improving the service purchasing system,performance appraisal system,and payment review system along with supervision system.These measures are designed to make public hospitals in their public benefit nature with scientific system design,powerful institutional guarantee,and efficient execution strategies.

ObjectiveTo analyze the effects of three different display devices on the interpretation of imaging data in medieal imaging system.MethodsThe luminance-meter L100 and the test patterns provided by the American Association of Physicists in Medicine(AAPM)were used to assess the performance of three display devices(the spherical,flat panel and liquid crystal display devices).The corresponding luminance response was compared with the reference criteria provided by AAPM Task Group 18(AAPM TG18).DR chest photography was taken on CDRAD2.0 contrast-detail phantom using the following experimental parameters:100 mA,80 kVp,and the exposure times for 6,10,12,and 16 ms.After each exposure,the surface dose of phantom was recorded and the image quality factor(IQF)was calculated.Statistical analysis of IQF was performed using ANOVA. ResultsThe maximum luminance values in the spherical,flat panel and liquid crystal display devices were 170,59 and 231 cd/m2 and the luminance ratio was 257, 99 and 350 respectively.There was a significant difference in the IQF bewteen the liquid crystal display device and other two display devices.Conclusion The liquid crystal display device has the better image quality.

Objective To investigate the expression of inhibitors of apoptosis (IAPs) in non-small cell lung cancer (NSCLC) patients and its clinical significance. Methods The mRNA expression level and protein level of survivin, hIAP-1 (human IAP-1), hIAP-2 (human IAP-2), and XIAP (X chromosome-linked IAP) in 36 NSCLC patients and 36 controls were analyzed by semi-quantitative reverse transcription polymerase chain reaction (RT-PCR) and Western blotting, respectively. BACKGROUND: Previous studies have proved platelet-rich fibrin (PRF) with osteoinduction ability, and the centrifugal speed and time to prepare rabbit advanced PRF (A-PRF) with the most similar structure to that of human PRF have been determined.OBJECTIVE: To observe the histological changes during A-PRF-induced osteogenesis.METHODS: Thirty Japanese white rabbits were randomly divided into A-PRF and blank control groups (n=15 per group).The full-thickness defect models were established on the rabbit parietal bone, followed by implanted with A-PRF or nothing, respectively. The model rabbits were killed immediately, at 2, 4, 8 and 12 weeks after modeling, to grossly observe the bone formation, and the histological changes in the defect region were observed through hematoxylin-eosin staining, Masson staining and immunohistochemistry.RESULTS AND CONCLUSION: Unhealed defects were observed in the blank control group. Gross and histological observations showed that the speed, amount and maturity of bone formation in the A-PRF group were significantly better than those in the blank control group immediately, 4, 8 and 12 weeks after modeling (P < 0.05). Our findings suggest that the rabbit skull bone defect is successfully established. A-PRF can induce osteogenesis, and more mature newly born bones appear with time. Additionally, osteoclasts can act with osteoblasts synergically under the A-PRF induction to promote the bone formation.

Objective To detect the expression of DBC2 and the methylation status of its gene promoter region in gastric cancer,so as to analyze their correlation. Methods Gastric cancer tissues and adjacent normal tissues were collected from 92 cases as research subjects. The DBC2 mRNA expression and the methylation status of DBC2 gene promoter region in gastric cancer were detected by using RT?PCR and MS?PCR respectively. In addition,the correlation of DBC2 gene methylation status with clinicopathological parameters was analyzed. Results In comparison with adjacent normal tissues,gastric cancer tissues displayed a low expression level of DBC2mRNA and a high methylation level of gene promoter region. Besides, the methylation level was significantly correlated with tumor invasion depth,differentiation and TNM staging(P<0.05). With deep invasion,poor differentiation or higher TNM staging,methylation level of gene promoter region was more significant in gastric cancer tissues. Conclusion High methylation of DBC2 gene promoter region may be a significant contributing factor leading to low DBC2 expression.

Objective: To investigate the effects of fosinppril on myocardial cell apoptosis and apoptosis-associated gene expression in chronic heart failure (CHF) rats.
Methods: CHF model was established by partially banding of abdominal aorta superior to renal artery. The experimental rats were randomly divided into 3 groups: Sham operation group and CHF group, the rats in both groups received stomach normal saline; Fosinopril group, the rats received stomach fosinopril 10 mg/kg?d.n=10 in each group and all animals were treated for 8 weeks. LVEDP, ±dp/dtmax and LVMI were examined, left ventricular myocardial cell apoptotic index (AI) was measured by TUNEL method, Bcl-2 and Bax protein levels were detected by immunohistochemistry and caspase-3 protein expression was assessed by Western blotting.
Results: Compared with Sham operation group, CHF group had increased LVEDP, LVMI, AI, elevated protein expressions of Bax and Caspase-3,P<0.01; while decreased ±dp/dtmax, reduced protein expression of Bcl-2 and the ratio of Bcl-2/Bax,P<0.01. Compared with CHF group, Fosinopril group presented decreased LVEDP, LVMI, AI, reduced protein expressions of Bax and Caspase-3,P<0.01; while increased ±dp/dtmax, elevated protein expression of Bcl-2 and the ratio of Bcl-2/Bax, P<0.01.
Conclusion: Fosinopril may inhibit myocardial cell apoptosis which occurred during CHF, by up-regulating Bcl-2 expression and down-regulating expressions of Bax and Caspase-3 in CHF rats, therefore improve the cardiac function.

For the purpose of resolving such current problems as low inclusion ratio and high exit ratio in the single-disease clinical pathway management,the paper proposed an idea of clinical pathways grouping management,in line with DRG management and based on traditional clinical pathway management practice and China' s conditions.Proposed in the paper are such measures for government service procurement,as quality control mechanism,price negotiation mechanism,performance-based payment mechanism,and supervision mechanism.These measures are designed to provide incentives for medical institutions and medical workers,control irrational rise of medical expense,optimize expense makeup,and improve both quality of care and patient satisfaction.Thanks to the China Rural Health Development Project in place in Henan province where DRG-based clinical pathway grouping management and government service procurement are in trials,the following outcomes are achieved.Namely,significant expansion of diseases coverage and population coverage,efficient control of per-patient/per-visit expenses,and a steady rise of surgery and treatment expenses which embody value of service of medical workers.Other benefits include 50％ to less than 30％drop of drugs proportion,gradual optimization of medical expenses makeup,obvious improvement of quality of care.These measures have made hospitals,insurance providers and patients satisfactory.
