1.Application of responsibility system for holistic nursing in high-quality nursing service in the department of respiratory medicine

Objective To evaluate the application efficacy of responsibility system for holistic nursing system in high-quality nursing service in the department of respiratory medicine.Methods Department of respiratory medicine unit which was the quality nursing service demonstrated unit in our hospital was selected.The nursing responsibility-bound grouping system was first performed and the nurse-incharge would provide the patients with whole and continuous nursing service.Before and 6 months after the application,the nursing job satisfaction was conducted and the nursing grouping system satisfaction was also evaluated among nurses and doctors.Results Six months after the application of nursing responsibility-bound grouping system,the patient satisfaction with nursing job was raised obviously in the unit.Conclusions The application of responsibility-bound grouping system can not only promote the activity but also mobilize the enthusiasm of nurses,and then high-quality nursing service was resulted.So the service system was worthy of clinical application.

Objective To investigate the effects of penehyclidine hydrochloride (PHCD) pretreatment on hypoxia-inducible factor-1alpha (HIF-1α) expression in lung tissues in rats with acute lung injury (ALI) induced by lipopolysaccharide (LPS).Methods One hundred and twenty female Sprague-Dawley rats,weighing 180-220g,were randomly divided into 3 groups (n =40 each):control group (group C),ALI group and PHCD pretreatment group (group P).ALI was induced by intraperitoneal LPS 5 mg/kg in ALI and P groups.The equal volume of normal saline was given intraperitoneally in group C.Group P received intraperitoneal PHCD 2 mg/kg at 30 min before LPS administration.Eight rats in each group were chosen at 2,4,8 and 24 h after LPS administration,and the lung was removed for determination of HIF-1α mRNA expression in lung tissues by RT-PCR.Eight rats in each group were chosen at 6 h after LPS administration,and the lung was removed for determination of W/D lung weight ratio and IL-6 content (by ELISA) and for microscopic examination.Results The W/D lung weight ratio and IL-6 content at 6 h after LPS administration and HIF-1α mRNA expression at each time point after LPS administration were significantly higher in groups P and ALI than in group C (P < 0.05),and lower in group P than in group ALI(P < 0.05).The pathological damage was significantly ameliorated in group P as compared with group ALI.Conclusion PHCD can reduce LPS-induced ALI through down-regulation of HIF-1α expression in lung tissues and inhibition of the inflammatory response in rats.
4.Isokinetic muscle strength test for the muscular force after repair of ruptured Achilles tendon

BACKGROUND:Restoration of tendon integrity and tenacity, and maintenance of ankle joint flexibility, play a crucial role on the rupture of Achiles tendon. OBJECTIVE: To evaluate the muscular force using isokinetic muscle strength test after the ruptured Achiles tendon is repaired, and to assess the repairing effect. METHODS: Muscle power including peak torque, peak torque/body weight, total work, average peak torque, set total work of flexor and extensor of 17 patients with Achiles tendon rupture were assessed after reconstruction using Biodex system 4 at 60 (°)/s and 120(°)/s. RESULTS AND CONCLUSION:The peak torque, peak torque/body weight, total work, average peak torque, set total work of flexor muscle group at the injured side were significantly decreased compared with uninjured side at 60 (°)/s and 120 (°)/s (P < 0.05). Isokinetic muscle strength test can reflect the muscular force after the ruptured Achiles tendon is repaired, and evaluate the repairing effect.

6.Relation of Fundus Changes and Urinary Microalbumin with TCM Syndrome Types of Hypertension Combined with Metabolic Syndrome

Objective To study the relation of different TCM syndrome types of hypertension combined with metabolic syndrome (MS) patients with retinal vascular changes and the urinary microalbumin, and provide objective basis for TCM treatment with syndrome differentiation. Methods Seventy-six patients of hypertension combined with MS (divided into four syndrome types) and 20 patients with simple hypertension were selected. General data, biochemical indexes, retinal vascular changes and the urinary microalbumin were detected and compared. Results In patients with hypertension combined with MS, blood lipid disorders and high blood sugar was mainly focused on the type of phlegm and blood stasis mutual and the type of qi and yin deficiency. The incidence of eye and kidney damage in hypertension with MS patients was higher than the simple hypertension patients. The positive rates of retinal blood vessels and renal damage in qi and yin deficiency type and phlegm and blood stasis mutual type were higher than that in other syndrome types. Conclusion Hypertension with MS patients should pay attention to the prevention and control of retinal vascular and renal vascular complications. Fundus examination and detection of early renal damage index can be used as the basis of TCM syndrome differentiation of hypertension with MS.
7.Investigation of Ascending Aortic Distensibility and Risk Factors in Pre-hypertension Patients

Objective: To explore the features of ascending aortic distensibility (AAD) and risk factors in pre-hypertension (PHT) patients. Methods: A total of 206 participants who received retrospective ECG-triggered coronary CTA by physical check-up or by clinically suspected coronary artery disease (CTA) were enrolled. The participants were divided into 2 groups: Control group, the subjects with normal blood pressure,n=85 and Pre-hypertension group,n=121. A 128 slice dual-source CT scanner was used and the image was automatically reconstructed at every 5% absolute phases in entire R-R interval. The beginning of left coronary artery plane was deifned as the reference and 25 mm above the reference plane was deifned as interested region. ADD value was calculated. Results: Compared with Control group, Pre-hypertension group had decreased AAD,P<0.01 and similar normalized cross-sectional area (Ss),P>0.05; ADD value was similar among different gender and blood lipid levels. Correlation analysis presented that AAD was negatively related to age (r=-0.69,P=0.001), systolic blood pressure (r=-0.37,P=0.001), pulse pressure(r=-0.43,P=0.001) and glycosylated hemoglobin (r=-0.43,P<0.05). Age and systolic blood pressure were the independent risk factors for AAD decline (standardized β=-0.66,P=0.001) and (standardized β=-0.44,P=0.001). Conclusion: Without additional contrast media consumption and radiation dosage, retrospective ECG-triggered coronary CTA may detect AAD changes with risk factors at the early stage in pre-hypertension patients which is helpful to distinguish the high risk individuals.

AIM: To study the effects of Liquestrazin on succinic dehydrogenase (SDH) and cytochrome oxidase (CCO) in the myocardial mitochondria of the ischemia-reperfusion rats and its mechanism. 9.Clinical analysis of the application of autologous blood transfusion in neurosurgery of primary hospital

Objectives To observe the clinical significance and application value of autologous blood transfusion in neurosurgery of primary hospital. Methods Four hundred and fourteen patients who underwent the neurosurgery operation and were subjected to intraoperative blood transfusion were selected, among whom 97 patients were subjected to autologous blood transfusion (observation group), and 317 patients were subjected to heterogenous blood transfusion (control group). The condition of intraoperative blood transfusion, changes of hemoglobin and hematocrit, blood transfusion related cost were compared between 2 groups. Results There were no statistical differences in operation time, infusion volume, rate of transfusion related complications and postoperation hemoglobin, hematocrit between observation group and control group (P>0.05). The patients in control group were infused with 189 000 ml, and the transfusion liquid volume proportion of total blood transfusion was 79.22%(189 000/238 580);13 patients in observation group were used the heterogenous blood transfusion with 5 400 ml, and the transfusion liquid volume proportion of total blood transfusion was 10.30%(5 400/52 430). Eighty-six patients (88.66%, 86/97) in observation group performed autologous blood collection and transfusion, the volume of autologous collection was 80 650 ml, and the volume of transfusion was 47 020 ml. Eleven patients in observation group did not perform autologous blood transfusion, among whom 6 patients was because of operational and mechanical reasons, and 5 patients performed collection but did not transfuse. The cost of heterogenous concentrated suspension red blood cell over 6 U was significantly higher than the cost of disposable material and injection of autologous blood:(2 287.06 ± 243.52) yuan vs. (1 595.08 ± 133.95) yuan, and there was statistical difference (P<0.05). The rate of heterogenous concentrated suspension red blood cell 6 U in control group was 14.83%(47/317), and the rate of over 6 U was 6.62%(21/317). Conclusions The autologous blood transfusion is safe and effective, and it is worth popularizing in neurosurgery of primary hospital. But in the process of its application, it is necessary to strengthen the user′s operating skills and ensure the quality of autologous blood transfusion.
10.An Analysis of Clinical Characteristics of Vertigo in Children

Objective To investigate the data of outpatient children,survey on the clinical characteristics of vertigo and provide baseline information for clinical diagnosis and treatment.Methods The questionnaires and clinical tests data of 553 children with vertigo were retrospectively analyzed.Results All the children and their parents received interviews,otological examinations and clinical audiological tests. Their ages were between 4 and 15,with the average at 9.51?2.83. ①The males were 341(61.66%) and female 212 (38.34%),with the gender ratio of male to female as 1.61:1. ②The peak of the age curves of treatment and the first onset of vertigo was 9.51 and 8.62-year-old,respectively. ③20.98% of children with vertigo had normal results with peripheral vertigo more common in the rest.④The semicircular canals were more likely to dysfunction if their relatives had vertigo or car sickness.Conclusion The age of children mostly with vertigo is between 6 and 11 years with more males than females. Vertigo itself can be a symptom but on the other hand,some show no positive signs even with vertigo.
