1.Application of responsibility system for holistic nursing in high-quality nursing service in the department of respiratory medicine

Objective To evaluate the application efficacy of responsibility system for holistic nursing system in high-quality nursing service in the department of respiratory medicine.Methods Department of respiratory medicine unit which was the quality nursing service demonstrated unit in our hospital was selected.The nursing responsibility-bound grouping system was first performed and the nurse-incharge would provide the patients with whole and continuous nursing service.Before and 6 months after the application,the nursing job satisfaction was conducted and the nursing grouping system satisfaction was also evaluated among nurses and doctors.Results Six months after the application of nursing responsibility-bound grouping system,the patient satisfaction with nursing job was raised obviously in the unit.Conclusions The application of responsibility-bound grouping system can not only promote the activity but also mobilize the enthusiasm of nurses,and then high-quality nursing service was resulted.So the service system was worthy of clinical application.

Objective To investigate the effects of penehyclidine hydrochloride (PHCD) pretreatment on hypoxia-inducible factor-1alpha (HIF-1α) expression in lung tissues in rats with acute lung injury (ALI) induced by lipopolysaccharide (LPS).Methods One hundred and twenty female Sprague-Dawley rats,weighing 180-220g,were randomly divided into 3 groups (n =40 each):control group (group C),ALI group and PHCD pretreatment group (group P).ALI was induced by intraperitoneal LPS 5 mg/kg in ALI and P groups.The equal volume of normal saline was given intraperitoneally in group C.Group P received intraperitoneal PHCD 2 mg/kg at 30 min before LPS administration.Eight rats in each group were chosen at 2,4,8 and 24 h after LPS administration,and the lung was removed for determination of HIF-1α mRNA expression in lung tissues by RT-PCR.Eight rats in each group were chosen at 6 h after LPS administration,and the lung was removed for determination of W/D lung weight ratio and IL-6 content (by ELISA) and for microscopic examination.Results The W/D lung weight ratio and IL-6 content at 6 h after LPS administration and HIF-1α mRNA expression at each time point after LPS administration were significantly higher in groups P and ALI than in group C (P < 0.05),and lower in group P than in group ALI(P < 0.05).The pathological damage was significantly ameliorated in group P as compared with group ALI.Conclusion PHCD can reduce LPS-induced ALI through down-regulation of HIF-1α expression in lung tissues and inhibition of the inflammatory response in rats.

5.Relation of Fundus Changes and Urinary Microalbumin with TCM Syndrome Types of Hypertension Combined with Metabolic Syndrome

Objective To study the relation of different TCM syndrome types of hypertension combined with metabolic syndrome (MS) patients with retinal vascular changes and the urinary microalbumin, and provide objective basis for TCM treatment with syndrome differentiation. Methods Seventy-six patients of hypertension combined with MS (divided into four syndrome types) and 20 patients with simple hypertension were selected. General data, biochemical indexes, retinal vascular changes and the urinary microalbumin were detected and compared. Results In patients with hypertension combined with MS, blood lipid disorders and high blood sugar was mainly focused on the type of phlegm and blood stasis mutual and the type of qi and yin deficiency. The incidence of eye and kidney damage in hypertension with MS patients was higher than the simple hypertension patients. The positive rates of retinal blood vessels and renal damage in qi and yin deficiency type and phlegm and blood stasis mutual type were higher than that in other syndrome types. Conclusion Hypertension with MS patients should pay attention to the prevention and control of retinal vascular and renal vascular complications. Fundus examination and detection of early renal damage index can be used as the basis of TCM syndrome differentiation of hypertension with MS.
6.Isokinetic muscle strength test for the muscular force after repair of ruptured Achilles tendon

BACKGROUND:Restoration of tendon integrity and tenacity, and maintenance of ankle joint flexibility, play a crucial role on the rupture of Achiles tendon. OBJECTIVE: To evaluate the muscular force using isokinetic muscle strength test after the ruptured Achiles tendon is repaired, and to assess the repairing effect. METHODS: Muscle power including peak torque, peak torque/body weight, total work, average peak torque, set total work of flexor and extensor of 17 patients with Achiles tendon rupture were assessed after reconstruction using Biodex system 4 at 60 (°)/s and 120(°)/s. RESULTS AND CONCLUSION:The peak torque, peak torque/body weight, total work, average peak torque, set total work of flexor muscle group at the injured side were significantly decreased compared with uninjured side at 60 (°)/s and 120 (°)/s (P < 0.05). Isokinetic muscle strength test can reflect the muscular force after the ruptured Achiles tendon is repaired, and evaluate the repairing effect.
9.XIAP modified adipose-derived stem cells improve cardiac function following myocardial infarction

Objective We want to study the therapeutic efficiency of autologous ADMSC transplantation in myocardial infarction.And we try to find out a good way to improve the therapeutic efficiency by using the combination of gene therapy and cell therapy.Anti-apoptotic protein XIAP was selected to fight against the ischemic environment of myocardic infaction.Methods ADMSC was isolated from rat inguinal fat tissue.ADMSC was cultivated with DMEM.XIAP experession plasmid was elertco-transduced to ADMSC.The anti-apototic function of XIAP was tested by serum stavation induced apotosis.The method of ligation of the left anterior descending artery was used to prepare the Myocardial infarction model.Then rats were randomly separated into three groups to receive direct epicardial injections of normal saline,or ADMSCs cell suspension or XIAP modified ADMSCs cell suspension at five sites in central zones of myocardial infarction and border zone.Cardiac function and the infarct size were evaluated 4 weeks after ADMSCs transplantation.Results West blotting suggest that,XIAP over-expression block serum starvation induced apotosis.It showed that there are significant statistic difference among XIAP modified ADMSC transplantation group,ADMSC transplantation group and control group 4 weeks after myocardial infarction (P < 0.05).Left ventricular ejection fraction(LVEF) showed a significant improvement in ADMSCs transplantation group compared to control group (P <0.05).Left ventricular end systolic diameter(LVDs) and left ventricular end diastolic diameter(LVDd) of ADMSCs group were smaller than control group(P < 0.05).The area of myocardial infarction was significantly reduced in the ADMSCs transplantation group compared to the saline group(P <0.05).Compared to ADMSCs transplantation group,effect of the XIAP modified ADMSC in rats with myocardial infarction is more obvious.The reduction of LVEF of XIAP modified ADMSCs group was signific antly lower(9%) than the ADMSCs group(16%) (P < 0.01).Infarction area in XIAP modified ADMSCs group(3.26 ±0.95)% was smaller than ADMSCs group(5.17 ±2.03)% (P <0.05).Conclusion Autologus ADMSC transplantation is an efficient therapeutic tool in myocardial infarction therapy.Over expression of XIAP can partly inhibit lowserum induced apotosis of ADMSC in vitro,and it can improve left ventricular function better in vivo.Over expression XIAP of ADMSC can improve the therapeutic efficiency compare to ADMSC transplantation.
10.Impact of laparoscopic surgery on incidence of deep venous thrombosis in lower limbs in patients undergoing hysterectomy

Objective To explore the impact of laparoscopic surgery and that of open surgery on the coagulation function and incidence of deep venous thrombosis(DVT) in lower limbs in patients undergoing hysterectomy .Methods From April 2014 to A‐pril 2015 ,110 eligible patients were recruited .There was 57 patients received laparoscopic surgery ,and 53 patients received open surgery for hysterectomy .All patients underwent surgery under the general anesthesia .The APTT ,PT ,FIB ,and D‐dimer was de‐tected at 5 time points:before surgery ,surgery completion ,24 h after surgery ,48 h after surgery ,and 72 h after surgery .All patients were scheduled to ultrasound exam in lower limbs to detect the DVT at 24 ,48 and 72 h after surgery .Results Both groups showed decreased PT at surgery completion as compared with other time points (P<0 .05) ,decreased APTT at surgery completion and 24 h after surgery as compared with other time points(P<0 .05) ,and increased FIB at surgery completion and 24 h after surgery as compared with other time points(P<0 .05) .No significant difference was found in APTT ,PT ,and FIB at each time points between two groups(P>0 .05) .Both groups showed increased D‐dimer after surgery compared with that before surgery ;the laparoscopic group showed higher D‐dimer at 24 h after surgery ,and lower D‐dimer at 48 h after surgery than open group(P<0 .05) .The inci‐dence of DVT in laparoscopic group and open group was 14 .04(8/57) and 5 .66% (3/53) ,respectively ,with no significant difference between two groups(P>0 .05) .Conclusion Both the laparoscopic surgery and open surgery will activate the coagulation system , and lead to DVT ;as compared with open surgery ,the laparoscopic surgery could not reduce the incidence of DVT in lower limbs in patients undergoing hysterectomy .
