1.Effect of Tegafur,Gimeracil and Oteracil Potassium Capsules on peripheral blood TPS,sMICA,HIF-1αand Her-2 in advanced cervical cancer

Objective To investigate the effect of Tegafur, Gimeracil and Oteracil Potassium Capsules ( TS-1 ) on peripheral blood tissue polypeptide antigen (TPS), soluble MHC class I chain-related molecules A (sMICA), hypoxia-inducible factor-1 (HIF-1α) and human epidermal growth factor receptor 2 ( Her-2 ) in the treatment of patients with advanced cervical cancer.Methods 80 cases with advanced cervical cancer were selected and divided into two groups, 40 cases in the control group were treated with routine clinical therapy, 40 cases in the experimental group were treated with TS-1.Tumor markers, TPS,sMICA,HIF-1αand Her-2 were compared before and after the treatment.Results Compared with the control group, the levels of tumor antigen TA-4,carcinoembryonic antigen (CEA) and squamous cell carcinoma antigen (SCCA) in the experimental group were lower (P<0.05); the TPS, HIF-1, sMICA and Her-2 were lower (P<0.05), and the total effective rate in the treatment group was higher (P<0.05).Conclusion TS-1 has a obvious clinical efficacy in the treatment of advanced cervical cancer.It is speculated that the possible mechanism may reduce the serum tumor marker levels via reducing the TPS and SMICA, HIF-1 alpha, HER-2 levels.
3.Arthroscopic treatment of ulnar impaction syndrome

Objective To investigate ulnar impaction syndrome imaging characteristics and wrist arthroscopy in the diagnosis and treatment effects.Methods From September 2006 to April 2014,41 patients with ulnar impaction syndrome were treated.Of all 41 patients,28 were male and 13 female with an average age of 34.5±3.9 years old (ranging from 15 to 52 years old).The preoperative values of wrist range of motion (ROM) were 44.17°±6.22° at volar flexion,42.72°±3.37° at dorsal extension,17.45°±4.11° at radial deviation,19.12°±3.70° at ulnar deviation,66.81°±4.28° at pronation,and 70.27°±5.30° at supination.Arthroscopic exploration was conducted in all patients,aiming to observe the ulnar variance,wrist osteonecrosis,articular cartilage damage and triangular fibrocartilage complex tear.Arthroscopic synovectomy were performed in all cases;33 cases of articular cartilage injury were treated with arthroscopic chondroplasty;16 cases of osteonecrosis were treated with arthroscopic drilling decompression;30 cases with tear of TFC were treated with arthroscopic repair;26 cases with ulna positive mutation were treated with arthroscopic distal ulna resection.Patients were followed at 3,6,and 12 months after operation.At 12 months follow-up,the values of wrist range of motion was evaluated for volar flexion,dorsal extension,radial deviation,ulnar deviation,pronation,and supination.GreenO'Brien method was used to evaluate the function of wrist joint.Results Primary healing was presented in all patients without neurovascular injury and joint infection.All patients were followed up 12-36 months (mean,17.5 months).At 12 months followup,the values of ROM compared with that preoperative increased with significant difference,49.13° ±3.47° at volar flexion,47.13°±2.39° at dorsal extension,19.33°±3.29° at radial deviation,25.17°±4.89° at ulnar deviation,75.73°±7.66° at pronation,and 77.22°±4.89° at supination.According to Green-O'Brien standard,the results were excellent in 21 case,good in 17 cases and fair in 3 case,with 93% excellent and good rate.Conclusion Wrist arthroscopy dose not only contribute to the diagnosis in ulnar impingement syndrome but also can be used in treating inflammatory synovium and cartilage lesions,bone drilling decompression,TFC repair and arthroscopic distal ulna resection with improvement in the range of motion of the wrist joint and function.
4.Treatment of atlantoaxial instability with atlantoaxial pedicle and lateral mass screw WANG

Objective To study the effect of atlantoaxial pedicle and lateral mass screw in treat-ment of atlantoaxial instability. Methods A retrospective analysis was done on 11 eases of aflantoaxi-al instability treated with atlantoaxial pedicle and lateralmass screw from June 2006 to December 2007. Results The postoperative radiographs verified good position of all screws, with satisfactory atlantoaxial reduction. Follow-up for 3-21 months (average 12 months) showed no spinal cord and vertebral artery in-jury or interfixation failure. Conclusion Atlantoaxial pediele and lateral mass screw technique is a convenient method with solid fixation for treatment of atlantoaxial instability.

6.The effect of aprotinin on inflammatory response to intraoperative blood salvage using cell saver in patients undergoing spinal surgery

Objective To investigate the effect of aprotinin on the inflammation response to reinfusion of shed blood during operation after being aspirated, filtered and washed using cell saver.Methods Twenty-four ASA I - II patients undergoing elective spinal surgery under general anesthesia were randomly divided into 2 groups: aprotinin group ( n = 12) and control group ( n = 12) .The patients were premedicated with intramuscular pentobarbital 0.1 g and atropine 0.5 mg. Anesthesia was induced with propofol 2 mg ? kg -1 , fentanyl 2-4 ?g ? kg-1 and vecuronium 0.1-0.3 mg ? kg-1 and maintained with isoflurane, propofol infusion and intermittent i. v. boluses of fentanyl and vecuronium. The patients were mechanically ventilated after tracheal intubation. In aprotinin group aprotinin 10 ? 105 IU was given before skin incision and another dose of aprotinin 10 ? 105 IU was continuously infused during operation. The blood shed during operation was collected, anticoagulated with heparin, filtered, washed and reinfused using AutoLog cell saver. Blood samples were taken from CVP line before skin incision (T1 , baseline), 30 min after reinfusion of salvaged bloods (T2) and at the end of operation (T3 ) for WBC and neutrophil granulocyte counts and determination of expression of CD11b and CD18 on the surface of neutrophil granulocytes using flow cytometry. Results The two groups were similar with respect to the general condition of the patients, duration of operation and account of shed blood reinfused. (676? 353) ml was reinfused in control group. Objective To optimize the extraction process of Jingzhi Guanxin Recipe. Methods With the amounts of tanshinone ⅡA,salvianolic acid B,paeoniflorin,ferulic acid and hydroxysafflor yellow A as observation parameters,uniform design method was used for the optimization of wet-shattering extraction process of Jingzhi Guanxin Recipe. Results The optimum extraction process was as follows:adding 6-fold 95 %alcohol for the first time of exaction,adding 6-fold 60%alcohol for the second time,adding 6-fold 20 %alcohol for the third time,and shattering for 10 minutes every time. Conclusion This optimized process is efficient,economic and simple.
8.Utilization of IQM data of GEM3000 blood gas analyzer for evaluating instrument performance

Objective To apply the IQM quality data of the GEM 3000 blood gas analyzer to evaluate the instrument performance and to compared it with the traditional evaluation method .Methods The precision and accuracy were calculated by using the IQM data ,the external quality control substance was adopted to verify the main performance indexes of the blood gas analyzer .Results The precision of PH ,PCO2 and PO2 calculated by using the IQM data was 0 .02% ,2 .16% and 0 .63% ,which by the traditional e-valuation method was 0 .07% ,1 .68% and 1 .28% respectively ,the accuracy of PH ,PCO2 and PO2 calculated by using the IQM data was 0 .00% ,1 .15% and 0 .49% ,which by the traditional evaluation method was 0 .11% ,2 .91% and 1 .07% respectively .Conclu-sion The intelligent quality management can provide objective and reliable indicators for the performance evaluation of the blood gas analyzer ,and compared with the traditional performance evaluation scheme which is more simple and convenient to operate .
9.Features of clinical and progression of multiple system atrophy

Objective To explore the features of clinical and progression of multiple system atrophy(MSA).Methods The clinic data of 28 subjects diagnosed as probable MSA according to Gilman diagnostic criteria were studied retrospectively.Three aspects of activities of daily living(ADL)(aid-requiring walking,wheelchair-bound state and bedridden state)were used to assess the progression of disease.Kaplan-Meier analysis was also used to estimate the difference between subgroups.Results Three systems were involved in 26 cases(92.9%)and autonomic functional disturbance was common.The calculated median time from onset to evolution to MSA was mean 2 years.The median times from onset to aid-requiring walking,wheelchair requirement and bedridden state were 3,5 and 7 years,respectively.The patients impaired both of motor and autonomic systems progressed fastly within 3 years from onset of the disease(P
