1.Current status of laboratory animal science and its future

Objective To summarize the progresses in laboratory animal science in civil and military fields,and explore the problems existed in development of laboratory animal science in PLA. Methods Literature concerning the laboratory animal science published domestically and abroad were retrospectively and descriptively analyzed. Integrated with the practices on laboratory animal science,the strategies and suggestions for future development of laboratory animal science in PLA were herewith presented. Results Considerable developments have been achieved in the laboratory animal science in PLA,including the improved productive facilities and experimental conditions,and preliminarily established system of traiming of personnal. However,compared with the development level of laboratory animal sciences at home and that of abroad,such disparities as heterogeneity in quality of experimental animals from different breeding institues,lower standard in experimental technology,poorer foundation in the development of animal model special for military purpose,and lack of strategic reserve of laboratory animal resources,etc could still be found. Conclusions The development of laboratory animal science in PLA should depend on a combination of military efforts and civilian support,with establishment of the mechanism to share resources and professional personnel. Meanwhile a long-term mechanism should be developed for professional training of personnel in breeding experimental animals for researches in the field of military medicine,in order to establish a truly fundamental discipline of animal science for research of military medical sciences.
2.Clinical Effect of Laparoscopy Combined with Gestrinone in Treatment of Ovarian Endometriosis Cyst and the Prognosis of Patients

Objective To study clinical effect of laparoscopy combined with gestrinone in treatment of ovarian endometriosis cyst and the prognoisi of patients. Methods A randomized, controlled, prospective study was used in this study, 78 patients with ovarian endometriosis cyst were selected in Sichuan province panzhihua city miyi people's hospital from august 2012 to august 2014, and were randomly divided into observation group and control group, 39 patients in each group. Patients in the observation group were treated with laparoscopy combined with gestrinone (2.5 mg/time, two times per week, for 6 months) intervention; patients in the control group were treated with laparoscopic intervention. After six months, the postoperative serum FSH (Follicle Stmi Ulating Hormone, FSH), Estradiol (E2) and Estradiol levels, ovarian biggest plane average diameter (D), the level of MMP-9 and TIMP -1 level, pregnancy outcome of two groups were compared and the outcome and the recurrence rate 18 months after treatment were followed up. Results Six months after treatment, the E2 and FSH levels in the observation group were better than control group, with statistically significant difference (P<0.01);the postoperative serum levels of MMP-9 and TIMP-1 in Observation group were better than control group, with statistically significant difference (P<0.01);there were 20 patients of D≤2 cm, 19 patients of D>2 cm, the postoperative ovarian shrink rate was 51.28% in the observation group; there were 5 patients of D≤2 cm, 34 patients of D>2 cm ovarian shrink ratio was 12.82% in the control group, the ovarian shrink ratio of observation group was obviously higher than control group, with statistically significant difference (P<0.01) . Follow-up for 18 months, the recurrence rate was 5.13%in the observation group, which was significantly lower than thatin the control group (25.64%), with statistically significant difference (P<0.05);the pregnancy rate was 66.66%and 30.77%in the Observation group and control group, respectively, the difference had statistical significance (P<0.01) . Conclusion Laparoscopic surgery combined with gestrinone can protect ovarian function of patients with ovarian endometriosis cyst, improve pregnancy outcomes, and reduce recurrence rate, and has good clinical value.
3.Pharmaceutical Care for One Case of Multi-target Treatment for Refractory ClassⅣ+ⅤLupus Nephritis

OBJECTIVE:To summarize the method and experience of clinical pharmacist participating in pharmaceutical care for a case of refractory class Ⅳ+Ⅴ lupus nephritis (LN). METHODS:Clinical pharmacists and physicians formulated individualized treatment together,i.e. prednisone 30 mg/d+MMF 0.5 g,q12 h+FK506 1 mg,q12 h;on the fifth day of treatment,the patient suf-fered from ardent fever and increase of blood pressure,and clinical pharmacist suggested using linezolid 0.6 g,ivdrip,q12 h+merope-nem1.0 g,ivdrip,q8 h to control pulmonary infection;using Amlodipine besylate tablet 5 mg,po,qd+one Valsartan hydrochlorothia-zide capsule, po,qd+Arotinolol hydrochlorid tablet 10 mg,po,bid to control blood pressure. RESULTS:Multi-target treatment for re-fractory class Ⅳ+Ⅴ lupus nephritis achieved efficacy:the patient got higher plasma albumin and lower 24 h urine protein quantity af-ter 14 days;the patient suffered from secondary pulmonary infection and high blood pressure;physicians adopted the pharmacist’s suggestion so that pulmonary infection and blood pressure had been controlled. CONCLUSIONS:To ensure the effectiveness and safe-ty of the multi-target treatment,clinical pharmacists should formulate individualized treatment,focus on the possible ADR and provide phamaceutical care in the field of blood pressure monitoring,infection control,etc. when participating in multi-target treatment.
4.Differentiation and regulation of the osteoclast

The osteoclast is a hematopoietic cell derived from CFU GM, which is regulated by systemic hormones, soluble factors in the phase of differentiation, fusion and proliferation. The major singnaling pathways involved in osteoclastic bone resorption and formation in response to cytokine and hormones include cAMP and R PTK. The advances in our understanding of the osteoclast make it possible for us to comprehend the development and the treatment of the bone metabolic diseases.
5.Need analysis of clinical research consultation for clinical epidemiology in a general hospital

Objective To understand the needs for and supply of clinical research consultation at the clinical epidemiological unit (CEU) in a general hospital. Methods Data were collected from work records of 2008 of CEU and were analysed. Results There were three faculties and three graduate students in the CEU, who altogether provided 272 consultations in 2008. 147 of the consultants were doctors (54.0%), and 90 were graduate students (33.1%). With regard to the content of consultation, 106 (39. 0%) were for bio-statistical methods, 96 (35. 3%) for project design, 36(13.2%) for paper writing and 24 (8.8%) for others. The professor of the CEU provided more consultation on project design (71/139, 51.1%) than assistant professors (22/151, 19.1%) and graduate students (3/18, 16.7%) (P<0.01). The assistant professors (66/151, 57.4%) and graduate students (10/18, 55.6%) provided consultation on bio-statistical methods more than the professor (30/139, 21.6%) (P<0. 01). Conclusion Doctors and graduate students in the general hospital need clinical research consultation service from the CEU. It is necessary and possible to establish a CEU in general hospital to meet the needs of clinical epidemiological consultation. The free consulting policy is effective in improving the accessibility and satisfaction of the consulting service.
6.New concepts in pathogenesis of HIV disease: hypothesis main pathogenic site of intestinal lymphoid tissue

Given its population of CCR5-expressing, immunologically activated CD4 +T cells, the gastrointestinal (GI) mucosa is uniquely susceptible to human immunodeficiency virus (HIV)-1 infection. Recent studies have shown that, as in macaques infected with simian immunodeficiency virus (SIV), intestinal CD4 +T cells are selectively and rapidly depleted in the intestine of HIV-infected patients. Depletion of intestinal CD4 +T cells occurred at all stages of infection regardless of highly active antiretroviral therapy (HAART). Here we discuss the important implications of the recent findings for our understanding of HIV pathogenesis, treatment, and vaccine design. The major significance is that it supports a simple hypothesis to explain the pathogenesis of HIV infection, that most HIV replication occurs in the intestine and that disease progression may correlate with turnover of specific cell subsets in mucosal tissues.
7.Association between development of CD4~+CD25~+ regulatory T cells and thymus CD4~-CD25~+ cells

AIM: To explore the correlation between development of CD4~+CD25~+ regulatory T cells (CD4~+CD25~+ Tr) and thymus CD4~-CD25~+ cells. METHODS: The ratios of CD4~+CD25~+ regulatory T cells to CD4~+ T cells in thymus, spleen, lymph node and peripheral blood of mice from birth to mature and also the ratios of CD4~-CD25~+cells to CD4~-T cells in thymus were measured by flow cytometry. Purified CD4~+CD25~+ T cells and CD4~+CD25~- T cells were labeled with CFDA-SE, and then stimulated with various kinds of stimulators. RESULTS: The percentages of CD4~+CD25~+ Tr in mouse spleen, lymph nodes and peripheral blood increased gradually, but not in thymus, from day one to week 10 of the age with rapid rising from day one to week 1. The percentages of CD4~-CD25~+ cells in mouse thymus were quite high on day one after birth, and decreased rapidly from day one to week 1. Both CD4~+CD25~+ Tr and CD4~+CD25~- T cells showed no proliferation in response to ConA, while CD4~+CD25~+ Tr showed a transient enlargement of cell size. Both CD4~+CD25~+ Tr and CD4~+CD25~- T cells underwent proliferation in response to PDB plus ionomycin. CD4~+CD25~- T cells, but not CD4~+CD25~+ Tr, showed a proliferative response to the stimulation of coated anti-CD3 plus soluble anti-CD28 antibody, however, CD4~+CD25~+ Tr showed significant proliferation and CD4~+CD25~- T cells showed a stronger response in addition of high dose of IL-2. CONCLUSION: The thymus CD4~-CD25~+ cells are probably the precursor of CD4~+CD25~+ Tr during cell development.
