1.Clinical study of Shiwei-Jinhuang Ointment and Chinese herbal external washing prescription for acute paronychia

Objective:To evaluate the clinical efficacy of Shiwei-Jinghuang Ointment combined with Chinese herbal external washing prescription in the treatment of acute paronychia. Methods:A total of 126 patients with acute paronychia in Xiyuan Hospital of China Academy of Chinese Medical Sciences from January to December 2019, who met the inclusion criteria, were divided into 3 groups according to the random number table method (42 in each group). The treatment group was treated with Shiwei-Jinghuang Ointment combined with Chinese herbal external washing prescription. The control group 1 and group 2 were simply applied with Chinese herbal external washing prescription and mupirocin ointment, respectively. All groups were treated for 2 weeks and followed up 2 months. Serum CRP level was determined by ELISA method, and neutrophil percentage (NEU%) was measured by automatic hematology analyzer. The subsided time of red swelling and inflammatory exudation in nail groove of the paronychia patients were observed and recorded. Results:The total effective rate was 90.5% (38/42) in the treatment group, 71.4% (30/42) in the control group 1 and 66.7% (28/42) in the control group 2. The statistical difference were significant among three groups ( χ2=7.350, P=0.025). After treatment, the serum CRP level and NEU% in three groups were decreased ( P<0.01), and the serum CRP and NEU% level in treatment group were significantly lower than those in the control group 1 and group 2 ( F values were 6.482, 33.619, respectively, all Ps<0.001). After 2 months of follow-up, the incidences of chronic paronychia were 50.0% (6/12) and 57.1% (8/14) in control group 1 and group 2, respectively, but no chronic paronychia occurred in the treatment group. The difference among three groups was statistically significant ( χ2=9.355, P=0.009). Conclusion:The combination of Shiwei-Jinghuang Ointment and Chinese herbal external washing prescription can promote paronychia red swelling, reduce the inflammatory exudation time and the incidence of chronic paronychia, effectively improve the in vivo decrease inflammation index and clinical efficacy.
2.Advances in research on influencing factors of visual impairment in the elderly

The definition of visual impairment, the current status of visual impairment in the elderly, influencing factors related with visual impairment and management control modes are reviewed.There are three international standards of visual impairment including the WHO standard in 1972, the United States standard, and the WHO standard in 2003. The domestic standard is consistent with the WHO standard in 1972. Currently, with the global aging, the international and the Chinese situations of visual impairment are severe,and it is urgent to reduce the incidence of visual impairment. The visual impairment of the elderly is affected by various factors. A variety of management and control modes of visual impairment for the elderly have begun to bear fruit. The aim of the review is to provide the basis to screen for high-risk groups of visual impairment and provide timely intervention to reduce the prevalence of visual impairment in the older population.

Therapies for uveitis include drug treatment and surgery.Medical therapies including proper administration of corticos- teroids,immunosuppressive drugs,nonsteroidal anti-inflammtion drugs and anti-infective drugs are important in management of the in- traocular inflammation and decreasing the risk of complications,furthermore,are the basis for surgical treatment of uveitis.
4.Effect of Continuous Passive Motion on Hand Spasm and Hand Function of Hemiplegic Patients

Objective To observe the effects of hand function training with continuous passive motion (CPM) on hand spasm and function of the hemiplegic patients.Methods 60 hemiplegic patients after stroke or brain trauma and with hand spasm were randomly divided into the treatment group and control group with 30 cases in each group. The cases in the treatment group were treated mainly with the method of CPM together with the routine rehabilitation procedures, while the those in the control group were only treated with the routine rehabilitation procedures. The changes of hand spasm and function of two groups were observed.Results After four weeks' treatment, the efficacy of the treatment group was obviously better than that of the control group ( P<0.05).Conclusion The rehabilitation training is an effective method for treatment of hemiplegia, and CPM is an important auxiliary method in the treatment. These two Methods combined together can shorten the course of treatment, and strengthen cure efficacy.
5.Immune function affer postoperative parenteral nutrition enriched with arginine in patients with gastrointestional carcinoma

Objectives:In order to study the immune function after postoperative parenteral nutrition enriched with arginine in patients with gastrointestinal carcinoma. Methods:Forty patients who underwent radical operation of gastrointestinal carcinoma were randomly divided into experimental group( n= 20),in which patients received TPN enriched with arginine,and control group (TPN, n =20).At day 1 before operation,day 1 and day 8 after operation,IgG,IgA,IgM,T cell subgroups.NK cell activity and IL 2 were dectected. Results:The immune function of patients with gastrointestinal carcinoma was depressed at different levels.At day 1 after operation,IgG,IgM,IgA,CD4 +,CD4 +/CD8 +,Nk cell activity and IL 2 decreased more significantly than those before operation.The immunefunction recovered rapidly and exceeded the level before preoperation in experimental group.The difference was significant( P
7.Electro-acupuncture up-regulates the expression of stromal cell-derived factor-1α mRNA and its protein and promotes revascularization in the brain after focal cerebral ischemia and reperfusion

Objective To investigate the mechanism by which electro-acupuncture (EA) promotes revascularization in the brain after focal cerebral ischemia and reperfusion.Methods The Sprague-Dawley rat model of focal cerebral ischemia was made by filament occlusion. The rats were randomly divided into a control group, a model group, and an EA group. The model and EA groups were each divided into 5 subgroups receiving reperfusion 1, 3,7, 14 or 21 days after ischemia. EA was given at the bilateral Hegn point (LI 4) in the EA group. The expression of stromal cell-derived factor-1α(SDF-1α) mRNA was detected using a RT-PCR in the 3, 7 and 14 day subgroups.The immunohistochemical method was employed to detect the expression of SDF-1α protein. Results Compared with the control group, expression of SDF-1α protein increased significantly in the model and EA groups. Compared with the model group, the expression of SDF-1α mRNA increased significantly in the 3, 7 and 14 day subgroups.SDF-1α protein expression and microvessel count increased slightly but not significantly in the 1d subgroup, but the increases were significant in the 3, 7, 14 and 21 day subgroups.Conclusions EA may promote angiogenesis in an ischemic area of the cortex by increasing the expression of SDF-1αmRNA and its protein after focal cerebral ischemia and reperfusion.
8.Study on the hospital acquired pneumonia prevention in intensive cancer patient using the bundle strategy of diagnosis and treatment

Objective To investigate the effect of bundle strategy of diagnosis and treatment on preventing hospital acquired pneumonia in intensive cancer patients.Methods Two hundred and sixty-five patients as control group were treated by regular preventive measures,and 292 cases as intervention group were treated by bundle strategy including the control application of antibiotics,hand hygiene,oral care,isolation of multi-drug resistant,elevation of bedside,management of respiratory tract and appropriate sedation and analgesia et al.Compare the hospital acquired pneumonia incidence,attribution mortality,duration of mechanical ventilation,stay-time in ICU,hospital acquired pneumonia antibiotics defined daily doses system and positive rate of multi-drug resistant infection between two groups.Results With the bundle treatment,the decreases of the hospital acquired pneumonia Incidence,duration of mechanical ventilation,stay-time in ICU and hospital acquired pneumonia Antibiotics daily doses system were significant(P< 0.01),and the positive rate of hospital acquired pneumonia attribution mortality and MDR infection were no statistical significance (P > 0.05).Conclusion The bundle strategy of diagnosis and treatment could reduce the hospital acquired pneumonia incidence and the length-time in hospital.
9.The experience in surgical treatment for serious pilon fractures

Objective To analyze the clinical methods of type Ⅲ Pilon fracture internal fixation. Methods The internal fixation with steel plate were performed in 36 cases with Ruedi-AllgoWer type, Ⅲ Pilon fracture and all patients had early post-operative rehabilitation functional exercise. Results Thirty-six cases were followed up for an average period of 16 months. The fracture healed completely in all cases and the successful rates of good function reached 88.9% (32/36) according to YU Guang-rong's evaluation standard. Conclusion For Ruedi-AUgower type Ⅲ Pilon fracture, appropriate surgical time combined with internal fixation with steel plate and early post-operative function exercise showed satisfactory therapeutic effect.
10.Comparison of minimally invasive plating osteosynthesis and intramedullary nail fixation in treatment of humeral shaft fractures

Objective To compare clinical outcomes of minimally invasive plating osteosynthesis (MIPO) and intramedullary nail stabilization in treatment of acute humeral shaft fractures. Methods From March 2007 to January 2009, 52 patients were treated in our department for acute fractures of middle and lower humeral shaft. Twenty-seven were treated with MIPO technique (group A) and 25 with intramedullary nail (group B) . In group A, there were 15 men and 12 women, aged from 18 to 65 years (average, 36. 7 years); in group B, there were 16 men and 9 women, aged from 25 to 63 years (average,39. 4 years). Operation time, amount of blood transfusion, hospital stay, time for union, complications, Mayo scores of the elbow and Constant scores of the shoulder were recorded in both groups and compared statistically. Results The mean follow-up time was 17. 8 months (range, 16 to 36 months) . There were no significant differences between the 2 groups in operation time, amount of blood transfusion, hospital stay, time for union, and Mayo scores of the elbow ( P > 0. 05) . There was no case of postoperative iatrogenic radial nerve palsy or non-union in group A, but there were 4 cases of non-union, 2 cases of iatrogenic radial nerve palsy and one case of varus malunion in group B. Compared with group B, group A showed significant advantages in rate of postoperative non-union and Constant scores of the shoulder functional recovery ( P < 0. 05) . Conclusion In treatment of humeral shaft fractures, MIPO technique has advantages of less operative invasion, quick bone union, satisfactory shoulder functional recovery and low risk of postoperative palsy of the iatrogenic radial nerve.
