Objective To analyze the constitutes of essential oil form Nervilia fordii. Methods GC-MS was used to analyze the essential oil. The relative content of each component was determined by area normalization. Results Fifty-five kinds of constitutes were identified,accounting 97.1 %of total contents. The content of 2-Pentadecanone,6,10,14-trimethyl is the highest,being 13.55 %,which had also been found in the same family. Conclusion 2-Pentadecanone,6,10,14-trimethyl can be used for the identification of the same family.

BackgroundThe morphological examination of macular disease is very important information for the early diagnosis.Spectral domain optical coherence tomography (SD-OCT) is a gold criteria in the diagnosis of macular diseases,but its clinical application is limited in the eyes with refractive medium opacity.The resolution ratio of conventional B-scan sonography is unsatisfactory.20 MHz B-scan sonography may be a new approach to maculopathy.ObjectiveThis study was to evaluate the clinical value of 20 MHz B-scan ultrasound by comparing morphological changes of different maculopathies observed by 20 MHz ultrasound and spectral-domain OCT.MethodsA prospective study was designed.51 eyes of 51 patients with macular diseases who were determined by Tianjin Medical University Eye Center were enrolled in this study.Spectral-domain OCT was used to image the macular area,and then the 20 MHz B-scan ultrasound was used to examine the same eye to assess the value of 20 MHz B-scan ultrasound diagnosis.The macular diseases were typed according to the OCT manifestation.The characteristics of 20 MHz ultrasound for the same maculopathy were analyzed.All the patients subscribe the informed consent before the study.ResultsIn 10 eyes with macular hole determined by spectral-domain OCT,shallow dome-shaped lesion with central hollow was exhibited on the ultrasound image.The macular edema,retinal neuroepithelium layer detachment and retinoschisis were found in 16 eyes,6 eyes and 4 eyes by spectral-domain OCT,but the 20 M Hz B-Scan ultrasound presented with the focal belt-shaped elevation with non-echo zone below.In contrast,in 15 eyes of retinal pigment epithelium detachment in OCT,spectral-domain OCT showed stronger and thicker belt-shaped echo.Conclusions20 MHz B-scan ultrasound has certain value in the diagnosis and differentiation for several common maculopathies.Although it is not accurate as OCT in recognizing diseases,it could play an important role in refractive media opacity eye.

BACKGROUND:Though the effects of conservative or traditional open reduction and internal fixation in the treatment of thoracolumbar fractures are reliable and satisfactory for most cases, two methods also have shortages. Minimal-invasive percutaneous pedicle screw system provides a new available method. OBJECTIVE:To investigate the clinical effect of Sextant percutaneous pedicle screw system in the treatment of thoracolumbar fractures. METHODS:A total of 55 patients, who had undergone percutaneous pedicle screw fixation using Sextant system (25 patients) or traditional open internal fixation (30 patients) for single-level vertebral body compression fractures in Department of Orthopedics of Renmin Hospital of Wuhan University from February 2011 to January 2013, were enrol ed in this study. RESULTS AND CONCLUSION:Except two patients in traditional open internal fixation group were lost after discharge, al other patients were fol owed up for 8-14 months. Operative time, intraoperative blood loss, postoperative drainage amount and hospital day were better in percutaneous pedicle screw fixation group than in the traditional open internal fixation group (P<0.05). Compared with preoperation, the sagittal Cobb angle, visual analogue scale and Oswestry disability index after operation were significantly lower (P<0.05), while anterior vertebral body height ratio was significantly higher in each group (P<0.05). No significant difference was found in terms of correction loss and Oswestry disability index between two groups at 8 months after operation (P>0.05). The results show that percutaneous pedicle screw fixation using Sextant system has a satisfactory outcome in the treatment of thoracolumbar fractures. However, obeying indication strictly is very important for clinical application.

Objective To investigate the status of knowledge and performance on Chinese Heart Failure Diagnosis and Treatment Guideline (2014 version) in general practitioners of Shanghai Pudong communities.Methods The survey was conducted from April to June in 2014 with a self-designed questionnaire.Total 390 general practitioners (GPs) in Pudong New Area were selected by cluster sampling method.The contents of questionnaire included:diagnosis and differential diagnosis,drug therapy,non drug therapy of chronic heart failure.Result Total 385 questionnaires were retrieved with a response rate of 98.7％ (385/390).The results showed that in aspect of diagnosis and differential diagnosis,373 (96.9％) Gps made the diagnosis based on history and physical examination,171 (44.4％)Gps never used BNP or NTPro-BNP tests,280 (72.7％)GPs did not know how to identify systolic or diastolic heart failure,86 (22.3％)Gps made the differential diagnosis according to the EF value.In aspects of drug therapy,the rate of beta blockers use was 10％-30％ in 284 (73.8％) Gps,149 (38.7％) Gps did not use beta blockers because of not knowing the contraindications,289 (75.1％) Gps used a maximum dose of betaloc for 25-50 mg,no one used 101-200 mg,242 (62.9％)Gps did not know the target dose of betaloc,the rate of ACEI/ARB use was 10％-30％ in 330 (85.7％) Gps,258 (67.0％) Gps would increase the dose but not knowing the target dose.The main reason for not using the target dose of Betaloc and ACEI/ARB was not knowing the dose.In aspect of non-drug therapy:240 (62.3％)Gps never heard of cardiac resynchronization therapy (CRT) and 271 (70.4％)Gps never heard of implantable cardioverter defibrillator (ICD).The senior rank GPs grasped the guideline much better than Gps with primary and intermediate professional ranks.Conclusion General practitioners in community health centers should further study the guideline of heart failure,particularly need to strengthen the knowledge and ability of drug therapy.

There were significant differences in operative time, intraoperative blood loss, postoperative drainage amount and length of hospital stay between two groups (P=0.000 0). The post-operative Cobb angle was significantly lower while anterior vertebral body height ratio was significantly higher at one week than those before fixation in two groups (P=0.000 0). No significant difference was found in correction loss between two groups at 8 months after operation. Experimental findings indicate that, percutaneous pedicle screw fixation using Sextant system has a satisfactory outcome in the treatment of thoracolumbar fractures. However, understanding the correct indications is very important for clinical application.

OBJECTIVE:To study the improvement effects of polydatin on renal fibrosis in rats and its mechanism. METH-ODS:50 rats was were randomly divided into sham operation group,model group,positive group(benazepril,5 mg/kg)and poly-datin high-dose and low-dose groups(100,50 mg/kg),with 10 rats in each group. Except for sham operation group,renal fibrosis model was induced by unilateral ureter obstruction. After modeling,administration groups were given relevant medicine intragastri-cally,and sham operation group and model group were given 0.5%sodium carboxymethyl cellulose solution once a day for consec-utive 4 weeks. The pathological change of renal tissue was scored. 24 h urinary protein and serum levels of urea nitrogen and creati-nine were determined,and the content of hydroxyproline,mRAN expression of TGF-β1 and FN were detected in renal tissue. RE-SULTS:Compared with sham operation group,pathological score,24 h urinary protein,serum levels of urea nitrogen and creati-nine,the content of hydroxyproline in renal tissue,mRNA expression of TGF-β1 and FN were all increased significantly (P<0.01). Compared with model group,24 h urinary protein,serum levels of urea nitrogen and creatinine and mRNA expression of FN in renal tissue decreased significantly in administation groups;the pathology scores,the content of hydroxyproline in renal tis-sue and mRNA expression of TGF-β1 of positive group and polydatin high-dose group were all decreased significantly(P<0.05 or P<0.01). CONCLUSIONS:Polydatin can prevent kidney fibrosis induced by unilateral ureteral obstruction,the mechanism of which may be associated with the mRNA expression down-regulation of TGF-β1 and FN in renal tissue.

OBJECTIVEA study was conducted to investigate the effects of Porphyromonas gingivalis lipopolysaccharide (Pg-LPS) on the expression of regulated upon activation normal T-cell expressed and secreted (RANTES) and fractalkine in human umbilical vein endothelial cells (HUVECs).

METHODSHUVECs were incubated with different concentrations of Pg-LPS (200, 500, and 1000 ng x mL(-1)) for 1, 6, 12, and 24 h, respectively. Then real time quantitative polymerase chain reaction (RT-PCR) and enzyme-linked immunosorbent method (ELISA) were adopted to detect the protein levels and mRNA levels of RANTES and fractalkine.

RESULTSThe RANTES protein levels and mRNA levels, as well as fractalkine mRNA levels, were significantly higher in all experimental groups of 1, 6, and 12 h than in the control group (P<0.05), except the expression of RANTES mRNA in 200 ng x mL(-1) group of 12 h and RANTES protein in 200 ng x mL(-1) group of 1 h. The expression levels of RANTES mRNA and fractalkine mRNA were highest in 1000 ng x mL(-1) group of 6 h and were 4.88- and 6.20-fold higher, respectively, than those in the control group. The expression levels of RANTES protein, mRNA, and fractalkine mRNA decreased 6 h after stimulation, and were significantly higher than those in the control group (P<0.05) in the RANTES and fractalkine in HUVEC, and such expression is important in the development of atherosclerosis 500 ng x mL(-1) group of 24 h. There was a significant difference between the expression of fractalkine mRNA in 1000 ng x mL(-1) group of 6 and 12 h than in the control group (P<0.05).

CONCLUSIONPg-LPS infection might up-regulate the expression of RANTES and fractalkine in HUVEC, and such expression is important in the development of atherosclerosis.

Objective To study the protective effects of ischemic postconditioning and preconditioning against ischemic reperfusion injury of skeletal muscle.Methods According to different treatment methods on ischemia-reperfusion injury,forty Wistar rats were divided into I/R group,IPost group,IPC group,IPC + IPost group,control group.Using a rat amputation-like model,Wistar rats underwent temporary amputation at the level of the femur,excluding the femoral vessels.By measuring MDA,MPO,the extent of skeletal muscle infarction,protective effects of postconditioning and preconditioning,postconditioning combined with preconditioning were observed.Results In the Ipost group,IPC group,IPC + Ipost group,MDA and MPO at one hour of reperfusion and extent of muscle infarction at 6 hour of reperfusion was lower than group IR (P ＜ 0.05).In the Ipost group,MDA,MPO and extent of muscle infarction was similar to group IPC + Ipost; In the Ipost group,MDA and MPO was lower than group IPC,extent of muscle infarction was similar to group IPC.Conclusion Ischemic postconditioning at the beginning of reperfusion can protect skeletal muscle against ischemic reperfusion injury.Preconditioning also protect skeletal muscle against ischemic reperfusion injury,but preconditioning combined with postconditioning don't offer additional benefit over preconditioning or postconditioning alone.
