Journal of Medical Postgraduates 2016;(2):148-152
Objective Microporous polysaccharide hemostatic powder ( MPPHP) is a most commonly used hemostatic mate-rial, but reports are rarely seen on its application in posterior spinal orthopedic surgery. The present paper aimed to investigate the val-ue of MPPHP in stopping bleeding in posterior spinal surgery. Methods Twenty-four 5-month-old New Zealand rabbits were equally randomized to an experimental and a control group. An 8 cm-long wound was made in the back muscle of animals to establish the model of spinal posterior surgery.MPPHP was applied to the wound of the experimental rabbits but none to that of the controls.Comparisons were made between the two groups of animals in the bleeding time, in vitro coagulation time, intra-operative blood loss, and results of pre-and post-operative blood routine and histopathological examinations. Results Statistically significant differences were observed between the experimental and control groups in the mean bleeding time ([15.36 ±1.42]s vs [347.58 ±32.43]s, P<0.05), in vitro coagulation time ([235.41 ±21.08]s vs [722.16 ±40.35]s, P<0.05), and intra-operative blood loss ([1.61 ±0.24]g vs [2.86 ± 0.37] g, P <0.05).After operation, the experimental animals showed a significantly higher count of erythrocytes than the controls ([5.68 ±0.46] ×106/L vs [4.47 ±0.43] ×106/L, P<0.05)and a higher level of hemoglobin than the latter ([12.67 ±1.18]g/L vs [10.64 ±1.05]g/L, P=0.00).There were no significant differences in the effectiveness and excellence rates of hemostasis between the experimental and control groups (100%vs 83.33%and 83.33%vs 66.67%, P>0.05).Pathological examination revealed mild bleeding and a small number of red blood cells in the muscle interstitium and blood vessel lumen of the experimental rabbits as compared with the controls. Conclusion Intraoperative topical ap-plication of MPPHP can reduce blood loss and achieve rapid and efficient hemostasis in posterior spinal surgery without affecting wound healing.

Objective To explore the relationship between Arg913Gln(G→A) polymorphism of solute carrier family 12 member 3 (SLC12A3) gene and diabetic nephropathy (DN) in type 2 diabetes mellitus (T2DM) in Han population of Shanghai. Methods Two hundred and fifty-eight Han ethnic people in Shanghai with T2DM (T2DM group) were divided into non-DN group (DN0 group, n=95) and DN group (n=163) according to 24 h urine albumin excretion rate (AER), and those in DN group were subdivided into microalbuminuria group (DN1 group, n=95) and macroalbuminuria group (DN2 group, n=68). Besides, 82 people with normal results of oral glucose tolerance test (OGTT), without diabetes mellitus and nephropathy were served as controls. PCR-sequencing was used to detect the genotypes of Arg913Gln polymorphism of SLC12A3 gene. Genotypic and allelic frequencies and clinical characteristics were compared among groups. Results Three genotypes (GG, GA and AA) were detected. The frequencies of GA+AA genotype and A allele in T2DM group were higher than those in control group, while there was no significant difference between groups (P>0.05). There was no significant difference in genotypic or allelic frequencies among subgroups of T2DM group (P>0.05). The level of triglyeeride (TG), AER, level of fasting insulin (FINS) and HOMA-IR in patients with GA+AA genotype were significantly higher than those in patients with GG genotype in T2DM group (P<0.05). Conclusion Arg913Gln(G→A) polymorphism of SLC12A3 gene is not significantly associated with T2DM and DN in Han population of Shanghai. The AER of people with GA+AA genotype is significantly higher than that with GG genotype. Arg913Gln (G→A) polymorphism of SLC12A3 gene may predict the risk of increase of albuminuria in patients with T2DM in Han population of Shanghai.

Objective To determine the function and clinical significance of OMOM capsule endos-copy. Methods To review the history and outcomes of 65 patients underwent capsule endoscopy from Oct. to Dec. 2004. Results OMOM capsule endoscopy failed to pass the pylorus within the effective working time in one case. The significant pathological findings were revealed in 47 patients among 64 patients (73. 4% ). Inflammatory small bowel diseases in 21 patients including Crohn's disease in 3 patients, small intestinal pol-yps in 8 patients including one Peutz-Jeghers syndrome, angiodysplasia in 9, diverticula in 3, hookworm in 3, submucosal tumor in 3 including one myosarcoma finally diagnosed by operation. OMOM capsule endosco-pies have working time 473 min (360-630) averagely. The duration from oral to anus was 1723 min (690-2370 min) averagely. Conclusion The capsule endoscopy is a highly useful technique in detecting small in-testinal diseases. It can be recommended as part of the routine work-up in patients with obscure bleeding.
