Ten years have been spent taking a firm grip on medical administration and high tech in the hospital.The skill of organ transplanting has been upgraded and new medical technonogy developed. The overall rising rate of full recovery and the lowering rate of death of serious and critical patients have shown a higher medical quality.All this attributes largely to the growing attention to medical administra- tion and quality control,tightening infection control,more strict practice of ward visits by three ranks of doctors,more care to serious and critical patients and the trianing of technical personnel.More actions have been taken to answer the call of develoPing the hospital through science and technology with special measures focusing on the basic construction required by the call and technical competitiveness of major departments and on the concentrating attacks on hot problems.
2.The analysis of clinical characteristics on eosinophilic granulomatosis with polyangiitis: a report of 23 cases

Objective To analyze the clinical features of eosinophilic granulomatosis with polyangiitis (EGPA).Methods The clinical characteristics,laboratory test,therapeutic regimen,pathology and imaging diagnosis in 23 cases with EGPA from the First Affiliated Changhai Hospital of Second Military Medical University within June 2007 to January 2013 were retrospectively analyzed.Results There were 14 males and 9 females,with the age ranged from 21 to 68 years.The mean age was (42±9) years.The overall prevalence of asthma was 78%(18/23).The allergic rhinitis accounted for 61%(14/23).The maxillary sinuses were the most frequendy involved,which accounted for 57%(13/23).Skin involvement was 57%(13/23),peripheral neuritis was 70%(16/23).Central nervous system involvement presented cerebrovascular event.Cardiac involvement accounted for 48%(11/23),digestive system involvement accounted for 17%(4/23).The outcome of auxiliary laboratory test revealed that 96%(22/23) patients expressed significantly higher levels of IgE and 70%(16/23) patients carried anti-neutrophil cytoplasmic antibodies (ANCA) which were presented as the perinuclear ANCA (p-ANCA).The patchy infiltrates of lung CT scan accounted for 69% (11/23).EMG showed mononeuritis multiplex and symmetric sensory motor neuropathy.The abnormal ratios of ECG and Echocardiography were 48%(11/23),79%(11/14) respectively.The pathological manifestations were necrotizing vasculitis,eosinophilic tissue infiltration,and extravascular granulomas.Conclusion Our results confirm the heterogenicity in clinical presentation and lack of specificity.Early diagnosis and treatment will be helpful for good prognosis.
3.Effect of midazolam combined with etomidate on liver function, MDA and SOD levels after laparoscopic cholecystectomy

Objective To analysis effect of midazolam combined with etomidate on liver function, malondialdehyde(MDA) , superoxide dismutase (SOD) levels after laparoscopic cholecystectomy.Methods 80 patients who were undergoing laparoscopic cholecystectomy were collected,all patients were randomly divided into control group and experimental group, 40 cases in each group,who were given the appropriate anesthesia induction drugs before surgery.After operation,the serum levels of MDA, SOD, glutathione peroxidase(GSH-Px) and the liver function were detected in all patients. Results After operation, compared with control group, the serum alanine aminotransferase(ALT) , aspartate aminotransferase (AST) and total bilirubin (TBIL)levels were lower in the experimental group (P<0.05);the serum MDA level was lower in the experimental group (P<0.05);the serum SOD and GSH-PX levels were higher in the experimental group (P<0.05) .Conclusion Midazolam combined with etomidate can reduce oxidative stress reaction in patients after laparoscopic cholecystectomy, which is conducive to the recovery of liver function.
4.Studies on the Quality standard of Multicomponent Dibiling

The Quality Standard of Multicomponent Dibiling, a nose drop for nasitis was studied.The presence of garlic, cassia and ephedra were demonstrated by TLC and the amouns of Allicin in the preparation was determined by HPLC. The method is simple, rapid and accurate.Average recoyery (n = 6 ) was 101.41 %and CV%was1 .76.
5.Clinical Significance of Antinucleosome antibody in systemic lupus erythematosus

Objective To investigate the clinical significance of antinucleosome antibody (AnuA) in the patients with lupus erythematosus (LE) through detecting the serum level of AnuA in different types of LE patients before and after treatment. Methods Enzyme-linked immunosorbent assay(ELISA) was used to detect the serum level of AnuA in 31 SLE patients, 26 SCLE patients, 7 DLE patients, 6 scleroderma patients, 5 dermatomyositis patients and 30 healthy controls. Among them, the serum levels of AnuA before and after treatment were detected in 15 SLE patients and 10 SCLE patients. Clinical data, laboratory test results and the scores of SLE disease activity index (SLEDAI) were analyzed with SPSS11.0 software. 6.Risk factor analysis of coronary artery lesions in children with Kawasaki disease

Objective To determine the risk factors for coronary artery lesions (CALs) in patients with Kawasaki disease (KD).Methods The clinical records of 2331 patients with KD from January 2005 to December 2014 in Beijing Children's Hospital,Capital Medical University were retrospectively analyzed.The relationship between the following factors and CALs was analyzed by univariate and multivariable Logistic regression analysis:age,gender,incomplete KD,total fever duration,intravenous immunoglobulin(IVIG) treatment resistance,white blood cell count,C-reactive protein (CRP),platelet count,sodium and albumin.Results The incidence of CALs was 36.0%(840/2331 cases).Univariate Logistic regression analysis indicated that male patients,incomplete KD,total fever duration ≥10days,IVIG treatment resistance,CRP>100mg/L,platelet count>300×109/L and albumin<35 g/L were associated with CALs (all P<0.05).Multivariable Logistic regression analysis identified that male patients (OR=1.698,95% CI 1.383-2.084,P<0.001),incomplete KD (OR=2.730,95% CI 2.121-3.515,P<0.001),total fever duration ≥10 days (OR=2.556,95% CI 1.975-3.307,P<0.001),CRP>100 mg/L (OR=1.556,95% CI 1.274-1.900,P<0.001) and albumin<35 g/L (OR=1.665,95% CI 1.323-2.096,P<0.001) were the independent risk factors for CALs.Conclusions The main damage in patients with KD is CALs.The male children with KD,incomplete KD,total fever duration ≥10 days,CRP>100 mg/L and albumin<35 g/L were prone to CALs.
7.Clinical significance of combined detection of serum SAA,CRP,PCT and TAP in early diagnosis and treatment of severe acute pancreatitis

Objective To analyze the changes of serum amyloid A (SAA) ,C reactive protein (CRP) ,procalcitonin(PCT) and trypsin activated peptide(TAP) in the patients with acute pancreatitis (AP) ,and to evaluate the above 4 indexes combined detection in the diagnosis and treatment of AP .Methods The levels of SAA ,CRP ,PCT and TAP were detected in 21 cases of severe AP (SAP) and 49 cases of mild AP(MAP) .The detection results were compared with those in the healthy control group (n=50) .Re‐sults The levels of SAA ,CRP ,PCT and TAP had statistical differences between the patients with acute stage of SAP and MAP groups with the healthy control group(P<0 .01);the levels of SAA ,CRP ,PCT and TAP in the SPA group were significantly high‐er than those in the MAP group with statistical difference(P<0 .01) .The diagnostic efficiency of 4‐index combined detection was higher than that of single index detection .Conclusion The combined detection of SAA ,CRP ,PCT and TAP is conducive to early diagnosis and disease condition judgement of AP ,and has an important significance to the diagnosis and treatment of SAP .
8.Effect of compound rhubarb gallnut gargle combined with periodontal basic therapy on inflammatory factors in chronic periodontitis

Objective To investigate the compound rhubarb gallnut gargle combined periodontal treatment effect on inflammatory factors in patients with chronic periodontitis.Methods 76 patients with chronic periodontitis from June 2014 to May 2015 in our hospital were selected and randomly divided into two groups,given periodontal treatment as control group,compound rhubarb gallnut gargle combined periodontal treatment as observation group,38 cases in each group,two groups were detected after treatment of gingival crevicular fluid levels of inflammation.Results After treatment,the gingival crevicular fluid of tumor necrosis factor-α(alpha TNF-α),high sensitive C-reactive protein(hs-CRP)and interleukin three(IL-6,IL-1 beta and IL-8)of two groups were significantly lower than those before treatment,and the observation group of inflammatory cytokine levels were significantly lower than the control group,the difference was statistically significant(P<0.05).The gingival index(GI)and gingival bleeding index(SBI)of the two groups were were lower than those before treatment,and the GI and SBI in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05).Conclusion In the treatment of chronic periodontitis,compound rhubarb gallnut gargle combined periodontal basic treatment effect,can significantly reduce the level of inflammation.
9.Comparison of transfection efficiency of lymphocytes from human peripheral blood by different methods

Objective To explore the transfection efficiency of primary lymphocytes from human peripheral blood by different methods to acquire the method with higher transfection efficiency.Methods Mononuclear cells from human peripheral blood were isolated using Ficoll-Hypaque.Cell viability was detected by Trypan blue staining.Suspending lymphocytes were sucked out and were incubated in 24-well plate after cultured in 6-well plate for 2 h.Activated lymphocytes were transfected by electroporation with plasmid(PEGFP-N1).Resting or activated lymphocytes were transfected by lentivirus vector(LVGFP) single infection or repeated infection,respectively.Green fluorescence protein (GFP) was detected under the fluorescence microscopy and percentage of positive cells was checked by flow cytometry at different time points after infection.At the same time,the effectiveness of lentivirus infection was compared under different conditions.Results Purity of mononuclear cells isolated by Ficoll-Hypaque was 95 % and its viability was over 95 %.The percentage of lymphocytes obtained with a uniform shape was 90 %-95 %.Scattered fluorescence was observed by electroporation under the conditions of voltage 2 100 V,pulse width 10 ms,pulse number 1 for lymphocyte,while fluorescent became weaker over time and no green fluorescent was observed after transfection for 72 h.After resting lymphocytes were infected once for 48 h by lentivirus vector,green fluorescent was not found and positive cells were less than 1%.1%-5 % of activated lymphocytes could express GFP after single lentivirus infection and the expression levels were enhanced with concentration increasing,while 5 %-10 % of activated lymphocytes showed strong green fluorescent by repeated lentivirus infection.In contrast with electroporation,the fluorescent with lentivirus infection was stronger over time.Conclusion Repeated lentivirus infection could efficiently transfect exogenous genes into activated lymphocytes for stable expression.
10.Primary Study on Hydrolysation of Centipedes Protein and Evaluation of Its ACE Inhibitory Activity

This study was aimed to analyze the major protein composition of centipedes (Scolopendra subspinipes mutilans L.Koch.), and the ACE inhibitory activity of its hydrolysis. Albumins, gulbulins, coixins and glutelins were sequentially extracted from centipedes flour with corresponding buffer and then quantified by Kjeldahl method and Brandford. Hydrolysation of four kinds of proteins of centipedes and the residues were conducted with pepsin. The hydrolysis was ultrafiltrated (MWCO=3 000) and lyophilized. The peptides (≤3 kD) were obtained to evalu-ate the ACE inhibitory activity by RP-HPLC. The results showed that the total protein content of centipedes was (62.69±1.41)%. Among which the contents of albumins, globulins, coixins, glutelins and residual were account-ing for (6.42±0.31)%, (7.94±0.24)%, (4.31±0.34)%, (40.66±0.56)% and (25.78±0.60)%, respectively. The inhibition rate of hydrolysis of four kinds of protein and residual were 50.28%, 57.37%, 31.15%, 58.99%, 80.81%, respectively. It was concluded that centipedes were rich in protein and the hydrolyzate of all proteins manifested ACE inhibitory activity at different extent. The residual and glutelins indicated strong ACE inhibitory potential by hydrolysis. This research provided valuable sights for exploring hypotensive activity and functional food from centi-pedes.
