1.Application of Pharmaceutical Care in the Treatment of Diabetes Mellitus at Home

OBJECTIVE:To explore the method of pharmaceutical care which influence the effect of the treatment for diabetes mellitus at home. METHODS:163 patients with diabetes mellitus in community who received treatment at home were selected, and then randomly divided into control group (58 cases),family visit group (51 cases) and group education group (54 cases);control group were given pharmaceutical intervention with traditional dispensing window;family visit group received door-to-door service or telephone follow-up service;group education group were given collective education and interactive way. The latter two groups were all given regular drug education,and the advisory desk was set up and provided consulting services about medication knowledge. RESULTS:After providing pharmaceutical care,the regular medication rate of 3 groups were 43.1%,64.8% and 78.4%;medication adherence were 51.7%,70.4% and 82.4%;the rates of blood glucose at target level were 29.3%,48.1% and 54.9%;the rates of glycosylated hemoglobin at target level were 13.7%,31.5% and 31.4%,ADR occurance rates were 31.0%, 13.0%,5.9%. These index of family visit group and group education group were all better than those of control group significantly (P<0.05). CONCLUSIONS:Humane pharmaceutical care can improve the quality and effect of the treatment for diabetes mellitus.

3.SELF— REGULATION OF MESANGIAL CELLS (MsC) PROLIFERATION BY MsC — DERIVED

This study was designed to investigate whether there is an antagonistic effect of MsC—derived IL—1 and PGE2 on MsC proliferation. Experimental results showed that: (1)MsC stimulated by LPS could produce IL—1; (2) MsC stimulated by MsC—derived IL—1 could release PGE2; (3) exogenous PGE2(100ug/ml) inhibited the MsC proliferation induced by MsC—derived IL—1; (4) indomethacin(2ug/ml) which inhibited the production of endogenous PGE2, enhanced the MsC proliferation induced by MsC—derived IL—1. The results suggest that there is a self—regulation effect on MsC proliferation by MsC—derived IL—1 and PGE2. Clinically, the beneficial effect of PGE2 should be protected, so that the abuse of indomethacin in the treatment of glomerulonephritis should be avoided.

Objective To investigate the expression of MMP-2 in rabbit carotid artery injured by airing drying. Methods Fifty-four rabbits were assigned to normal diet (n=18), fat diet (n=36), 18 of fat diet rabbits underwent airing drying of carotid intima after one-week fat diet. Then fed on fat diet after operation. 5.Assessment of the microbiologic and clinical effects of ultrasonic scaling combining with simultaneous antimicrobial agent irrigation

Objective:To assess the clinical and microbiologic effects of ultrasonic scaling with simultanous irrigation with chlorhexidine(CHX), conventional ultrasonic scaling or subgingival irrigation. Methods: 20 teeth in 7 patients with periodontitis were equally divided into test and control group. Patients in the test group were treated by irrigation with CHX during ultrasonic scaling(Master400),those in the control with irrigating after scaling(MiNi ). Clinical assessments of bleeding on probing (BOP),probing depth (PD) were made just before and 7 and 14 days after treatment. The subgingival microbiologic examination was performed before and 0,7 and 14 days after treatment. 6.Application of three-level storage pattern to PACS in basic level hospitals

Objective:To explore the application of three-level storage pattern(on-line,near-line,off-line)to picture archiving and communication system(PACS),and to find a solution to the long-term storage of medical imaging data in PACS of the basic unites.Methods:To realize the on-line storage of medical imaging data by the redundant array of independent disk(RAID)of host-server;To realize the near-line storage of medical imaging data by the network attached storage(NAS);To realize the off-line storage of medical imaging data by the tape-drive.Results:The long-term storage of medical imaging data in PACS has been available in our hospital for one year since the three-level storage pattern used in PACS.It not only realizes the on-line access of enormous amount of medical imaging data,but also protects the imaging data's integrity when the on-line data damaged by human's activities or fire,etc.Conclusion:The application of three-level storage pattern in PACS,which is characterized as low expense,dependable performance,simple maintenance,can become perfect pattern in the basic Level hospital.
7.Factors of postoperative intracranial infection after endoscopic repair of cerebrospinal fluid rhinorrhea

Objectives To investigate risk factors for postoperative intracranial infection after nasal endoscopic re-pair of cerebrospinal fluid rhinorrhea, in order to provide a reference for later treatment. Methods Selected 80 cases treated by nasal endoscopic repair of cerebrospinal fluid rhinorrhea from January 2010 to 2015 as study object, ret-rospective analyzed the clinical data such as the patient's age, gender, etiology, leak size, the leak location, prior in-tracranial infection, prophylactic antibiotics and postoperative application of antibacterial drug > 7 d, the number of operations, whether or not the operation using artificial materials, repair materials, repair of cerebrospinal fluid rhin-orrhea failure and by nasal endoscopic repair of cerebrospinal fluid rhinorrhea occurred between intracranial infec-tion. Results 4 of them (5.00 %) occurred intracranial infection. CSF bacterial culture show 8 strains of pathogenic bacteria were isolated, including 3 cases of leather of gram negative bacteria and 5 strains of leather of gram positive bacteria and gram positive bacteria were mainly Staphylococcus aureus and accounted for 40.00 %. Univariate anal-ysis showed that repair of cerebrospinal fluid rhinorrhea failure, improper surgical procedures, previous intracranial infection history, the choice of restorative materials and the size of the leak diameter > 1 cm and after endoscopic repair of cerebrospinal fluid rhinorrhea postoperative intracranial infection have some relevance ( < 0.05). Logistic multivariate analysis showed that repair of cerebrospinal fluid rhinorrhea failure, improper surgical procedures, previ-ous intracranial infection history, the choice of restorative materials and the size of the leak diameter > 1 cm of intranasal endoscopic repair of cerebrospinal fluid rhinorrhea independent risk factor for postoperative intracranial infection ( < 0.05). Conclusions Skull size of the leak diameter > 1 cm, previous history of intracranial infection, cerebrospinal fluid rhinorrhea repair a variety of factors fail, surgery failure factors are likely to increase patient after endoscopic repair of cerebrospinal fluid rhinorrhea intracranial infection the incidence.
8.Effect of ptxA and ptxB genes of phosphotransferase system on growth of Streptococcus mutans.

OBJECTIVEThis study aims to evaluate the effect of ptxA and ptxB genes, which are important genes in the L-ascorbate phosphotransferase system (PTS) of Streptococcus mutans (S. mutans).
METHODSThe ptxA-, ptxB-, and ptxAB-double deficient mutant as well as ptxAB-complemented strain were constructed. Quantitative real-time polymerase chain reaction analysis was performed to evaluate the expression of the target genes of wild-type S. mutans when L-ascorbate was used as the sole carbohydrate source. The OD₆₀₀ values of the wild type, deficient, and complemented strains were continuously monitored, and their growth curves were constructed to compare growth capacity.
RESULTSPolymerase chain reaction and sequencing analyses suggested that deficient and complemented strains were successfully constructed. The expression levelsof ptxA and ptxB significantly increased (P < 0.01) when L-ascorbate was used as the sole carbohydrate source. The growth capacity of the deficient mutants decreased compared with that of the wild-type strain. However, the wild-type phenotype could be restored in the complemented strain.
CONCLUSIONptxA and ptxB genes are associated with L-ascorbate metabolism of S. mutans. The construction of deficient strains and complemented strain lay a foundation for further mechanism study on L-ascorbate metabolism in S. mutans.

BACKGROUND:The previous researches indicate that, shock waves can enhance the proliferation of T-cells and the expression of interleukin (IL)-2 through a mechanism that involves p38 mitogen activated protein kinase (MAPK)activation.OBJECTIVE: To investigate if adenosine triphosphate (ATP) release is an underlying mechanism through which low-density shock waves (LDSWs) augment T-cell function.DESIGN: Controlled repetitive measurement by groups, taking cells as subject.SETTING: Department of Orthopedics, the First Hospital of Jilin University.MATERIALS: KDE-2001 Extracorporeal Shockwave Lithotripter (Beijing Zhongke Jian An Meditechs Co., Beijing, China).p38 MAPK inhibitor SB203580 1 mg (BioSource Inc., Camarillo, CA); p38 MAPK kit for detecting phosphorylation (Cell Signaling Technology, Inc. U.S.A.); P2 receptor inhibitor suramin 50 mg (BIOMOL Research Laboratories Inc., PA) was prepared into 0.02 mol/L solution by 1.749 2 mL IMDM. ATP enzyme: apyrase 200 U (Sigma, U.S.A.); P2X7 receptor antagonist KN-62 (BioSource Inc., Camarillo, CA); ATP Bioluminescence Assay Kit CLS Ⅱ (Roche Diagnostics GmbH,Mannheim, Germany).METHODS: The experiment was carried out in the Orthopedic Laboratory of the First Clinical Hospital in Jilin University from January 2005 to December 2006. ①An Extracorporeal Shockwave Lithotripter (at 7 kV generator voltage, 0.3 μF capacitance, 23 MPa positive pressure, 0.18 mJ/mm2 energy flux density) was applied for LDSWs treatment ranging from 50 to 400 impulses. ②ATP release into the culture supernatant from Jurkat T-cells or human peripheral blood mononuclear cells (PBMCs) was determined with a specific ATP Bioluminescence Assay Kit. ③Negative control group excluded antagonist or inhibitor. Human PBMCs were used to determine the effect of LDSWs on activated T-lymphocyte proliferation. Human Jurkat cells were used to study the effects of LDSWs on IL-2 expression. Expression and phosphorylation of p38 MAPK in Jurkat T-cell were measured by Western Immunoblotting with anti-p38 MAPK antibodies and anti-p38 MAPK phospho-specific antibodies that recognized the phosphorylation (on Thr180/Tyr182).MAIN OUTCOME MEASURES: extra-cellular ATP release, IL-2 expression in cell suspension, cellular proliferation and the phosphorylation of p38 MAPK.RESULTS: ①ATP release under the condition without LDSWs was obviously lower than that with LDSWs of 100, 150,200, 250, 300, 360 and 400 impulses (P < 0.01), and ATP release increased with the LDSWs impulse.②Compared with negative control group, the additions of apyrase, KN-62 or suramin attenuated the 3H-TdR incorporation of the phytohemagglutinin-stimulated PBMCs or CD3/CD28-stimulated Jurkat T-cells, which were effected with LDSWs of 100,150, 200, 250, 300, 330 impulses at 0.18 mJ/mm2 (P< 0.01). IL-2 expression in the cellular supernatant was also significant increased (P < 0.01). ATPase, KN-62 or suramin all decreased the effect of LDSWs on p38 MAPK of Jurkat T-cells.CONCLUSION: ①LDSWs deform cellular membranes but have no effect on organelle, which results in ATP release from Jurkat cells. Exogenous ATP release activates P2X7 receptor and p38 MAPK, and increases IL-2 expression. LDSWs enhance T-cell proliferation and IL-2 expression through a mechanism that involves ATP release, P2X7 receptor and phosphorylized p38 MAPK activation. ②The release of ATP plays a key role in the mechanism through which LDSWs regulate the function of T cells.
10.Body volume measurement in female college students and selection of optimal regression equations in Zhejiang province

BACKGROUND: Human body is an irregular geometrical one, so it is very diffcult to measure its volume.OBJECTIVE: To establish multiple regression equations by taking body height and body mass as the independent variables and body volume as the dependent variable, calculate the body volume of female college students and select optimal regression equations.DESIGN: A single-sample univariate analysis.SETTING: Department of Physical Education of Lishui College, Department of Physical Education of Zhejiang University and Academy of police in Zhejiang. PARTICIPANTS: Eighteen female students aged 18-22 years were selected from Zhejiang Lishui College.METHODS: Both the body height and body mass indexes of the female students were measured with the nation-ratified constitutional test instrument, and the body volume index was measured with a self-made iron container with a diameter of 0.95 m and height of 1.20 m. There was a scale mark for height in the container, water was poured to a fixed height, then the student slowly immersed herself into the water completely and the height difference was recorded. Body volume (m3)=(0.95÷2)2×3.141 59×height difference. The measured data were statistically calculated. Regression equations were established by taking body height and body mass as the independent variables and body volume as the dependent variable,and the optimal regression equation was selected.MAIN OUTCOME MEASURES: The measured data of body height,body mass and body volume of female students and the calculated results of the regression equations were observed.RESULTS: The measured data of body height, body mass and body volume of 18 female students all entered into the in result analysis ① A regression equation in two unknowns for calculating body volume was established: (y)=-0.031 016+0.000 761×body mass+0.000 267×body height. ②A regression equation in one unknown for body volume and its optimal selection: The linear equation was y=0.001×body mass+0.000 8; the logarithm equation was y=0.005 1Ln (body mass)-0.15; the power equation was (y)=0.001×body mass0.9909; the exponent equation was y =0.0192×e0.0188x.and the multiple correlation coefficient R2=0.9497-0.9591, all were close to 1, indicating that the body volume predicted by models was highly correlated with the actual one (r > r0.001(18-2), P < 0.001), the predicted values of the 4 models were not different from the actual one. ③ Analyzing from the simplicity of calculation and the economic way of index measurement, Logarithm equation is the best in the 5 regression equations. ④ The body volume covered body shape, physical function and Physique.CONCLUSION: The index of body volume is one of the important indexes, which cannot be neglected in the study of Physique. Analyzing from the simplicity of calculation and the economic way of index measurement, Logarithm equation is the best.
