1.Research and development of aphasia recovery

OBJECTIVE: About 70% patients with stroke have dysfunction at various degrees in clinic, especially aphasia would affect their social communication and even offer a serious economic load for family and social. This paper aims to summarize the relevant articles on aphasia recovery so as to provide researching evidences in the future. DATA SOURCES: Relevant articles were retrieved from the Medline with the key words of "aphasia, recovery, language, recovery" in English from January 1980 to January 2004. STUDY SELECTION: All articles were selected firstly, those which were related to aphasia recovery were selected, and the non-randomly clinical papers were excluded. The full text of rest was looked up to determine whether they were randomized controlled studies or not. Randomized controlled researches were regarded as inclusion criteria regardless of blind way. DATA EXTRACTION: Among 22 articles on aphasia recovery, 12 were selected according to inclusion criteria and 10 were excluded because of duplicated papers.DATA SYNTHESIS: Recovery of language function was related to sex,age, ducational degrees, characteristics of lesion, range of lesion, duration from episode to language recovery, insight and self redress of language disorder. Therefore, there were various findings and views for previous re searches on mechanism of aphasia recovery. CONCLUSION: There is not uniform conclusion on mechanism of aphasia recovery. Compliance of language function in brain may be a mechanism of aphasia recovery.
2.The allogeneic bone marrow mesenchymal stem cells improve rats cutaneous wound healing

Objective This experiment used the allogeneic bone marrow mesenchymal stem cells to cure the wound in rats .And to observe the effect and safety of bone marrow mesenchymal stem cells injection for wound heal -ing.Methods The rats with wound were divided into two groups .The rats bone marrow mesenchymal stem cells were isolated,cultured,purified,amplified and passaged.Flow cytometry instrument was used to identify the immune pheno-type of rats bone marrow mesenchymal stem cells .Local injected the rats-bone marrow mesenchymal stem cells in the rats.The difference of wound healing rate between the experimental group and control group were statistically ana -lyzed.Western blot was used to detect the expression of keratinocyte growth factor (KGF) in two groups.Histopatho-logical change of the wound was observed .Results The wound healing rate between the two groups had significantly statistical difference.After 3 days,7 days,14 days,the wound healing rates of the experimental group were (21.95 ± 8.11)%,(56.25 ±4.96)% and (79.85 ±5.44)%,respectively and which were better than those of the control group[(12.11 ±4.87)%,(30.24 ±1.53)%,(58.51 ±6.52)%](t=4.693,3.853,2.559,all P <0.05). According to the grading standards of the healing skin after HE staining ,the experimental group was better than the control group .According to the results of Western blot , the KGF protein expression of the experimental group was higher than the control group .Conclusion The allogeneic bone marrow mesenchymal stem cells may be an effective and safe method for the wound in rats .
3.Cytokines expression and significance in the peripheral blood from mice with temporal lobe epileptic in the acute phase

Objective To study the expression changes of IL-1β、FN-α6、IFN-γ and TNFR-m18 in peripheral blood within 2 hours in epileptic mice. Methods Semi quantitative real-time PCR was used to test the mRNA expres?sion level of IL-1β、FN-α6、IFN-γand TNFR-m18 in peripheral blood from normal and pilocarpine-induced epileptic mice at different time points (10 min, 30 min, 1 h and 2 h). Results The mRNA expression level of IL-1βincreased at 30 min(1.8±0.07), 1 h(2.9±0.98)and 2 h(1.45±0.11)after pilocarpine-induced status epilepticus comparing with that of control and SE 10 min(0.81±0.09)(P<0.05). The IFN-α6 mRNA expression level was lower at 10 min(0.59±0.05, P<0.05) than that of control. IFN-γmRNA expression level was higher at 10 min(2.85±0.11) than that of control and at oth?er time points during SE(P<0.01). TNFR-m18 mRNA expression level was higher at 1h(2.84±0.15) than that of control, and at other time points during SE(P<0.01). Conclusion The immune system of epileptic state is active, the imbalance of cytokine expression in peripheral blood may be related to the immune pathological process of acute stage of epilepsy.
4.Comparative research on application between power peripherally inserted central catheters and double cavity central venous catheter with the patients in intensive care unit

Objective To compare the safety and effectiveness of power peripherally inserted central catheters (Power PICC) and double cavity central venous catheter (CVC) application in intensive care unit (ICU). Methods 458 cases were reviewed during January to September in 2014 and divided into two groups: Power PICC group (245 cases) and CVC group (213 cases) , and average retention time, successful rate of inserting catheter and the incidence of complications were compared. Results The average retention time of Power PICC group was (21.6±5.8) days which was longer than (13.1±3.4) days of CVC group (t=2.234, P <0.05). No statistics difference of successful rate between two groups (P>0.05). No significant difference for the total incidence of complications between two groups as 14.69% (36/245) and 19.72%(42/213)(P>0.05). No significant difference for the total incidence of complications in the operation time between two groups as 5.31% (13/245) and 4.23% (9/213)(P>0.05). But rate of catheter malposition for Power PICC group [ 2.86% (7/245) ] was higher than CVC group 0 (X2=4.428, P <0.05). Rate of the total incidence of complications in the retention time Power PICC group [ 9.39%(23/245) ] was lower than CVC group [ 15.96%(33/213)(P<0.05). And rate of catheter related blood stream infection of CVC group [3.29%(7/213)] was much more higher than Power PICC group (0)(X2=6.139,P<0.05). Conclusions Power PICC and CVC are both applicable for ICU, and Power PICC has more advantage regarding safety and effectiveness than CVC and can be one replacement for CVC.
5.EFFECT OF PU-ERH TEA ON LIPOGENESIS AND EXPRESSION OF RELATIVE GENES IN OBESE RAT FED WITH HIGH FAT DIET

Objective To examine the effect of Pu-Erh tea extract(PTE) on genes expression of lipogenesis in white adipose tissue of rats fed high fat diet.Method Thirty male SD rats were randomly divided into three groups(n=10):the control group(basal diet);the high fat group(high fat diet);the PTE group(high fat diet + Pu-Erh tea extract).Body weight and adipose tissue were measured.Expression of genes regulating lipid metabolism was assessed in adipose tissue.Results PTE supplementation prevented diet-induced increases in body weight and adipose tissue.Diacylglycerol acyltransferase-1(DGAT1),stearoyl-CoA desalurase-1(SCD1) and sterol regulatory element binding protein-1c(SREBP-1c) mRNA levels were markedly decreased in adipose tissue of rats fed PTE.Conclusion This study shows for the first time that Pu-Erh tea extract prevents diet-induced obesity,and this effect is partly mediated via a direct influence on adipose tissue.
6.Optimization of a floating osmotic pump system of ambroxol hydrochloride using central composite design-response surface methodology and its pharmacokinetics in Beagle dogs.

This paper reported that a new type of floating osmotic pump of ambroxol hydrochloride was designed. Third method apparatus (Chinese Pharmacopeia 2010, appendix XD) was employed to simultaneously evaluate the release and floating behavior in vitro. The system was optimized using central composite design-response surface methodology. Similar factor (f2) between the release profile of self-made formulation and the target release profile was chosen as dependent factor. The amount of glucose (A, mg), pore former (B, %) and weight of coating (C, %) were employed as independent factors. Optimized formulation was: A (100.99 mg), B (1.70%), C (4.21%). The value of f2 (89.14) was higher than that of market capsules (69.02) and self-made tablets (72.15). It was showed that self-made capsules possessed character of zero-order release (r = 0.994 4) and drug release completely (>90%). It was showed in result of in vivo study that tmax and Cmax of self-made capsules were significantly lower than that of market capsules and self-made tablets. The correlation coefficient between the fraction of absorption in vivo and the release rate in vitro was 0.985 1, and relative bioequivalence of self-made capsules was 110.77%. Accordingly, self-made capsules displayed obviously characteristics of controlled release both in vivo and in vitro.
7.Expression of serum sHLA-G in systemic lupus erythematosus

Objective To investigate the expression of serum sHLA-G in systemic lupus erythematosus (SLE) patients and the association with the disease activity.Methods The serum concentration of sHLA-G in SLE patients and healthy controls was measured with enzyme-linked immunosorbent assay.Results Significant higher sHLA-G levels were detected in patients of SLE than control group (P<0.01),The serum concentrations of sHLA-G in active SLE patients were markedly higher than stable SLE patients (P<0.01).The expression level of sHLA-G showed positive correlations with SLE activity index (SLEDAI)(r=0.30,P=0.01).There was no correlation between sHLA-G levels and serum concentration of Anti-dsDNA,C3,C4 and Anti-ANA in SLE patients (P>0.05).Conclusion The level of serum sHLA-G is significantly increased in SLE patients.Positive correlations are observed between sHLA-G levels and SLEDAI.These data indicated that sHLA-G may play certain roles in the pathogenesis and progress in SLE.
8.Effect of Rifabutin Combined with Multi-drugs in the Treatment of Multi-drug Resistant Tuberculosis with Long-term Therapy

Objective:To evaluate the effect and safety of rifabutin combined with multi-drugs in the treatment of multi-drug resist-ant tuberculosis with long-term therapy. Methods:Totally 86 cases of patients with multi-drug resistant tuberculosis were divided into the control group and the treatment group with 43 ones in each according to a random number table method. The two groups were trea-ted with levofloxacin, pasiniazid, ethambutol, protionamide and amikacin etc. The control group was treated with rifapentine, and the treatment group was treated with rifabutin additionally. After 18-month treatment, the negative conversion ratio of sputum smear and sputum mycobacterium tuberculosis culture, lesion absorption rate and cavity closure rate of X-ray chest radiograph and adverse reac-tions in the two groups were compared. Results:The negative conversion ratio of sputum smear and sputum mycobacterium tuberculosis culture in the treatment group was 41. 86% and 32. 56%, respectively, which were similar with those in the control group ( P >0. 05). There were no significant differences in lesion absorption rate and cavity closure rate of X-ray chest radiograph and adverse re-actions between the two groups (P>0. 05). Conclusion:Rifapentine or rifabutin combined with multi-drugs in the treatment of multi-drug resistant tuberculosis can improve the negative conversion rate of sputum mycobacterium and lesion absorption and cavity closure with high safety.

10.Analysis of clinical effects of iliolumbar fixation in treating sacrum fracture of Denis type II.

OBJECTIVETo evaluate the clinical effects of iliolumbar fixation for the sacrum fractures of Denis type II.
METHODSThe clinical data of 86 patients with sacrum fracture of Denis type II treated by iliolumbar fixation from January 2008 to January 2012 were retrospectively analyzed. There were 55 males and 31 females, aged from 17 to 55 years old with an average of 39.1 years. Among them, 73 cases complicated with pelvis fracture and 13 cases with acetabular fracture; 37 cases with sacral neurological symptoms and 49 cases without sacral neurological symptoms. Fracture healing time, nerve function, clinical function and complications were observed in the patients.
RESULTSIn 86 cases, 6 cases were out of followed-up and 80 cases were followed up from 24 to 71 months with an average of 36 months. The mean fracture healing time was 13 weeks (ranged, 10 to 38 weeks). According to Gibbons scoring to evaluate the neurological function, preoperative nerve rehabilitation, lower limbs feeling, lower limbs activity,bladder and rectum function,total score respectively were 0.62 +/- 0.04, 1.54 +/- 0.35, 1.12 +/- 0.18, 0.23 +/- 0.01, 3.46 +/- 0.47 and postoperative respectively were 0.82 +/- 0.12, 0.36 +/- 0.04, 0.05 +/- 0.01, 0.03 +/- 0.01, 1.25 +/- 0.22, there were statistically significant differences between preoperative and postoperative (P < 0.05). According to Majeed scoring to evaluate the clinical function, postoperative pain, standing, sitting, sexual life, work ability, total score respectively were 22.54 +/- 4.02, 27.93 +/- 5.46, 8.47 +/- 3.61, 2.54 +/- 1.33, 16.46 +/- 4.34, 81.32 +/- 8.73, 60 cases got excellent results, 17 good, 3 fair. The main complications including fracture nonunion of 5 cases,deep incision infection of 1 case, and screw prominence resulting uncomfortable of 8 cases.
CONCLUSIONIliolumbar fixation has the advantages of stable fixation, satisfactory functional rehabilitation, less complications, and is a good method in treating sacrum fracture of Denis type II.
