AIM: To analyze the characteristics of neurological ophthalmology manifestation of patients with neurosyphilis. · METHODS: Retrospective and nonrandomized case analysis were used. Totally 22 cases of 39 eyes were included. They were 17 males and 5 females, aged from 34 to 65 years old. The average age were 49. 6 years old. ·RESULTS: The optic nerve atrophy presented in 11 cases of 22 eyes. One eye of them accompanied by left eye oculomotor nerve palsy;5 eyes in 3 cases expressed as optic neuritis acute phase;neuroretinitis appeared in 4 cases of 6 eyes; 1 case of 2 eyes expressed as chorioretinitis accompanied by optic disc edema; central retinal artery occlusion were found in 1 case of 1 eye. Argyll-Robertson pupil was as only manifestation in 2 cases of 3 eyes. In all cases, Argyll-Robertson pupil signs can be seen in 19 eyes. Treponema pallidum particle agglutination test ( TPPA) were positive in all 22 cases. Syphilis rapid plasma reactin test ( RPR ) were positive in 19 of 21 cases. All patients underwent lumbar puncture and cerebrospinal fluid were detected for RPR, cerebrospinal fluid protein, white blood cell count. Cerebrospinal fluid RPR were positive in 13 cases. Cerebrospinal fluid protein were greater than 450mg/L in 18 cases. Cerebrospinal fluid white blood cell count were greater than 5/mm3 in 13 cases. · CONCLUSION: Neurosyphilis involving neuro -ophthalmology often occurs in middle-aged men and subacute onset. Both eyes can suffered from optic nerve disease simultaneously or sequencely. A few can be expressed as other cranial nerve palsy, which may lead to misdiagnosis. Considering medical history, clinical manifestations, ophthalmic examination, serum and cerebrospinal fluid laboratory tests can improve the diagnostic rate.

Objective To evaluate the feasibility of ETView visual endotracheal intubation in elderly patients with a potentially difficult airway under general anesthesia.Methods A total of 80 patients scheduled for elective surgery,with at least three characteristics indicative of an increased risk for difficult tracheal intubation and aged 65-86 years with American Society of Anesthesiologists (ASA) grade [Ⅱ or Ⅲ,were selected and randomly assigned into 2 groups:the control group (Group C) and the ETView visual endotracheal intubation group (Group E) (n=40 for each).After induction with propofol 0.5-1.5 mg/kg,sufentanil 0.2 μg/kg,and rocuronium 0.6mg/kg,regular tracheal intubation and ETView visual endotracheal intubation were conducted in Group C and Group E respectively.Blood pressure and heart rate were recorded before induction (T0),before initial intubation (T1),at successful intubation (T2),and 5min after successful intubation (T3).Duration of catheterization,number of intubation attempts,rate of successful intubation at first attempt and complications within 2 days of intubation were also recorded.Results The duration of catheterization and the number of intubation attempts were lower in Group E than in Group C[(34±6) s vs.(48± 22) s,(1.0±0.0) vs.(1.4±0.4),t=2.484 and 2.373,respectively,P=0.017 and 0.023,respectively].The rate of successful intubation at first attempt was higher in Group E than in GroupC (100％ or 40 cases vs.75％ or 30 cases),x2=5.714,P=0.017).There was no significant difference in hemodynamic changes and the rate of postoperative hoarseness between the two groups (P＞0.05 for both).Conclusions ETView visual endotracheal intubation shows excellent safety in elderly patients with a difficult airway under general anesthesia,with shorter catheterization duration and a higher rate of successful intubation at first attempt than regular intubation.

OBJECTIVE:To establish the optimal technical condition for the extraction of volatile oil from the volatile herbs of Huoxiang zhengqi prescription with supercritical CO2 fluid(SFE-CO2).METHODS:The technical condition for the extraction of volatile oil from the volatile herbs of Huoxiang zhengqi prescription were optimized using L9(34)orthogonal design table with extraction pressure,the extraction temperature,the pressure and temperature of separator Ⅰ as factors and with the yield of the volatile oil as index.RESULTS:A highest extraction yield of volatile oil from the volatile herbs of Huoxiang zhengqi prescription was achieved(2.057%)under an optimal technical condition as follows:with extraction pressure at 25 MPa,the extraction temperature at 30 ℃,the pressure for separator Ⅰ at 9 MPa and the temperature of separator Ⅰ at 45 ℃.CONCLUSION:Extraction of volatile oil from the volatile herbs of Huoxiang zhengqi prescription by SFE-CO2 is characterized by high extraction yield,stable,pure operation,safe,and by which the biological activity of the volatile oil can be maintained,thus it is a potential extraction and separation method.

Objective To evaluate the safety and efficacy of Holmium laser resection for primary non-muscle invasive bladder cancer (HoLRBt) compared with transurethral resection of bladder tumor (TURBt). Methods Data of 212 patients with primary non-muscle invasive bladder cancer were collected retrospectively. The patients were divided into HoLRBt group(n= 101) and TURBt group (n= 111). The patients in each group were stratified into 3 risk groups (low, intermediate and high risk) according to prognostic factors for recurrence based on EAU guideline. Then, the safety of HoLRBt and TURBt groups were compared, concerning the intraoperative complications and postoperative recovery. Efficacy indicated by recurrence-free survival of the 2 groups was analyzed and compared by Kaplan-Meier technique. Results Patients' demographics including age, gender, tumor characteristics, and recurrence risk of tumor between the 2 groups were comparable(P＞0.05). No obturator nerve reflex occurred in the HoLRBt group. Meanwhile, 7 out of 111 patients in the TURBt group experienced this complication resulted bladder perforation in 3 patients. The proportion of patients needing postoperative bladder irrigation in the HoLRBt group was lower compared to the TURBt group (P＜0.05). HoLRBt was associated with shorter postoperative catheter drainage period (P＜0.05). The mean postoperative follow-up was 34 months (range 18 to 43). Recurrence-free survival after HoLRBt was similar with that of TURBt (P = 0. 283). Conclusions Compared with TURBt, HoLRBt is a feasible, safe and effective alternative for the management of primary non-muscle invasive bladder cancer with similar therapeutic efficacy and fewer perioperative complications.HoLRBt can be widely used in clinical practice in the treatment of primary non-muscle invasive bladder cancer.

Objective To investigate and analyze the death situation of children aged under 5 year old in a class 3A hospital during 2007-2015 to provide a basis for formulating corresponding interventional measures to reduce the disease mortality of children.Methods The medical records of children death in this hospital during 2007 2015 were retrospectively analyzed.Results The mortality rate of children under 5 years old in this hospital during 2007-2015 showed a decreasing trend year by year.There was the statistically significant difference in the mortality rates among different years,between male and female (3.58 ％vs.2.10％) and between rural family registers and non-rural family registers(P＜0.05);the neonatal mortality rate was highest (50.45 ％);the top 5 places of death causes were premature delivery or low birth weight newborns,respiratory system diseases,nervous system diseases,congenital diseases and accidents.Conclusion The key for reducing the children mortality rate is to strengthen the perinatal health care,strengthen prenatal examinations and pregnant examinations,prevent the occurrence of respiratory system disease,pay attention to the influence of various factors on children health and prevention and treatment of accidents.

Clinical features of 2 cases of ectopic ACTH syndrome with manifestation of severe hypercortisonism are described with the aim of exploring effective therapies.

Objective To investigate the relationship between serum homocysteine and post-stroke depression in elderly. Methods 62 elderly first stroke patients with post-stroke depression, 20 first stroke patients without depression, and 100 healthy elderly as control were detected their fasting serum homocysteine with fluorescence polarization immunoassay. Results Serum homocysteine were (25.59±9.47) μmol/ L, (18.56±7.84) μmol/L, and (12.67±6.45) μmol/L in the depression group, non depression group and healthy group, respectively (P<0.05). The level of homocysteine correlated with the score of Hamilton Depression Scale (r=0.8953, P<0.05) and age (r=0.8462, P<0.05). Conclusion The level of serum homocyteine might be one of risk factor of depression in patients with stroke.
