1.Expression of MAGE-10 genes in human hepatocellular carcinoma and its clinical significance

Objective To study the relationship between the expression of MAGE-10 genes and the clinical parameters of metastasis and recurrence of patients with Hepatocellular carainama (HCC). Methods The expression of MAGE-10 genes in tumor tissues,liver tissues adjacent to tumor and hepatocirrhosis tissues was examined by RT-PCR method.The relationship between positive expression rate of MAGE-10 genes and clinical parameters of metastasis and recurrence (such as AFP, AFU, Anti-HCV, HBsAg, AFPmRNA, diameter of tumor) in patients with HCC were studied. Results The positive expression rate of MAGE-10 genes significantly higher in tumor tissues than that in the surrounding tumor tissues (51 6% vs. 0%, ? 2=18 95,P 0 05).There was no expression of MAGE-10 in hepatocirrhosis tissues of 10 cases. Conclusions There was a high frequency of MAGE-10 genes expression in HCC,but it is unrelated to the tumor markers,metastasis and recurrence of HCC.

3.Diagnostic Value of 64-slice Spiral CT Coronary Angiography in the Parietal Coronary Artery

Objective To study the imaging manifestations of parietal coronary artery and the diagnostic value with 64-slice spiral CT.Methods 1000 patients with clinically suspected coronary heart disease underwent 64-slice spiral CT scan,the images were dealed with curved planar reconstruction and analysed at workstation.Results Parietal coronary arteriae were found in 380 patients,among them,314 cases were at the LAD,32 cases were at the CX,13 cases were at the RCA and multiple arteriae involved in 21cases.Conclusion 64-slice spiral CT coronary angiography is an effective methed in diagnosis of parietal coronary artery.
4.Evaluation of the effects of low-dose ketamine on postoperative analgesia

Objective To investigate the effects of low-dose ketamine combined with fentanyl or sulfentanyl in postoperative patient-controlled intravenous analgesia.Methods Two hundred cases with ASA Ⅰ~Ⅱ undergoing elective orthopedic operations were randomly allocated into four groups(50 cases for each group): 0.4?g/(kg?h) fentanyl(group F);0.2?g/(kg?h) fentanyl+120?g/(kg?h) ketamine(group FK);0.04?g/(kg?h) sulfentanyl(group S);and 0.02?g/(kg?h) sulfentanyl+120 ?g/(kg?h) ketamine(group SK).8mg of ondansetron was added to each recipe,and then diluted into 100ml with normal saline.The loading doses of all the patient-controlled-analgesics were 2ml administered 30min before the end of the operation.The background infusion doses were 2ml/h,while the pressing doses were 2ml,with intervals of 30min.Heart rate(HR),blood pressure(BP),respiration rate(RR),pulse O2 saturation and visual analogue scales(VAS),sedation score(SS) with adverse effects and the total times of pressing PCA pump button were observed and recorded in all patients respectively for 48h after operations.Results No statistically significant differences were found in HR,BP and RR among the four groups.VAS and the total pressing times in group F were higher than that in the other groups significantly(P
5.Influence of different combination of anesthetics on BP and HR during operation in aged patients with serious illness

Objective To evaluate the influence of different combination of anesthetics on blood pressure (BP) and heart rate (HR) of aged patients with serious illness during operation. Methods Forty patients, aged 80~92, with physical status Ⅱ~Ⅲ as categorized according to American Society of Anesthesiologists (ASA) undergoing selective surgery, were randomly divided equally into two groups: in group A ancsthesia was induced with midazolam, fentanyl and maintained with isoflurane, and in group B anesthesia was induced and maintained with etomidate, sevoflurane and nitrous oxide. Mean arterial pressure (MAP), HR and bispectral index (BIS) were observed at the points of preinduction (T0), induction (T1), intubation (T2), incision (T3), 30 minutes after incision (T4) and extubation (T5). Results After induction, BIS decreased from 95?2 to 52?4, while MAP and HR decreased from 110?11mmHg and 73?8/min to 75?9mmHg and 59?4/min respectively, in group A. While in group B BIS decreased from 94?3 to 58?3, MAP and HR decreased from 103?8 mmHg and 76?6/min to 87?7 mmHg and 67?5/min, respectively. 6.Long-term Follow-up of Patients with Varicose Veins of the Lower Extremities Treated by Electrocoagulation

Objective To summarize the therapeutic efficacy of electrocoagulation for varicose veins of the lower extremities.Methods Totally 168 cases(238 limbs)of varicose veins of the lower extremities were treated with electrocoagulation in our department from April 2000 to April 2007.Through a small ankle incision,electrocoagulation probe was inserted ascendingly into the groin via the great saphenous vein.After high ligation of the great saphenous vein,the varicose veins and great saphenous trunk were electrocoagulated.Results The patients were discharged from hospital in one to two days after the operation.None of them had incisional infection.The ulcers at the ankle were healed in four weeks.Postoperative complications in this series,included Ⅰ degree burn injury in one limb,Ⅱ degree burn injury in one limb,superficial phlebitic pain in two limbs(two cases);and numbness around the ankle in 14 limbs(10 cases).Totally 148 cases with 208 limbs were followed up for 2 to 9 years with a mean of 6.6 years.During the period,the rate of recurrent varicosity was 3.4%(7/208);and healing rates of ache,dermatitis and eczema,ankle pigment,swelling around the calf and ankle were 95.3%(162/170),85.7%(12/14),41.7%(15/36)and 36.4%(16/44)respectively.Conclusion Electrocoagulation is a minimally invasive and effective method for the treatment of varicose veins of the lower extremities,with quick recovery and low rate of recurrence.
7.Application of mechanical embolectomy in acute ischemic stroke

Acute ischemic stroke is a common disease serevely threatening the public health. The main management involves intravenous thrombolysis, intra-arterial thrombolysis and mechanical embolectomy. Recently, endovascular mechanical embolectomy is attracted more attention because it provides a good outcome for patients either ineligible not responsible for IV thrombolytic therapy. The article reviews the relative aspects with summerization in this field.
8.Application of Gait Analysis for Stroke Patients with Hemiparesis

Objective To explore the characteristics of hemiparetic gait for stroke patients. Methods 49 stroke patients were allocated to two groups: experimental group (n=33) received 3D-video gait analysis, and control group (n=16) received the gait analysis with ankle-foot orthosis (AFO). The kinematic parameters were compared between two groups, and the experimental group with normal gaits. Results Compared with the normal gaits, the step, stride, step frequency, gait velocity, and the percentage single support in gait cycle decreased significantly,and gait width, and the percentage in gait cycle of double support increased significantly in the experimental group (P<0.01). There was no significant difference in kinematic parameters beween the experimental group and the control group (P>0.05). Conclusion The major kinematic characteristics of hemiparetic gait are slow velocity, long cycle, relative decrease of single support of weak limb, and increase of double support.
9.Comparison of two prediction models for mortality ofacute paraquat poisoning

Objective:
To compare the effects of Cox proportional hazard regression model (Cox model) and extreme gradient boosting model ( XGBoost model ) on the prediction of the mortality of acute paraquat poisoning (APP).
Methods:
The APP cases admitted to Qingdao Eighth People's Hospital and Shandong Provincial Hospital from January 1st of 2018 to December 1st of 2020 was recruited and divided into a training group and a verification group by a random number table. The Cox model and XGBoost model were established to select the predictors for APP mortality. Receiver operating characteristic ( ROC ) curve was drawn to analyze the predictive power of the two models, and the calibration was evaluated using Hosmer-Lemeshow test.
Results:
Totally 150 APP cases were recruited. There were 75 cases each in the training group and in the verification group, with 52 and 55 cases died respectively, accounting for 69.33% and 73.33%. The Cox model showed that paraquat intake, the time from taking poison to seeing a doctor, the time for the first perfusion, the time for the first vomiting, aspartate aminotransferase, alanine aminotransferase, serum creatinine, blood urea nitrogen, white blood cell, lactic acid, creatine kinase isoenzymes, glucose, serum calcium and serum potassium were the predictors of APP mortality ( all P<0.05 ). The XGboost model showed that the predictive power of the factors in a descending order were the time from taking poison to seeing a doctor, the time for the first vomiting, the time for the first perfusion, lactic acid, white blood cell, paraquat intake, serum creatinine, serum potassium, serum calcium, creatine kinase isoenzymes, glucose, aspartate aminotransferase, blood urea nitrogen and alanine aminotransferase. The area under curve ( AUC ) of the XGBoost model for predicting was 0.972, which was greater than 0.921 of the Cox model ( P<0.05 ). The predicted results of the Cox model and XGBoost model were consistent with the actual situation ( P>0.05 ).
Conclusion
The Cox model and XGBoost model are consistent in predicting the mortality of APP, but the latter is better.
10.Evaluation of lipid accumulation in muscle fibers in inflammatory myopathies

Objective To study the frequency and correlate factors of lipid accumulation in muscle fibers in inflammatory myopathies Methods Muscle biopsy specimens were routinely processed for histopathological and histochemical studies Excepting inflammatory changes, lipid droplets were observed by ORO staining According to the amount of lipid droplets in the muscle fibers, these cases were separated into two groups, and then the differences in muscle power, serum CK level, morphologic changes of muscle, courses and corticosteroids administration between two groups were compared Results 37 7% specimens showed lipid accumulation in muscle fibers distinctly As compare with the lipid normal group, in the lipid increasing group, the generalized muscle fiber degenerating were more common and muscle weakness were more prominent Although more patients have long term therapy with steroid in lipid increasing group, there were no significant differences between these two groups Conclusions Lipid accumulation in muscle fibers was present in some patients with inflammatory myopathies Increasing of lipid droplets might result from the muscle fiber degenerating and might turn to impair the muscle function reversely
