1.Pulmonary rehabilitation can improve the exercise capacity and dyspnea of patients with chronic obstructive pulmonary disease

Objective:To investigate the effect of pulmonary rehabilitation therapy on the exercise capacity and dyspnea of persons with chronic obstructive pulmonary disease (COPD).Methods:One hundred COPD patients were randomly divided into a control group and an observation group, each of 50. Both groups were given routine medication, while the observation group was additionally provided with health guidance, oxygen therapy, respiratory physiological therapy and exercise for 3 months. Before and after the intervention, both groups′ forced expiratory volume in 1 second (FEV1) and the first and second forced expiratory volume as a percentage of FEV (FEV1%) were measured. The subjects′ motor functioning was evaluated using the 6-minute walk test. Enzyme-linked immunosorbent assays and immunoturbidimetry quantified their expression of inflammatory factors. And their ability in the activities of daily living (ADL) was evaluated using the Barthel index. The COPD quality of life questionnaire (CRQ) was also used to assess their life quality.Results:After the intervention, the average clinical efficacy in the observation group was 96%, significantly higher than that of the control group (80%). Moreover, the average FEV1, FEV1%, 6-minute walk test time of the former group were all significantly better than before the intervention and better than the control group′s results after the intervention. Their average CRP, IL-6 and TNF-α levels were all significantly lower as well. After the intervention, the observation group′s average total CRQ score and its average scores on the instrument′s emotion, fatigue, wheezing and disease control components were all better than the control group′s averages. The observation group′s average ADL score was also significantly higher than that of the control group.Conclusions:Supplementing conventional medication with pulmonary rehabilitation therapy can effectively improve the lung function, motor functioning and life quality of COPD patients. It can also lower their level of serum inflammatory factors.
2.Establishment of a miniature pig model of mandibular osteoradionecrosis.

OBJECTIVETo establish a miniature pig model of mandibular osteoradionecrosis (ORN).
METHODSA single dose of irradiation (IR25 Gy, 28 Gy) was delivered via an electronic linear accelerator to the right mandible of 24 miniature pigs by utilizing three dimensional conformal radiotherapy (3D-CRT). The first molar of the mandible was extracted 2 months after radiation. Mandibular ORN was diagnosed through gross observation, X-ray film, CT, and histopathologic examination.
RESULTSAt 3-4 months after radiation, all animals suffered from mandibular ORN after dental extraction. The symptoms of ORN were more severe in the 28 Gy group than in the 25 Gy group.
CONCLUSIONA miniature pig model of mandibular ORN can be established through 25 Gy irradiation + dental extraction.

3.Preliminary study on treatment of mandibular osteoradionecrosis by autologous bone marrow mesenchymal stem cells in miniature pigs

Objective To investigate the treatment effect of mandibular osteoradionecrosis(ORN) by autologous bone marrow mesenchymal stem cells(BMMSC) in miniature pigs.Methods Six miniature pigs with mandibular ORN(25 Gy) were used.BMMSC were separated and cultured in vitro and then implanted on to the premolded hydroxyapatite-tricalcium phosphate(HA-TCP).The BMMSC-HA-TCP complexes were implanted into the defective area of mandibular ORN in 4 animals.Implantation of only HA-TCP in 2 animals served as control.Gross observation, spiral CT, coronary CT and histopathologic examination were carried out.Results Fistula disappeared in animals with BMMSC-HA-TCP complexes 3 months after implantation, while fistula was found in controls.Spiral CT analysis showed that cortical bone repair were found 4 months after BMMSC-HA-TCP complexes implantation, while bone damage(cortical and cancellous bone fracture) increased in controls.Histopathologic examination revealed that 4 months later after BMMSC-HA-TCP complexes implantation, new bone formation and bone cells could be observed, but there was a large number of fibrous tissue and no new bone in controls.Conclusions Transplantation of autologous BMMSC with HA-TCP may have therapeutic effect in the treatment of mandibular ORN.
4.Research on Diagnosis and Treatment Behavior for Chronic Obstructive Disease among Physicians of South HenanArea

Objective:To investigate the current situation of basic knowledge and diagnosis and treatment behavior for chronic obstructive pulmonary disease (COPD) among physicians of South Henan area .Methods:Questionnaire was designed for a research ,which was conducted among physicians from 15 hospitals located in 9 counties ,and the data were analyzed by using SAS 6 .12 software .Results:A total of 135 valid questionnaires were collected .And 80 .7% (109/135)of the physicians understood the epidemiology of COPD ,while 31 .8% (43/135) of them understood the disease characteristics .The proportion of physicians who understood the disease characteristics in respiratory professional group was 44 .4% (20/45) ,which was significantly higher than that (25 .5% ,23/90) in non‐respiratory professional group (P<0 .05) .Furthermore ,66 .6% (90/135) of the physicians diagnosed disease by clinical symptoms ,while 33 .3% (45/135) of them diagnosed disease by clinical symptoms and pulmonary function .The proportion of physicians who diagnoses disease by clinical symptoms and pulmonary function in respiratory professional group was 60 .0% (27/45) ,which is significantly higher than that 20 .0% (18/90) in non‐respiratory professional group (P< 0 .05) .Therapy for acute exacerbation of COPD was nearly conformed to the treatment guidelines ,however ,50 .3% (68/135)of physicians did not completely understand mechanical ventilation .For therapy of stable COPD ,57 .7% (78/135) of physicians recommended inhaled glucocorticosteroids combined with beta‐2‐receptor agonist .The proportion of physicians who recommended inhaled glucocorticosteroids combined with beta‐2‐receptor agonist in respiratory professional group was 88 .8% (40/45) ,which was significantly higher than that 42 .2% (38/90) in non‐respiratory professional group (P<0 .05) .Non‐respiratory professional group were lack of knowledge of home oxygen therapy ,respiratory exercises , and nutritional support .Only 11 .1% (10/90) physicians in the non‐respiratory professional group understood the knowledge comprehensively .Conclusions:It is key to enhance the education of COPD and popularize the treatment guideline among the physicians ,especially non‐respiratory physicians ,in South Henan area .
