1.Performance verification of portable Celercare M1 analyzer in CRP determination

Objective To verify the analytical performance of the portable Celercare M1 analyzer in the C-reactive protein(CRP) quantitative determination.Methods According to the standard program of the American Clinical and Laboratory Standards Insti-tute (CLSI),The portable Celercare M1 analyzer was performed the verification on the CRP detection in the aspects of the linearity range,precision,accuracy,interference test and method comparison.Results The CRP detection showed good linearity in the range of 0.5-200 mg/L and the regression equation was Y =0.979X +0.456.The within-run coefficients of variation(CV)of samples with low and high CRP levels were 5.9% and 2.0%,respectively.The between-day CV of samples with low and high CRP levels were 6.3% and 2.2%,respectively.The accuracy of the assay was admirable,and bias of CRP results at low and high levels were 2.3% and 0.7%,respectively.No significance interference was observed when serum bilirubin ≤340 μmol/L,triglyceride ≤10 mmol/L and hemoglobin ≤8 g/L in this method.The regression equation of Celercare M1 analyzer(Y )and the IMMAGE 800 (X) for detecting CRP was Y =0.944X +0.206,R 2 =0.996(P <0.05).The relative bias of CRP results in three medical decision levels (3,10,100 mg/L)was 1 .0%,4.0% and 5.0%,repectively,which was lower than 1/2 of total error allowance.Conclusion The linearity,precision,accuracy and interference of the Celercare M1 analyzer in CRP determination are admirable and can be accepted in clinical practice.
2.Comparative analysis of common biochemical assay results for venous blood and peripheral blood

Objective To investigate the comparability of the results of common biochemical items detected by Celercare M1 an‐alyzer in the peripheral blood and the venous whole blood .Methods The samples of peripheral blood and venous whole blood were collected from subjects .The biochemical items including Mg2+ ,Cl- ,tCO2 ,K+ ,Na+ ,Ca2+ ,α‐HBDH ,LDH ,AST ,CK ,CK‐MB ,TP , ALB ,TBIL ,ALT ,GGT ,ALP ,UREA ,GLU ,UA ,CHOL ,and HDL‐C were determined by Celercare M1 analyzer ,and the results were compared .Results The tCO2 results of venous blood was significantly higher than that of peripheral blood (P<0 .05) .How‐ever ,the results of α‐HBDH ,LDH ,CK and CK‐MB of venous blood samples were significantly lower than those of peripheral blood samples ,and the difference was statistically significant (P<0 .05) .Conclusion The peripheral blood can replace venous blood for biochemical analysis on Celercare M1 analyzer ,except for the electrolyte test items and cardiac enzyme items such as α‐HBDH , LDH ,CK and CK‐MB .
3.Identification of two newly discovered subgenotypes of hepatitis B virus genotype C

Objective To analyze the genotypic characteristics of hepatitis B virus (HBV)C/D recombinant of two types of newly discovered HBV genotypea found in Western China.Methods The whole genomes of 17 HBV strains isolated from Western China were amplified by polymerase chain reaction (PCR). Bioinformatic softwares were used for the analysis of full genome structure,genetic distances and recombination points.Results The heterogenicity of the HBV C/D recombinant was more than 8% compared with genotype A,B,D,E or F,but 3.8% 0A-8.O% compared with genotype C Based on phylogenetie analysis, a11 C/D recombinant strains clustered within genotype C.but were rouped into two other clusters within the genotype C independently from C1-C5 subgenotypes,which were two kinds of new HBV/C genotypea.Condusion The HBV C/D recombinant could he considered as tWO kinds of new subgenotypea of HBV genotype C which are different from subgenotype C1-C5 based on the genetic distances analysis.
4.The influence of social support and family function in elderly preoperative patients psychological situation

Objective To understand family function ,social support ,preoperative anxiety and depression status among elder‐ly operative patients ,and to explore their relationship .Methods 295 elderly preoperative patients were investigated by family AP‐GAR index (APGAR) ,social support rating scale (SSRS) ,self‐rating anxiety scale (SAS) and self‐rating depression scale(SDS) , descriptive and inferential analysis .Results Among 295 elderly preoperative patients ,there were 208 people with good family func‐tion ,accounting for 70 .51% ,67 people with moderate disability family function ,accounting for 22 .71% ,and 20 people with severe disability family function ,accounting for 6 .78% ;The total score of social support was 40 .42 ± 9 .03 and the scores of subjective support was 23 .32 ± 5 .42 ,objective support with 9 .45 ± 3 .73 and support availability with 7 .65 ± 1 .86 ;152 people were preoper‐ative anxiety ,accounting for 51 .53% ,98 people were preoperative depression ,accounting for 33 .22% .The rate of anxiety and de‐pression good family function group was lower than a family dysfunction group(P<0 .05) .The incidence of depression high social support group is higher than low group (P<0 .05);Among the scores of social support ,subjective support ,objective support and support availability ,we find that patients with family well‐functioning group was higher than patients with family dysfunction group (P<0 .05) .Multiple logistic regression analysis showed that influence factors of preoperative psychological abnormality had educa‐tion level ,whether the tumor diseases ,the total score of social support and family functioning condition .Conclusion Elderly preop‐erative patients have anxiety and depression to a certain extent .The anxiety and depression of patients were related to the family function and social support .We Should pay attention to the effect of farmily function and social support on mental health of elderly patients with operation ,and effective use .
5.Association between rs689466 in COX-2 gene and aspirin resistance in cerebral infarction patients

Objective To investigate the association of single nucleotide polymorphisms (SNPs) in COX-2 with aspirin resistance in Chinese cerebral infarction patients. Methods A total of 150 Chinese cerebral infarction patients were recruited. Platelet aggregation response was measured by light transmission aggregometry method and four SNPs located in COX2 gene were genotyped by sequencing method. Results Sixty patients of the total were classified as aspirin non-responders. For clinical variables , concentrations of high homocysteine and the frequency of recurrence cerebral infarction were significantly higher in aspirin non-responders when compared with aspirin responders. Univariate analysis of SNPs showed that rs20417 , rs689465 and rs689466 were significantly associated with aspirin resistance. Multivariate analysis indicated that after adjusting other SNPs and clinical risk factors, rs20417 and rs689466 were still significantly associated with aspirin resistance. Conclusions Rs689466 is significantly associated with aspirin resistance in Chinese cerebral infarction patients even after the adjustment of rs20417. By combining rs689466 , rs20417 and other clinical risk factors , we may better classify the aspirin non-responders from aspirin responders.
6.The clinical application of endoscopic thyroidectomy through two-areola axillary approach

ObjectiveTo discuss the more aesthetic and minimally invasive surgical approach of endoscopic thyroidectomy.Methods50 cases of endoscopic thyroidectomy via breast areola-axillary approach were retrospectively analyzed.Among these cases,12 received unilateral partial lobectomy,30 received unilateral total lobectomy and 8 received bilateral partial lobectomy.Results Endoscopic thyroidectomy was completed in all the patients.The intraoperative blood loss was ( 12.77 ± 20.12 ) ml.The operation time was (67.00 ± 21.28 )min.The postoperative hospital stay was (3.77 ± 0.50)days.Hoarseness occurred in 2 cases.All the 50 cases were followed up from 1 to 6 months and no other complication occurred.ConclusionEndoscopic thyroidectomy via breast areola-axillary approach is more aesthetic and minimally invasive; however,the operator needs an adaptive process for a better surgical view angle.
7.Clinical efficacy and safety of percutaneous microwave ablation guided by multimodal ultrasound in the treatment of benign thyroid nodules

Objective To analyze the clinical efficacy and safety of percutaneous microwave ablation guided by multimode ultrasound in the treatment of benign thyroid nodules.Methods The clinical data of 100 cases of benign thyroid nodules admitted from June 2017 to December 2018 were retrospectively analyzed.50 cases of benign thyroid nodules were treated by traditional open surgery as the control group,and 50 cases of benign thyroid nodules were treated by ultrasound-guided microwave ablation as the observation group.The complete clearance rate of lesion tissue and the incidence of comphcations were compared between the two groups.Results (1) The nodule clearance rate of the observation group was higher than that of the control group,with significant statistically difference (P < 0.05).Compared with the control group (transient thyroid dysfunction 16%,recurrent laryngeal nerve injury 10%),the incidence of complications (transient hypothyroidism 2%,recurrent laryngeal nerve injury 0%) was significantly lower in the observation group (P < 0.05).(2) The quality of life scores of the observation group and the control group were significantly higher than those before the treatment (P < 0.05);The quality of life score of the observation group was significantly higher than that of the control group (P < 0.05).Conclusions Compared with traditional open thyroidectomy,multimodal ultrasound-guided percutaneous microwave ablation has better efficacy and safety,which is worthy of wide application and promotion.
8.Hepatitis B virus replication and viral gene expressions do not affect CDC37 level in hepatocytes in vitro.

OBJECTIVETo study the influence of hepatitis B virus (HBV) replication and expressions of different viral genes on CDC37 level in hepatocytes.
METHODSWe amplified and cloned 6 HBV genes (P, preS1, preS2, S, C and X) into pCMV expression vectors, which were transfected in Huh7 and HepG2 hepatoma cell lines, and CDC37 expression level in the cells was detected using Western blotting. Wealso cloned the promoter sequence of CDC37 into pGL3 vector, and co-transfected pGL3 with pCMV recombinant plasmids into Huh7 and HepG2 cells and the fluorescent signals were detected. To study the influence of HBV replication on CDC37 expression, we constructed 1.28-copy overlength genomes of HBV genotypes B, C, D and CD recombinant. The overlength HBV genomes were transformed into Adeasier-1 cells for recombination and into 293 cells for packaging. Huh7 and HepG2 cell lines infected with the packaged HBV recombinant adenoviruses were examined for CDC37 expression with Western blotting.
RESULTSWestern blotting showed that the expression of different HBV genes did not obviously affect the protein level of CDC37 in the hepatocytes. The protein expression of HBV genes had no effect on the activity of CDC37 promoter. Huh7 and HepG2 cells infected with 1.28-copy HBV replicon showed no significant changes in the expression level of CDC37.
CONCLUSIONHBV replication and its gene expression have no effect on the level of CDC37 in hepatocytes in vitro.

9.Mutation analysis of NTRK1 gene in a family affected with congenital insensitivity to pain with anhidrosis.

OBJECTIVETo screen for mutations of NTRK1 gene in a Chinese family affected with congenital insensitivity to pain with anhidrosis (CIPA).
METHODSGenomic DNA was extracted from the proband and her family members. All of the 17 exons and intron-exon boundaries of the NTRK1 gene were analyzed by direct Sanger sequencing. For the deletional mutation, the PCR products were subjected to T-A cloning and sequencing to verify the mutation.
RESULTSNTRK1 gene analysis revealed that proband has carried a c.1786C>T (p.Arg596*) nonsense mutation inherited from her mother and a novel deletional mutation c.1928-2028+23del from her father. Her elder brother only carried the deletional mutation.
CONCLUSIONThe diagnosis of CIPA relied on typical clinical symptoms of no pain, anhidrosis and intellectual disability and detection of the biallelic NTRK1 mutations. The novel deletional mutation has enriched the spectrum of NTRK1 mutations.

10.Analysis of stylized operation step in the neck endoscopic thyroidectomy for 51 cases of papillary thyroid microcarcinoma

Objective:To summarize experience and effect of the stylized operation step in the neck endoscopic thyroidectomy for papillary thyroid microcarcinoma (PTMC).Methods:The clinical data of 51 patients who underwent transthoracic breast approach scarless neck endoscopic thyroidectomy (SET) from January 2018 to September 2019 in Tungwah Hospital Affiliated to Sun Yat-sen University were retrospectively analyzed. The operative procedure used stylized operation steps: establishment of operating space, three-step method thyroidectomy, central lymph node dissection (CLND), close operating space.Results:Among 51 patients, 25 cases underwent unilateral thyroidectomy plus isthmic resection, 26 cases underwent total/near-total thyroidectomy, and 16 cases underwent preventive CLND. The recurrent laryngeal nerve (RLN) was exposed in all 51 cases, the external branches of the superior laryngeal nerve (EBSLN) was exposed in 35 cases, no procedure was converted into open neck incision thyroidectomy. Forty-two cases were followed up, 2 cases had postoperative hoarseness and unilateral recurrent laryngeal nerve paralysis, which were recovered 55 and 75 d after operation respectively. There was no bucking, voice blunt or permanent low calcium. One patient had abnormal sensation of neck skin after operation, and recovered gradually 3 months after operation.Conclusions:The stylized operation step in the SET can increase safety of surgery.
