Objectives:To research the relation of high blood glucose,GCS and prognosis after craniocerebal injury. Methods:63 patients were divided into three groups,“GCS” 3~8,“GCS” 9~12,and GCS 13~15.Blood glucose of these groups were tested at admission of all patients and one week after hosptialization. Results:The level of blood glucose and the mortality in GCS 3~8 group were significantly high than those in the other two groups. Conclusions:After cranidcerebral injury,the blood glucose is an index of the injury degree.We should try to control the blood glucose in normal levels to improve the prognosis of patients with craniocerebral injury.

Chronic atrophic gastritis (CAG) is a common and refractory disease of the digestive system in clinic.Apoptosis is the important reason which relates CAG pathological changes.As one of the important means of clinical prevention and treatment of digestive diseases, traditional Chinese medicine (TCM) in the role and mechanism of CAG related research has become an important direction in recent years.Based on the collection of related literatures and data, the paper analyzed the possible mechanism of CAG pathogenesis grasped the key aspect of apoptosis and clarified the research status of TCM on the regulation of gastric mucosal cell apoptosis in CAG in recent years through focusing on the specific signaling pathways in mitochondrial pathway, death receptor pathway, endoplasmic reticulum pathway.Through the above analysis, the paper provided some ideas for further systematic and in-depth research and explored CAG effective TCM program.
4.Binary cardiac rehabilitation in patients with acute myocardial infarction and the measurement of 6-minute Walk Test

Objective To study the effects of binary cardiac rehabilitation (CR) composed of hospital-based and home-based CR in patients after acute myocardial infarction (AMI) evaluated by six-minute walking distance (6MWD). Methods A total of 38 post-MI patients were divided into two groups, namely conventional CR group (n=20) and binary CR group (n=18). In the first seven days, CR training was carried out in all 38 patients in our hospital, and then 20 patients were kept at CR training in the hospital (hospital-based CR group), whereas the remaining 18 patients (binary CR group) got tele-monitored walking training at home for eight weeks (binary model). Before discharge from hospital based CR or after 8 weeks CR at home, all patients received Cardiopulmonary Exercise Testing and Six-minute Walk Test, and the data of patients' body mass index (BMI),thyroid function, serum lipid metabolism and echocardiography were collected before and after CR. Results After CR, BMI and low density lipoprotein (LDL) decrease in both groups compared with those before CR[hospital-based group: BMI, (25.99±3.36)kg/m2 vs. (23.04±3.72) kg/m2,P<0.05; LDL, (3.40±1.38) mg/dl vs. (2.04±0.73)mg/ dl,P<0.01; binary group: BMI, (24.84±2.70) kg/m2 vs. (22.88±2.56) kg/m2,P<0.05; LDL, (3.40±1.01) mg/dl vs. (92.11±0.37) mg/dl,P<0.01]. After 8 weeks CR at home or hospital based CR until discharge, the anaerobic threshold (AT), maximum rate of oxygen consumption (VO2max),metabolic equivalent (MET) and 6MWD improved significantly in both groups compared with those at 7 days after CR in hospital [Hospital-based group: AT, (12.37±1.53) mL/(kg. min) vs. (14.77±1.57) mL/(kg. min); VO2max (17.87± 1.66 mL/(kg. min) vs. (20.73±2.14) mL/(kg. min); MET (5.02±0.36) vs. (6.09±0.53); 6MWD (500±53.36) m vs. (582.5±57.6) m,P<0.01; Binary group: AT, (12.56±1.11) mL/(kg·min) vs. (14.30±1.23) mL/(kg. min); V02max, (17.28±1.38) mL/(kg. min) vs. (20.02±1.37) mL/(kg. min); MET, (5.07±0.47)vs (5.94±0.46); 6MWD,(511.4±50.96) m vs. (590.3±56.1)m,P<0.01]. There was no significant difference in CR effects observed between two groups (P>0.05). Conclusions In post-MI patients,a binary model of CR training improved physical capacity and was a similarly effective form of CR as a entirely hospital-based approach. A home-based tele-monitored program facilitated patients' adherence to CR. The 6 minute walk experiment is economical and good evaluation on the CR effect of binary cardiac rehabilitation.
5. Assessment of pulmonary infectious disease treatment with Mongolian medicine formulae based on data mining, network pharmacology and molecular docking

Objective: Pulmonary infectious diseases (PID) include viral pneumonia (VP) and pulmonary tuberculosis (PT). Mongolian medicine (MM) is an effective treatment option in China, however, the core group medicines (CGMs) in the treatment of PID and their underlying therapeutic mechanisms remain unclear. In this study, through the method of data mining, the CGMs of MM for the treatment of PID were excavated, and the possible mechanism of action of the CGMs in the treatment of PID was explored by using network pharmacology. Methods: First, 89 MM formulae for the treatment of pulmonary infectious diseases collected from Gan Lu Si Bu, Meng Yi Jin Kui, People's Republic of China Ministry of Health Drug Standards (Mongolian Medicine Volume), Standard of Mongolian Medicine Preparations in Inner Mongolia (2007 Edition), and Standard of Mongolian Medicine Preparations in Inner Mongolia (2014 Edition). The CGMs of MM for PID were excavated through association rule analysis and cluster analysis. Then, the active ingredients and potential targets of the CGMs were obtained from TCMSP, TCMIP, BATMAN-TCM databases. PID targets information was collected from OMIM, GeneCards, and DrugBank databases. The possible targets of CGMs treatment for PID were obtained by intersection. The PPI network was constructed through the STRING database, and the topology analysis of the network was performed. Through the enrichment analysis of the intersection targets by R language, the main action pathways and related target proteins of CGMs in the treatment of PID were screened out. The results were verified by molecular docking. Results: A total of 89 formulae were included, involving 164 MM herbs. The efficacy of the drugs was mainly cough-suppressing and panting-calming herbs, and heat-clearing herbs. The nature and flavor were mainly bitter and cold. The CGMs of MM to treatment of PID was excavated as the classic famous formula Sanzi Decoction (Toosendan Fructus-Chebulae Fructus-Gardeniae Fructus). A total of 28 candidate components and 237 predicted targets of CGMs were collected, and 61 common targets with PID were obtained, including key compounds such as quercetin, kaempferol, β-sitosterol and stigmastero and key targets such as VEGFA, IL6, TP53, AKT1. KEGG enrichment analysis yielded AGE-RAGE signaling pathways, IL-17 signaling pathways, and TNF signaling pathways. Molecular docking results showed that the key targets were well matched with the potential active ingredients of CGMs. Conclusion: This study found that MM commonly used cough-suppressing and panting-calming herbs in combination with heat-clearing herbs to treat PID, and the CGMs for the treatment of PID is “Toosendan Fructus-Chebulae Fructus-Gardeniae Fructus”. CGMs mainly play a role in the treatment of PID by acting on VEGFA, IL6, TP53, AKT1 and other targets, regulating AGE-RAGE signaling pathways, IL-17 signaling pathways, and TNF signaling pathways.
6.Rapid Health Technology Assessment of Tirofiban in the Treatment of Acute Coronary Syndrome

OBJECTIVE:To evaluate the benefit and risk of tirofiban in the treatment of acute coronary syndrome (ACS),and to provide evidence-based reference for clinical drug selection and decision. METHODS :Retrieved from domestic and foreign database as PubMed ,the Cochrane Library ,CNKI and Wanfang database ,during the establishment of database to Apr. 2020,two researcher independently screened the literature based on inclusion and exclusion criteria and extracted the data. After the quality evaluation of the included literatures ,based on rapid health technology assessment ,the extracted results were classifiedly evaluated and comprehensively analyzed. RESULTS :A total of 13 researches of systematic review/Meta-analysis and 1 research of pharmacoeconomics were included. Compared with placebo ,tirofiban could significantly reduce all-cause mortality [OR =0.68, 95%CI(0.54,0.86),P=0.000 1] and the incidence of major adverse cardiac events (MACE)in patients with ACS [RR =0.24, 95%CI(0.14,0.40),P<0.01],and increased the incidence of TIMI 3 [OR=5.73,95%CI(2.99.10.97),P<0.01]. Tirofiban and eptifibatide had similar therapeutic efficacy in the treatment of ACS ,but tirofiban significantly increased the risk of TIMI small bleeding in patients with ACS [RR =0.61,95%CI(0.38,0.98),P=0.04]. For ACS patients with non-ST elevation (NSTE-ACS), compared with placbo ,tirofiban significantly reduced the incidence of MACE [RR =0.76,95% CI(0.61,0.96),P=0.018],but significantly increased the risk of bleeding [OR =1.49,95%CI(1.12,1.98),P=0.006],while there was no significant difference in its effects on the all-cause mortality of NSTE-ACS patients (P>0.05). For STEMI patients ,compared with placebo ,tirofiban significantly reduced the all-cause mortality [RR=0.61,95%CI(0.35,1.05),P=0.007] and the incidence of MACE [RR =0.63,95% CI(0.44,0.90),P=0.007]. When combined with liposuction ,tirofiban also significantly reduced the incidence of MACE [RR = 2.05,95%CI(1.71,2.46),P<0.01],and significantly increased the incidence of TIMI 3 [OR=3.18,95% CI(2.4,4.22),P< 0.01],but there was no significant difference in its effects on bleeding risk (P>0.05). The included pharmacoeconomic study showed that patients treated with bivalutine could get 10.07 QALYs,patients treated with heparin combined with tirofiban could get 9.98 QALYs,and the incremental cost-effectiveness ratio bivalutine compared to the latter one was 28 575.77 yuan/QALYs,which was lower than 3 times of the per capita GDP of some cities. CONCLUSIONS :Tirofiban has good efficacy in the treatment of ACS,but it can increase the risk of bleeding than eptifibatide and placebo. Domestic bivalirudin treating for ACS has a cost-effectiveness advantage over tirofiban combined with heparin.
