1.Study on relationship between helicobacter pylori infections and homocysteine

Objective To explore the relationship between helicobacter pylori infections and homocysteine (Hcy) in health checkup population .Methods A total of 930 health checkup people in our hospital physical examination center during December 2012 to September 2013 were selected as the research object and were divided into Hp positive group (465 cases) and the negative group (465 cases) according to the Hp infection by the immune colloidal gold method .The level of Hcy was measured in two groups by fluorescence polarization immunoassay method .The relationship between helicobacter pylori infections and homocysteine was analyzed .Results The abnormal rate of high serum level of Hcy in Hp positive group was 68 .39% ,While 44 .30% in negative group ,the difference was statistically significant (P<0 .05);the abnormal rate of male was 88 .25% ,female 18 .80% ;the abnormal rate of male was 59 .76% ,and that of female was 5 .30% ,the difference of two groups had statistical significance (P<0 .05 ,P<0 .01) .Conclusion Hp infection may be a risk factor of high Hcy levels ,and taking measures actively to prevent and control Hp in‐fection may reduce the of the cardiovascular and cerebrovascular diseases .
2.Use of cystatin C in the evaluation of post-transplant renal allograft function

Objective To study the correlation between the post-transplant renal allograft function and the variation of serum cystatin C (CyC) concentration in renal allograft recipients. Methods One hundred and ninety-three renal allograft recipients accepted the same combination immunosuppressive regimen of tacrolimus, mycophenolate and prednisone were enrolled into the study. Patient's serum and urine samples were collected on day 5 post-transplant to detect serum cystatin C, serum and urine creatinine (SCr). Correlation analysis was used to analyze correlation between CyC concentration and SCr concentration or the calculated creatinine clearance rate (CkCCr) by using the Cockcroft-Gault equation and urine creatinine clearance rate (CCr). Specificity and sensitivity of using the CyC concentration to evaluate glomerular filtration rate (GFR) were calculated as well.Results The mean concentrations of serum CyC and SCr on day 5 post-transplant were (1.91±1.2)mg/L and (174.0±129.1)μmol/L, respectively. While the CCr and CkCCr were (67.9±27.3)ml/min and (68.1±27.8)ml/min, respectively. Forty-two patients had a CyC concentration below 1.25 mg/L, 102 patients'CyC concentrations were between 1.25 and 2.0 mg/L and 49 patients'CyC concentrations were above 2. 0 mg/L. As for SCr, 62 patients had a concentration below 125 μmol/L, 83 patients'concentrations were between 125 and 200 μmol/L and 48 patients'concentrations were above 200 μmol/L. For CkCCr, there were 52 cases with a concentration above 80 ml/min, 96 cases with a concentration between 80 and 60 ml/min and 45 cases with a concentration below 60 ml/min. Serum CyC concentration had a negative correlation with CkCCr (r=-0. 907, P<0. 001) and had a significantly positive correlation with SCr concentration (r=0. 886, P<0. 001). SCr had a significantly negative relationship with CkCCr (r=-0. 889 ,P<0. 001). Serum CyC had higher correlation with CkCCr than the correlation between SCr and CkCCr. The ROC curves showed that areas under curve of CyC, SCr, CCr and CkCCr were 0. 877, 0. 771, 0. 832 and 0. 909, respectively. Specificity and sensitivity of CyC, SCr,CCr and CkCCr were 69.3%, 96.1%, 77.1%, 71.3%and 91.6%, 52.2%, 67.5%, 84.6%, respectively.Conclusions Serum CyC concentration elevates earlier than SCr concentration when there is slight renal function impairment. Serum CyC concentration might become a more sensitive marker to evaluate the post-transplant renal allograft function in renal transplant recipients.
3.Expression pattern of microRNA-379 during the embryo heart development and differentiation of P19 cells

Objective To observe the expression changes in microRNA (miR)-379 in the developmental process of the mouse heart and during the differentiation of P19 cells into cardiac myocytes,and to explore the possible relationship between miR-379 and the differentiation of cardiacmyocytes.Methods Heart tissues were collected from fetal mice in pregnant ones at their gestational age (8.5,11.5,14.5 and 18.5 days) respectively.Heart tissue sections of the fatal mice were obtained to observe the heart development process.Then total RNA was isolated from heart tissues by using the TRIzol method.Complementary DNA was synthesized from 1 μg total RNA by using a Reverse Transcriptase Kit.Finally,real-time PCR (RT-PCR) was employed to detect the expression of miR-379.At the same time,P19 cells were cultured with 10 mL/L Dimethyl sulfoxide in suspension for 4 days to form cell aggregation,and these aggregations were transferred into 6-wells plate for culturing by adherence.Beating cells were detected with microscopy on the 10th day after induction.Afterwards,total RNA was extracted from cultured P19 cells at different time points.Reverse transcription was executed to get DNA.At last,RT-PCR was used to explore the expression of miR-379 on 0,4,6,10 days after aggregation.Results The expression level of miR-379 was down-regulated gradually in the developing heart (at gestational age of 8.5,11.5,14.5,16.5 days,respectively),and there were significant differences on the different days (F =21.13,P < 0.05).On the other hand,myocardial markers of troponin T represented an increasing trend during the process of P19 cells induction,which demonstrated that P19 cells were successfully induced into cardiomyocyte-like cells.Meanwhile,miR-379 showed a low expression on day 0 of P19 cells aggregation.On day 4,miR-379 demonstrated a higher level.Afterwards,miR-379 proved to be down-regulated gradually.Conclusions miR-379 plays a role in the process of the heart development,but the specific mechanisms need further research.
4.Distally based perforator-plus sural fasciocutaneous flap for repair of lower leg soft-tissue defects in pediatrics and adults

Objective To compare the effect of distally based perforator-plus sural fasciocutaneous flap for soft tissue reconstruction of the lower leg in pediatric and adult populations.Methods Data of 165 patients treated with distally based perforator-plus sural fasciocutaneous flap between April 2001 and February 2011 were retrospectively reviewed.There were 125 males and 40 females,at age range of 3-78 years.The patients were divided into pediatric group (< 14 years,n =50) and adult group (≥ 17 years,n =115) according to the age.The two groups were compared in terms of flap-viability-related complications (partial necrosis,marginal necrosis and overall complication rate) and their potential risk factors (sex,location of defect,causes of defect,pivot point site and top-edge location).For the patients followed at least 12 months,the reconstruction outcomes,donor-site morbidities(hypertrophic scar,pruritus,pigmentation,numbness of skin graft area and paresthesia of skin graft area),and swelling of the limb were compared between the groups.Results All patients were followed up for 2 weeks to 72 months.Partial necrosis of the flap,marginal necrosis of the flap,and overall complications in pediatric group accounted for 14%,4% and 18% respectively,higher than 11.3%,1.7% and 13.0% in adult group,but the differences were insignificant (P > 0.05).Sex ratio,pivot point site and top-edge location were not significantly different between the groups (P > 0.05).Pediatric group versus adult group presented a significantly lower proportion of pretibial defects,but significantly higher proportions of the defects over the achilles tendon and heel (P < 0.05).A significantly higher proportion of spoke injury and lower proportions of tamp injury and sports injury were observed in pediatric group than in adult group (P <0.05).Reconstruction outcomes were not significantly different between the groups (P > 0.05).Pediatric group versus adult group presented significantly higher incidences of hypertrophic scar and pruritus at the donor site,but a significantly lower incidence of limb swelling (P < 0.05).Conclusion Reliability and repair effect of the distally based perforator-plus sural fasciocutaneous flap are similar between pediatrics and adults,while the pediatrics are more likely to have hypertrophic scar and pruritus at the flap donor site.
5.Cardiopulmonary resuscitation in the delivery room for very/extremely low birth weight infants: a systematic review and meta-analysis

Objective:To evaluate the outcomes of cardiopulmonary resuscitation in the delivery room (DR-CPR) at birth for very/extremely low birth weight infants (VLBWI/ELBWI).Method:PubMed, Embase, Cochrane Library, CNKI, VIP database and Wanfang database were searched. The search time limit is from the establishment of the database to October 26, 2020. Search and screen all the literature on the short-term and long-term outcomes of VLBWI/ELBWI who require DR-CPR and conduct quality evaluations. Review Manager 5.3 software was used to perform the Meta analysis. Egger's test in Stata Software 15.0 was used to draw a funnel plot and validate publication bias.Result:A total of 16 studies were included, all in English. 5 661 VLBWI/ELBWI received DR-CPR, and 73 438 VLBWI/ELBWI did not receive DR-CPR. The Meta analysis showed: DR-CPR for VLBWI/ELBWI was associated with an increased risk of mortality ( RR=2.30, 95% CI 1.89~2.82, P<0.05), grade 3 or 4 intraventricular hemorrhage (IVH) or periventricular leukomalacia (PVL) ( RR=1.92, 95% CI 1.56~2.36, P<0.05),bronchopulmonary dysplasia (BPD) ( RR=1.18,95% CI 1.04~1.33, P<0.05), neurodevelopmental impairment (NDI) ( RR=1.25, 95% CI 1.14~1.38, P<0.05). However, it did not increase the risk of retinopathy of prematurity (>grade 2)( RR=1.31, 95% CI 0.96~1.79, P=0.09). The ELBWI was analyzed in subgroups, and the results were consistent with the overall results. Conclusion:CPR at birth for VLBWI/ELBWI was associated with higher risk of mortality, IVH (grade 3 or 4) or PVL, BPD, NDI.

The "student-centered" concept highlights the dominant position of students in the teaching process and makes up for the deficiency of traditional teaching mode.Based on the above idea,the teaching team of the author's department tried to improve teaching effect through effective teaching design in biochemistry.Firstly,the teaching goal (the goal of knowledge,ability and emotion) and the important situational factors the learning situation analysis,the characteristics of the course and the class,etc)should be identified.suitable teaching strategies he inspiration strategy,exploration strategy and problem solution strategy,etc.),teaching method (discussion,case study,problem based learning,etc.) and proper assessment (such as the summative assessment with formative evaluation) should be determined to organize classroom teaching.At the end of the class the timely feedback and the teaching reflection should be carried outto extend classroom teaching to extra-curricular class.The results showed that effective teaching designismore likely to stimulate students' learning enthusiasm,promote students' deep learning and achieve better teaching effects.
7.Post-marketing immunogenicity and safety of domestic 23-valent pneumococcal polysaccharide vaccine: a multicenter study

Objective:To evaluate the post-marketing safety and immunogenicity of a 23-valent pneumococcal polysaccharide vaccine (PPV23).Methods:From September 2020 to June 2021, a clinical trial of single-dose PPV23 was conducted in people ≥3 years old in Centers for Disease Control and Prevention of Guizhou, Hunan and Fujian provinces. Blood samples were collects from the subjects before and 30 d after vaccination. ELISA was used to quantitatively detect IgG antibodies against capsular polysaccharides of 23 Streptococcus pneumoniae serotypes in serum samples. The adverse events (AEs) were monitored within 7 d after vaccination. Results:A total of 409 subjects were enrolled and included in safety analysis. Except for one with antibody level inversion, the other 408 participants were included in immunogenicity analysis. The levels of antibodies against the 23 Streptococcus pneumoniae serotypes were all increased after vaccination by an average of 4.24 folds. The two-fold growth rates of the antibodies ranged from 51.72% to 96.81% with a total two-fold growth rate of 78.59%. The overall rate of AEs was 27.14% (111/409). Local AEs were mainly pain, induration, redness and swollen. No serious adverse events related to vaccination occurred. Conclusions:This study preliminarily demonstrated the good immunogenicity and safety of PPV23 vaccine.
8.Laparoscopic and endoscopic cooperative surgery with supine position under general anesthesia in the operation of type Ⅰ Mirizzi syndrome with choledocholithiasis

Objective:To explore the value of laparoscopic and endoscopic cooperative surgery with the patient lying on supine position under general anesthesia in the operation of type I Mirizzi syndrome with choledocholithiasis.Methods:From Jan 2018 to Jan 2020, 53 cases of Mirizzi syndrome with choledocholithiasis undergoing laparoscopic and endoscopic cooperative surgery (preLC+ ERCP+ EST) at the Second Affiliated Hospital of Kunming Medical University were retrospectively analyzed.Results:53 patients successfully underwent LC without conversion to open surgery, and 2 patients failed in ERCP + EST attempt, with a success rate of 96.2%. One patient developed pancreas pseudocyst as a result of post-operative hemorrhagic necrotizing pancreatitis. Two patients suffered from chronic pancreatitis. Three patients complaining postoperative upper abdominal discomfort were finally diagnosed as stump cystic duct inflammation by MRCP, and no abnormalities were found in the follow-up of the remaining cases.Conclusion:Laparoscopic and endoscopic cooperative surgery in the treatment of patients with type I Mirizzi syndrome combined with choledocholithiasis is minimally invasive and effective.
9.Mechanism of Zhizi Prescription in Protection of CCl4-induced Acute and Subacute Liver Injury in Mice

ObjectiveTo investigate the protective effect of Zhizi prescription (ZZP) on carbon tetrachloride (CCl4)-induced acute and subacute liver injury and its mechanism. MethodAcute and subacute liver injury animal models were induced. C57 mice were randomly divided into a normal group, model group, obeccholic acid group, ZZP high-dose (0.5 g·kg-1) group, and ZZP low-dose (0.25 g·kg-1) group. According to the experiment design, the serum and liver tissue of mice were collected after the last administration. Hematoxylin-eosin (HE) and Sirius staining was used to observe the liver pathological changes. Serum alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin (TBIL), liver homogenate hydroxyproline (Hyp), malondialdehyde (MDA), and superoxide dismutase (SOD) levels were determined by kit. The levels of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) in the liver tissue were determined by enzyme-linked immunosorbent assay (ELISA). Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) was used to detect the mRNA expressions of collagen 1A1 (Col1a1), collagen 3A1 (Col3a1), fibronectin (FN), transforming growth factor β receptor Ⅱ (Tgfbr2) and α-smooth muscle actin (α-SMA) in the liver tissue. ResultIn terms of the acute liver injury, as compared with the normal group, the levels of ALT, AST, TBIL and MDA in the model group were significantly increased (P<0.01), while the activity of liver SOD was significantly decreased (P<0.01). Compared with model group, the ZZP high-dose and low-dose groups both significantly reduced the degree of liver cell injury, and protected the acute liver injury induced by CCl4. The ZZP high-dose group had a better effect than the ZZP low-dose group. In terms of the subacute liver injury, the levels of ALT, AST, MDA,TNF-α and IL-6 in the model group were significantly increased (P<0.01), while the activity of liver SOD was significantly decreased (P<0.01). As compared with the model group, liver Hyp content in the ZZP high-dose and low-dose groups was significantly decreased (P<0.01), and the collagen deposition in liver of both groups was significantly reduced. The ZZP high-dose group also significantly down-regulated the mRNA expressions of α-SMA, Col1a1, Col3a1, FN, and Tgfbr2 in the liver of mice (P<0.05, P<0.01). ConclusionZZP effectively protects the acute and subacute liver injury induced by CCl4, and the protective effect is proportional to its concentration. The mechanism may be related to the increase of the activity of antioxidant enzymes in the liver tissue, the decrease of the level of lipid peroxidation, and the inhibition of inflammatory response, thus reducing collagen deposition and improving early liver fibrosis.
