1.Effect of Acupuncture on the Quality of Life in Patients with Irritable Bowel Syndrome Discussed Based on Brain-gut Interaction

ObjectiveTo testify the therapeutic efficacy of acupuncture in treating diarrhea-predominant irritable bowel syndrome (D-IBS) based on brain-gutinteraction.MethodEighty D-IBS patients were randomized into brain-gut interacted acupuncture group (treatment group) and a group with acupoints selected based on conventional syndrome differentiation (control group), 40 in each group. The treatment group was intervened by acupuncture by selecting the thirteen ghost acupoints (invented by SUN Si-miao) plus Tianshu (ST25), Dachangshu (BL25), etc., while the control group was by acupuncture based on conventional point selection, including Zusanli (ST36),Tianshu (ST25), Dachangshu (BL25), Ganshu (BL18), Pishu (BL20), etc., three times a week, and 10 sessions as a treatment course. The clinical efficacies were evaluated after 2 treatment courses.ResultThe two groups were generally equal to each other incomparing each dimension of the Short-form Health Survey (SF-36) before treatment, while the scores were increased after treatment in both groups; the treatment group was significantly superior to the controlgroup in comparing the scores ofrole-physical,bodily pain, general health, vitality, social role functioning, and reported health transition from the SF-36 (P<0.05).ConclusionAcupuncture based on brain-gut interaction can produce a more significant effect in treating D-IBS compared to conventional acupuncture.
2.THE TREATMENT OF TESTICLE EMBRYONAL RHABDOMYOSARCOMA

PURPOSE 10 cases of testicle embryonal rhabdomyosarcoma (RMS) were treated in the Cancer Hospital of Shanghai Medical University from 1971 to 1988. All cases were treated by orchiectomy followed by retroperitoneal node dissection and three of them did not have lymph node metastases. Methods 2 cases were given postoperative irradiation, 7 cases received adjuvant chemotherapy. Results The 2-. 5-year overall survival were 50% and 30% respectively. Conclusion This report analyzes the prognosis of adult testicle embryonal rhabdomyosarcoma. The treatment is a combination of surgery、 chemotherapy and radiotherapy. Intensive chemotherapy should be administered. The prognosis of RMS in adult seems to be worse than in childhood. Patients with negative lymph nodes has better outcome than those with node metastases. Retroperitoneal lymph node dissection after radical inguinal orchiectomy is unnecessary in patients without CT evidence of nodal involvement.
3.Effect of Transverse Position and Numbers on the Stability of the Spinal Pedicle Screw Fixation during the Pedicle Cortex Perforation.

Objective To evaluate the effect of transverse position and numbers on the stability of the spinal pedicle screw fixation during the pedicle cortex perforation. Methods The vertebral compression fracture was performed in the L1 vertebral body using the Chiba's method from 60 fresh thoracic and lumbar vertebrae samples of sheep(T13-L3),which were randomly divided into 6 groups(A,B,C,D,E,and F)using a lottery method. Bilateral pedicles of vertebral arch of T14 and L2 were inserted pedicle screws,connecting titanium rods to fix T14-L2 segments. Then the samples of groups B,C,D,E,and F were removed a quarter of right side of lateral T14 thoracic pedicle cortical,which were considered the pedicle cortex perforation model. Finally,each group was fixed on different numbers of crosslinks:group A(0 crosslink,Intact),group B(0 crosslink,NCL),group C(1 crosslink,1/2 of the rods,MCL),group D(1 crosslink,1/3 of the rods close to T14,PCL),group E(1 crosslink,2/3 of the rods away from T14,DCL)and group F(2 crosslinks,1/3 and 2/3 of the rods respectively,TCL). After all samples were subject to 10 000 times of fatigue test with biomechanics test machines,the axial compressive stiffness,range of the motion(ROM)of the 6 directions(flexion,extension,lateral bending,and axial rotation),and the maximum pullout of the screws of the T14 pedicle cortex perforation were measured and compared among these 6 groups. Results The axial compressive stiffness in groups A,C,D,E,and F was significantly higher than that in group B(all P<0.05),and group A was significantly higher than group F(P<0.05) . The maximum pullout in groups A,C,D,E,and F were significantly higher than that in group B(all P<0.05),and group A was significantly higher than group F and groups C,D,and E were significantly lower than group F(all P<0.05). ROM of flexion,extension,lateral bending,and axial rotation in groups A,B,C,D,E,and F were significantly lower than that in group B(P=0.000),and ROM of left and right axial rotation in groups C,D,and E were significantly higher than in that group F(P=0.000). Conclusions During the pedicle cortex perforation,adding of one crosslink can improve the stability of the pedicle cortex perforation,and adding of two crosslinks can approximately achieve the same stability as the pedicle screw fixation with no pedicle cortex perforation. The location of the crosslink has no obvious effect on the short segment of spinal fixation.
4.Inhibitory effects of autophagy inhibitor 3-MA on high glucose-induced proliferation of human retinal pigment epithelium cells

Background Diabetic retinopathy (DR) is one of the common complications of diabetes.Retinal pigment epithelium (RPE) cells,as important constituent cells of the retina,play important roles in the development and progression of DR.Objective This study was to investigate the suppressive effects of autophagy inhibitor 3-MA on the proliferation of human retinal pigment epithelium cells (hRPECs) following high glucose culture.Methods HRPECs were divided into control group,high-glucose group and 3-MA+high glucose group.The cells were cultured by the DMEM/F12 with 5 mmol/L glucose in the control group,and by the DMEM/F12 with 5 mmol/L glucose in the high glucose group and by the DMEM/F12 with 10 mmol/L 3-MA (for 1 hour firstly) and 30 mmol/L glucose in the 3-MA+high glucose group.The cells were inoculated into 24-well plate with the content of 1 ×105/well,and then the cells were consecutively cultured for 24 hours with DMEM/F12 containing 0.5% fetal bovine serum after achieved attached 80% confluence.The morphology and uhrastructure of the cells were examined by optics microscope and transmission electronic microscope.The proliferative rate of the cells was assayed by CCK-8 kit.The expression of autophagy-related gene microtubule associated protein light chain 3B (LC3B) in the cells was detected by Western blot and LC3B-Ⅱ/LC3B-Ⅰ value was quantitatively evaluated among the groups.Results The cells grew well with uniform size and regulatory arrangement in the control group.The cells in the high glucose group enlarged and the number of cells evidently increased.In the 3-MA+high glucose group,the cells decreased with a disorder arrangement.Under the transmission electron microscope,the cells were normal with the round-or oval-like nucleus and normal organelles in the control group,and autolysosome could be seen in the cells in the high glucose group.In the 3-MA+high glucose group,some autophagic bodies were found.The proliferative rate of the cells was (100.0±2.0) %,(116.9-±5.2)% and (103.7 ±4.7)% in the control group,high glucose group and 3-MA+high glucose group respectively,showing a significant difference among the groups (F =13.526,P =0.006).The proliferative rate was considerably raised in the high glucose group compared with the control group and 3-MA+high glucose group (both at P<0.05),but there was no significant difference in the proliferative rate of the cells between the control group and 3-MA+high glucose group (P>0.05).Compared with the control group,the expressing intensity of LC3B-Ⅰ protein was weakened and that of LC3-Ⅱ protein was enhanced,and the expression intensity of LC3B-Ⅰ and LC3B-Ⅱ proteins in the 3-MA+high glucose group was similar to that in the control group.The LC3-Ⅱ/LC3-Ⅰ ratio was 0.131 ±0.065,2.504±0.097 and 0.274±0.007 in the control group,high glucose group and 3-MA+high glucose group,respectively,with significant differences among the groups (F =1 694.676,P =0.000),and the LC3-Ⅱ/LC3-Ⅰ ratio was increased in the high glucose group in comparison with the control group and the 3-MA+high glucose group (all at P<0.05).No significant difference was found in the LC3B-Ⅱ/LC3B-Ⅰ ratio between the control group and 3-MA + high glucose group (P > 0.05).Conclusions High glucose culture of hRPECs can activate autophagy process and promote cell proliferation.3-MA,an autophagy inhibitor,suppresses the high glucoseinduced growth of HRPECs by inhibiting autophagy process.
5.Multidimensional spatial triangular area as an index for the evaluation of the release-absorption correlation of multiple component traditional Chinese medicines

The paper is aimed to provide a novel index, named as multidimensional spatial triangular area, for the evaluation of the release-absorption correlation of multiple component traditional Chinese medicines (TCMs). The applicability of the method was demonstrated by the example data. The method and standard practice for evaluation of the release-absorption correlation for western medicines with single compound could not be applied to TCMs with multiple components. The release percentage or absorption percentage of the multiple components for TCMs at the sampling time was a point in the multidimensional space. The area of the triangle formed byt the sequential three points rrepresented the changing characteristics of the components' release and absorption kinetics. The side lengths of the triangle could be calculated from the spatial distances between each two of the sequential three points. Then the triangle area could be obtained by the side lengths. The in vitro release-in vivo absorption correlation of the multiple components could be represented by the correlation between the integrating values of the release triangle areas and that of the absorption triangle areas. The results of the examples indicated that the multidimensional spatial triangular area method could treat the multiple components in a holistic way, in line with the holism the hi he TCMs. Therefore, the multidimensional spatial triangular area method provided new methodology for the release-absorption correlation of the TCMs with multiple components.
6.The application of a high-performance bloodstain testing reagent in forensic scene

Objective To develop a highly sensitive luminescent reagent for bloodstain testing at forensic crime scenes.Methods Based ontheprincipleof ECL luminescence and the ping-pong conjugate activation principle of chemical electronic chain,this project developed a new type of highly sensitive luminescent reagent for bloodstain testing by usingthe uniform design of experimental methods to optimize the conditions andsynthesize several new compounds.Results The bloodstain testing luminescent reagentdeveloped in this project has high sensitivity andlongluminescence time.In the case of blood samples diluted by 1,000 times,reading the fluorescence withChemiScope 3300 chemiluminescence imaging system,the maximumvalue of gray scale reached 56,and the luminescence time lasted for 10 minutes.Conclusion The project has successfully developed a highly semitivebloodstain testing reagentthat could be applied to crime scene investigation.
9.Application of continuous monitoring of jugular venous oxygen saturation in the course of mild hypothermia treatment of severe traumatic brain injury

Objective To discuss significance of continuous monitoring of jugular venous oxygen saturation(S_(jv)O_2)in the course of mild hypothermia treatment(MHT)for severe traumatic brain injury (sTBI).Methods Intracranial pressure(ICP),S_(jv)O_2 and brain tissue pressure(P_(bt)O_2)were contin- uously monitored in 36 cases with sTBI for analyzing the correlation between S_(jv)O_2 and P_(bt)O_2.Results (1)There was negative linear correlation between P_(bt)O_2 and ICP(r=-0.978,P<0.05),negative lin- ear correlation between S_(jv)O_2 and ICP(r=-0.947,P<0.05)and positive linear correlation between P_(bt)O_2 and S_(jv)O_2(r=0.965,P<0.05)within 24 hours and at 36 hours and 48 hours after injury.(2) The cases with decreased S_(jv)O_2 value had a worse outcome than those with normal S_(jv)O_2.meanwhile,the cases with abnormal increase of S_(jv)O_2 value had worse prognosis.Prognnsis was improved significantly with increase of S_(jv)O_2 in certain range(P<0.05).Conclusion Continuous monitoring of S_(jv)O_2 can reflect the condition of hemicerebral oxygen metabolism and guide treatment and predicting outcome.
10.HYPOMAGNESEMIC CONVULSIONS OF THE NEWBORN

This paper reports 17 cases of hypomagnesemic convulsiens of the newhorn that were admitted from Se-Ptember 1981 to January 1983. Only 2 patients were breast-fed.Symptoms and signs of hypomagnesemia are indistinguishable from these of hypocalcemia unless the serum magne-sium is determ ined. Serum magnes iumlevels had been determined in 50 normal children. The average value-2 standard deviation=2.17-2?0.34=1.49mEq/L.We defined hypomagnesemia as the serum magnesium lcvels below 1.48mEg/L. The serum magnesium levels of 17 patients varied from 0.65 to 1.46m Eq/L. Of 10 cases serum calcium le-vel6mg/dl.2.5%MgSO_4 was given intraveno-usly by continuous infusion in a dose of 2-4ml/kg every 12 hours. After the convulsions had been controlled a dose of 25% MgSO_4 was given intramuscul-arly in a dose of 0.4ml/kg twice daily Convulsions usually ceased after 1--4doses of MgSO_4, but the serum magne-sium levels did not rise to normal le-vels until 2-6 days. The convulsions could not be controlled by repeated ad-ministrations of calcium gluconate in 5 patients who had both hypomagnes-emja and hypocalcemia. Only after theadministiation of MgSO_4 did the serum calicum levels rise to the normal level and the convulsions cease.Electrocardjograms recorded in 7 patients all were abnormal but 1 case,so we should pay attention to the inf-luence of magnesium upon the heart.
