Objective To study the feasibility of preoperative acute hypervolemic hemodilution for decreasing perioperative blood use. Methods Thirty two patients(ASAI-II) undergoing spinal surgery were randomized into AHH group and control group. After anesthesia 6% HES(200/0.5) was given through internal jugular vein for AHH. The blood volume was increased by about 25%. During the operation, blood loss was replaced by HES in equal volume. Blood was transfused when the Hct dropped below 25%. The HR, MAP, CVP, SPO2, Hb, Hct., Plt, FIB, APTT, PT, amount of blood loss, blood transfused and urine output, volume of drainage 24h after operation and VAS score (0 means no effect, 10 means maximal effect) were observed before AHH, at the end of AHH, at the end of operation, and 24h after the operation. AIM: To establish a RP-HPLC method for determining ephedrina hydrochloridum and pseudoephedrine hydrochloride in Chuanliting Spray(Herba Ephedrae,Folium Artenisiae argyi,Radix et Rhizoma Asari,etc.). METHODS: HPLC conditions were as follows: C_(18)(4.6 mm?250mm,5?m) column,0.2% phosphoric acid—acetonitrile(96∶4) as mobile phase with flow rate of 1mL/min,and the detection wavelength at 205nm.(RESULTS:) The calibration curve of ephedrina hydrochloridum was linear between 0.448-3.416?g(r=(0.999 8,) n=7).The average recovery was 100.26% and RSD=2.17%(n=5).The calibration curve of pseudoephedrine hydrochloride was linear between 0.16-1.12?g(r= 0.999 1,n=7).The average recovery was 101.60% and RSD =2.19%(n=5). CONCLUSION: The method is convenient and efficient,and can be used for quantitative analysis and quality control of this preparation.

Objective To establish a method for the determination of Puerarin in Huanglian Jiangtang Tablets. Methods The sample was extracted with 30 %ethanol. The chromatographic conditions were as follows:Diamonsil C18 chromatographic column(250 mm?4.6 mm,5?m)with a mobile phase of acetonitrile-0.5 %glacial acetic acid,the detection wavelength being at 250 nm and the flow rate being 1.0 mL?min-1. Results A linearity was obtained from 0.338 ?g to 2.336 ?g of Puerarin in Huanglian Jiangtang Tablets with a good correlation (r=0.999997,n=7).The average recovery was 100.2% and RSD=1.80%(n=6). Conclusion This method for determination of Puerarin in Huanglian Jiangtang Tablets is easy,sensitive,specific and accurate.

Objective To establish a quality control method for tanshinone ⅡA in Niaotong Tablets.Methods The content of tanshinone ⅡA was determined by TLC-scanning method. Benzene-ethyl acetate(19 ∶1) was used as the developer, ?s = 470nm,?R = 620nm,SX=3.Results A linearity was obtained from 0.44?g to 2.20 ?g of tanshinone ⅡA in Niaotong Tablets (r=0.9993,n=5);the average recovery rate was 98.08 %,RSD=3.29 %.Conclusion This method is simple, sensitive and reproducible for the determination of tanshinone ⅡA in Niaotong Tablets and Radix Salviae Miltiorrhizae.

Seven sinapine analogs (6a-6g) were synthesized using cinnamon acid or benzoic acid and their derivatives as starting materials, which obtained from substituted benzaldehyde and malonate. The structures of target compounds were characterized by IR, 1H NMR and elemental analysis. The effects of compounds 6a-6g on the smooth muscle of intestine isolated from rabbit were studied, and the experimental results showed that compounds 6a, 6d and 6g had diastolic action, while 6f had contractile action.

Objective To establish a method for the determination of 5-O-methylvisamminol in Shufeng oral liquid.Methods The sample was extracted with methyl and HPLC method was used.The chromatographic conditions were as follows: Alltima-C18 column(150 mm ? 4.6 mm,5 ?m)with a mobile phase of methanol-water(41∶59),the detection wavelength at 293 nm and the flow rate being 1.0mL?min-1.Results A linearity was obtained from 0.12 ?g to 0.84 ?g of 5-O-Methyvisammioside in Shufeng oral liquid with a good correlation(r = 0.99998,n = 7).The average recovery was 99.8 % and RSD = 2.05 %(n = 5).Conclusion This method for determination of 5-O-Methyvisammioside in Shufeng oral liquid is simple,sensitive,specific and accurate.

A patient diagnosed with myoepithelial carcinoma was recently treated in our department. The neoplasm was huge, located in the left maxillofacial region, blocking both eyes, impeding feeding. About one month before admission, the tumor began to bleed frequently, about 100 ml each time, causing headache, dizziness, fatigue, and cold sweats. CT showed the maximum diameter of the tumor was about 23 cm, with uneven density, and maxillofacial bone destruction. MRI revealed internal bleeding and necrosis inside the tumor. After admission, blood routine test showed erythrocyte count 3.64 x 10(12)/L(↓), hemoglobin 106 g/L(↓), hematocrit 0.320 (↓), serum iron 6.2 μmol/L(↓). After surgery, the patient recovered smoothly.

Objective To evaluate the epidemic status of main non-communicable chronic diseases among the residents of Jingan district in Shanghai.Methods Multistage cluster sampling was adopted.960 residents were investigated with questionnaire and physical examination.Results The prevalence rates of Hypertension,diabetes mellitus,Cerebral vessels and Heart-blood were 25.6%,5.8%,3.1%and 5.7%,respectively.The multivarate logistic regression analysis showed that the related risk factors to four non-communicable chronic diseases included family history,the OR(95%CI)1.6(1.2～2.3),6.9 (3.5～13.6),4.5(2.0～10.0),2.2(1.2～4.0)separately.Conclusion Hypertension,diabetes are the main non-communicable disease hazarding the public health.The government and professional institutes should raise the attention on this serious problem and finance more as well as supplying policy support.
