1.Clinical comparison of cerivastatin with simvastatin for aged hyperlipidemia

OBJECTIVE To assess the efficacy and safety of Lipobay (cerivastatin) and Jingbishuxin (simvastatin) on lipid profiles.METHODS 40 patients with hypercholesterolemia were randomizedly divided into Lipobay group (0.3 mg,QN) and Jingbishuxin group (domestic simvastatin tablet) (20 mg,QN)for 4 weeks.Total cholesterol (TC),low density Lipoprotein cholesterol (LDL-C),triglyceride (TG) and high density lipoprotein cholesterol (HDL-C),blood sugar,hepatic and renal function,blood and urine regular test were measured before and after the treatment.RESULTS After 4 weeks of treatment,TC,LDL-C and TG were reduced by 29.02%,29.95%,17.21% in Lipobay the group and 22.48%,28.68%,14.74% in Jingbishuxin group,respectively.Compared the measurements after treatment with that before treatment,there were significant differences in both groups,but there was no significant difference between them.No serious adverse effect was found.CONCLUSION Lipobay had a same efficacy and safety on lipoprotein profiles as that of Jingbishuxin in elderly.It has a good tolerance with smaller dose.
2.Expression and role of Twist and N-cadherin in pancreatic cancer

ObjectiveTo investigate the expression of Twist and N-cadherin in pancreatic cancer,and to study its relationship with clinicopathological parameters and patients' prognosis.Methods The expression of the Twist and N-cadherin in 62 tissue samples from patients with pancreatic ductal adencocarcinoma and 10 normal pancreatic tissue samples was determined by using immunohistochemistry MaxVision two step method,and the relationship with clinicopathological parameters and patients' prognosis was analyzed.ResultsThe positive expression rate of Twist in pancreatic cancer was higher than that of normal pancreatic tissues (96.8% vs.30%,P <0.01 ),and the positive expression rate of N-cadherin was higher than that of normal pancreatic tissues (75.8% vs.0,P <0.01),but there was no correlation between them (r =0.100,P =0.441 ).The expressions of Twist and N-cadherin was significantly correlated with TNM stage,lymph node metastasis,infiltration of portal vein or nerves and tumor location ( P <0.05 ),but not with age,gender and degree of differentiation ( P > 0.05 ).The post-operative survival of patients decreased with the increasing Twist expression,but the survival was not associated with the expression of N-cadherin.TNM stage,the expression of Twist was independent predictive factors of prognosis for pancreatic cancer patients.ConclusionsTwist and N-cadherin are highly expressed in pancreatic cancer,and the expression was associated malignant behavior of pancreatic cancer.The abnormal expression of Twist may be a potential marker for prognosis evaluation of pancreatic cancer patients.
3.Relationship between the expression of Fas or FasL and the apoptosis of peripheral blood T and B lymphocytes in systemic lupus erythematosus patients

Objective To investigate the relationship between the expression of Fas or FasL and the apoptosis of peripheral blood T and B lymphocytes in systemic lupus erythematosus (SLE) patients. Methods The expression rates of Fas and FasL and apoptosis rates of peripheral blood T and B lymphocytes were detected by flow cytometry (FCM) in 30 active SLE patients and 30 inactive SLE patients and 30 healthy controls. Results There were significant difference in the expression rates of Fas of T and B lymphocytes among three groups (P0.05). In addition, the activity of SLE had no correlation with the expression rates of Fas or FasL on T or B lymphocytes but has positive correlation with the apoptosis rates of T or B lymphocytes. And no correlation was found between the expression rate of Fas or FasL and the apoptosis rate of neither T nor B lymphocytes. Conclusion The apoptosis of peripheral blood lymphocytes is influenced by many factors in SLE patients, but is abnormally increased at last, especially in active SLE. It is inferred that the increase of apoptosis rate of peripheral blood lymphocytes plays an important role in the pathogenesis of SLE.
4.Curative effect comparison of hysteroscopy combined with laparoscopy operation and transvaginal operation treatment of patients after cesarean section uterine incision diverticulum

Objective To investigate the clinical efficacy of uterine incision diverticulum type repair after cesarean section treated by laparoscopy and transvaginal combined with repair in.order to provide the reference for the clinical operation scheme selection.Methods Fifty-two patients who underwent uterine incision diverticulum type repair after cesarean section in the Second People's Hospital of Jingzhou were randomly divided into observation group and control group with 26 cases in each.Patients in the observation group were treated with laparoscopy combined with repair treatment,and patients in the control group were treated with transvaginal repair treatment.The operation periods,cut bleeding volume,postoperative anal exhaust time for the first time,postoperative highest temperature and length of hospital stay were recorded to evaluate the repair and uterine diverticula recovery at 3 months after operation.Results The operation periods in the observation group was (119.91 ± 35.73) min,longer than that in the control group ((62.32 ± 31.83) min ; t =3.514,P <0.05).There were on significant differences between two groups in terms of the amount of bleeding,anal discharge first time,the highest postoperative temperature and contrast after a few days in hospital (P> 0.05).After treatment,the observation group menstrual recovery rate was 84.62% and 88.46% in the control group.The differences were not statistically significant (x2 =1.14,1.76,P > 0.05).Conclusion Laparoscopy hemiorrhaphy and transvaginal repair are effective treatment for uterine incision diverticulum after caesarean operation.The corresponding treatment plan can be made according to the patient's condition.
5.Clinical study on effect of common methylenetetrahydrofolate reductase gene mutation on coronary artery disease in hypertension

Objective To investigate the association between the MTHFR gene C677T mutation and CAD in patients with hypertension. Methods Of the 103 cases of patients with hypertension who underwent coronary angiography, 58 were CAD and 45 were not. MTHFR genotypes were identified by PCR and restriction fragment length polymorphism analysis with Hinf I digestion. Plasma folate was determined by radioimmunoassay. Plasma HCY was determined by HPLC. Results The frequencies of T677 allele (0.45) in the CAD group were markedly higher than those in the control group (0.33, P=0.03), plasma homocysteine level in the CAD group (17.73?2.22 ?mol/L) was also significantly higher than that in the control group (12.18?1.23 ?mol/L, P=0.037). There was no significant difference in the concentrations of folate for the two studied groups. Conclusion The MTHFR gene T677 variant is the genetic high risk factor for CAD in patients with hypertension. It can induce the hyperhomocysteinemia, potentially contributing to the pathogeny of CAD.
6.Role of autophagy in human mesenchymal stem cells in response to irradiation

BACKGROUND:Bone marrow mesenchymal stem cells can survive under the lethal dose of radiation in response to hematopoietic stem cells, and stil maintain the typical characteristics of stem cells to promote hematopoietic recovery after radiation. However, autophagy is an important mechanism for cellular adaptation under stress, which may be involved in radiation tolerance of bone marrow mesenchymal stem cells. OBJECTIVE:To investigate the role of autophagy in human bone marrow mesenchymal stem cells in response to irradiation. METHODS:Passage 3 bone marrow mesenchymal stem cells cultured in vitro at logarithmic phase were col ected and randomized into control, 3-mehyladenine, rapamycin, irradiation, irradiation+3-mehyladenine and irradiation+rapamycin groups. The autophagy reactions of bone marrow mesenchymal stem cells were regulated by 5 mmol/L 3-mehyladenine and 200 nmol/L rapamycin for 12 hours in the 3-mehyladenine and rapamycin groups, respectively. Two-hour 6 Gy X-ray irradiation was performed in the irradiation group and two complication groups undergoing 12-hour corresponding drug intervention. RESULTS AND CONCLUSION:The proportions of cells with autophagic vacuoles and apoptotic cells were higher in the irradiation group than the control group, moreover, autophagic+apoptotic cells were increased in the irradiation group. 3-mehyladenine intervention could decrease the proportion of cells with autophagic vacuoles,and increase the number of apoptotic cells. But there was no difference in the proportion of autophagic+apoptotic cells between the 3-mehyladenine and irradiation groups. After rapamycin intervention, the proportion of autophagic cells was higher than that in the irradiation group, but no difference in the proportion of apoptotic cells between the two grups, as wel as there were no autophagic+apoptotic cells. 9.The experimental study of the artificial esophagus composed of resorbable porcine thoracic aortas acellular matrix

Objective To investigate the feasability of resorbable porcine-derived, xenogeneic aortas acelullar tissue matrix (ACTM) to be used as scaffold for cervical esophageal defect repair in a dog model. Methods Esophageal segmental defect measuring 5cm in length were created by surgical resection in 6 mature Chinese mongrel health dogs, then the defect were repaired with aorta acelullar tissue matrix (ACTM). The animals were kept alive for periods. The healing process was observed. Results There no death after perioperative period, one dog had anastomotic leakage. one dog was died of esophagus rupture as the result of esophageal dilation. Submucosal tissue regeneration and abundant neovascularization were found at 2 weeks. The squamous epithelium covered the most part of grafts surface in dogs killed after 4 weeks. 12 weeks later the squamous epithelium comprising 8 to 12 layers, both esophageal glands and muscle tissue were found. The graft material itself was mostly absorbed and could not be identified by naked eyes in the dogs killed after 12 weeks. Conclusion ACTM show promise as an ideal treatment option for esophageal repair.
10.Minimally invasive pie-crusting technique combined with arthrolysis for the treatment of the stiff knee.

OBJECTIVETo study the effectiveness of pie-crusting technique in improving the stiff knee.
METHODSFrom February 2012 to December 2013, 13 patients with stiff knee were reviewed retrospectively. There were 6 males and 7 females, ranging in age from 39 to 70 years old (averaged, 55.6 years old). Of the 13 cases, 8 patients had stiffness following fracture (comminuted tibial plateau fracture in 4, femoral supracondylar fracture in 3 and patellar fracture in 1), 5 patients had TKA-related stiffness.
RESULTSA follow-up lasted 8 to 12 months (mean 10 months)in 13 cases. The mean maximum flexion increased from (37 ± 6)° preoperatively to (52 ± 7)° after arthrolysis, and (108 ± 7)° after pie-crusting. At the final follow-up, mean maximum flexion was (105 ± 6)°. According to Judet evaluation system, 10 patients got an excellent result and 3 good. No major complications, such as extensor lag, skin necrosis, deep infection, dislocation of the patella or recurrent stiffness were found.
CONCLUSIONThe percutaneous technique of pie-crusting is a simple, minimally invasive and effective treatment for knee stiffness.
