Strabismus and amblyopia are the commonly eye diseases.Great progress has been achieved in the treating strabismus and amblyopia in recent years.But some essential problems should be considered to gain better therapeutic results for strabismus and amblyopia patients.For example,what is the goal of strabismus surgery? What is the timing(when to treat strabismus and when to treat amblyopia) for strabismic amblyopia? What are the guidelines of amblyopia treatment? Here are summaries and comments round above problems which are expected to be useful for strabismus and amblyopia treatment.
2.Diagnostic value of combined LAVA and DWI for pancreatic carcinoma

Objective To evaluate the combination of LAVA and DWI in diagnosis of pancreatic carcinoma.Methods MRI of 24 patients pathologically confirmed to have pancreatic carcinoma were retrospectively analyzed.MRI sequences included FS T1WI,FS T2WI,enhanced multi-phasic LAVA scanning and DWI.Images quality of all sequences was assessed.SIR of tumor/pericancerous tissue in LAVA,DWI,T1WI and T2WI was compared.ADC values of the cancer,pericancerous tissue were statistically analyzed.The detection rate and accuracy of single sequence and those of the combination of LAVA and DWI were compared.The LAVA evaluation of peripancreatic vascular involvement was compared with surgical findings.Results Images quality of LAVA was higher than the others(P
3.TIPS combined with gastric coronary vein embolization for cirrhosis portal hypertension complicated by upper gastrointestinal bleeding

Objective To compare the clinical curative effect between transjugular intrahepatic portosystemic stent-shunt (TIPS) and TIPS together with gastric coronary vein embolization (GCVE) in treating cirrhosis portal hypertension (PHT) associated with upper gastrointestinal hemorrhage (UGH),and to discuss the necessity,feasibility and clinical curative effect of TIPS plus GCVE.Methods The clinical data of 38 PHT patients with UGH,who were admitted to authors' hospital during the period from April 2010 to May 2012,were retrospectively analyzed.Only TIPS was employed in 15 patients (group A),and TIPS plus GCVE was adopted in 23 patients (group B).Before and after operation,the indexes,hemodynamics of portal vein and spleen,the morphology of spleen,and the degree of gastrointestinal varices were determined and analyzed.The patients were followed up to observe the occurrence of postoperative complications.Results In both groups,the postoperative portal vein pressure showed an obvious reduction with accelerated velocity of flow,and the splenic venous congestion index was decreased,these changes were statistically significant when compared with the preoperative ones (P<0.05),and which was more obvious in group B than in group A (P<0.05).After the treatment,the esophagogastric varices (EGV) was obviously improved,the improvement rates of group B and group A were 94.7% and 66.6% respectively,the emergency hemostasis rates of group B and group A were 100% and 75.0% respectively,The rates of re-bleeding were 4.3% and 28.5% respectively;the above results of group B were statistically better than those of group A (P<0.05).No statistically significant differences in liver function indexes existed between preoperative values and postoperative ones in the same group as well as in group comparison (P>0.05).The differences in the postoperative hepatic encephalopathy (HE) occurrence and in primary patency rate of stent between the two groups were not statistically significant (P>0.05).In both groups,the re-intervention patency rate was 100%.The incidence rate of HE in patients in whom the distal end of stent was located in the left branch of portal vein was strikingly lower than that in patients in whom the distal end of stent was located in the right branch of portal vein (P<0.05).Conclusion For the treatment of PHT associated with UGH,TIPS combined with GCVE carries reliable curative effect,this therapy is superior to simple use of TIPS.
4.THE PATHOLOGICAL CHANGES OF ULTRASTRUCTURE IN BRONCHIAL MUCOSA OF TYPE 2 DIABETIC PATIENTS

Objective To investigate the pathological changes of ultrastructure of mucosa in various bronchial segments from type 2 diabetic patients. Methods Sixteen cases of type 2 diabetic patients were selected,2-3 pieces of bronchial mucosa and submucosal tissue of the lesion were taken from various bronchi during bronchoscopy and these samples were observed under a transmission electron microscope. Results The basal lamina of bronchial capillary were diffusely thickened and mostly showed onion-skin like change,protein deposited around and mixed with basal menbrance;irregular highly electron dense materials were found to deposite around capillary,capillary lumen became narrow or even collapsed,neutrophilic leucocyte marginated in lumen and adhered with endothelium;protein deposited in the interstitial;endothelial cells and pericytes had dark cell changes.The cistern of rough endoplasmic reticulum dilated and vesicle formed.Conclusion Bronchial mucosa and its surrounding tissues show characteristic pathological changes of diabetes,bronchial is also the target organ of chronic diabetic damage.
5.Effect of 3-n-butylphthalide on expression of matrix metalloproteinase andβ-APP in rats

Objective To study the effect of 3-n-butylphthalide on expression of MMP-2 ,MMP-9 andβ-APP in rats following chronic cerebral hypoperfusion at molecular level .Methods Seventy-two male Wistar rats were randomly divided into sham operation group ,ischemic group ,and bu-tylphthalide group (24 in each group) .Rats in ischemic group and butylphthalide group were di-vided into postischemic 7 ,14 ,30 ,60 d subgroups (6 in each group) .Expressions of β-APP ,MMP-2 , MMP-9 in each group were detected by Western blot .Results The β-APP expression level was significantly higher in postischemic 14 d ,30 d ,60 d subgroups than in postischemic 7 d subgroup (P<0 .05 ,P<0 .01) ,and in postischemic 30 d ,60 d subgroups than in postischemic 14 d sub-group (P< 0 .01) .The β-APP expression level differed greatly in 30 d and 60 d butylphthalide subgroups (P<0 .01) .The MMP-2 expression level was significantly higher in postischemic 60 d subgroup than in postischemic 30 d subgroup (P<0 .05 ,P<0 .01) .Theβ-APP and MMP-2 ex-pression levels were significantly higher in ischemic group and butylphthalide group than in sham operation group (P<0 .05 ,P<0 .01) ,and significantly lower in butylphthalide group than in is-chemic group (P< 0 .01) .Conclusion MMP-2 and β-APP tend to change similarly and are in-volved in blood brain barrier destruction and amyloid deposition .Butylphthalide intervenes in am-yloid deposition by downregulating the expression of MMP-2 andβ-APP .
6.Comparison of anterior segment parameters with Lenstar 900 and Sirius system

AlM: To compare the anterior segment measurements derived from optical low coherence reflectometer ( Lenstar LS900 ) and combined Scheimpflug - Placido disk topographer ( Sirius) .
METHODS: ln this study, we enrolled healthy myopic subjects 54 ( 87 eyes ) . The central corneal thickness (CCT), anterior chamber depth (ACD), flat keratometry ( FK) readings , steep keratometry ( SK ) readings and white to white ( WTW ) were measured by LS900 and Sirius. Evaluation and analysis were performed using paired t tests, the Pearson correlation, and Bland-Altman analyses.
RESULTS: The CCT and ACD measurements were significantly lower whereas FK, SK and WTW measurements were higher with LS900 (P<0. 001). Mean differences were -6. 11 ± 6. 32μm,-0. 09 ± 0. 07mm, 0. 18 ± 0. 25D, 0. 21 ± 0. 36D and 0. 25 ± 0. 39mm with statistical significant (P<0. 01). Pearson correlation analysis showed high correlation between the 2 devices for all measurements (P<0. 001). On Bland-Altman analysis, 95% limits of agreement for all measurements were 6. 26 to -18. 49μm; 0. 04 to 0. 22mm; 0. 68 to 0. 32D; 0. 92 to 0. 50D and 1. 00 to 0. 5mm.
CONCLUSlON: Anterior segment parameters evaluated with LS900 and Sirius systemare correlated well and achieve good agreement. However, there are significantly statistical differences which may be caused by the different measurement modes, so they may not be interchangeable use under certain clinical circumstances.
7.Repair of rabbit facial nerve with collagen conduits and expanded polytetrafluoroethylene

ObjectiveTo evaluate the effects of expanded polytetrafluoroethylene (e-PTFE) and collagen tubes in the repair of facial nerve in rabbits.MethodsThe facial nerves of rabbits were transected and removed for 5 mm. The nerve ends were then repaired with an e-PTFE or collagen tube, and with an autologous nerve graft as control. After 7 days, 1 month and 3 months, the animals' neural conductive velocity were determined. And then, the nerve specimens were taken out, and the myelinated fibers across the specimen were counted with histological examination.ResultsOn the 7th day, no neural regeneration was observed. But new forming neural fibers across the biocompatible materials and autologous nerve grafts were seen in the following 3 months, while the number and conductive velocity of myelinated fibers varied significantly at the different time points.Conclusione-PTFE and collagen conduits are effective in the repair of peripheral nerves.
8.Clinical application of intravenous myocardial contrast echocardiography in patients undergoing percutaneous coronary interventions

Objective To study the clinical application of intravenous myocardial contrast echocardiography(IMCE) in assessing myocardial reperfusion by using intravenously infused Levovist before and after percutaneous coronary interventions(PCI). Methods IMCE was performed in 20 patients with coronary artery disease(CAD) before and after PCI. The power Doppler signals of the myocardial segments were analyzed and the time-amplitude curves were plotted during Levovist infusion and incremental triggering. 9.A preliminary study on the correlation between gastric lymphoid follicles and Helicobacter pylori infection

Objective To investigate the correlation between H.pylori strain possessing cagA gene and the formation of gastric lymphoid follicles(GLF),and the effect of H.pylori infection on the occurrence of GLF.Methods Antral biopsy specimens from 655 patients (chronic gastritis:n=479,peptic ulcer:n=176)were used for detection of H.pylori infection and histological analysis. CagA gene was examined in 70 clinical isolates by means of PCR- amplification. Results The incidence of lymphoid follicles in gastric mucosa is significantly higher(60.14% ) in patients with H.pylori infection than those without infection(17.06% ).GLF is easier to be detected in patients with active gastritis than in those with inactive gastritis. There is no significant difference in the presence of GLF among H.pylori associated gastroduodenal diseases,such as chronic gastritis ,gastric ulcer. Moreover, there is also no significant relationship between H.pylori strains possessing cagA gene and GLF. Conclusion The presence of GLF might directly related with H.pylori infection, and can be observed as a constant morpholgical marker of H.pylori related gastroduodenal disease ;The formation of gastric lymphoid follicles is not related to the cytotoxin of cag A gene of H.pylori.
10.Clinical study on microvascular reserve by myocardial contrast echocardiography using intravenously infused Levovist

Objective To establish the feasibility and clinical applications of intravenous myocardial perfusion imaging with Levovist in assessing the coronary microcirculaton reserve.Methods Intravenous myocardial contrast echocardiography (IMCE) was performed pre and post dipyridamole injection in 10 cases with normal coronary angiography (group I) and 8 cases with 50% to 70% stenosis in the lift anterior descending branch(LAD) (group Ⅱ) respectively.A set of special instrumental setting and an integrated imaging approach including power Doppler imaging,incremental triggering imaging and harmonic imaging were utilized to record the apical four chamber view.The power Doppler signals of the septum were analyzed and the time amplitude curves were plotted during Levovist infusion and incremental triggering.The maximal amplitude score (A),the mean ascending slope of the curve(?) and the product of A?? were measured.The reserve of A,? and A?? were also calculated.Results At baseline,both group I and group Ⅱ had normal myocardial perfusion.After dipyridamole infusion,contrast score A increased slightly in group I and decreased in group Ⅱ without statistical significance.Both ? and A?? were increased significantly after dipyradamole (P
