Objective: A capillary electrophoresis for the determination of berberine hydrochloride and palmatine hydrochloride in Ruyijinhuang Powder(Rhizoma Curcumae Longae, Radix et Rhizome Rhei, Cortex Phellodendri, Rhizoma Atractylodis, etc.) was established. Methods: The berberine hydrochloride and palmatine hydrochloride were determined using benzoic acid as an internal standard its detection wavelength was at 230nm. The optimized condition was achieved with a fused\|silica capillary 100?m?56.5cm, 10mmol?L -1 phosphate buffer(pH=4.80) containing 30% acetonitrile, a constant voltage of 20kv and temperature at 30℃. Results: Calibration curve was constructed in the range of 5?g?mL -1 ～100?g?mL -1 for berberine hydrochloride and palmatine hydrochloride, respectively. The regression equations were Y=0.02964X -0.00771 ( r =0.9998) and Y =0.01092 X -0.02939 ( r =0.9997) for berberine hydrochloride and palmatine hydrochloride, respectively. The average recovery was 98.7% and 102.1%( n =5), respectively. Conclusion: This method is sample, quick and sensitive.

OBJECTIVE: To explore the therapeutic effects of Ziyin Xiehuo Recipe (ZYXHR) for nourishing yin and lowering fire and Yishen Tianjing Recipe (YSTJR) for nourishing kidney and replenishing essence on regulating the gonadotrophic and somatotrophic functions of hypothalamic-pituitary axis, and to reveal the mechanisms of ZYXHR and YSTJR in modulating the course of pubertal development of children with precocious puberty. METHODS: The pubertal rats were fed with ZYXHR or YSTJR for 30 days, and the parameters of rats were monitored as the followings: The content of gonadotropin-releasing hormone (GnRH), the frequency and amplitude of GnRH impulse releasing, the releasing amounts of aminoacid neurotransmitters, and neuropeptide Y (NPY) and beta-endorphin (beta-END) in the gonadotrophic area of the hypothalamus were detected with neurobiological methods (push-pull perfusion, homogenate, incubation of brain slices, and immunohistochemical staining). The levels of gene and protein expressions of GnRH, growth hormone releasing hormone (GHRH) and somatostatin (SS) in hypothalamus, and follicular stimulating hormone (FSH), luteinizing hormone (LH) and growth hormone (GH) in adenohypophysis as well as insulin-like growth factor I (IGF-I) in metaphysis were determined with real time reverse transcription-polymerase chain reaction (RT-PCR). RESULTS: The ZYXHR could reduce the activity of GnRH neurons in hypothalamus through inhibiting the release of central exciting aminoacid neurotransmitters, whereas promoting the release of central inhibiting aminoacid neurotransmitters, NPY and beta-END in gonadotrophic area of hypothalamus. The expression levels of GnRH, FSH and LH mRNAs were down-regulated while the expression level of SS mRNA in hypothalamus was up-regulated in the ZYXHR-treated group. The GH mRNA in hypophysis and the IGF-I mRNA in metaphysis were both down-regulated by ZYXHR. The YSTJR could promote the activity of GnRH neurons in hypothalamus through inhibiting the release of NPY in gonadotrophic area of hypothalamus, up-regulating the expression levels of GnRH, FSH, LH and GH mRNAs in hypophysis, and IGF-I mRNA in metaphysis, while down-regulating the expression level of SS mRNA in hypothalamus. CONCLUSION: The ZYXHR and YSTJR could both regulate the gonadotrophic and somatotrophic functions of hypothalamic-pituitary axis through modulating the neuroendocrine regulation and the gene expressions of GnRH and SS in hypothalamus, GH, FSH and LH in hypophysis, and IGF-I in metaphysis. These may be the chief mechanisms of ZYXHR and YSTJR in modulating the course of pubertal development and ameliorating the skeletal development in children with precocious puberty.

Objective To investigate the effect of acute blood glucose on the level of glutathione peroxidase (GSH-Px),interleukin-6 (IL-6),and the expression of nuclear factor-κB (NF-κB).Methods The Wistar rats were infused intermittently or consistently with 50％ glucose solution.The level of GSH-Px was investigated with colorimetry.The level of IL-6 was measured with enzyme-linked immunosorbent assay (ELISA).The expression of NF-κB was investigated with immunohistochemisty.Results The level of GSH-Px in aorta homogenate in the acute blood glucose fluctuation group [(6.26 ± 0.38) nmol/mgprot] was evidently lower than that of consistently high blood glucose group [(8.98 ± 0.56) nmol/mgprot] and the control group [(10.02 ±1.10)nmol/mgprot] (P ＜0.05).The level of IL-6 [(20.56 ±3.78)pg/ ml] and the expression of NF-κB in the aorta in the acute blood glucose fluctuation group [(16.35 ±2.45) pg/ml] were evidently higher than the other two groups (P ＜ 0.05).Conclusions Acute glucose fluctuations induced the enhancing of oxidative stress and expression of inflammatory cytokines in the aorta,which leads to start NF-κB signaling pathway.

Objective To analyze the clinical and radiographic outcomes of staged reduction and fixation in a consecutive series of patients with the old Lisfranc injuries. Methods Fifty patients (16 feet) with Lisfranc injuries were treated with staged reduction. Mean duration between injury and surgery was 4. 8month ( 3 to 8 month) . In first stage an external fixator was applied across the Lisfranc joint and distraction was done at 1 milliliter per day to 2 milliliter per day. In the second staged the ORIF ( open reduction and internal fixation) was doneand we were able to reduce all the fractures and dislocations. Extra-Articular screws and staple fixation were used for fixation. We compared categorical variables using Fisher’ s exact test and continuous variables using paired t-test or Wilcoxon signed-rank test. Results All patients were followed up 1 to 3 years ( mean 2. 2 years) in the clinic. The visual analogue scale score averaged 3. 1 points at the final follow-up, the average AOFAS scores for these patients were 55. 8 points ( range, 43 to 98 points), with a significant increase than before surgery ( P=0. 001). The mean duration between two surgeries was 3. 2 weeks (range 2. 5-4. 5 weeks). Anatomic reduction was obtained in all 15 patients. At the last follow-up, 2 patients had lost reduction. Posttraumatic osteoarthritis was observed in 5 patients, and all of them were scheduled for arthrodesis because of persistent pain. Conclusions The study have displayed that staged reduction and Extra-Articular fixation should be considered for old Lisfranc injuries with a good reduction, the firm stability, low risk of intraoperative fracture. The short-term effectiveness is good, but the long-term effectiveness needs further follow-up.

Objective To investigate the change in quality of life (QoL) following laparoscopic Roux-en-Y gastric bypass (LRYGB) for T2DM with obesity.Methods A total of 46 overweight T2DM cases (mean baseline BMI (32.7 ± 3.9) kg/m2) undergoing LRYGB were followed by QoL questionnaires (SF-36) before and after 6 and 12 months.Results The preoperative physical QoL scales (physical component summary,PCS) was significantly different from the general population(GP) scales (P ＜ 0.01),and the improvement in QoL was significant at 6 months postoperative and continued to consistently increase up to 12 months (P ＜ 0.01).For mental component summary (MCS),the preoperative scales was not significantly different from the GP scales,however,the 6 and 12 postoperative scales improved significantly (P ＜0.01).As for physical function,role physical,role emotion,the 6 (78 ± 8,76 ± 17,70 ± 23) and 12 months' (85 ± 7,82 ± 18,77 ± 18) postoperative scales were markedly higher than those in the preoperative controls (P ＜ 0.01),and there were no differences between the 12 month's postoperative scales and GP scales.The preoperative bodily pain,general health and social function (66 ± 14,55 ± 12,63 ±15) was different when compared with (76 ± 12,75 ±6,75 ± 13) at 6 months and (82 ±9,79 ±6,94 ±9) at 12 months (P ＜0.01).The preopertive mental health (71 ± 12) was not significantly different from GP scales,nor there were significant difference when compared with (71 ± 10) at 6 months and (73 ±8) at 12 months.Conclusions There were improvements in physical and mental QoL in T2DM patients with obesity after LRYGB.

OBJECTIVETo evaluate the effectiveness of hypnotic and musical relaxation therapy and psychological consultation for parents of children with cleft lip and/or palate (CLP) and to provide a scientific basis of clinical-psychological treatment options.

METHODSSixty-six subjects with children with CLP participated in this study. The subjects were randomly assigned to a test group (n = 33) and a control group (n = 33). The test group was treated with hypnotic and musical relaxation therapy; the control group were subjected to psychological consultation. Anxiety and depression states were evaluated by using a self-rating anxiety scale (SAS) and a self-rating depression scale (SDS) before and after treatment was administered.

RESULTS1) The test group demonstrated a significant decrease in SAS and SDS scores (t = 2.855, P < 0.01; t = 2.777, P < 0.01). The control group showed a significant decrease in the SAS score (t = 1.831, P < 0.05) but failed to show a significant change in the depression score (t = 0.909, P > 0.05). 2) The test group yielded a higher percentage of remission indicated by the SDS scores than the control group (test group = 75.76%; control group = 60.61%; P < 0.05). The test group also displayed a higher percentage of remission indicated by the SAS scores than the control group (test group = 78.79%, test group = 69.70%; P < 0.05).

CONCLUSIONHypnotic and musical relaxation therapy can more effectively reduce the scores of the anxiety and depression states of the parents of patients with cleft lip and/or palate than psychological consultation.

Objective To observe the impact of laparoscopic Roux-en-Y gastric bypass on bowel habits in patients with type 2 diabetes mellitus(T2DM).Methods 70 cases of T2DM undergoing laparoscopic Roux-en-Y gastric bypass were studied.Changes in bowel habits, frequency and odor of flatulence, and social life were estimated at least 6 months after surgery using a self-administered questionnaire.Results 67.1%of the patients had normal bowel habit, 68.6%of patients maintained normal flatus before undergoing surgery, and visual ana-logue scale reveals bowel and flatus habit would cause little trouble on daily life.47.1% of patients maintained their normal bowel habit, and 45.7%of patients had loose stools and diarrhea after surgery.The number of pa-tients with loose stools significantly increased(28/70, 40% after surgery vs 5/70, 7.1% before surgery), with statistical difference( P<0.001) .42.9%patients believed that eating high-fat diet was related with loose stools (P<0.001).Patients with constipation decreased significantly after surgery(5/70, 7.1% vs the preoperative 16/70, 22.9%), with statistical difference(P=0.016).Visual analogue scale showed that 57.1% of patients thought their daily life and social activities were not affected(P=0.05).50%of patients considered an increase flatus, and 55.7%had malodorous flatus, which had statistical significance compared with those before surgery ( P<0.001) .A visual analogue scale showed that 60%of patients thought that this change would not affect their daily life and social activities( P=0.212) .Conclusions After laparoscopic Roux-en-Y gastric bypass surgery, some patients had loose stod, diarrhea, increased flatus and and offensive odor, but after proper treatment these changes do not affect their daily life and social activities.

A method was developed for the determination of zeranols (α,β-zearalanol,α,β-zearalenol,zear-alanone,zearalenone) with RRLC-MS/MS in the plant tissue. The samples were extracted with acetonitrile and reextracted with aqueous alkaline solution,cleaned up with MAX column and then determined by RRLC-MS/MS using multiple reaction monitoring ( MRM) scan mode. The results showed that the working curves were linear in the range of 0 -20 μg/kg. The limits of detection ( LOD) of zeranols were 0.5 μg/kg and the limits of quantification (LOQ) was 1. 0μg/kg. Extraction recoveries for zeranols ranged from 75. 8% to 105.4% and RSD was between 2.4% and 12.1%. The method is suitable for the determination of zeranols in the plant tissue.
