1.Understanding the development status of ophthalmic artificial intelligence, holding the challenges and opportunities

With the rapid development of artificial intelligence (AI), especially deep learning, AI research in the field of ophthalmology has presented a trend of diversification in disease types, generalization in scenarios and deepening in researches. The AI algorithm has showed a good performance in the studies of diabetic retinopathy, age-related macular degeneration, glaucoma and other ocular diseases, yielding up the great potential of ophthalmic AI. However, most studies are still in their infancy, and the application of ophthalmic AI still faces many challenges such as lack of interpretability for results, deficiency of data standardization, and insufficiency of clinical applicability. At the same time, it should also be noted that the development of multi-modal imaging, the innovation of digital technologies (such as 5G and the Internet of Things) and telemedicine, and the new discovery that retina status can reflect systemic diseases have brought new opportunities for the development of ophthalmic AI. Learn the current status of AI research in the field of ophthalmology, grasp the new challenges and opportunities in its development process, successfully realizing the transformation of ophthalmic AI from research to practical application.
2.Clinical features and prognostic risk factors of candidemia in non-intensive care unit wards

Objective To investigate clinical features,pathogen distribution and prognostic risk factors of candidemia in patients from non-intensive care unit (ICU) wards.Methods Seventy two patients with blood culture-confirmed candidemia were admitted in non-ICU wards in the First Affiliated Hospital of Zhejiang University during January 1 st 2011 and January 31 th 2014.The clinical data of patients,including clinical features,pathogen distribution,treatment and prognosis were retrospectively analyzed.The risk factors for the infection and the fatality of patients were assessed by chi-square test,Fisher exact probability test and multivariate Logistic regression analysis.Results Among 72 patients with candidemia,29 (40.28%) were related to Candida albicans and 43 (59.72%) were related to non-Candida albicans.Univariate analysis showed that elderly people (x2 =6.109,P < 0.05),male (x2 =5.258,P < 0.05),solid tumor (x2 =7.186,P < 0.01),parenteral nutrition (x2 =6.512,P < 0.05),surgery within 30 d (x2 =4.284,P < 0.05),indwelling gastric tube (x2 =4.164,P < 0.05),and indwelling urethral catheter (x2 =3.952,P < 0.05) were risk factors of Candida albicans-related candidemia;while patients with hematologic malignancies (P < 0.0l) and neutropenia (P < 0.05) were prone to non-Candida albicansrelated candidemia.Fourteen cases died within 28 days from the diagnosis of candidemia with an overall 28-d fatality rate of 19.44%.Multivariate Logistic regression analysis indicated that septic shock(OR:84.818,95% confidential interval:3.560-2020.567,P < 0.01)and acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ) score (OR:1.252,95% confidential interval:1.035-1.515,P < 0.05) were independent risk factors of death,while removal or replacement of central venous catheter (OR:0.002,95 % confidential interval:0-0.135,P < 0.01) was a protective factor.Conclusions There are more non-Candida albicans-related candidemia than Candida albicans-related candidemia in non-ICU wards.Patients with high APACHE Ⅱ score and septic shock usually have poor prognosis,while removal or replacement of central venous catheter may improve the prognosis.
3.Diagnosis of fetal truncus artery by echocardiography

Objective To investigate the clinical value of fetal echocardiography in prenatal diagnosis of fetal truncus artery. Methods Retrospective analysis was made in 10 cases with fetal truncus artery for their fetal echocardiography and follow-up findings. Echocardiographic characteristics was summed up in various fetal heart views. Results Truncus artery was confirmed by autopsy in 7 fetuses,and 3 cases were correspondingly verified by postnatal echocardiography. Echocardiographic characteristic images of fetal truncus artery was larger ventricular septal defect, only 1 great artery arising from heart, and pulmonary artery arising from truncus artery. Conclusions Echocardiography has important value in accurate diagnosis of fetal truncus artery and caution should be paid in differential diagnosis with other fetal complicated congenital heart diseases.
4.Individual influencing factors of the normal adult kidney size

Objective To explore the relationship between the size of the kidney and gender,age,height,weight,waist circumference then derive an estimation formula of a normal kidney size for different people.Methods We investigated 1 000 normal cases who accepted the examination in Tianjin Medical University General Hospital from December 2011 to April 2012,including 462 males,538 females,aged 21-78 years.All the investigated subjects were healthy except for hypertension,diabetes,coronary heart disease.Blood urea nitrogen (BUN) and creatinine (Cr),fasting glucose,uric acid,routine urine test were all in the normal range.Height,weight,and waist circumference were measured for all the subjects.The renal length and transverse diameter in supine coronal sections,anteroposterior diameter in vertical cross-section of the renal hilum were measured by the same technical experts with Philip iU22 C5-1,3.5 MHz convex array probe.Results The right and left kidney size both are significantly related to height (right r=0.845,left r=0.876,P<0.01).By multiple regression analysis,there was a significant association between height,weight,body surface area and kidney size (R2>0.5).Linear regression formula for the kidney length and the height(H,cm):Kidney length of men:right 0.059×H+0.144; left:0.061 ×H+0.287.Kidney length of women:right 0.039×H+3.679; left:0.035×H+4.454.Regardless of gender,the formula of left kidney length:0.052× H+0.721.Linear regression formula about the left kidney length and the height,body weight and body surface area (unit:H cm,W kg,BSA m2):0.114×H+0.139×W-10.287×BSA+2.112.Conclusion There is the best correlation between kidney length and height.Height,weight,body surface area have great influence on kidney size.
5.Effect of Fuxin Tablet on Thrombus Formation in the Arteries in Rats and Aggregation of the Blood Platelet in Rabbits

Objective To investigate the effect of Fuxin tablet on thrombus formation in the arteries in rats and aggregation of platelet in rabbits. Methods To observe the effect of OT in the arteries through electric stimulation of common carotid artery. To observe the aggregation of platelet by using the PRP of the rabbit and ADP as the accelerant. PAG-1, PAG-3, PAG-5 and PAG-M were examined in the normal saline group and high-dose, middle-dose, low-dose groups of Fuxin tablet. Results High-dose and middle-dose groups of Fuxin tablet could prolong OT and decrease the aggregation of platelet. Conclusion Fuxin tablet could significantly inhibit the formation of thrombus in the arteries and maximal aggregation rate of platelet.
6.EFFECTS OF EXPERIMENTAL HYPERINSULINEMIA ON THE LEVELS OF THE MAIN APOLIPOPROTEINS AND ON THE PROCESS OF REVERSE CHOLESTEROL TRANSPORT IN RABBIT

This paper presented the effects of experimental hyperinsulinemia on the main apolipoproteins (At, B) and on tbe process of reverse cholesterol transport in rabbit, which consisted of 1) uptake of cholesterol from peripheral tissue by HDL; 2) cholesterol esterification and 3) cholesterol ester transfer. The results showed that insulin could significantly decrease both serum and lymph Apo A1 levels and inhibit all the three steps of reverse cholesterol transport. The larger the dose of insulin, the greater the effects. Apo B did not seem to be influenced by insulin. It was further demonstrated by a series of statistical analyses that insulin had both direct and indirect inhibitory actions, to different extent, on each of the three main steps of reverse cholesterol transport.
7.Influence of microsurgical operation through lateral fissure-insula lobe approach in the treatment of hyperten-sive cerebral hemorrhage in basal ganglia region

Objective To explore the impact of Microsurgical operation through lateral fissure-insula lobe approach in the treatment of hypertensive cerebral hemorrhage in basal ganglia region on the clinical curative effect.Methods Selected 90 cases of patients with hyper-tensive cerebral hemorrhage in basal ganglia region in our hospital from April 2013 to December 2015.According to the random number table method,they were divided into the observation group and the control group.Regarded the even numbers as the observation group while the odd numbers as the control group,with 45 cases in each group.Patients of the two groups were all performed general anaesthesia with tracheal in-tubation.And patients of the control group were given large trauma craniotomy,while patients of the observation group were treated by micro-surgical operation through lateral fissure-insula lobe approach.Recorded the situation of surgery and postoperative recovery of patients in the two groups.In addition,compared the postoperative complications and evaluated living conditions of the two groups through activity of daily living scale(ADL)half a year after surgery.Results The good rate of recovery in the observation group was 84.44% (38 /45),while it was 51.11%(23 /45)in the control group,and there was statistically significant difference between the two groups(χ2 =11.447,P =0.000).Be-sides,the mortality rate of the observation group was 0(0 /45)half a year after surgery,while it was 8.89%(4 /45)in the control group,and there was statistically significant difference between the two groups(χ2 =4.186,P =0.041).The operation time of observation group and the automatic opening time in the observation group were obviously shorter than that of the control group(P <0.05).Moreover,the hematoma clearance rate and GCS score of the observation group 1 week after operation were significantly higher than those of the control group (P <0.05).The incidence of postoperative complications of the observation group was 8.89%(4 /45),which was significantly lower than 31.11%(14 /45)in the control group,and there was significant difference(P <0.05).Conclusion It has many advantages such as shorter operative time,faster postoperative recovery,and higher hematoma clearance rate to apply microsurgical operation through lateral fissure-insula lobe approach in the treatment of hypertensive cerebral hemorrhage in basal ganglia region.Besides,it pays attention to protecting cerebral vascular of patients.The clinical treatment effect is ideal and safe.Therefore,it is worth popularizing in clinical application.
8.The effect of microwave ablation combined with dendritic cell tumor vaccine treatment on mouse mammary carcinoma and its immunological mechanism

Objective To investigate the therapeutic effects of microwave ablation combined with dendritic cell (DC) tumor vaccine on a murine breast cancer model and to explore its immunological mechanism.Methods Forty-eight Balb/c mice were used to establish models of mouse mammary carcinoma,and then randomly divided into a sham-operation group,a microwave ablation treated group,a DC tumor vaccinated group and a microwave ablation plus DC tumor vaccinated group.Microwave ablation was conducted with the microwave ablation group on day 14 after establishing the model.DC tumor vaccination was performed on day 7 in the vaccinated group.In the microwave ablation plus DC tumor vaccinated group,DC tumor vaccination and microwave ablation were consecutively performed on days 7 and 14,respectively.Tumor growth,lung metastasis and survival were observed.The proportion of CD3+ and CD4+ T cells in splenocytes were analyzed by fluorescence Activated Cell Sorter (FACS) on the 14th day after microwave ablation.Serum levels of interferon-γ (IFN-γ) and interleukin-4 (IL-4) were also detected using ELISA.Results The survival time (56.4 ± 4.3 days),tumor size (67.3 ± 6.1 mm2) and lung metastasis index (0.6 ± 0.4) in the microwave ablation plus DC tumor vaccinated group were all significantly lower than the corresponding averages in the microwave ablation and DC tumor vaccinated group.The proportions of splenic CD3 + T cells (27.8 ±10.3 %) and CD4+ T cells (16.7 ± 8.3 %) in the microwave ablation plus DC tumor vaccinated group were significantly larger than in the control group.Average serum levels of IFN-γ (41.8 ± 22.9 pg/ml) in the microwave ablation plus DC tumor vaccinated group were also significantly larger than in the control group,while the average serum level of IL-4 (15.4± 6.4 pg/ml) was significantly lower.Conclusions Microwave ablation and DC tumor vaccination together could have synergistic effects in treating experimental breast cancer,which might be closely related to the enhanced immune response.
