1.Biomechanical changes after implant fixation for femoral condyle fracture

BACKGROUND:Clinical treatment of epicondyle fractures and intercondylar comminuted fractures is quite tricky due to instability and spread to the articular surface. Common complications contained bone delayed union, nonunion, broken nails and broken boards. Internal fixation for fractures is various, but reasonable choice for clinical fixation plays a decisive role for repair of fractures.
OBJECTIVE:To explore the repair effects of various fixation methods on femoral condyle fracture from different aspects such as fracture type, bone healing, functional recovery and biomechanics.
METHODS:First author searched PubMed database and China National Knowledge Infrastructure for articles about various fixation methods in repair of femoral condyle fracture published from January 2000 to April 2014. Key words were“femoral condyle fracture, internal fixation, biomechanics, load-displacement, axial stiffness, horizontal shear stiffness”. Total y 142 articles were retrieved, but 39 articles met the inclusion criteria.
RESULTS AND CONCLUSION:Femoral condyle fracture caused the damage to normal anatomic structure of knee join, changed normal anatomical axis and the mechanical axis of knee joint. Therefore, anatomic reduction and rigid internal fixation are necessary. During treatment, the advantages and disadvantages of various fixation methods should be known. It is necessary to ful y evaluate the type of intercondylar fractures of the femoral condyle and soft tissue injury. According to biological characteristics of the human body and mechanical property, internal fixation device should be reasonably used. Individual treatment programs should be provided. Thus, the occurrence of postoperative complications should be reduced utmostly, resulting in a satisfactory repair outcome.

3.Denoising of Fetal Heart Sound Based on Empirical Mode Decomposition Method.

Fetal heart sound is nonlinear and non-stationary, which contains a lot of noise when it is colleced, so the denoising method is important. We proposed a new denoising method in our study. Firstly, we chose the preprocessing of low-pass filter with a cutoff frequency of 200 Hz and the resampling. Secondly, we decomposed the signal based on empirical mode decomposition method (EMD) of Hilbert-Huang transform, then denoised some selected target components with wavelet soft threshold adaptive noise cancellation algorithm. Finally we got the clean fetal heart sound by combining the target components. In the EMD, we used a mask signal to eliminate the mode mixing problem, used mirroring extension method to eliminate the end effect, and referenced the stopping rule from the research of Rilling. This method eliminated the baseline drift and noise at once. To compare with wavelet transform (WT), mathematical morphology (MM) and the Fourier transform (FT), the SNR was improved obviously, and the RMSE was the minimum, which could satisfy the need of the practical application.

4.Realization of Heart Sound Envelope Extraction Implemented on LabVIEW Based on Hilbert-Huang Transform.

We proposed a research of a heart sound envelope extraction system in this paper. The system was implemented on LabVIEW based on the Hilbert-Huang transform (HHT). We firstly used the sound card to collect the heart sound, and then implemented the complete system program of signal acquisition, pretreatment and envelope extraction on LabVIEW based on the theory of HHT. Finally, we used a case to prove that the system could collect heart sound, preprocess and extract the envelope easily. The system was better to retain and show the characteristics of heart sound envelope, and its program and methods were important to other researches, such as those on the vibration and voice, etc.

5.Dynamic change of cystathionine β-synthase during cerebral ischemia-reperfusion and its effect in rats

Objective:To observe the dynamic change of cystathionine β-synthase during cerebral ischemia-reperfusion and its effect in rats.Methods:The ischemic model was established with line embolism to block the middle cerebral artery .The reverse tran-scription-polymerase chain reaction ( RT-PCR) and Western blot assay were used to assess the expression of cystathionine β-synthase (CBS) in SHAM group,I group,and IR group.ELISA assay was performed to detected the homocysteine (HCY) level in plasma.After treating with the inhibitor of cystathionine β-synthase called hydroxyla mine(HA),the expression of hemeoxygenase 1(HO-1) and the pathologic change of the brain was evaluated .Results:As compared to sham group ,the expression of CBS was significantly up-regulated in ischemia-reperfusion group at 12 h post-reperfusion.Meanwhile,it existed the lowest level of HCY at 12 h post-reperfusion,comparing to sham grouzp ( 5.73 ±1.17 vs 2.88 ±0.93 , F=25.56 , P=0.001 ) .When inhibited the activity of CBS via using HA , the down-regulation of HO-1 protein and further damage in neuron were observed .Conclusion:Cystathionine β-synthase serves as an protective factor during cerebral ischemia-reperfusion.
6.Clinical features and micro-invasive reshaping repair of calcified epithelioma in head and face

Objective Calcified epithelioma is a rare benign tumor in head and face. It is seldomly seen canceration. But it has local invasion behavior. The aim of this study was to explore the methodology in the diagnosis and treatment of calcified epithelioma in head and face, and to improve the accuracy of clinical diagnosis. Methods A retrospective analysis was carried out in 26 patients with calcified epithelioma in head and face. Results 53. 8 % patients with calcified epithelioma were less than 13-year-old. The disease was easily misdiagnozed. In this group of patients the misdiagnosis rate reached to 42. 3 %. Btype ultrasonographic diagnosis possessed a high level of concordance with histological diagnosis. There were three major types of calcified epithelioma: type 1, a mass with internal echoic foci; type 2, a hyperechoic mass with high beam; type 3, no calcification in the parenchyma. Piomatricoma was described as a skin tumour histologically composed of shadowcells, basophillic cells, foreign body giant cells and intracellular and stromal calcification. The treatment was excision and no recurrences were found by followup. Conclusions It is important for clinicians to know the clinical features of calcified epithelioma. Imaging studies including ultrasonography would help establish the diagnosis. Surgical excision with clear margins is recommended for this tumor.
7.Synergistic Killing Action of Suicide Gene System Combined with Medicated Serum of Liuwei Dihuang Bolus on Hepatoma Cells

Objective To investigate the possibility of establish a combined regimen of Chinese medicine and suicide gene therapy,we observed the killing action of medicated serum of Liuwei Dihuang Bolus (LDB)combined with HSV-tk/GCV suicide gene therapy system on rats hepatoma cell line CBRH7919.Methods The HSV-tk/GCV suicide gene therapy system was constructed and the working solution of ganciclovir (GCV)at 39.2 ?mol/L was detected firstly.Then medicated or non-medicated serum was prepared from SD rats treated with or without LDB by gavage.Meanwhile,the effects of medicated and non-medicated serum on cell proliferation were detected.The control groups without serum added were blank control group,GCV group and suicide gene therapy (SGT)group.Blank serum groups,blank serum + GCV groups,blank serum + SGT groups,medicated serum groups,medicated serum+GCV groups and medicated serum+SGT groups all had two serum concentrations of 5% and 7.5%,respectively.Six double holes were set for each group.CBRH7919/ tk+ and CBRH7919/ tk-were mixed together,and the tk+ cell percentage was adjusted to 0%,5% and 10%,respectively.Then the mixture of CBRH7919/ tk+ and CBRH7919/ tk-was implanted into the 96-hole plates at a density of 3?103 cells/hole,cultured with complete medium for 24 hours,and then treated with medicated or non-medicated serum for 12 hours and sequentially with GCV at 39.2?mol/L for another 60 hours.The number of surrival cells was determined by MTT assay.Q value (the ratio of measured and theoretical pharmacological action)was used to analyze the synergism of Chinese medicine and suicide gene therapy:additive action when 0.85≤Q1.15.Results No cytotoxicity was found when blank serum or medicated serum was below the concentration of 20% (volume fraction).When the concentration of serum was at 5% and 7.5%,the killing action of SGT + medicated serum group was stronger and the cell survival rate was higher those that of medicated serum group and SGT + blank serum group (P1.15).Conclusion HSV-tk/GCV suicide gene therapy system combined with LDB has a synergistic killing action on rats hepatoma cells.
8.Hepatic resection for huge primary liver carcinoma

Objective To evaluate the safety and feasibility of hepatic resection for huge primary liver carcinoma (PLC). Methods 216 cases of huge PLCs(mean diameter of 14.2cm) were resected. The hepatectomies were performed under intermittent occlusion of hepatic inflow. Results All 216 cases were successfully resected. The mean time of occlusion of hepatic inflow was 19min, the mean blood loss was 743 ml. No serious complications occurred, and only seven patients died of hepatic failure and upper gastrointestinal haemorrhage postoperatively in this series. Conclusions Although resection of huge PLC is quite difficult, but if suitable surgical techique and perioperative management are adopted ,it is safe and feasible .
9.Food-Grade Nisin Controlled Gene Expression System NICE of Lactic Acid Bacteria

Lactic acid bacteria (LAB) are traditional dairy strains which have a long history of safe use. They are a kind of food-grade microorganisms that can be used in industry to produce lots of food and drink, such as yoghourt. In the last two decades the physiology and genetics of these bacteria have been thoroughly studied. Because of their genetic accessibility and easy to handle, LAB, in addition to their traditional applications, have been extensively developed and used for the expression of heterologous genes. So they have an important foreground in agricultural, medical and many other fields. People have developed a series of food-grade gene expression systems of LAB. LAB, especially their model strain Lactococcus lactis and their most useful food-grade induced expression system--nisin controlled gene expression system NICE and its food-grade inducer nisin, food-grade host, expressing vectors and applications in expressing heterologous proteins of NICE will be introduced.
10.Ultrasonographic characteristics of ovarian yolk sac tumor

Objective To investigate ultrasonographic features of primary and metastatic ovarian yolk sac tumors.Methods Ultrasonographic features of 19 primary lesions and 33 metastatic lesions in 35 patients were retrospectively analyzed.Results Primary tumors were sized (14.6±3.6)cm in maximum diameter,manifesting as cysti-solid masses.Solid components of primary tumors were mainly hypoechoic or isoechoic(16/19)with rich blood supplies.Thirty-three metastatic lesions were located in pelvoceliac cavity(26/33) and liver parenchyma(7/33),sized (9.4±4.5)cm,(9.2±4.9)cm and (5.6±1.6)cm in maximum diameter respectively.Metastatic lesions in pelvoceliac lesions mainly demonstrated as hypoechoic masses(21/26), however lesions in the liver were mainly hyperechoic(5/7).Anechoic regions could be found in 9/26 of the pelvoceliac lesions.Blood supply was found less rich in metastatic masses than that in primary ones.Elevated serum level of alpha fetoprotein (AFP) was observed in all patients; ranging from 217 to 211 682 μg/L.Conclusions Primary and metastatic lesions of ovarian yolk sac tumor have obvious ultrasonographic characteristics.Combined with serum AFP level,the accuracy of diagnosis could be improved.
