2.Surgical treatment of the cervical spine fractures combined with ankylosing spondylitis

Objective To study effects and other related problems of surgery for patients with cervical spine fracture and ankylosing spondylitis.Methods Twelve patients with cervical spine fracture and ankylosing spondylitis were treated with surgery from April 1986 to April 2004.All eases were studied retrospectively.They were followed up for an average of 67.5 months and their complete clinical data were kept.The neurological function was evaluated by ASIA (American Spinal Injury Association) impairment scale,and the image analyzing software (Image-Pro Plus 5.1) was used to measure the angles of superior and inferior vertebral bodies of the fusion segment at flexion and extension positions.The difference between flexion and extension angles,?,served as the parameter of interspinal movement.According to the definition of spinal fusion by FDA (Food and Drug Administration),the?≥4?was considered as nonfusion.Other related problems were discussed by descriptive study.Results The average improvement in the nine patients with neurological injury was 1.3 ASIA grades.The injured segments in 10 cases were treated with fusion,of whom nine were fused by internal fixation.The fusion rate was 100%.Three cases were scheduled to have laminoplasty,but two had to receive laminectomy instead because of intraoperative complete fracture at the hinged side.Ten patients were complicated by insufficient function of major organs preop- eratively,and endotracheal intubation was difficult to perform in eight cases.Postoperative complications occurred in three cases,but fortunately healed completely.There were no deaths or fatal complications.Conclusions The neurological function can be improved by surgery for patients with cervical spine fracture and ankylosing spondylitis. Although the cervical spine is instable for most of the patients,fusion with internal fixation is indicated and can be successful.Those who had preoperative systemic diseases are likely to suffer from postoperative complications.The difficult endotracheal intubation is a common intraoperative problem.

Objective To investigate the oxidative stress,microinflammation and their relationship in patients with MHD(maintenance hemodialysis)and to evaluate the effect and safety of vitamin E supplementation in patients with MHD.Methods Fifty-eight patients with MHD and 20 healthy volunteers were involved in this study.The patients were administered 400 mg/d of vitamin E for 6 weeks.The levels of oxidative vitamin E(Vit E),malondialdehyde(MDA),advanced oxidation protein product(AOPP),superoxide dismutase(SOD),activity of glutathione peroxidase(GSH-PX),microinflammation cytokines C-reactive protein(CRP),interleukin-6(IL-6),interleukin-10(IL-10) and tumor necrosis factor(TNF-?)in plasma were measured at the beginning and the end of administering Vit E.Results 400 mg/d of Vit E supplementation decreased the levels of plasma AOPP,MDA,CRP,TNF-?(P
4.A clinical analysis of 20 cases with bone marrow granulomas

Objective To study the clinical feature and original diseases of bone marrow granulomas. Methods A total of 5217 bone marrow biopsies were retrospecitively analyzed in Peking Union Medical College Hospital from January 2001 to December 2007. Results Bone marrow granulomas were present in 20 cases, representing an incidence of 0.38% in the series and an annual incidence of 2. 9 cases per year. Finally, 13 of these cases (65%) were diagnosed with tuberculosis. Hematological neoplasms and viral hepatitis were found in 4 and 2 cases respectively. Acute interstitial nephritis was diagnosed in one case. Conclusions The finding of a granuloma in a bone marrow biopsy is not common and unspecific.Tuberculosis, hematological neoplasms and viral hepatitis are the most common underlying diseases. Bone marrow biopsy is an important method for the diagnosis of disseminated tuberculosis.

Objective To study the threshold effect of government health expenditure on residents' health.Methods Using threshold panel data model to perform the empirical research.Results There is threshold effect of government health expenditure on residents' health.There are two threshold values.As the increase of the proportion of the government health expenditure to the total fiscal expenditure,it has two changes of the impact on the health of residents.Conclusion The government should increase the health expenditure,and increase the proportion of the government health expenditure to the total fiscal expenditure,but the government should not blindly pursue the high input of health expenditure,focus on the efficiency of health expenditure,and avoid of resource waste.
6.Introduction of “Bar Code Checking Program”for the Drug Dispensing in Our Hospital

OBJECTIVE:To reduce errors in drug dispensing so as to improve the accuracy of drug use.METHODS:The application of bar code technology in both domestic and foreign counties was overviewed,the flow-sheet and the chief function of the “bar code checking program”for the drug dispensing developed in our hospital were introduced.RESULTS &CONCLUSION:Connecting “bar code checking program”to hospital information system in the drug dispensing and adding machine judgment procedures can reduce the occurrence of error while improving quality of drugs dispensing.
7.ANTAGONISTIC EFFECT OF ROSA DAVURICA PALL JUICE ON CANCER

The influence of Rosa davurica pall juice on pulmonary adenoma induced by urethan was observed in the experiment, which lasted for 20 weeks. 250 mice were divided into 4 groups at random. Group A was set as control receiving basal diet and tap water only. Another 3 groups were given basal diet, however, group B was given Rosa davurica pall juice 0.2ml/20g boody weight once a week, group C 0.1% urethan solution for drinking freely and group D 0.1% urethan solution plus the juice. The mouss body weight was weighed once a week; the activities of superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px), the level of lipoperoxides (LPO), lymphocyte transformation and hemolysin were determined once a month, the pulmonary pathological examination was made also once a month. The obtained data were carried out with a 2?2 factorial analysis. 8.Case report of cervical intraspinal primitive neuroectodermal tumor and literature review

Objective To investigate the clinical characters and therapy of primitive neuroectodermal tumor(PNET).Methods A retrospective analysis was conducted.A 36-year-old female patient was showed pain and numbness of the right upper limb and back for 6 months.The cervical spine MRI showed a spindleshaped intradural mass right ventrolateral of spinal cord at C5-7 with in homogeneously enhancing.Surgery and pathologic examination confirmed that was PNET.Combiled with a series of literatures to analyse the clinical characters Results Surgery was performed to remove the tumor and decompression combined with radiotherapy.The pathologic examination and immunohistochemical analysis revealed that it was PNET.MRI identified local recurrence in spinal canal at 3 month later after surgery.Conclusion Spinal PNET is an uncommon intraspinal tumor with poor prognosis.Histopathology is the evidence of diagnosis.Optimal therapy has not yet been found.Surgical resection with the combination of chemo-radiotherapy or radiotherapy might get the better outcomes.Multidisciplinary treatment should be further clinical required.
9.Application of Point Zigong to Gynecological Diseases and Its Modern Theoretical Basis

Point Zigong is an empirical point for acupuncture treatment of gynecological diseases. Now it is widely applied to clinical treatment and has a marked therapeutic effect. The therapeutic effect of point Zigong stimulation on gynecological diseases is closely related to the anatomical position, morphostructure specificity and action effect of the acupoint. In the present study on this acupoint, the single acupoint is seldom applied and the mechanism is not completely clear. It is needed to conduct a thoroughstudy in the future.
10.Diagnosis,treatment,and prognostic analysis of thirty-one cases with primary bronchopulmonary carcinoid

Objective:We aimed to explore the clinical features, computed tomography (CT) findings, treatment, and prognosis of bronchopulmonary carcinoid. Methods:Clinical data of 31 patients with primary carcinoid tumor of the lung were retrospectively re-viewed. The prognostic factors were analyzed via Cox univariate and multivariate analyses. Results: Clinical symptoms included coughing or expectoration in 17 of the 31 cases, hemoptysis or blood-stained sputum in 7 cases, and chest pains or shortness of breath in 8 cases. Six cases were asymptomatic. The CT scans showed round or oval nodules with clear boundaries, and enhancement CT scans indicated mild, homogeneous enhancement. Immunohistochemistry results revealed the positive expression rates of synaptophy-sin (Syn), chromogranin A (CgA), and neuron-specific enolase (NSE) were 90.3%(28/31), 87.1%(27/31), and 90.3%(28/31), respec-tively. Therapy and prognosis results were as follows:28 of the total number of patients underwent surgery, among which 3 underwent postoperative adjuvant therapy, 2 received chemotherapy; and only 1 refused treatment. The 1-year overall survival rates were 100%(18/18) and 92.3%(12/13), whereas the 3-year survival rates were 94.4%(17/18) and 69.2%(9/13) in the typical and atypical carcinoid cases, respectively. Cox univariate analysis results revealed that lymphatic metastasis (P=0.02), tissue types (P=0.017), TNM stage (P=0.005), and therapies (P=0.01) were the prognostic factors. Cox multivariate analysis results showed that lymphatic metastasis (P=0.032) and tissue types (P=0.002) were the independent prognostic factors. Conclusion:Compared with other lung cancers, the bron-chopulmonary lung carcinoid has no special clinical manifestation in clinical and radiographic images. The diagnosis was mainly based on histopathology results. Surgery was the main and effective treatment, whereas chemotherapy and radiotherapy showed unsatisfactory results. The overall prognosis was satisfactory. However, the atypical carcinoid was inferior to the typical carcinoid in terms of progno-sis. Pathological typing and lymph node metastasis were significant prognostic factors.
