2.Nutrition Status of Premature Neonates in Early Postnatal Stage in Neonatal Intensive Care Unit

Objective To study the nutrition status of premature infants in the early postnatal stage.Methods According to the physical development standard,the premature neonates for treatment in our NICU from Jun.2007 to Jun.2009 were enrolled and divided into normal growth group and extrauterine growth restriction(EUGR) group.The nutrition status of these premature neonates were evaluated respectively.SPSS 13.0 software was used to analyze the data.Results Compared with EUGR group,the durations of regaining birth weight were shorter in normal growth group(t=-13.83,P0.05).Conclusions Premature neonates are prone to encounter extrauterine growth retardation.Extrauterine growth retardation had great impact on premature neonates.Premature neonates should be strengthened with nutrition supplement in order to reduce the incidence of extrauterine growth retardation.

4.Alpha 1-antitrypsin Matrix Attachment Region Enhances RNA PolymeraseⅡ-dependent Transcription

Eukaryotic DNA element called Matrix Attachment Regions (MARs) can function on regulating the structure and activity of chromosome. Traditional quantitation in vitro and indirect functional analysis can not always reflect MAR-involved physiological state. In order to study transcription regulation and make a try in methodism,? 1-antitrypsin MAR (?1-AT MAR) is cloned and incorporated into pEGFP-C1 vector. Non-MAR-containing and MAR-containing plasmids were then transfected into HEK-293 cells with LipofectamineTM 2000 respectively. Positive cell clones were assayed after 20 days of selection by G418. Semiquantitative RT-PCR and fluorescence microscope analysis show that this MAR has a positive effect on modulating nearby gene expression. Further, co-localization with newly CMV promoter and RNA polymeraseⅡ(RNAPⅡ) was detected by chromatin immunoprecipitation (ChIP), The PCR result demonstrates that more RNAPⅡwas recruited to the CMV promoter to initiate transcription in presence of MAR. ChIP can be used to confirm the MAR-mediated transcriptional activation and provide more reliable information than RT-PCR in real time. The technology is also providing a platform for our research in gene expression regulation.
5. Protective effect of human erythropoietin on acute renal injury caused by abdomen open injury plus artificial seawater immersion in rats

Objective: To investigate the protective effect of erythropoietin (EPO) on the acute renal injuries caused by abdomen open injury plus seawater immersion in rats. Methods: Sixty healthy male Wistar rats (clean grade) were evenly randomized into four groups,namely,EPO pre-treatment group,observation group,low-dose EPO treatment group and high-dose EPO treatment group. Acute renal injury was induced by abdomen open injury plus artificial seawater immersion (22°C). The serum creatine, BUN, creatine kinase, creatine kinase isoenzyme, TNF-α, IL-6, complement C3a, C-reactive protein, renal homogenate superoxide dismutase (SOD) , and the renal pathological changes were observed and compared between different groups. Results: Acute renal injury was observed in all groups 3 hours after abdomen open injury plus seawater immersion,with increased serum creatine and BUN,but the rats survived after treatment. The serum creatine,BUN,creatine kinase,and creatine kinase isoenzyme in EPO pre-treatment group were significantly lower than those of the other 3 groups; the levels of TNF-α,IL6, complement C3a ,and C-reactive protein were also obviously decreased; the renal homogenate SOD was obviously increased; and the score of renal proximal tubule necrosis was obviously decreased. However,no significant differences were found between the high- and low-dose EPO groups concerning all the parameters (P>0.05). Conclusion: EPO pre-treatment has a protective effect on the acute renal injury induced by abdomen open injury plus seawater immersion in rats.
6. Clinical observation on 125I seed brachytherapy combined with chemotherapy and thermotherapy for mid-advanced tumor

Objective: To investigate the clinical value of brachytherapy with radioactive 125I seed implantation combined with chemotherapy and thermotherapy for mid-advanced tumor. Methods: Twenty two patients with mid-advanced tumor were implantated radioactive 125I seed followed by chemotherapy and thermotherapy. Their clinical data was retrospectively reviewed and analyzed. Total 445 125I seeds of (1.48-2.59) × 107 Bq were implanted into 26 foci with the guidance of surgery or CT. Combined chemotherapy and thermotherapy was performed after brachytherapy. All the patients were followed up. The blood biochemical index, immune function and biological parameters of oncology and imaging changes were measured. The short-term efficacy was evaluated. Results: The oncological parameters significantly decreased 3 months after operations. There were 5 patients with complete response (CR), 12 with partial response (PR), 2 with no change (NC), 3 with progressive disease (PD). The objective response rate was 77.3% (17/22). Local foci control rate was 95.5% (21/22). One patient had pneumothorax during 125I seed implantation and the other patient had seed migration in lung. No other severe complications were observed. Conclusion: Radioactive 125I seed implantation combined with chemotherapy and thermotherapy has good clinical effects in treating mid-advanced tumor with low occurrence of complications. It is a simple, safe, and effective method for combined treatment of mid-advanced tumor.
8.Application of PDCA Cycle Management Method in Pharmacy Management of Three-level General Hospi-tals Re-accreditation in Our Hospital

OBJECTIVE:To investigate the application and feasibility of the PDCA cycle management method in pharmacy management of three-level general hospital re-accreditation in our hospital and promote continuous improvement of hospital pharma-cy management. METHODS:Referring to 2011 edition of Detailed Rules for Three-level General Hospital Accreditation,the work of hospital pharmacy management was checked and continually improved based on the PDCA cycle management method. RE-SULTS:After the application of PDCA,prescription error rate in outpatient pharmacy dropped from 0.0314% to 0.0192%;the number of dispense error in center pharmacy decreased by 60.2% and the accuracy rate of drug stock increased from 87.14% to 97.32%;the number of dispensing errors in pharmacy intravenous admixture service reduced from 5.6 bags/week to 1.8 bags/week, and the error rate reduced by 67.86%;the proportions of irrational prescriptions of glucocorticoid drugs dropped from 19.26% to 4.23%. CONCLUSIONS:The application of PDCA cycle management method in pharmacy management of three-level general hos-pital reaccreditation and the establishment of relevant system can obviously improve the work of pharmacy management continuously.
9.Therapeutic effect observation on acupuncture plus medication for post-stroke depression

Objective:To observe the clinical efficacy of acupuncture with medication for post-stroke depression.Methods:A total of 96 patients with post-stroke depression were randomized into an observation group and a control group,with 48 cases in each group.The control group was treated with oral fluoxetine hydrochloride capsules,and the observation group was treated with acupuncture on the basis of the treatment of the control group.After 6 months of treatment,the traditional Chinese medicine (TCM) symptom scores,and Hamilton depression scale (HAMD) component scores and total scores of the two groups before and after treatment were observed,and the clinical efficacy and adverse reaction rate were compared between the two groups.Results:The total effective rate was 95.8% in the observation group and 83.3% in the control group,and the difference between the two groups was statistically significant (P<0.05).After treatment,the intra-group differences in TCM symptom scores and HAMD score of both groups were statistically significant (all P<0.01).In the observation group,the scores of indifferent expression,emotional restlessness,and sentimentality in the TCM symptoms were statistically different from those in the control group (all P<0.01).The scores of desperate factor and sleep disorder factor and total score in HAMD in the observation group were significantly different from those in the control group (all P<0.01).The incidence of adverse reactions was 6.3% in the observation group and 4.2% in the control group,and the difference was not significant between the two groups (P>0.05).Conclusion:Acupuncture plus medication is an effective method for post-stroke depression,and it can further improve the patient's bad mood.
10.Contrast-enhanced ultrasound in assessing renal cortical and medullary perfusion in acute renal failure in rabbits

Objective To assess the application of contrast-enhanced ultrasound (CEUS) in evaluation of the renal cortical and medullary perfusion changes before and after acute renal failure (ARF) in rabbits. Methods Rabbit ARF models were established with intramuscular 50% glycerin (12-15 ml/kg) injection into rabbits' thighs. One day before and after ARF models establishment, CEUS was performed on each rabbit. The renal perfusion time-intensity curve (TIC) was analyzed, including parameters like AT (arrival time), TTP (time to peak intensity), A (amplitude of peak intensity), AUC (area under the curve), β (slope rate of TIC) of renal cortex and medulla. Results The value of A and β before model establishment was (17.36±13.73) dB and (5.38±2.08) dB/s, respectively, both was significantly higher than those after glycerin injection ([6.59±4.25] dB and [1.58±1.41] dB/s, respectively) (P<0.05). The value of TTP and AUC before model establishment was (2.46±1.76) s and (329.31±171.70) dBs, both was significantly lower than those after injection ([5.93±4.80] s and [722.28±354.14] dBs, respectively) (P<0.05). No significant difference was found in AT of renal cortex and AT, A, TTP, AUC, β of renal medulla, nor in SCr or BUN. Conclusion CEUS can display the changes of perfusion image of the renal cortex of the ARF earlier than SCr and BUN.
