1.Ribavirin combined with dioctahedral-smectite in treatment of rotavirus enteritis

Objective:To analyze the curative effect of ribavirin combined with dioctahedral-smectite(smecta,DS) on ro-tavirus enteritis. Methods:Totally 168 children with rotavirus enteritis, aged from 6 month to 2 years, were randomly divided into 4 groups (3 treatment groups and a control group). All cases were treated with liquid infusion and other supportive treatment. Group A(n = 41) was treated with ribavirin intravenous injection, group B(n = 40) was treated with DS orally, and group C(n = 45) was treated with ribavirin and DS. The effective rate, symptom amelioration time, average hospital stay and side effects were analyzed. Objective To explore the effect of preservation of inclined fractured permanent molars by transverse fixation with screw post,and provide a practical method for reserving the inclined fractured permanent molars.Methods Clinical data of 50 patients with inclined fractured permanent molars by transverse fixation with screw post in the Department of Stomatology of Tianjin First Central Hospital between January 2010 to December 2011 were reviewed retrospectively.After fixing the crown with steel-wire binding and root canal treatment,the molars were drilled a hole buccolingually and placed screw post.The flowable light-curing composite resin was used to seal the crack and bond screw post to prevent the relative movement.All treated molars were restored with full crown.All patients were followed up for 10-60 months.The therapeutic effects were observed.Results After 5 year-follow-up,41 cases were cured completely,6 cases were cured clinically,and 3 cases failed treatment.Conclusion The transverse fixation with screw post and bonding technique can preserve the inclined fractured molars to gain chewing function and avoid or reduce the extraction of affected teeth,which provides a better restoration effect.

Objective To investigate the efficacy of relaparoscopic common bile duct exploration in choledocholithiasis.Methods 50 patients who underwent LC/OC/LCBDE/OCBDE for biliary surgery with choledocholithiasis were randomized into two groups:Group A (n =25) laparoscopic approach and Group B (n =25) open approach.The operation time,hospital stay,cost of hospitalization and postoperative complications were compared.Results There was no significant difference in the operation time,liver functional index,postoperative bile leakage rate and cost of hospitalization between the two groups.The postoperative hospital stay in group A was shorter than that in group B (7.1 ± 1.5 vs 12.4 ±4.3 days,P <0.05),as was the volume of intraoperative blood loss (58.3 ± 24.2 ml vs 108.6 ± 35.7 ml,P < 0.05),recovery of gastrointestinal function (26.3 ±3.6 vs 58.2 ±6.3 hours,P <0.05),postoperative analgesia (7/25 vs 17/25,P<0.05) and wound infection rate (1/25 vs 6/25,P<0.05).Conclusions Relaparoscopic commonbile duct exploration for recurrent choledocholithiasis appeared to be a safe,feasible,and efficacious procedure when carried out by expert laparoscopic surgeons.The procedure is worth promoting.
6.Primary closure of common bile duct following laparoscopic reexploration and choledocholithotomy in 31 patients with a history of biliary tract operation

Objective To investigate the feasibility and safety of primary closure of common bile duct following laparoscopic common bile duct reexploration and choledocholithotomy for recurrent or residual choledocholithiasis in patients with prior biliary surgery.Methods We retrospectively studied the clinical data of 31 patients who were diagnosed recurrent or residual common bile duct (CBD) stones after prior bile duct surgery.All of 31 patients had undergone primary closure following laparoscopic common bile duct reexploration between Jan 2010 and Jan 2014.Results In all the 31 patients,there was no conversion to open surgery.There were no complications due to port placement,postoperative bleeding and mortality.The operation time was (106 ±32) min; the volume of intraoperative blood loss was (52 ± 16) ml; the time of postoperative gastrointestinal function recovery and postoperative hospital stay were (23 ± 3) h,(5.5 ± 1.5) d respectively.Duodenal injury was found and repaired intraoperatively laparoscopically in one patient.Bile leakage occurred in 2 cases recovered by conservative therapy.No postoperative stricture of bile duct,recurrent or residual stones were observed during a period of 1 month to 1 year (mean follow-up 6 months).Conclusions Primary closure following laparoscopic common bile duct reexploration for patients with a past history of biliary tract operation is safe,feasible,and effective procedure in hands of experienced laparoscopic surgeons.
7.Implementation and management of pharmacy intravenous admixture services

Objective:To evaluate the common nursing problems in pharmacy intravenous admixture services(PIVAS).Methods:By strengthening nursing staff training,nurses were required to carry out checking system strictly,use the super-clean work platform collectly and prevent minute granule contamination.Results:Among 170 000 pack of intravenous liquid prepared in PIVAS in the last six months,no transfusion reaction occurred.Medical cost decreased and nursing occupation protection was strengthened.Conclusion:Strict Personal and system management can guarantee the effectiveness of PIVAS.
8.Fragmentation pathways of five estrogens using electrospray ionization quadrupole time-of-flight mass spectrometry.

The fragmentation pathways of five estrogens (estradiol, estrone, equilin sulfate, 17 a-dihydroequilin sulfate and equilenin sulfate) have been studied with high resolution and high mass accuracy using electrospray ionization quadrupole time-of-flight mass spectrometry (ESI-Q-TOF/MS) in the negative ion mode. Molecular weights were obtained from [M-H](-) ions in the product ion spectra. The results indicate that the five structurally similar estrogens have similar fragmentation pathways. Using their stable isotope forms as internal reference compounds, the accurate mass and composition of the fragment ions were determined. During collision-induced dissociation (CID), cleavage is initiated by loss of oxygen atoms from carbon-17, after which D and C rings cleave sequentially and rearrange to finally form stable conjugate structures with highly abundant characteristic fragment ions at m/z 183 (accompanied by m/z 181), m/z 169 and m/z 145 (accompanied by m/z 143). Understanding these characteristic fragmentation pathways of estrogens will be helpful in identifying the structures of steroid hormones in general.

9.Sectional anatomy at any orientation of the pancreas and its surrounding structures

Objective To establish the three-dimensional model of pancreas and its surrounding structures.Methods The three-dimensional pancreas model was established and analyzed by using PC computer platform based on the data set from the first Chinese visible human female.Results The three-dimensional pancreas model,established successfully,could be rotated at any angles and could be sectioned at any orientation.The structures of each section could be displayed clearly.Conclusion Sectional anatomy at any orientation of the pancreas and its surrounding structures can be performed on our pancreas model,which has laid the foundation for virtual surgery of the pancreas.
10.Three-dimensional reconstruction of pancreas and its adjacent structures

Objective To build three-dimensional model of the pancreas and peripanercatic organs to provide morphological basis for imaging diagnosis and operation selection of pancreas diseases. Methods Serial cross-sectional images from the first Chinese visible human dataset were reviewed and the structures of pancreas and peripancreatic organs were reconstructed three-dimensionally by using 3D-Doctor software on PC. Results Three-dimensional structures of pancreas and peripancreatic organs were reconstructed successfully. All reconstructed structures could be displayed in multiple methods and color modes. Conclusions The internal and adjacent struc-tures of pancreas can be clearly shown on the reconstructed three-dimensional images. The visible model is a useful reference both for clinical diagnosis and surgical practice.
