1.The application of low dose CT with automatic tube current modulation in the diagnosis of pneumoconiosis

Objective To explore the feasibility and application value of low dose CT with automatic tube current modulation (ATCM) in the diagnosis of pneumoconiosis.Methods Forty-five patients with pneumoconiosis confirmed by the occupational agency underwent standard dose CT scanning with a fixed tube current of 150 mA and low dose CT scanning with ATCM (50-200 mA) with noise index (NI) of 14.The CT dose index volume (CTDIvol) and dose length product (DLP) under the different scanning condition were recorded.The image quality,specific image manifestations of pneumoconiosis (shadow size,shadow concentration,pneumoconiosis stages) were reviewed in double-blind methods by two radiologists,and the statistical analysis was performed.Results CT characteristics of pneumoconiosis could be detected efficiently with low dose CT scanning with ATCM.There were no significant differences in showing the shadow size,shadow concentration,pneumoconiosis stages and the image quality between the two groups (P>0.05).The CTDIvol and DLP of standard dose CT scanning were 13.53 mGy and (337.13 ± 13.53) mGy/cm respectively,and CTDIvol and DLP of low dose CT scanning with ATCM were (7.39 ± 0.45) mGy and (188.78 ± 1.80) mGy/cm,there were statistical differences (P < 0.05).Conclusions Low dose CT scanning with ATCM can reflect the characteristics of pneumoconiosis,and it also can significantly reduce the radiation dose with diagnostic acceptability of the image quality.So low dose CT scanning with ATCM can replace the standard dose CT scan in diagnosing and screening of the pneumoconiosis.
2.Study on the effect of collaborative care on job burnout and turnover intention among township hospital phy-sicians

Objective To learn the burnout and turnover intention of physicians from township hospitals in Xi County,Henan province,for reference of health administrators and healthcare institution managers.Methods Cluster sampling method and qualitative interviews were used and all physicians from such hospitals were sampled for questionnaire survey in 2014,with 1 70 scales distributed.The questionnaire was the Markov Burnout Inventory generic version revised by Li Chaoping,and turnover intention questionnaire was that revised by Liang Kaiguang.Results 140 physicians from such hospitals were found with zero or low burnout (94.6%);Collaborative care in the county poses great positive effect on promoting professionalism and interpersonal relations, yet has limited positive impact on reducing workload and performance appraisal stress and increasing their income.Conclusion It is imperative to enhance training of managers of such hospitals for them to learn the appraisal method with organizational scale,and build a humanistic concern mechanism for physicians,for purposes of improving the organizational environment and minimize the burnout and turnover intention of the physicians.
3.Cloning and expression analysis of HMG-CoA reductase from Aquilaria sinensis (Lour.) Gilg.

3-Hydroxy-3-methylglutaryl-coenzyme A reductase (HMGR) is the first rate-limiting enzyme for sesquiterpene synthesis in the mevalonate (MVA) pathway. The specific primers were designed according to the transcript sequence of AsHMGR2 from the Aquilaria sinensis (Lour.) Gilg transcriptome database. The full-length cDNA of AsHMGR2 was cloned by RT-PCR and rapid amplification of cDNA ends (RACE) technology, and was analyzed at bioinformatics levels; AsHMGR2 expression profiles in different tissues and in responds to different treatments were analyzed by real-time PCR. The length of AsHMGR2 Open Reading Frame (ORF) was 1 749 bp, encoding 582 amino acids. The GenBank accession number is KC140287. Tissue expression analysis indicated that AsHMGR2 was mainly expressed in root and shoot tips, followed by stem, and was lowest in leaves. Inducible-experiments showed that the genes were induced by mechanical wound as well as chemical liquid induction, and reached the highest expression level at 6 h and 8 h, separately. The full-length cDNA of AsHMGR2 and its expression patterns will provide a foundation for further research on its function in agarwood sesquiterpene biosynthesis.
4.Short-term impact of fast track surgery combined with minimally invasive surgery on elderly patients with colorectal cancer

Objective To analyze the short-term impact of fast track surgery (FTS) combined with minimally inva-sive surgery on elderly patients with colorectal cancer. Methods 200 patients with colorectal cancer received laparo-scopic surgery from July 2013 to July 2015 were selected and randomly divided into control group and FTS group. Patients in control group received conventional perioperative treatment and patients in FTS group received perioper-ative management according to fast track surgery. Status of intraoperative, postoperative recovery and the postopera-tive complication rate were compared between the two groups. Patients' quality of life were analyzed after two weeks of operation. Results Compared with the control group, the blood loss, operation time and number of lymph node dissection of patients in FTS group showed no significant difference ( > 0.05). Flatus time, ambulation time, start eating time, the infusion time and length of hospital stay in FTS group was significantly shorter than the control group, the difference was statistically significant ( <0.05). The total cost of hospitalization of FTS group was signifi-cantly lower than the control group ( <0.05). The incidence of sore throat, abdominal distension, lung infections and overall complications incidence rate in FTS group were significantly lower than the control group, the difference was statistically significant ( <0.05). The satisfaction rate in FTS group was 96.0%(96/100), and was significantly high-er than the control group 82.0 %(82/100), the difference was statistically significant ( < 0.05). QLQ-C30 results showed that scores of overall health, emotional function and social function of FTS group were significantly higher than the control group ( <0.05), and scores of nausea and vomiting, pain, insomnia, anorexia, constipation were sig-nificantly lower than the control group ( < 0.05). Conclusion Laparoscopy combined with FTS is safe and effective in treatment of colorectal cancer in the elderly patients, which can promote postoperative rehabilitation, improve pa-tient satisfaction and quality of life in short term.
5.The impact factors analysis of hospitalized patients with coronary heart disease accompanying depression disorders

Objective To explore the status and impact factors of hospitalized patients with coronary heart disease(CHD) accompanying depression disorders.Methods Adopt HRSD,SDS and general questionnaire for survey tools to evaluate the 300 hospitalized patients with CHD.Using SPSS13.0 software to build database,and the data was analyzed by descriptive statistics analysis and correlation analysis.Result There were 148 people accompanying depression disorders among the hospitalized·patients with CHD(47.30% ),and 68 people were treated (47.89%).Multivariate logistic regression indicated that the age ( OR =-0.415,P < 0.05,CI =0.443 ~0.984),smoking history( OR =0.384,P < 0.05,CI =1.118 ~ 1.928 ),course of disease ( OR =-0.250,P <0.05,CI =0.608 ~ 0.996),myocardial infarction ( OR =0.676,P < 0.05,CI =1.082 ~ 3.576 ),family history ( OR =-0.744,P < 0.05,CI =0.231 ~ 0.978 ) were related to coronary artery disease accompanying depression disorders.Multivariate correlation analysis indicated that anxiety factor and gender were closely related,weight factor and age,educational level,myocardial infarction had closed relationship,cognitive factors were closely associated with the course of disease,day and night changes were in relation to age,block factor and gender,age,education,drinking history,smoking history were related closely,sleep factor and gender,onset form,hypertension relations closed,despair factor have closed relationship with gender,age,educational level,course of disease.Conclusion Age,smoking history,course of disease,myocardial infarction,family history were important risk factors of depression disorders in hospitalized patients with CHD.The HRSD's seven symptoms factors are closely associated with multipling influence factors.
6.The reliability and validity of the Chinese version of a quality of life questionnaire for traumatic brain injury

Objective To study the reliability and validity of the Chinese version of an instrument for assessing quality of life after traumatic brain injury (QOLIBRI).Methods A total of 49 patients with traumatic brain injury were studied.The test-retest reliability of the instrument was examined by computing intraclass correlation coefficients and its internal consistency was examined using Cronbach's α statistic.Its criterion-related validity was examined using reference instruments such as the HADS,SF-36 and Glasgow outcome scale-extended (GOSE).The Kaiser-Meyer-Olkin measure and Barlett's test of sphericity as well as factor analysis were used for construct validity testing.Results The test-retest reliability of the QOLIBRI subscales was stable,with Pearson's correlation coefficients ranging from 0.906 to 0.987.Good internal consistency was demonstrated by the Cronbach's αs ranging from 0.498 to 0.921.The Kaiser-Meyer-Olkin measure of QOLIBRI was 0.813.Factor analysis yielded four factors,and their cumulative contribution was 68.4%.The QOLIBRI scores were highly negatively correlated with HADS scores,moderately correlated with GOSE scores and positively correlated to SF-36 scores.Conclusions The Chinese QOLIBRI instrument has good reliability and validity.
7.Effects of early hyperbaric oxygen treatment combined with mild hypothermia treatment on patients with severe traumatic brain injury

Objective To investigate the clinical effects of early hyperbaric oxygen (HBO) treatment combined with mild hypothermia treatment on patients with severe traumatic brain injury (TBI).Methods A total of 45 participants with severe TBI were randomly divided into combination group (15 cases),mild hypothermia group (15 cases) and control group (15 cases).In control group the patients were managed with treatments for reducing the intracranial pressure (ICP) and controlling the hemorrhage and gastric acid,and with administration of neurotrophic treatment and nutritional support.In addition to above mentioned interventions,the patients in mild hypothermia group received mild hypothermia treatment; while those in the combination group received mild hypothermia plus HBO treatment.The scores of Glasgow coma scale (GCS) were measured before and after treatment.The ICP and pressure of oxygen in brain tissue (PbtO2) were recorded during the process of treatment.Results After treatment,the scores of GCS in 3 groups all increased significantly,but the improvement of the GCS scores in combination group was the highest(P <0.05).The ICP in combination group were significantly lower than that in mild hypothermia group and control group since the 5th day (P < 0.05).The PbtO2 in combination group were significantly higher than that in mild hypothermia group and control group since the 7th day(P <0.01).Conclusions Early HBO treatment combined with mild hypothermia treatment could improve the therapeutic efficiency in patients with severe TBI.

BACKGROUND: The essential component of Plumbago zeylanica Linn is plumbagin, which plays a role in anti-leukemia, antibiosis, antifertility, as well as cardiovascular. OBJECTIVE: To investigate the effects of Plumbago zeylanica Linn on the healing process of experimental rabbit fracture. DESIGN, TIME AND SETTING: Randomized control experiment of animal was performed at the Department of Pharmacology, Guilin Medical College between December 2006 and February 2007. MATERIALS: Eight New Zealand rabbits, 80 to 85-day-old, weighing 2.1-2.5 kg, half male and half female. METHODS: Eight rabbits were randomly divided into the control and experimental groups, with 4 animals in each group. Under sterile conditions, the experimental bone fracture model of rabbit was created by making a longitudinal incision in the medial of the middle portion of right foreleg, cutting off a point-foot radius with bone clamp, and suturing the incision. The rabbits in the experimental group were treated externally with the extraction of Plumbago zeylanica Linn in 50% of ethanol, twice a day for 15 days. The sodium chloride was administrated into rabbits in the control group. MAIN OUTCOME MEASURES: The serum alkaline phosphatase (S-ALP), Ca2+, P3+, K+, Na+, Cl-, liberation and total calcium of the rabbit were measured at 15 and 30 days after drug administration, X-ray of the bones were performed at the fracture healing. 10.Retroperitoneal Laparoscopy Deroofing for Parapelvic Cyst:Report of 16 Cases

Objective To explore the efficacy of retroperitoneal laparoscopy in the treatment of parapelvic cyst.Methods From August 2005 to December 2008,16 cases of parapelvic cyst were treated by retroperitoneal laparoscopic deroofing and decompressing in our hospital.Under general anaesthesia,an operational space was established via retroperitoneal route.And then,along the urethra,the parapelvic cyst was explored.Afterwards,the cyst was unroofed 3 to 5 mm from the normal renal tissues.Results The operation was completed in 15 of the patients with a mean operation time of(75?20) minutes and mean blood loss of(28?8) ml.No injuries to the kidney pedicle or pelvis occurred.Another patient was converted to open surgery because of difficulty in exposure of the cyst.This series were followed up for a mean of 13 months(range,6 to 20 months).No one had recurrence during the period.Conclusions Retroperitoneal laparoscopic deroofing is minimally invasive,safe and effective or parapelvic cyst.It should be the first choice for the disease.
