2.Training demand for chronic wound management of general practitioners in Beijing

General practitioners from community health service institutions in Shijingshan,Mentougou,Haidian,Chaoyang,Fengtai and Dongcheng Districts of Beijing were selected to take the examination of chronic wounds management.Questionnaire survey was also performed on the status of community chronic wounds management,medical service motivation,influencing factors and their demand for training.Among 204 general practitioners,55 (27.0%) had mastered basic knowledge,98 (48.0%) passed the examination and 51 (25.0%) failed.The results showed that 179 (87.7%) had changing dressing service,36.8% offered out-patient consulting service,17.6% performed surgical debridement,only 10 (4.9%) provided home chronic wounds care service and 2 (0.9%) of them carried out long-term follow-up.According to the motivation survey,all the practitioners expressed the willingness to help the patients and win their trust.Most of the practitioners claimed no supporting from their institutions(91.7%) and chronic wounds management was excluded from daily routine.Furthermore,heavy work load(82.8%) and lack of professional knowledge(80.9%)were also considered to be main obstructive factors.The survey showed that the majority of physicians required classroom teaching (90.1%),case analysis (86.8%),operating video (92.6%) and other teaching forms.More low level doctors(97.1%)had the desire to pursue further specialist training than higher level physicians (56.7%).The results of our investigation indicate that general practitioners in community heath service institutions have strong learning motivation for chronic wounds management and systematic training of knowledge and practical skills in this field should be provided.
3.Effect of calcitonin on matrix metalloproteinase 1 of cartilage of rabbit knee joint with osteoarthritis

[Objective]To assess whether calcitonin exerts an chondroprotective action in unstable knees which induce experimental osteoarthritis and its effect to MMP-1.[Method]Twenty 6-month New Zealand rabbits underwent anterior cruciate ligament transaction(ACLT) and were divided into two groups (ACLT group and ACLT+CT group) randomly after transaction on day 7 postsurgery. ACLT+CT group rabbits received a daily subcutaneous injection of salmon calcitonin at a dose of 5 IU per kg of body weight for six weeks.ACLT group were treated with the normal saline at the same dose. All animals were sacrificed 7 weeks postsurgery. The articular cartilage degeneration of medial femoral condyle was evaluated at mascroscopically and microscopically. 5.Mechanisms of carbapenem resistance in Enterobacteriaceae and the strategies for control of carbapenem-resistant Enterobacteriaceae

Monitoring data of antibiotic susceptibility from both home and abroad showed that carbapenem-resistant Enterobacteriaceae (CRE) are being on the rise,which represents an increasingly urgent clinical danger and an emerging public health concern worldwide.In order to effectively prevent CRE transmission in healthcare settings,we should not only pay high attention to the capability building of the laboratory of clinical microbiology,but also actively explore into the mechanisms of carbapenem resistance in Enterobacteriaceae.What's more,we should substantially raise the detection capacity of laboratory on CRE,effectively carry out the antimicrobial stewardship program,and strictly implement hand hygiene and contact precautions.(Chin J Lab Med,2013,36:300-302)
6.The effect and mechanism of Sorcin silencing on drug resistance of human ovarian cancer SKOV3/CDDP cell lines

Background and purpose:Multi-drug resistance is a major reason for the chemotherapy failure of human ovarian cancer. Sorcin was found overexpression in drug resistance tumors and it may be the target of multi-drug resistance reversal. The present article was aimed to study the effect and mechanism of Sorcin silencing on drug resistance of human ovarian cancer SKOV3/CDDP cell lines. Methods:The stable Sorcin silencing SKOV3/CDDP cell lines were established. MTS assay, flow cytometry was used to analyze the intra-cellular Rh-123 content, cells apoptosis and cycle. Real-time, Western blot and report gene assay were used to analyze the expression changes of genes, including MDR1, MPR1, Survivin, Bcl-2, p-Akt and NF-κB. Results:Sorcin inhibition enhanced the drug sensitivity of SKOV3/CDDP cells, increased the intra-cellular Rh-123 content and cell apoptosis, and arrested cell cycle in G2/M. The protein levels of MDR1, MPR1, Survivin, Bcl-2, p-Akt were down-regulated, the mRNA levels of MDR1 of Sh-Sorcin-1 and Sh-Corcin-2 group were decreased to 42.3%and 26.5%of untransfected control, MRP1 were decreased to 33.2%and 18.9%of untransfected control, Survivin were decreased to 36.2%and 29.6%of untransfected control, Bcl-2 were decreased to 54.6%and 46.7%of untransfected control, transcriptional activity of NF-κB were decreased to 56.3%and 38.4%of untransfected control respectively inSKOV3/CDDP cell lines after Sorcin silence. Conclusion: Sorcin silencing could reverse SKOV3/CDDP drug resistance and enhance drug sensitivity, which involves the decreased phosphorylation level of Akt and transcriptional activity of NF-κB.
7.Comparative observation of fundus ophthalmoscopic image characteristics of central serous chorioretinopathy

Objective To observe ophthalmoscopic image characteristics of central serous chorioretinopathy (CSC).Methods Twenty-one eyes of the 18 patients diagnosed with CSC were enrolled in this study.The patients included 12 males (14 eyes) and six females (seven eyes).The patients ages ranged from 26 to 47 years,with a mean age of (39.1±5.4) years.There were nine patients (11 eyes) with acute CSC,seven patients (seven eyes) with chronic CSC,and two patients (three eyes) with recurrent CSC.All the patients were examined using color fundus photography including infrared (IR),autofluorescence (AF),near infrared ray-auto-fluorescence (NIR-AF),fluorescein angiography (FA) and indocyanine green angiography (ICGA) photography.The ophthalmoscopic image characteristics of CSC were comparared.Results The circular serous retinal detachments of 21 eyes were depicted in color images of the ocular fundus,which in the IR showed the hypo-fluorescence.Ten eyes displayed mottled hyperfluorescent spots associated with serous retinal detachments corresponding to the leakage points.The serous retinal detachments of 15 eyes in the AF images showed hypo-fluorescence,six eyes showed hyperfluorescence.Fourteen eyes presented with hypo-or hyper-fluorescent spots corresponding to the leakage points,seven eyes presented without abnormal fluorescence corresponding to the leakage points.In addition,three eyes with acute CSC showed many scattered hyper-fluorescent spots,which showed hypo-fluorescence in the ICGA.The serous retinal detachment of 15 eyes exhibited hypo-fluorescence in the NIR-AF images,six eyes showed hyper-fluorescence.Fourteen eyes presented with hypo-or hyper-fluorescent spots corresponding to the leakage points,seven eyes presented without abnormal fluorescence corresponding to the leakage points.Twenty-one eyes in FA identified the leakage.Eight eyes showed regional choroidal delayed filling,13 eyes exhibited regional choriocapillary dilatation during 1 5 minutes after injection of ICGA.During 1-5 minutes after injection of ICGA,six eyes showed more lesions than FA,three eyes showed obvious patchy hypo-fluorescence whereas the FA were normal.Conclusions CSC has its own characteristic fundus images in the IR,FA and NIR-A.FA is still the photographic method of choice,but ICGA can reveal lesions of the choroid in CSC.IR,FA and NIR-AF are not as good as FA and ICGA for detecting of leakage points.

Serious and/or continuous cerebral hemodynamics disorder is the main cause of neonatal brain injury.Monitoring its changes by using simple,accurate and effective techniques has guiding significances in maintaining the stability of neonatal cerebral hemodynamics.This paper aims to introduce several cerebral hemodynamic monitoring techniques,including color Doppler ultrasound,near infrared spectroscopy,noninvasive magnetic resonance imaging,and provide references to the clinical application in neonatal field.
9.The clinical effect of Sitagliptin Phosphate Tablets combined with shenmai on type 2 diabetes

Objective To observe the effect of Sitagliptin Phosphate Tablets combined with shenmai on type 2 diabetes.Methods Sixty diabetes patients were divided into two groups randomly:treatment group were received Sitagliptin Phosphate Tablets and shenmai;the control group were given acarbose.14 weeks later,the indications FBG,2HBG,HbA1 C,TC,TG,LDL-C,HDL-C,hypoglycemia happen rate,BMI,blood rheology,transcranial Doppler ultrasound(TCD)were tested to see if there is any change.Results After treatment,two groups of FBG,2hBG,HbA1 C,TC,TG,LDL-C,HDL-C levels compared with those before treatment were statistically significant(P<0.05 or P<0.01),but the treatment group on lipid control more than those in the control group;treatment group therapy in reducing the risk of hypoglycaemia and weight gain than the control group(P<0.05);the whole blood viscosity,plasma viscosity,low viscosity and hematocrit of treatment group were significantly lower(P<0.05),while the control group showed no significant change;the abnormal rate of treatment group was 52.6%,significantly better than the control group 86.5%(P<0.05).Conclusion The effect of Sitagliptin Phosphate Tablets combined with shenmai on type 2 diabetes was significant,compared with the traditional antidiabetic drugs,the combination is effective hypoglycemic control fat and less adverse reactions.
