1.Periopertive nursing of five children with floating thumb treated with composite tissue flap grafting

Objective To summarize experience of periopertively nursing children with floating thumb treated with composite tissue flap transplantation. Method Health education was done to 5 children with floating thumb treated with composite tissue flap grafting as well as their families including preparing preoperational checks, preventing crisis of blood circulation, medication and pain nursing and paying attention to rehabilitation instruction. Result All thumbs survived successfully and one patient developed with crisis of blood circulation, which disappeared in 15 minutes. Conclusion Good preoperative nursing and effective health instruction are critical for nursing of the children with floating thumb treated with composite tissue flap transplantation.

3.Endostatins combined radiotherapy and chemotherapy in the treatment of advanced non-small cell lung cancer

Objective To preliminary study clinical efficacy and toxicity of endostatin (ES) combined radiotherapy and chemotherapy in advanced non-small cell lung cancer (NSCLC),and evaluate its effectiveness and safety.Methods We retrospectively reviewed 64 patients with Ⅲ B-Ⅳ stage NSCLC of Harbin Medical University Cancer Hospital from February 2009 to February 2012.The patients were divided into two groups:chemoradiotherapy group,39 patients and ES add chemoradiotherapy group,25 patients.The short-term effect,the total efficiency,median survival time,progression-free survival time and disease-free survival time were compared.Results The total effective rate of chemoradiotherapy group was 76.9%,while the total effective rate of ES add chemotherapy group was 84.0% (x2 =0.47,P =0.492).Chemoradiotherapy group,compared to ES add chemotherapy group,the median survival time,median progression-free survival time,median disease-free survival time were 11.52 months vs 16.51 months (x2 =3.74,P =0.042),7.32 months vs 10.37 months (x2 =5.32,P =0.025) and 5.21 months vs 7.57 months (x2 =4.56,P =0.035) respectively.The mainly adverse drug reactions were hematologic toxicity and gastrointestinal reactions,but there were no significant differences between the two groups; radiotherapy side effects mainly showed the grade 1 to 2 radiationinduced lung injury and radiation esophagitis (chemotherapy group had one case of grade 3 radiation-induced lung injury),but also had no significant differences between the two groups.Conclusion ES combined chemoradiotherapy can achieve a better short-term clinical efficacy without increasing adverse effects of radiotherapy or chemotherapy in advanced non-small cell lung cancer.
4.Effects of mindfulness behavior training on emotional disorder and immunological function in elderly patients after hip fracture fixation surgery

Objective To investigate the effects of mindfulness behavior training on emotional disorder and immunological function in elderly patients after hip fracture fixation surgery.Methods Sixty elderly patients with fixed hip fracture by operation were randomly divided into a training group and a control group with 30 cases in each group.The training group received mindfulness behavior training in addition to routine rehabilitation training,while the control group only received routine rehabilitation training.The mindfulness behavior training was administered 90 min for once a week,lasting for 5 weeks.The emotional disorder was assessed with the profile of mood states (POMS),and the immune system function were evaluated by the number of CD3 + 、CD4 + 、CD8 + in T cell subgroup before and after 5 weeks of training.Results Before training,there was no significant difference in scores of POMS and the cells number of CD3 +,CD4 +,CD8 + (P > 0.05) between the two groups.After treatment,scores of POMS including tension-anxiety (13.14 ± 3.36 vs 15.90 ± 4.28),depression-dejection (18.10 ± 6.24 vs 20.06 ± 5.42),fatigue-inertia (16.53 ± 3.50 vs 18.98 ± 4.88),vgor-activity (26.68 ± 5.22 vs 22.43 ± 5.20) (P < 0.05),the level of mindfulness practice(58.00 ± 4.78 vs 54.00 ± 4.89) and the cells number of CD3 +,CD4 +,CD8 + (75.75 ± 5.40vs 69.91 ± 4.42 ;39.54 ± 3.29 vs 34.44 ± 4.21 ; 39.82 ± 3.55 vs 36.82 ± 3.55) in experimental group improved more than those in control group (P < 0.05).Conclusion Mindfulness behavior training can improve the patients' mood state and have positive promotion on immunological function.
5.Using the first page data of medical record to analyze medical service quality of different disease entities

Objective:To search for a method for analyzing medical service quality of different disease entities using the first page data of medical records.Methods: Based on documents review and real data analysis,the basic principles,methods and steps of disease entity dividing and selecting,evaluation index selecting,and calculating were introduced.Special attention was paid to certain problems in the discussion.Results: When the inpatients of selected disease entities covered 60% of total hospitalized patients,the medical quality information included exceeded 80% of the total information.For 97.66% disease entities,their inpatient cost and length of hospital stay were logarithmic-normally distributed.Conclusion: Dividing disease entities by fixed digits of ICD-9 code is easily performable.This method should be used when the disease entities include about 60% of total inpatients,and geometric mean should be used in calculating average length of hospital stay and average cost.
6.Clinical application of transdermal fentanyl for 30 patients with moderate or severe cancer pain

Purpose:To study analgesia effect ,adverse reactions and life quality changes of transdermal fentanyl in patients with cancer pain.Methods:30 patients with moderate or severe cancer pain were treated by transdermal fentanyl .The pain intensity, quality of life and adverse reactions were observed before and after the treatment.Results:After using transdermal fentanyl ,all the patients obtained moderate to good remission.The excellent,marked and moderate rates were 43.33%,53.33% and 3.33% respectively. The adverse reactions included nausea, vomiting,constipation,dizziness and lethargy.The rate of the adverse reactions was less.The quality of life improved markedly. Conclusions:Transdermal fentanyl is a safe,effective and simple way to relieve cancer pain in patients with moderate or severe pain.It can improve the quality of life markedly.Its adverse reactions are mild.

Objective To review the progresses in epidemiology worldwide,and to summarize the achievements in military epidemiology in recent five years,so as to provide an orientation of development of epidemiology in the future. Methods Research articles in the field of epidemiology published were retrieved by information research method. The progress in basic theory,research methods and field application of epidemiology were summarized and the developmental tendency of the subject was analyzed. Results As a basic subject of preventive medicine,epidemiology had made rapid progress in principles and study methods in recent years,and played an important role in diseases prevention and control. The advances in epidemiology mainly encompassed three aspects. Firstly,the study field had extended from only focusing on human diseases to human health and all health-related public health events. Secondly,several branches of epidemiology developed quickly,including field epidemiology,macroepidemiology,fundamental epidemiology and human genome epidemiology. The related study methods had been used for investigating the risk factors of diseases,controlling disease epidemics,decreasing impact of health events,and evaluating the effects of intervention. Thirdly,many new methods and technologies,such as molecular biological technologies and spatial information technologies,had been applied in epidemiological researches. Military epidemiology had obtained remarkable achievements in surveillance of diseases as well as epidemiological studies and prevention of common infectious diseases,natural focus diseases,injury and mental diseases in the armed forces. Conclusions In future,infectious disease epidemiology is still the most important subject in military epidemiology. More attention should be given to noninfectious diseases,especially traumas and mental health problems of the servicemen. Also,introduction of advanced technologies and methods will be helpful for prevention and control of related diseases.
9.The inhibitory effects of dexamethasone on cisplatin induced apoptosis of human lung adenocarcinoma cell SPC-A1 and its molecular mechanism.

The aim of this study is to investigate the apoptotic inhibition and its molecular mechanism of dexamethasone (DEX) acting on cisplatin (CDDP)-induced apoptosis of human lung adenocarcinoma cell SPC-A1; SPC-A1 cells were pre-cultured in vitro for 24 hours with DEX in different concentrations and then CDDP was added in different concentrations for culturing for further 48 hours. The survival rates of the cells were determined by MTT. The expression of serum/glucocorticoid-induced kinase (SGK-1) and mitogen-activated protein kinase phosphatase-1 (MKP-1) in SPC-A1 cells after being cultured by 1 micromol/l DEX at different time was detected by semi-quantitative RT-PCR technology. The expression of glucocorticoid receptor (GR) in SPC-A1 cells was measured by immunohistochemistry (IHC) with biotin-labeled anti-GR. The results of MTT showed that SPC-A1 cells had resistance to CDDP-induced apoptosis with pre-cultured DEX and the resistance intensity presented DEX concentration-dependent. The expressing quantity of SGK-1 in SPC-A1 cells stimulated by DEX could be elevated and increased with intention of time, but the express of MKP-1 was not detected. Up-regulated expression of GR in SPC-A1 cells stimulated by DEX was detected by IHC. The number of cells expressing GR in SPC-A1 cells was significantly higher than that in the control group. The results showed that DEX inhibited apoptosis of SPC-A1 cells induced by CDDP. The possible molecular mechanism is that elevated expression of GR induced by DEX up-regulates the expression of SGK-1 which locates at the downstream of anti-apoptosis pathway. The apoptosis resistance of SPC-A1 cells may account for all above the factors.

10.The effects of fructose-1, 6-diphosphate after cerebral ischemia and reperfusionin in rabbits

Eighteen adult rabbits were randomly divided into three groups: normal, control and experimental group. The model of global cerebral ischemia was produced by four-vessel occulsion. The cerebral temporal cortex samples were collected to determine volume of the water, LPO and Ca~(2+) concentration,SOD activity,and pathologic examinations of the cortex were made by optical and electronic microscope. The results showed that fructose-1, 6-diphosphate (FDP) reduced the water content and intracellular Ca~(2+)level, restrained the LPO production, enhanced the SOD activity of the brain. It is concluded that FDP protects cerebral biological membrane to a certain degree from ischemia and reperfusion injury.
