The aim of this study is to investigate the apoptotic inhibition and its molecular mechanism of dexamethasone (DEX) acting on cisplatin (CDDP)-induced apoptosis of human lung adenocarcinoma cell SPC-A1; SPC-A1 cells were pre-cultured in vitro for 24 hours with DEX in different concentrations and then CDDP was added in different concentrations for culturing for further 48 hours. The survival rates of the cells were determined by MTT. The expression of serum/glucocorticoid-induced kinase (SGK-1) and mitogen-activated protein kinase phosphatase-1 (MKP-1) in SPC-A1 cells after being cultured by 1 micromol/l DEX at different time was detected by semi-quantitative RT-PCR technology. The expression of glucocorticoid receptor (GR) in SPC-A1 cells was measured by immunohistochemistry (IHC) with biotin-labeled anti-GR. The results of MTT showed that SPC-A1 cells had resistance to CDDP-induced apoptosis with pre-cultured DEX and the resistance intensity presented DEX concentration-dependent. The expressing quantity of SGK-1 in SPC-A1 cells stimulated by DEX could be elevated and increased with intention of time, but the express of MKP-1 was not detected. Up-regulated expression of GR in SPC-A1 cells stimulated by DEX was detected by IHC. The number of cells expressing GR in SPC-A1 cells was significantly higher than that in the control group. The results showed that DEX inhibited apoptosis of SPC-A1 cells induced by CDDP. The possible molecular mechanism is that elevated expression of GR induced by DEX up-regulates the expression of SGK-1 which locates at the downstream of anti-apoptosis pathway. The apoptosis resistance of SPC-A1 cells may account for all above the factors.

Objective: To investigate the effects of surface sealing on the shear bond strength(SBS) of the brackets. Methods: Twenty extracted premolars for orthodontics were cut into the buccofacial teeth pieces and etched by Transbond~TM Plus self-etching primers for 30 seconds firstly, then divided into two groups with 10 teeth pieces in each group randomly. ① experiment group, the buccal surface of the enamel was sealed with Filtek Flow 1400A3 flowable composite resin before bracket bonding;②control group, the bracket was bonded to the buccal surface of the enamel directly. All specimens were stored in deionized water at 37 ℃ for 48 h, thermocycled for 1 000 times at 30-second intervals between 5 ℃ and 55 ℃ water baths, and debonded with a universal testing machine. The amounts of resin remaining on the teeth pieces after debonding by using the resin remnant index (RRI), which were examined under 10×magnification stereo-microscope. Results: The SBS(MPa) between experiment group and control group was 8.08±1.38, 8.34±1.37 respectively, whereas the RRI was 2.80±1.03, 3.20±1.03 respectively. The SBS, RRI between experimental group and control group revealed no statistically significant differences. Conclusion: Surface sealing shows adequate bond strength, which is considered a clinically acceptable bond strength. The debonding of bracket is not affected by the surface sealing.

Objective To investigate the effects of mindfulness behavior training on emotional disorder and immunological function in elderly patients after hip fracture fixation surgery.Methods Sixty elderly patients with fixed hip fracture by operation were randomly divided into a training group and a control group with 30 cases in each group.The training group received mindfulness behavior training in addition to routine rehabilitation training,while the control group only received routine rehabilitation training.The mindfulness behavior training was administered 90 min for once a week,lasting for 5 weeks.The emotional disorder was assessed with the profile of mood states (POMS),and the immune system function were evaluated by the number of CD3 + 、CD4 + 、CD8 + in T cell subgroup before and after 5 weeks of training.Results Before training,there was no significant difference in scores of POMS and the cells number of CD3 +,CD4 +,CD8 + (P ＞ 0.05) between the two groups.After treatment,scores of POMS including tension-anxiety (13.14 ± 3.36 vs 15.90 ± 4.28),depression-dejection (18.10 ± 6.24 vs 20.06 ± 5.42),fatigue-inertia (16.53 ± 3.50 vs 18.98 ± 4.88),vgor-activity (26.68 ± 5.22 vs 22.43 ± 5.20) (P ＜ 0.05),the level of mindfulness practice(58.00 ± 4.78 vs 54.00 ± 4.89) and the cells number of CD3 +,CD4 +,CD8 + (75.75 ± 5.40vs 69.91 ± 4.42 ;39.54 ± 3.29 vs 34.44 ± 4.21 ; 39.82 ± 3.55 vs 36.82 ± 3.55) in experimental group improved more than those in control group (P ＜ 0.05).Conclusion Mindfulness behavior training can improve the patients' mood state and have positive promotion on immunological function.

Objective To preliminary study clinical efficacy and toxicity of endostatin (ES) combined radiotherapy and chemotherapy in advanced non-small cell lung cancer (NSCLC),and evaluate its effectiveness and safety.Methods We retrospectively reviewed 64 patients with Ⅲ B-Ⅳ stage NSCLC of Harbin Medical University Cancer Hospital from February 2009 to February 2012.The patients were divided into two groups:chemoradiotherapy group,39 patients and ES add chemoradiotherapy group,25 patients.The short-term effect,the total efficiency,median survival time,progression-free survival time and disease-free survival time were compared.Results The total effective rate of chemoradiotherapy group was 76.9％,while the total effective rate of ES add chemotherapy group was 84.0％ (x2 =0.47,P =0.492).Chemoradiotherapy group,compared to ES add chemotherapy group,the median survival time,median progression-free survival time,median disease-free survival time were 11.52 months vs 16.51 months (x2 =3.74,P =0.042),7.32 months vs 10.37 months (x2 =5.32,P =0.025) and 5.21 months vs 7.57 months (x2 =4.56,P =0.035) respectively.The mainly adverse drug reactions were hematologic toxicity and gastrointestinal reactions,but there were no significant differences between the two groups; radiotherapy side effects mainly showed the grade 1 to 2 radiationinduced lung injury and radiation esophagitis (chemotherapy group had one case of grade 3 radiation-induced lung injury),but also had no significant differences between the two groups.Conclusion ES combined chemoradiotherapy can achieve a better short-term clinical efficacy without increasing adverse effects of radiotherapy or chemotherapy in advanced non-small cell lung cancer.

Objective To summarize experience of periopertively nursing children with floating thumb treated with composite tissue flap transplantation. Method Health education was done to 5 children with floating thumb treated with composite tissue flap grafting as well as their families including preparing preoperational checks, preventing crisis of blood circulation, medication and pain nursing and paying attention to rehabilitation instruction. Result All thumbs survived successfully and one patient developed with crisis of blood circulation, which disappeared in 15 minutes. Conclusion Good preoperative nursing and effective health instruction are critical for nursing of the children with floating thumb treated with composite tissue flap transplantation.

Objective To investigate the emotional intelligence training training's impact on medical students' ethics education.Methods Givingnthe undergraduate and graduate students of Grade 2009,2010 and 2011 emotional intelligent training and distributing questionnaires on ethics education and comparing the result with that of 2006,2007,2008 undergraduate and graduates.Covering issues of medical ethics and the understanding of students' role in alleviating the doctor-patient relationship in medical ethics,evaluation of medical ethics in today's society,the doctor of professional ethics,medical ethics education approaches and problems and so on.By comparing the results of the questionnaire students' knowledge of medical ethics are evaluated and their emotional intelligence training in ethics education role are analyzed.Research data for entry to a database of Excel 2013,using SPSS 19.0 statistical software for analysis,count data taken x2 test,P＜0.05 was considered statistically significant.According to the questionnaire topic A representative proportion of medical students is selected to measure the level of ethics and the role of ethics education in medical students training in emotional intelligence.Results the students of 2006,2007 and 2008 who selected A of five topics were 54.78％(1 278/2 333),79.47％(1 854/2 333),68.41％(1 596/2 333),86.28％(2 013/ 2 333),13.12％(306/2 333); while the students of 2009,2010 and 2011 who selected A of five topics were 73.70％(1 777/2 411),88.68％(2 138/2 411),81.54％(1 966/2 411),93.78％(2 261/2 411),71.75％(1 730/2 411).The difference was statistically significant,indicating that 2009,2010 and 2011 medical students' cognitive level of ethicshas been nore significantly improved than the students of Grade 2006,2007 and 2008.Conclusion EQ training can significantly improve the effectiveness of medical ethics education for medical students and it is worth promoting.

Objective　To evaluate the diagnostic value of 99Tcm-HL91 in experimental tumor and inflammation models comparing with 99Tcm-MIBI. Methods　Three kinds of solid neoplasm bearing mice (Ehrlich carcinoma bearing mice, H22 carcinoma bearing mice and human ovarian COC1 neoplasm bearing nude mice) and two inflammation models (chemical and bacterial inflammation) underwent static whole body planar imaging at 1 and 4 hours after injection of 99Tcm-HL91. Two kinds of neoplasm bearing mice (Ehrlich carcinoma bearing mice,H22 carcinoma bearing mice) and two inflammation models (chemical and bacterial inflammation) underwent static planar imaging after injection of 99Tcm-MIBI, at early phase (10～20 minutes) and delayed phase (2 h). All of the mice were sacrificed at 4 h. The tumors, or inflammatory lesions, blood and contralateral normal muscles served as controls were removed, weighted and the radioactivity was measured. ROIs were drawn around tumor, inflammatory lesions and contralateral muscles in planar images, and the radioactivity ratios of target (tumor or inflammatory lesions)-to-blood (T/B), target-to non target (contralateral muscles) i.e. T/NT were calculated. Results　Neoplasms were clearly visible in planar images at 1 and 4 h after injection of 99Tcm-HL91 in all tumor models. At the same time inflammatory lesions could not be seen clearly. Neoplasms were seen in delayed phase in 99Tcm-MIBI group, but it was not easy to distinguish them from inflammation. The T/B ratios and T/NT ratios of 99Tcm-HL91 tumor model groups were significantly higher than that of 99Tcm-MIBI tumor model groups. The T/NT ratios of tumors were significantly higher than that of inflammatory lesions in 99Tcm-HL91 groups. Conclusion　Compared with 99Tcm-MIBI imaging, 99Tcm-HL91 is of much more diagnostic value in detection of certain solid neoplasms, and can distinguish neoplasm from inflammation.

The etiology of pulmonary fibrosis is complex. The occurance and development of this disease involve many cell signal transduction pathways, including MAPK signaling pathway, JAK-STAT signaling pathway, Smad signaling pathway, PI3K-Akt signaling pathway, NF-κB signaling pathway and so on. Numbers of experiments have shown that intervention in signal transduction pathways with TCM can delay the progression of fibrosis, which provides good therapeutic methods for this disease. This article reviewed study on TCM intervention in different signal transduction pathways in the pulmonary fibrosis in China.

OBJECTIVE:To provide reference for rational use of Fentanyl transdermal patch in the clinic. METHODS:By ex-tracting data from hospital information system and digital medical record management system,medical records of 150 cancer pain inpatients receiving Fentanyl transdermal patch were collected from our hospital during Jun.-Dec. 2014,and then investigated and analyzed retrospectively. RESULTS:Numeric rating scale (NRS) score of 150 cancer pain patients receiving Fentanyl transdermal patch were as follows:pain scores of 17 patients (11.3%) were 1-3 points,those of 63 patients (42.0%) were 4-6 points and those of 5 patients(3.3%)were 7-10 points;the rest of the patients were not rated. Among those 150 cases,88 cases were well tol-erated with opioid and 62 patients were considered to be opioid intolerant. Those intolerant patients were who could not be treated orally or could not bore side effects of opioid hepatic or renal dysfunction. 25 cases used Fentanyl transdermal patch without defi-nite evidence. As to drug combination,22 cases additionally received Morphine hydrochloride injection;12 cases additionally re-ceived Bucinnazine hydrochloride injection;8 cases additionally received Morphine tablet;10 cases additionally received Oxycodo-ne hydrochloride sustained-release tablets and Morphine hydrochloride sustained-release tablets;8 cases additionally received Oxyco-done and acetaminophen tablets;3 cases additionally received Carbamazepine tablets. CONCLUSIONS:Fentanyl transdermal patch are widely used for cancer pain treatment in our hospital. There still is irrational use of it,such as absence of pain score before use,no medication indication,irrational drug combination. Physicians should obey medication indication strictly,and clinical phar-macists should strengthen communication with physicians and pay attention to medication education for patients so as to guarantee rational drug use.
