Background:Gastroesophageal reflux disease(GERD)is a common disorder of digestive system, however, some of patients are not responding to conventional 4-8 weeks proton pump inhibitor therapy. Aims:To investigate the effect of digital music gastric electrical pacing on clinical symptoms, anxiety and depression and esophageal motility in patients with refractory GERD. Methods:Fifty-three patients with refractory GERD from Jan. 2014 to Oct. 2015 at the Third Affiliated Hospital, the Third Military Medical University were recruited. All of them fulfilled the Montreal definition of GERD. Digital music gastric electrical pacing was performed for 15 days and the efficacy was evaluated by clinical symptom scoring,Hamilton anxiety scale( HAMA),Hamilton depression scale( HAMD)and esophageal motility manometry. Results:After 15-day treatment, the main symptoms, including heartburn, daytime and nocturnal acid reflux, upper abdominal pain, nausea, and sleep disorder were significantly improved(P < 0. 001). A great proportion of patients were complicated with anxiety and/ or depression at recruitment, and after treatment the scores of HAMA and HAMD were decreased significantly(P < 0. 001). Meanwhile, the lower esophageal sphincter resting pressure(LESP),distal wave amplitude, peristaltic wave duration, speed of peristaltic wave and distal contraction integral after treatment were significantly increased as compared with the baseline values(P < 0. 05). Conclusions:Digital music gastric electrical pacing is effective for treatment of refractory GERD with increase of LESP and esophageal body motility. The clinical symptoms and anxiety and depression are improved simultaneously. Digital music gastric electrical pacing is expected to be a new choice of non-medicine treatment for esophageal motility disorders.

Objective To test the infeciton efficiency of recombinant adenovixal vector carrying HBsAg-HSP70 chimeric gene and to abserve its biological characteristics. Methods Peripheral blood mononuclear cells (PBMC) were separated from healthy blood donor and they were infected by Ad-HSP70-HBsAg on the first day of isolation. DCs were induced in medium with cytokines IL-4, GM-CSF and TNF-α in vitro. The biological characteristics of DC induced were analyzed by inverted fluorecent microscope, RT-PCR, flow cytometer(FACS), and mixed lymphocyte reaction (MLR). Results The traced gene-GFP were abserved in DCs by inverted fluorecent microscope and HSP70-HBsAg gene mRNA expression was detected by RT-PCR after the Ad-HSP70-HBsAg infection. FACS analysis shown that the expression of CD1a,CDSO,CD86 and HLA-DR on surfece of two groups of DCs were similar. MLR showed that there are not a statitie difference of stimulated index (SI) between two groups. Conclusion Results indicated that Ad-HSP70-HBsAg can effectively infected DCs without affecting its biological characteristics.

Objective To investigate the types of dysphagia after stroke (DAS) calling for neuromuscular electrical stimulation (NMES) and to explore the probable mechanisms of the treatment. Methods Sixty patients with DAS diagnosed by videofluoroscopie swallowing study (VFSS) were enrolled in this study. They were randomly divided into a treatment group (n = 30) and a control group (n = 30). VFSS, misaspiration, laryngeal elevation, food residues and food intake scores of the two groups were evaluated and compared before and after 10 days of treat-ment. After that, both groups were divided into mild, moderate and severe sub-groups separately according to their VFSS scores. The VFSS scores of the six subgroups were then compared. Results There was no significant differ-ence between the two groups with regard to VFSS scores and misaspiration, laryngeal elevation, food residues, food intake scores before treatment. After 10 days of treatment, VFSS scores in the treatment group were significantly high-er than in the control group, and miaspiration and laryngeal elevation scores were significantly lower. There were no significant difference between the two groups in terms of food residues and food intake scores. The VFSS scores of pa-tients with moderate DAS in the treatment group were significantly higher than those in the control group, but there was no significant difference between patients with mild and moderate DAS in the two groups. Conclusions NMES could be an effective treatment for DAS. NMES treatment is most effective for moderate DAS, but has no advantage in treating the mild cases. NMES may improve laryngeal elevation and decrease misaspiration.
