1.Transobturator or retropublic passage tension-free vaginal tape for 254 female patients with stress urinary incontinence

This study was to retrospectively report the clinical effects of retropublic passage tension free vaginal tape(TVT)vs.transobturator tape(TVT-O)on 254 female patients with stress urinary incontinence(SUI).Our findings showed that mean operative time was significantly lower in TVT-O group than that in TVT group[(19±4)min vs.(27±6)min;P<0.05].Perioperative complications were more commonly seen in the retropubic procedure(2.9% vs.8.8%;P<0.01).There was no significant difference in intraoperative blood loss[(19±6)ml vs.(31±8)ml in TVT-O or TVT groups;P>0.05]and in-hospital stay[(4.1±1.5)d vs.(4.5±1.8)d in TVT-O or TvT groups;P>0.05]between the two groups.Grouts-Blaivas Score demonstrated that those two treatment groups did not differ significantly in 24-hour(93.6% vs.91.2%;P>0.05)or long-term postoperative cure rate(94.5% vs.94.3%;P>0.05).We suggested that TVT or TVT-O seemed to be equally effective in surgical treatment of SUI.However,TVT-O had advantage of relatively lower overall perioperative complication rate.
2.Influencing factors for complete resection and operation time in endoscopic submucosal dissection for colorectal tumors

Objective To explore influencing factors for complete resection and operation time of endoscopic submucosal dissection( ESD) for colorectal tumors. Methods This retrospective study included 95 consecutive colorectal tumors in 88 patients whose pathological diagnosis was adenoma and carcinoma, treated with ESD at the Department of Endoscopy of the First Hospital of Jilin University from January 2013 to December 2014. Multiple logistic regression analysis was conducted on the factors related to complete resection and operation time. Results Average tumor size was 28. 7±14. 1 mm(range,8?80 mm), and the average procedure time was 80. 72±63. 90 min. The rate of complete resection was 92. 6%(88/95),and the rate of incomplete resection was 7. 4%(7/95). Multivariate logistic regression analysis revealed that fibrosis (P=0. 012,OR=52. 473, 95%CI:2. 571?1140. 438) contributed to incomplete resection. Fibrosis ( P=0. 001, OR=0. 045, 95%CI:0. 007?0. 289) ,tumor size ( P=0. 035,OR=0. 170, 95%CI:0. 033?0. 884) ,granular?type laterally spreading tumor ( P=0. 013, OR=34. 432, 95%CI:2. 138?554. 476 ) , non?granular?type laterally spreading tumor(P=0. 044,OR=31. 715, 95%CI:1. 093?919. 904) were independent factors for extending operation time of colorectal ESD. Conclusion The severer fibrosis can induce higher rate of incomplete resection. The more severe fibrosis is, the larger tumor size is, and the longer operation time is.
3.Sinus heart rate turbulence in patients with different subtypes of essential hypertension

Objective To explore sinus heart rate turbulence (HRT) in patients with different subtypes of essential hypertension (EH), and analyze the relationship between HRT and autonomic nervous system function damage in these patients. Methods The study consisted of 107 patients with EH (EH group) and 46 controls (control group). Based on 24 hours dynamic electrocardiogram, all patients were divided into dipper,non-dipper,and anti-dipper blood pressure group. The indexes about HRT and heart rate variability (HRV) among these groups were calculated and compared,and the relationship between turbulence onset (TO),turbulence slope (TS) and 24 hours mean systolic blood pressure,diastolic blood pressure was analyzed. Results There were significant differences in TO,TS,SDNN between EH group and control group(P < 0.05 ). TO in non-dipper and anti-dipper blood pressure group was significantly higher than that in control group( P < 0.05 ), and TS was lower than that in control group(P < 0.05 ). There was no significant difference in TO,TS between dipper blood pressure group and control group (P > 0.05). TO in non-dipper and anti-dipper blood pressure group was significantly higher than that in dipper blood pressure group, but TS was lower than that in dipper blood pressure group (P <0.05). Correlation analysis showed that TO had positive relationship with average heart rate and age (rs = 0.265, P = 0.004;rs = 0.217, P = 0.018 ), but had negative correlation with SDNN and left ventricular ejection fraction (LVEF) (rx = -0.287,P = 0.002;rx =-0. 179, P = 0.049). Whereas, TS had negative correlation with average heart rate and age (r = -0.335, P =0.015 ;r = -0.238,P= 0.009), but had positive relationship with SDNN and LVEF(r = 0.540,P = 0.001 ;r =0.432,P = 0.001 ). Conclusions HRT of EH patients becomes significantly low. It suggests that the autonomic nerve function in EH patients be injured seriously.
4.Tetrasubstituted Sulphonated Aluminum Phthalocyanine-Cat ionic Surfactant System for Fluorimetric Determination of Ethanol in Alcoholic Beverage

A new fluorimetric method was developed for the determi nation of ethanol in alcoholic beverage. The method is based on the principle that tetrasubstituted sulphonated aluminum phthalocyanine (AlS4Pc), a red-r egion fluorescent reagent, is induced to associate in the presence of cationic s urfactant, cetyltrimethylammonium bromide (CTMAB), thus its fluorescence is quen ched, and then the aggregate is disaggregated by the a ddition of ethanol and the fluorescence is recovered. This method has a linear determination range of 0.5%~90.0%(V/V), the detection limit is 0.48%(V /V). The method has been used to determine real alcoholic beverage samples w ith satisfactory results.
5.The use of iterative reconstruction technique in low-dose CT angiography of renal artery

Objective To assess the feasibility of computed tomography angiography (CTA) in renal artery using the iterative reconstruction technique (iDose4) in Brilliance iCT (Philips Ltd.).Methods 90 patients (body weight 60-70 kg) supposed to undergo renal artery CTA were selected and randomly divided into three groups:A(120 kV,120 mAs),B(120 kV,60 mAs) and C(100 kV,100 mAs).CTA images from Group A were obtained using filtered back projection and those from the rest groups were generated by iDose4.Then CT number of renal artery,noise (denoted as standard deviation SD),signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) of images for each group were measured and recorded.Images were post-processed using volume reformation (VR) 3D imaging and image quality were assessed in five-level grade (1-5).Results The five parameters (CT number of renal artery,SD value,SNR,CNR and VR grade) of group A and group B were statistically insignificant (P > 0.05).While SD values of group C were comparable to those of group A and B,the rest four parameters of group C were apparently larger,which was statistically significant (tCT value =-12.326,tSNR =-7.136,tCNR =-7.649,ZVR =-2.690,PVR < 0.05).Conclusions Image quality could still fit the diagnosis requirements using iDose4 when mAs decreased lower than 50%.The image quality increases when radiation dose is halved at 100 kVp kV.
6.In vivo study on the body motion during the Shi's cervical reduction technique with 3D motion capture.

OBJECTIVEThe clinical effect of the Shi's cervical reduction technique for cervical spondylosis and related disorders has confirmed, however, there were few studies on the body motion during manipulation in vivo study. This study is to summary the law of motion and the motion characteristics of the right operation shoulder, elbow, knee and ankle joints by data acquisition and analysis with the 3D motion capture system.
METHODSThe markers were pasted on the head, trunk, left and right acromion, elbow joint, wrist joint inner side and the outer side of the inner and the outer side and the lateral upper arm, forearm lateral, anterior superior iliac spine, posterior superior iliac spine, trochanter, femoral and tibial tubercle, inner and outer side of knee, ankle, fibular head, medial and lateral in first, 2,5 metatarsal head, heel and dual lateral thigh the calf, lateral tibia of one manipulation practioner, and the subject accepted a complete cycle of cervical "Jin Chu Cao and Gu Cuo Feng" manipulation which was repeated five times. The movement trajectory of the practioner's four markers of operation joints were captured, recorded, calculated and analyzed.
RESULTSThe movement trajectories of four joints were consistent, while the elbow joint had the biggest discrete degree. The 3D activities of the shoulder and elbow were more obvious than other two joints, but the degree of flexion and extension in the knee was significantly greater than the rotation and lateral bending.
CONCLUSIONThe flexibility of upper limb joint and stability of lower limb joint are the important guarantees for the Shi's cervical reduction technique, and the right knee facilitated the exerting force of upper limb by the flexion and extension activities. The 3D model built by the motion capture system would provide a new idea for manipulation teaching and further basic biomechanical research.

8.Point-of-care testing for sensitive cardiac troponin Ⅰ in early diagnosis of acute myocardial infarction

Objective To evaluate the diagnostic performance of a point-of-care testing for sensitive cardiac troponin Ⅰ (POCT-cTnI) in early diagnosis of chest pain patients who had a high pretest probability of acute myocardial infarction (AMI).Methods Total of 127 patients with new-onset chest pain at the emergency department were enrolled.Blood samples were drawn for the routine blood test,and determined POCT-cTnI and central laboratory testing for high sensitive cardiac troponin T (CLT-hscTnT) at admission,three and then at six hours after admission.All patients were divided into AMI group and non-AMI group according to the final diagnosis,which was adjudicated independently by two physicians who reviewed all available medical records for the 90-day follow-up period,and they were unaware of the results of the investigational assays of cardiac troponins.The receiver operating characteristic (ROC) curves were constructed to assess and compare the diagnostic performance of AMI of two cardiac troponin assays.The comparison of areas under the ROC curves (AUC) was performed by DeLong test,and the sensitivity,specificity,negative predictive values (NPV) and positive predictive values (PPV) for the target markers were calculated by applying a maker-specific cutoff value.Results The final diagnosis of AMI was made in 40 of 127 patients (31.5 %).The diagnostic accuracy of the two assays oBtained at presentation,as quantified by AUC,was no statistically differences (AUC for POCT-cTnⅠ,0.901,95% CI,0.901 to 0.947;and for CLT-hscTnT,0.907,95% CI,0.842 to 0.951;Z =0.235,P =0.745).The AUC for POCT-cTnI at 3 hours after admission was significantly higher than that on admission (0.931 vs.0.858;Z =-2.038,P =0.042),while there was on further improvement at 6 hours after admission (0.931 vs.0.949;Z =-1.435,P =0.151).With use of POCT-cTnI (cutoff value 0.023 ng/mL,which was the 99th percentile upper reference limit) on adimission,the clinical sensitivity was 77.5%,and the specificity was 94.2%.A single sample of POCT-cTnI at 3 hours after admission improved the diagnostic accuracy,with a sensitivity of 96.4%,a specificity of 92.0%,and a NPV of 98.6%,a PPV of 81.8%.While,with use of CLT-hscTnT (cutoff value 0.014 ng/mL,was the 99th percentile upper reference limit) at 3 hours after admission,the NPV reached to 100%.Conclusions The use of a POCT-cTnI assay in chest pain patients can identify and exclude the AMI rapidly and exactly at three hours after admission,and the diagnostic performance is equivalent to CLT-hscTnT.
9.An Analysis of Snoring Sound Pressure Level Parameters in Obstructive Sleep Apnea Hyperpnoea Syndrome Patients with Different Obstructive Site

Objective To investigate the relationship between sound pressure level parameters of snoring sound and obstructive sites in patients with media or severe obstructive sleep apnea hypopnea syndrome(OSAHS). Methods Forty-six patients with media or severe OSAHS were included in this study.All underwent continuous upper airway pressure measurements and snoring sound pressure level recording simultaneously.The correlations between obstructive site,AHI,BMI and sound pressure level parameterssuch as equivalent continuous sound level (LAeq )and maximum sound pressure level (L10 )were analyzed.Results Both LAeq and L10 were affected only by AHI,whereas unrelated to obstructive site and BMI.Conclusion There is no definite relationship between the site of obstruction and sound pressure level parameters of snoring sound.It appears that LAeqand L10may be helpful to e-valuate the severity of OSAHS,but may have no clinical value on distinguishing obstructive sites.
10.Preincubation with low dose of hydrogen peroxide enhances anti-oxidative stress potential of mouse BMSCs

Objective To investigate the effects of preconditioning with low-concentration hydrogen peroxide ( H2O2 ) on oxidative stress-induced bone marrow mesenchymal stem cells ( BMSCs ) apoptosis and its mecha-nism.Methods Mouse bone marrow mesenchymal stem cells ( BMSCs) were isolated and purified by differential centrifugation, and were treated with 0,200,250,300, 500 μmol/L H2O2 after preincubation with 50 μmol/L H2O2 or control medium.Apoptosis of these cells was measured by flow cytometry, and the expression of phos-phorylated PI3K, Akt and mTOR was analyzed by Western blot; BMSCs were also primed with PI3K inhibitor LY294002 for 30 min, then preincubated with 50 μmol/L H2O2 or control medium for 12 h before treatment with 300 μmol/L H2O2.Expression of apoptosis proteins Bcl-2, Bax, caspaase-3, cleaved-caspase-3 and the key pro-teins of the PI3K/Akt/mTOR pathway were detected by Western blot .Results H2O2 induced BMSCs apoptosis in a dose-dependent manner ,and pretreatment of BMSCs with low concentration of H2O2 significantly decreased H2O2-induced apoptosis of the BMSCs .Western blot results revealed that preconditioning with low-concentration H2O2 re-markably reversed the decrease in Bcl-2, total and phosphorylated PI3K, Akt and mTOR levels, and increased in Bax, cleaved-caspase-3 expression after high-dose H2O2 treatment.Such effects were antagonized by PI3K inhibitor LY294002 .Conclusions Preincubation with low-concentration H2O2 may indnce resistance of BMSC to oxidative stress, and such effect may be mediated by inhibition of pro-apoptotic proteins and activation of the PI 3K/Akt/mTOR pathway .
