1.Application of Standardized Patient Technique and Multiple Station Examination of Clinical Skills

The paper reviews introduction of standardized patient (SP) technique to Chinese medical education and its uses and experiences of applying multiple station examination of clinical skills (MSECS) at Jiujiang Medical College (now Jiujiang University Medical College). Authors state that SP's role-play of patient, instructor and evaluator is essential for the teaching and evaluation of clinical skills. The paper analyzes and describes phases of the SP technique development in China and its use in Clinical Diagnosis course and MSECS on graduation, as well as the establishment of Clinical Skills Laboratory (CSL) and its uses in MSECS. Contents and results of the MSECS are reported.
2.Application of 3-D ultrasound in bladder diseases

Objective To evaluate the clinical application of 3 D ultrasound in the diagnosis of bladder diseases. Methods In 21 patients with bladder disease,3 D images were studied with a Voluson 530D diagnostic system.At first,the two dimensional images of lesions were collected,then defined 3 D volumes of interest were stored,reconstructed and analyzed. Results 3 D ultrasound provided a clear stereoscopic view and depicted the appearance of the 21 bladder diseases,displayed the internal structures,and showed invasive depth within and outside the bladder wall and also metastasis of the adjacent organs. Conclusions 3 D ultrasound can clearly demonstrate the morphologic characteristics and internal structures of the bladder disease and provide much more reliable information for the diagnosis of diseases.
3.Laparoscopic Subtotal Cholecystectomy without Using Titanium Clips

Objective To explore the feasibility of laparoscopic subtotal cholecystectomy without using titanium clips.Methods Laparoscopic subtotal cholecystectomy was performed without using titanium clips on 48 patients,who had serious adhesions at the Calot's triangle.The cystic duct was ligated by silk.Results The operations were completed uneventfully in all the cases without converting to open surgery.No massive hemorrhage or biliary leakage occurred during the operation.The patients were followed up for 3 months to 3 years(mean,18 months),during which no one developed abdominal pain,fever,or jaundice.Conclusions Laparoscopic subtotal cholecystectomy without using titanium clips is a safe and simple method for patients with serious adhesions at the Calot's triangle.The rate of postoperative complications is low.
4.Expressions of Connexin43 and E-cadherin and their significance in invasive ductal carcinoma

Objective To explore the correlation of Connexion43(Cx43),E-cadherin(E-cad)in breast infiltrating ductal carcinoma tis-sue. Methods The expressions of Cx43 and E-cad proteins were detected in 89 cases breast infiltrating ductal carcinoma tissue,48 cases partition groups. by immunohistochemistry Elivision method. Results The expressions of Cx43 and E-cad has a better consistency in the tumor area,the border area and far cancer area of breast infiltrating ductal carcinoma. For both the negative expression in tumor area same time,the rate of lymph node metastasis was highest. Conclusion Cx43 and E-cad in breast invasive ductal carcinoma has a certain synergy in the process of the occurrence and development,which related to its metastasis occurred.
5.Multicentre study on treatment of unstable tibiofibular fracture with percutem clamp therapy.

OBJECTIVETo objectively evaluate the clinical efficacy, safety and technical operating standard of percutem clamp fixation (PCCF), an operation designed by imitating bone setting method of TCM, in treating unstable tibiofibular fracture (UTF).
METHODSA prospective controlled multicentre clinical study was carried out on 200 patients with UTF, they were assigned to the treated group (99 cases) treated with PCCF and the control group (101 cases) treated with unilateral multifunctional external fixator (UMFEF).
RESULTSPCCF showed better effects than UMFEF in reducing the time of fixing operation and healing time; and the immediate effect after removing the fixating apparatus and long-term efficacy as well as safety in the treated group were also superior to those in the control group (P < 0.01).
CONCLUSIONIt is convenient, safe and effective to treat UTF by fixation with percutem clamp.

7.Dynamic change of parasites in the brains of BALB/c mice infected with Angiostrongylus cantonensis

To observe the dynamic change of parasites in the brains of BALB/c mice infected with Angiostrongylus cantonensis in order to explore its possible mechanism of pathogenesis', BALB/c mice infected with the III stage larvae of A.cantonensis were observed and killed in different times after infection. The number and distribution of parasites in the brains of the infected mice were observed microscopically and macroscopically. It was found that the larvae of A.cantonensis were distributed in the cerebrum and cerebellum of mice in accordance with the rule of parasitization of worms in the host, i.e.multiplication at first and then dropping in number. And the places where the parasites located were damaged due to mechanical action or inflammatory reactions. The time of onset of symptoms, such as ataxia and twitch was coincided with the dynamic changes in the brains of the infected mice.

AIM: To evaluate the dynamic changes of the function and ultrastructure of gastric parietal cells under stress and their relation with acute gastric mucosal lesions. METHODS: Thirty-two male SD rats were randomly divided into four groups, which were control group and 1,2,4 h groups under water restraint stress (WRS). The gastric fluid pH value and gastric mucosal ulcer index(UI) were measured. The ultrastructural changes of parietal cells were observed by transmission electronic microscopy (TEM). 9.Gas-liquid mass transfer in three-phase inverse turbulent bed reactor.

The characteristics of gas-liquid mass transfer of three-phase system comprising air, tap water/wastewater, and hollow glass beads were studied in a laboratory-scale inverse turbulent bed reactor. The influence of operational factors and liquid property on volumetric liquid-phase mass transfer coefficient kLa was investigated under the conditions of superficial gas velocity (0.53mm xs(-1) - 10mx s(-1) solid hold-up (0 - 0.3), and superficial liquid velocity (0 - 0.2mm x s(-1)). The results showed that the coefficient value was 0.0456 - 1. 414min -, which increased with superficial gas velocity and liquid velocity. The coefficient attained the maximum value at solid hold-up of 0.05 - 0.08. Compared with the coefficient value in tap water, that in synthetic wastewater and industrial wastewater is decreased by 39.0% and 50.9%, respectively. These data have provided a basis for the process analysis and mathematical simulation of inverse turbulent bed reactor.

10.Assessment of the diffusion-weighted imaging for the pathology and renal function in IgA nephropathy

Objective To study the correlation between the diffusion-weighted imaging (DWI) measurements and glomerular filtration rate (GFR), Katafuchi scores in IgA nephropathy. Methods Thirty-five patients with IgA nephropathy (IgAN group) and twenty healthy volunteers (control group) were enrolled in this study. All of the subjects underwent bilateral renal DWI measurements with 3.0T MRI scanner. The values of apparent diffusion coefficient (ADC) of renal cortex and medulla were measured. GFR of IgAN group was detected with 99Tcm-DTPA scintigraphy. Based on the Lee classification and the Katafuchi score system, the pathological grading was carried out in patients of IgAN group. The ADC values were compared between control group and different grades of IgAN group. The correlations between ADC and GFR values were analysed in defferent groups. The correlations between ADC values and Katafuchi scores were analysed in IgAN group. Results The renal cortical ADC values were significantly higher than medulla ADC values in both control group and IgAN group (P < 0.05). There were statistically significant differences in renal cortical ADC values and medulla ADC values between control group and IgAN subgroups (P<0.05). But there was no significant difference in renal cortical ADC value between IgANⅠgroup and control group (P>0.05). There was a positive correlation between the renal cortical and medulla ADC values and the GFR values in IgAN group (P<0.01). Negative correlation was found between the renal cortical and
medulla ADC values and the Katafuchi scores in IgAN group (P<0.05). Conclusion The diffusion-weighted imaging can reflect the physiological functions of kidney. It was feasible for application DWI in IgA nephropathy, which can be used for assessing the renal filtration function and the pathological damage. However, DWI measurement is not sensitive to early renal disease.
