1.Value of contrast -enhanced ultrasound in evaluating peripancreatic vascular invasion and resectability of pancreatic cancer

Objective To investigate the value of preoperative contrast -enhanced ultrasound (CEUS)in evaluating peripancreatic vascular invasion and resectability of pancreatic cancer.Methods A retrospective analysis was performed for the clinical data of 25 patients with pancreatic cancer confirmed by surgical exploration or postoperative pathological examination,who visited Shengjing Hospital of China Medi-cal University from December 2012 to October 2014,and all the patients underwent CEUS before surgery.The relationship between the le-sion and the peripancreatic vessels was observed,the degree of invasion was evaluated,and related scores were measured.These results were compared with the results of intraoperative exploration and postoperative pathological results.The rate of peripancreatic vascular invasion, sensitivity and specificity of preoperative CEUS in evaluating the degree of peripancreatic vascular invasion,and sensitivity,specificity,pos-itive predictive value,negative predictive value,overall coincidence rate,and Youden index of CEUS in evaluating the resectability of pan-creatic cancer were calculated.Results Preoperative CEUS showed that 42 vessels were invaded,while intraoperative exploration showed 41 vessels were invaded.A total of 17 patients were considered resectable by CEUS,while 15 patients were considered resectable by intraop-erative exploration.In evaluating the resectability of pancreatic cancer,CEUS had a sensitivity of 93.3%,a specificity of 70.0%,an over-all coincidence rate of 84.0%,a false positive rate of 30.0%,a false negative rate of 6.7%,a positive predictive value of 82.4%,a nega-tive predictive value of 87.5%,and a Youden index of 0.633.Conclusion Compared with intraoperative exploration,CEUS has higher sensitivity and specificity in evaluating the degree of peripancreatic vascular invasion and higher sensitivity,specificity,and coincidence rate in evaluating the resectability of pancreatic cancer.CEUS can accurately evaluate the degree of peripancreatic vascular invasion and provides a new method for preoperative evaluation of the resectability of pancreatic cancer.

In a case study of Phoenix Healthcare Group' s role in Beijing Mentougou District Hospital's reform,this paper systematically reviewed the PPP application in public medical service,revealing the problems found in the reform including concept deviations for public benefit,and sustainability of the business pattern.On such basis,the authors recommended from the perspectives of the government,hospital and society.Their recommendations cover policies on improving the supporting policies,reconstruction of the hospital mechanisms of motivation,income distribution and supervision,fostering efficient private medical enterprises and the third-party appraisers.These recommendations aim at improving PPP model in the reform of public hospitals in China.
3.THE IMMUNOHISTOCHEMICAL OBSERVATION OF DENDRITIC CELLS AND MACROPHAGES IN THE SPLEENS OF SARS PATIENTS

Objective To analyse the number of dendritic cells and macrophages and the size of macrophages in the spleen of SARS patients so as to provide evident for the study of pathology and pathogenesis of SARS. Methods Immunohistochemical method with four specific antibodies(S-100,CD68,HLA-DR,CD83) were used to detect the dendritic cells and macrophages in the spleens of six dead patients of SARS and six accidental deaths as the controls,The number or the size of these positive cells was analysed with image analysis system. Results In the spleens of SARS patients,the number of S-100~+ dendritic cells in the white pulp was reduced by 80.4% on average,and even disappeard.The number of CD68~+ macrophages in the red pulp was reduced by 39.48% in SARS spleens,and the average size of individual macrophages was increased by 2.21 times.HLA-DR~+ antigen presenting cells(APC) reduced remarkably in the SARS spleen white pulp.CD83~+ mature dendritic cells did not exist in either SARS spleens or the control spleens.Conclusion The function of antigen presentation had been damaged severely,which supports that SARS should be categorized as a viral immune deficiency disease.SARS virus doesn't induce the maturation of DC.The increase in the size of macrophages indicated that they were in an activated state and may play a role in the pathogenesis of SARS.
4.IMMUNOHISTOCHEMICAL QUANTITY ANALYSIS OF THE SPLENIC LYMPHOCYTES IN THE PATIENTS WITH SEVERE ACUTE RESPIRATORY SYNDROME

Objective To investigate the distribution and the number of splenic lymphocytes in the patients with severe acute respiratory syndrome(SRAS).Methods Immunohistochemical method with four specific antibodies(CD3,CD4,CD8,CD20) was used to detect the distribution of lymphocyte special antigens in the spleens of six cases who died of SARS and six normal cases as the controls.The numbers of lymphocytes were analyzed with image analysis system.Results In the spleen of SARS patients,splenic corpuscle and periarterial lymphatic sheath (white pulp) were damaged severely. In the number of periarterial lymphatic sheath decreased 93.39% and in the number of splenic capsule decreased about 80% even disappeared at all. Red pulp hemorrhage necrosis was widely spread. In red pulp the number of CD3~(+)T cells decreased 71.76%,even disappeared at all,in the number of CD4~(+) and CD8~(+)T cells markedly decreased 86% and 84% respectively.The number of CD20~+ B cell decreased more than 80%. Conclusion T cells and B cells in spleens of SARS patients decreased widely spread and markedly in number,indicating that the immune system had been damaged severely,and the destroyment of the immunosystem may be the primary lesion of SARS.
5.A SCANNING ELECTRON MICROSCOPE STUDY ON THE IMMATURE DENDRITIC CELLS BEFORE AND AFTER THE ELECTRANSFECTION OF HUMAN HEPATIC CANCER CELL RNA

Objective To observe the morphologic changes of immature dendritic cells(imDCs) before and after the electransfection of human hepatic cancer cell RNA. Methods Monocytes were purified from human peripheral blood,and induced into imDCs.Then human hepatic cancer cell RNA was electransfected into monocyte-derived imDCs.ImDCs were identified by the immunocytochemical method with 7 specific antibodies before and after electransfection.These dendritic cells were observed by scanning electron microscopy. Results After electransfection of human hepatic cancer cell RNA there were few changes of molecule expressions in imDCs.ImDCs were in round,oval and irregular shapes before electransfection.Their sizes were not identical but all bigger than monocytes.There were many protrusions in different shapes which looked like dendrite,or/and bubble,veil cloud on the surface of these imDCs.Although there were some cell fusions and cell deaths after electransfection,most imDCs recovered from the damage.Electransfecting human hepatic cancer cell RNA into imDCs would make pores on cell membrane.Conclusions The pores on cell membrane make it possible that the exogenous material enters imDCs.This study can prove the possibility of electransfecting human hepatic cancer cell RNA into imDCs to make cancer vaccine,which provides a new way for tumor biologic therapy.
6.Effects of tanshinone ⅡA on transforming growth factor beta 1/Smads signaling pathway in cardiac fibroblasts

BACKGROUND: One of important mechanisms underlying myocardial fibrosis is that transforming growth factor β1(TGF-β1) stimulates the proliferation and differentiation of cardiac fibroblasts via Smads signaling pathway.Previous studies have confirmed that tanshinone ⅡA can effectively inhibit myocardial fibrosis.But whether blockage of TGF-β1/Smads signaling pathway is involved in this process remains unclear. OBJECTIVE: To investigate the effects of tanshinone ⅡA on TGF-β1 signal transduction in rat cardiac fibroblasts. METHODS: Neonatal rat cardiac fibroblasts were harvested by trypsin digestion and differential attachment and treated with 5 μg/L TGF-βI and different concentrations of tanshinone Ⅱ A(106,10-5 and 10-4 mol/L).At 6,12,and 24 hours after TGF-β1 application,fibronectin expression was detected by reverse transcription-polymerase chain reaction and Western blot analysis.At 15,30,60,and 120 minutes after TGF-β1 application,Smads protein expression was determined by Western blot analysis. RESULTS AND CONCLUSION: Fibronectin mRNA and protein expression began to increase at 6 hours after TGF-β1 application and was 1.3 and 1.8 times higher than initial level,respectively(P < 0.01),at 24 hours after TGF-β1 application.Phosphorylated Smad2/3 protein expression began to increase at 15 minutes after TGF-β1 application,peaked at 1 hour,decreased at 2 hours,but it was still 3.9 times higher than initial level(P < 0.01).Tanshinone ⅡA(10-5 and 10-4 mol/L)pretreatment downregulated fibronectin and phosphorylated Smad2/3 expression(P < 0.05 or P < 0.01)in a dose-dependent manner.These findings demonstrate that TGF-β1 induced fibronectin protein and mRNA expression and Smad2/3 protein expression in a time-dependent manner.Tanshinone ⅡA against myocardial fibrosis was likely related to its inhibition of TGF-β1-induced Smad2/3 phosphorylation and blockage of TGF-β1/Smads signaling pathways within cardiac fibroblasts.
7.Preliminary studies on effect of polysaccharides from Tegillarca granosa as an adjuvant on DNA vaccine against Schistosoma japonicum infection

Objective To discuss the effect of polysaccharides from Tegillarca granosa as an adjuvant on pcDNA3.1?Sj26GST vaccine against Schistosoma japonicum infection. Methods Sixty SPF BALB/c mice were divided into four groups randomly(15 mice each group),including a control group,a polysaccharides group,a vaccine group,and a vaccine plus poly?saccharides group. In the 0,2th and 4th week of the experiment,the mice in the above four groups were immunized for 3 times with 100μl PBS,100μg polysaccharides from Tegillarca granosa,100μg Sj26GST vaccine,and 100μg Sj26GST vaccine plus equivalent polysaccharides,respectively. Two weeks after the last immunization,all the mice were infected with 40 ± 1 Schistosoma japonicum cercariae through the skin of the abdomen. After the infection for 6 weeks,all the mice were sacrificed, and their serums,livers and the adult worm of Schistosoma japonicum in them were collected,the specific sera IgG antibodies were detected by ELISA,and the worm reduction rates and egg reduction rates in the liver were calculated. Results Six weeks after the infection,the IgG antibody levels of the mice in the vaccine group and the vaccine plus polysaccharides group were 18.26 ± 0.42 mg/ml and 20.21 ± 0.89 mg/ml respectively,the difference between them were statistically significant,and both of the IgG levels of the above groups were significantly higher than those in the control(both P<0.05). The worm reduction rate and egg reduction rate in the vaccine group were 28.60%and 35.84%,respectively,and the rates in the vaccine plus polysac?charides group were 38.04% and 49.74%,respectively,the differences between the worm reduction rates and egg reduction rates were both statistically significant(both P<0.05). Meanwhile,the two rates in the two above groups were all significantly higher than those in the control group(all P<0.01). Conclusion Polysaccharides from Tegillarca granosa using as an adju?vant can increase the protection effect of pcDNA3.1?Sj26GST vaccine against Schistosoma japonicum infection.
8.The exploration of TTAG method in determinating the states of Eustachian tube functiong of healthy group

Objective:To explore the feasibility and validity of tubo-tymanoaerodynamic graphy(TTAG)determinating the Eustachian tube function of health adult group.Mthod:The ventilation function of Eustachian tube was measured by the TTAG method in health adlut group(132 ears),the results and the graphs were also analyzed.Reslut:The positive rate in health adult group using Valsalva method was 93.93%(124/132).The positive ears were divided into typeⅠand typeⅡ, the mean value of nasopharynx press of type Ⅰand Ⅱ has no significant differences(P>0.05),but the mean value of external auditory canal press had significant differences(P<0.01).Conclusion:TTAG meathod has the clinical value in determinating the states of Eustachian tube function of health group.

We reported a case of adult Drosophila melanogaster parasitized in nasal cavity of a 81-year-old woman who was living in Xuancheng City, Anhui Province now. She was admitted for treatment of cerebral infarction and water accumulation in the lungs in 2014 June. The patient was also suffering from secretory otitis media, a history of hypertension and heart stents were placed in 2007. A foreign body was found in the left nasal cavity during the preoperative examination process, and then the part of the inflammatory tissue was removed through the nasal endoscopy, and sent to our department for identification. There are three adults of Drosophila in paraffin-embedded biopsy specimens. The parasites length is approximately 3mm, with huge red compound eyes. The end of the body is tip, with 5 ring lines in back, has no dark spots. The abdomen of the parasites have seven sections. Tarsus of foot I have no sex comb on base, and they are male adult of Drosophila melanogaster after identification. After a thorough reviewing of medical history, we knew the patient began to sneeze violently and frequently six years ago. But there was no clear or purulent nasal discharge flowing, therefore did not attract attention. After removing the parasites the sneezing symptoms were relieved, and had no abnormal symptoms in the follow-up 6 months.
