1.Wet Smear Compared With Gram Stain Diagnosis of Bacterial Vaginosis in Asymptomatic Pregnant Women

Objective To compare wet smear and Gram stain diagnoses of bacterial vaginosis among asymptomatic pregnant women.Methods Between September 1,2006 and January 31,2008,asymptomatic women who initiated prenatal care in our obstetric clinics were invited to participate.Clinical diagnosis that required two of three positive criteria for bacterial vaginosis(vaginal pH,whiff test,and clue cells on wet smear)was compared with Gram stain diagnosis(Bacterial vaginosis score 7-10 by Nugent criteria).Results Population characterstics included an average matemal age of(27.3±6.6)years,26 nulliparas.The mean gestational age at entry was(15.6±7.6)weeks.Twenty-seven percent(18/67)of the study population was diagnosed with bacterial vaginosis by definitive Gram stain.Using Gram stain diagnosis of bacterial vaginosis as the standard,clinical diagnosis had sensitivity of 56%(95% confidence interval[CI]32%,78%),aspecificity of 96%(95% contidence interval[CI]90%,100%),a positive predictive value of 83%,and a negative predictive value of 85%.Conclusion In asymptomatic pregnant women,bacterial vaginosis can be diagnosed reliably by Gram stain.

3.Research on the Influences of the Implementing Essential Medicine System on Compensation Mechanism of Primary Medical Institutions:analysis on system dynamics

Objective: To analyze the influences of implementing essential medicine system on primary medical institutions and give suggestions. Methods: Using system dynamics method to construct the compensation mechanism model for primary medical institutions. Compare the growth in revenue and expenditure of primary medical institutions before and after essential medicine system implementation. Results: After the implement of essential medicine system, the growth in total expenditure and total income of primary medical institutions is steadier; when the annual growth rate of government financial subsidies invested more than 13%, the growth in total receipts will keep balance. Conclusion: It’s better to guarantee the government financial subsidies invested in time, raise medical service price properly and establish comprehensive compensation mechanism for implementing compensation mechanism of primary medical institutions.
4.Comparison between CA19-9,CEA and specific N-Glycans in early middle stage pancreatic carcinoma serum

Objective To test and compare CA19-9,CEA with Specific N-Glycans in early-middle stage pancreatic carcinoma serum to find a better tumor marker in early pancreatic carcinoma .Methods To find the difference of N-Glycans structure ,DSA-FACE was performed in 35 early middle stage pancreatic carcinoma patients and 50 healthy human .Meanwhile ,we tested CA19-9 and CEA in the pancreatic carcinoma patients at the same stage.Result The serum N -glycan profiles of pancreatic carcinoma was identified by the DSA -FACE technique.The results showed that between pancreatic carcinoma patients and healthy persons ,there were significant differences in N -glycans.The peak 13,14 and 17 were the most significant peaks which would be most likely picked as a new tumor marker of pancreatic carcinoma .Taking log(p14 ×p17/p13)as indicator of the ROC curve analysis,and the area under ROC curve was 0.799 ±0.050 with 84.9% sensitivity and 68% speci-ficity.Meanwhile,the sensitivity of CA19-9 was 61.2%,the sensitivity of CEA was 11.7%.Conclusion The Peak 13,14 and 17 are the most significant peaks which would be picked as a new tumor marker of pancreatic carcinoma.Espicially,its sensitivity is superior to CA19-9、CEA for early middle stage patients .
5.Significance of postoperative structural changes in serum N-glycans in pancreatic cancer patients

Objective To investigate the structural changes in specific serum N-glycans in pancreatic cancer patients and to identify the specific serum maker of pancreatic cancer.Methods The pancreatic cancer patients who visited the Third Affiliated Hospital of Harbin Medical University from June 201 1 to December 2013 were assigned to preoperative serum group (123 cases)and postoperative serum group (78 cases);healthy controls whose serum samples were collected in the Physical Examination Center were selected as control serum group (271 cases).DNA sequencer-aided fluorophore-assisted carbohydrate electrophoresis (DSA-FACE)was used to analyze serum N -glycans and compare them between the three groups.Results The serum N-glycan profiles in pancreatic cancer patients were identified by DSA-FACE.The results indicated that N-glycan peak 8 in preoperative serum group was significantly lower than those in control serum group (t=2.735,P<0.05)and postoperative serum group (P<0.05),but no significant difference was found between the postoperative serum group and control serum group.Conclusion N-glycan peak 8 can be considered as a serum marker of pancreatic cancer.

A 46-year-old female patient presented to our hospital with history of pharyngeal discomfort for 6 months. Physical examination showed that she had facial asymmetry, loss of the left nasolabial fold, rightward de- viation of uvula, swelling of the left soft palate. Magnetic resonance imaging revealed a bigger neoplasm in left pa- rapharyngeal space which invaded into left lateral skull base. The left internal carotid artery, vein and the styloid process was jostled backward. The primary clinical diagnosis is pleomorphic adenoma of the parapharyngeal space.

7.Cytotoxicity evaluation of self-made nickel-chromium ceramic alloy using cell culture method

BACKGROUND: Nickel-chromium ceramic alloy with well anti-causticity has been widely used in clinic; however, it should be paid much attention to the toxic and side effects of the released nickel and glucinium ions. Therefore, studies about modifying alloy component, enhancing physical and chemical performance, and improving biocompatibility are hot topics recently. OBJECTIVE: To measure the in vitro cytotoxicity of self-made nickel-chromium ceramic alloy comparing with nickel-chromium alloy and pure titanium metal. DESIGN, TIME AND SETTING: A contrast observational experiment was performed at Central Laboratory of China Medical University from May to December 2006. MATERIALS: L-929 cells at logarithmic phase were used in this study. Self-made nickel-chromium ceramic alloy was provided by Metal Institute of Chinese Academy of Science, pure titanium was provided by Matsukaze Japan Co., Ltd., and nickel-chromium alloy was provided by Heraeus Kulzer Dental Co., Ltd. METHODS: L-929 cells were assigned into nickel-chromium ceramic alloy, pure titanium, nickel-chromium alloy, positive control, and negative control groups. The cells were cultured in 96-well culture plate. Every five wells were randomly selected from each group, and 12 wells were used for the duplication. Cell suspension (100 μL) at concentration of 1.0×108/L was added in each well. Leaching liquor (100 μL) was added in self-made nickel-chromium ceramic alloy, pure titanium alloy, and nickel-chromium alloy groups, respectively, lead leaching liquor (100 μL) was added in positive control group to make the final concentration at 50%, and RPMI 1640 culture solution (100 μL) was added in the negative control group. MAIN OUTCOME MEASURES: Absorbance was measured at day 1, 2, 3, 4 and 5 using MTT colorimetric assay to calculate relative growth rate. RESULTS: There were no significant differences in absorbance between three metal groups and negative control group (P > 0.05), but there were significant differences in absorbance between three metal groups and positive control group (P < 0.05 or P < 0.01). Cell growth rate was > 100% and cytotoxicity was grade 0 in the pure titanium group; while, cell growth rate ranged from 86.36% to 99.49% and cytotoxicity was grade 1 in both self-made nickel-chromium ceramic alloy and nickel-chromium alloy groups. At day 3 after culture, cytotoxicity was grade 2 in the positive control group. CONCLUSION: Self-made nickel-chromium ceramic alloy has a little cytotoxicity and requires for the clinical application.

Objective To understand the modulating mechanisms in ciliary motility by NO signal pathway. Methods L arginine, the preferred substrate of NOS, was employed to act upon the cultured rat ciliated epithelia. After pre incubating with L NMMA, a NOS inhibitor, or ODQ,a sGC inhibitor, or Rp 8 Br cGMPS, an antagonist of PKG respectively, tissues were contacted with L arginine again. Ciliary beating frequency (CBF) was measured by phase contrast microscope and videotape analysis. Results L arginine increased CBF siginficantly. The effect of L arginine on CBF was blocked by L NMMA, ODQ or Rp 8 Br cGMPS.Conclusion L arginine may increase CBF via NO sGC cGMP PKG pathway.
