Objective To explore the correlation factors of cranial magnetic resonance diffusion weighted imaging(DWI) in patients with acute cerebral infarction.Methods A total of 350 patients with acute cerebral infarction who underwent MR imaging were divided into DWI positive group and negative group according to whether the first diffusion weighted imaging was abnormal or not.The clinical and radiological data of the two groups were retrospectively collected and analyzed.patients with negative DWI on initial MRI were re-examined 1 week later.Result Thirty-four(9.7%) patients were DWI-negative in 350 patients with acute cerebral infarction on initial MRI,and 5 patients in DWI-negative group had positive lesions on the re-examine MRI.Multivariate logistic regression analysis indicated the patients with first blood pressure of admission≥ 140/90 mmHg(P=0.033),first blood glucose of admission≥ 7.0 mmol/L(P=0.028) and the time from the onset to initial MRI>24 h were more likely showed DWI-positive on initial MRI(P=0.013).Patients with posterior circulation infarcts were more likely to have negative DWI findings than patients with anterior circulation infarction(P=0.001).Conclusion Some patients with acute cerebral infarction may show DWI negative results on initial MRI,which is related with the time from onset to initial MRI and the site of cerebral infarction.Patients with posterior circulation infarcts are more likely to have first negative MR findings than patients with anterior circulation infarction.Patients with elevation of the admission blood pressure or blood glucose are more likely to have DWI positive lesions.Patients with acute cerebral infarction within the time window should received be thrombolytic therapy after carefully assessment to avoid delay in treatment even if the DWI negative results.

The number, scale, compiling institutes, subject distribution, languages, and application of medical controlled vocabularies covered in Taxonomy warehouse were investigated, the developments of the main medical controlled vocabularies such as LMLS, MeSH and ICD were described, which showed the following tendencies of foreign medical controlled vocabularies,such as ontological transformation,intelligent update and application,coopera-tive compilation,dynamic integration and disintegration, distribution as linked data.

Objective:To establishing model of anterior disc displacement(ADD) of temporomandibular joint(TMJ) by persistent inducement of traction through orbit. Methods:The inferior and posterior walls of orbit of right side in 8 rabbits were exposed. The anterior part of disc was vertically passed through using a suture needle with an elastic rubber. The double strands of the elastic rubber was stretched forward from 7 mm to 16 mm and fixed in the hole drilled in the zygomatic arch bellow inferior orbit. The rabbits were killed 1,2,4 and 8 weeks after operation respectively, then both TMJs of each were resected. The paraffin sections were made and stained with hematoxylin & eosin. Results:The mandible of the rabbits deviated to the left side with inclined attribution of incisors. The disc was displaced forward and the degree of ADD in the rabbits was similar. Conclusions:The disc anterior displacement of temporomandibular joint can be induced by persitent anterior traction of the disc.

Objective To explore the psychological status of the patients with breast cancer and its influencing factors.Methods The psychological health status in 896 patients with breast cancer and 1 419 patients with benign breast diseases were surveyed and analyzed by adopting the Self-reporting Inventory(SCL-90).At the same time,SCL-90 was also used to analyze the differences of psychological health status among breast cancer patients with different occupations,degrees of education and family support attitudes.Results The each factor scores in breast cancer patients were higher than those in benign breast disease patients(P＜0.05).The scores of obsessive symptoms and somatization factor in breast cancer patients were higher than those of other factors(P＜0.05).In breast cancer patients,the proportion of workers and cadres with obsessive symptoms was higher than that of farmers.The family support attitude was good,and the proportion of obsessive symptoms was lower.The higher the education level,the lower the proportion of somatic symptoms.Conclusion Aiming at the existence of mental problems of higher somatization and obsessive symptoms,conducting the psychological counseling and intervention can improve the quality of life in the patients with breast cancer.

Objective:To assess the life quality of the patients with tongue cancer after operation.Methods:80 cases of tongue cancer following surgery during 1994 to 1998 were reviewed with questionaire.Scords of University of Washington Quality o f Life (UW-QOL) were analysed to evaluate their life quality.Results:(1) 3-year survival rate was 72.5%,higher survival rate was observed in the patients with I and II stage lesion than those with Ⅲ and Ⅳ stages.(2) 58 cases of survivers had UW-QOL average score of 618. Some patients had serious disability in mastication, swallowing,speaking and shoulder movement.(3)Patients retaining mandible or with skin flap repair had higher quality of life than the others.Conclusion:Following surgery,tongue cancer patients still maintain comparatively high quality of life.Reconstruction of mandibular or tongue is very importent for the life quality of the patients.

Objective To observe the efficacy of No. 1 Baiban granules in the treatment of vitiligo using skin confocal laser scanning microscopy. Methods The study was approved by the hospital ethics committee. Nine-two patients with qi stagnation and blood stasis were collected from Department of Dermatology of the Affiliated Hospital of Tianjin Academy of Traditional Chinese Medicine and were randomly assigned to two groups. The experimental group (n=49) received oral Baiban granuleΙ, 3 g, twice a day, transfer factor capsules 6 mg, 3 times a day, 0.1%mometasone furoate cream (excluding the face) for external use, once a day, or 0.1%tacrolimus ointment (Afacial), twice a day for 3 months. The control group (n=43) only received oral transfer factor capsules 6 mg, 3 times a day, 0.1%mometasone furoate cream (except for the face) for external use, once a day, or 0.1%tacrolimus ointment (face), twice a day, treatment for 3 months. The therapeutic effect was observed in two groups. CLSM was used to monitor the compound color and pigment recovery level, and changes of the number of dendritic cells and inflammatory cell infiltration in skin. Results The compound color was improved significantly after treatment in experimental group and control group. The total effective rate was higher in experimental group than that of control group (81.63% vs. 69.77%,χ2=3.947,P<0.05). Results of CLSM observation showed that the pigment recovery of skin lesion, the proportion of dendritic cells and inflammatory cells were significantly higher in experimental group than those of control group (P < 0.05). Conclusion The treatment effect of No. 1 Baiban granules is more obvious for vitiligo of qi stagnation and blood stasis.

BACKGROUND:The relationship of fibroblast growth factor receptor 3 (FBGFR3) and malignant tumor has been confirmed. However, domestic and foreign studies on whether FBGFR-3 plays a role in odontogenic tumor with special biological characteristic are few.OBJECTIVE: To observe the expression of FBGFR-3 in odontogenic tumor.DESIGN: A randomized controlled observation.SETTING: Department of Stomatology, Medical College, Medical College of Huzhou Teachers College; Stomatology Hospital Affiliated to Zhejiang University.MATERIALS:This experiment was carried out in the Stomatology Hospital, Medical College Affiliated to Zhejiang University between January 2003 and December 2004.All the samples were from the odontogenic tumor patients admitted to Department of Pathology, Stomatology Hospital Affiliated to Medical College of Zhejiang University between year 1999 and year 2000, including 29 patients with ameloblastoma, 20 keratocyst and 36 primordial cyst. Normal dental crypt or residual dental lamina epithelium tissue served as control. All the samples were embedded by paraffin, sliced into 3 to 5 μm-thickness sections, placed on polylysine-spread slide, and dried for later use.METHODS: The expressions of FBGFR-3 in the normal dental crypt or residual dental lamina epithelium tissue,odontogenic ameloblastoma, keratocyst and primordial cyst were detected by immunohistochemical method.MAIN OUTCOME MEASURES: Expressions of FBGFR-3 in ameloblastoma, keratocyst and primordial cyst.RESULTS: The expression of FBGFR-3 in the ameloblastoma, keratocyst and primordial cyst was positive, presenting 59%, 45% and 8%, respectively, and there were significant differences in expression of FBGFR-3 among these three;FGGFR-3 was not expressed in the normal dental crypt and residual dental lamina epithelium tissue. FBGFR-3-positive cells gathered in the maturation region of tumor cells.CONCLUSION: FBGFR-3 may be related to the pathogenesis of ameloblastoma and keratocyst, and terminal differentiation mechanism.

Objective:To study the diagnosis value of Transcranial Doppler ultrasound for intracranial artery stenosis in patients with cerebral infarction.Methods:A total of 140 patients with acute cerebral infarction,transient ischemic attack and posterior circulation ischemic attack in Department of Neurology,Xinjiang cardiovascular and cerebrovascular disease hospital from October 2014 to October 2016 were selected as research object,CT angiography (CTA) and TCD detection were performed in all patients.Used CTA examination results as the gold standard,the detection results of intracranial artery stenosis in two groups were compared,the diagnostic value of TCD and the diagnostic results of TCD to the degree of bilateral middle cerebral artery (MCA) stenosis were analyzed.Results:CTA diagnosis showed that 140 patients had a total of 105 patients with intracranial artery stenosis,in the anterior and posterior circulation vessel of 1155 intracranial segments,CTA detection showed that 249 vessels were narrow,TCD detection showed that 236 vessels were narrow.Com-pared with CTA,TCD was better in the diagnosis of patients (Kappa value＞0.75).The diagnostic sensitivity and positive predictive value of TCD for MCA were the highest,which were 91.26％ and 93.07％,the consistency was the best (Kappa value =0.917).CTA detection showed that 210 MCA vessels had 103 stenoses,mild stenosis 17,moderate stenosis 41,severe stenosis 45,TCD detection showed that the stenosis was 101,mild stenosis 16,moderate stenosis 40,severe stenosis 45.The Kappa test showed that the diagnostic results of TCD to the degree ofMCA stenosis was better consistency compared with CTA (Kappa value=0.884.Conclusion:TCD has a high diagnostic value for cerebral artery stenosis in patients with cerebral infarction,and it is consistent with the diagnosis of CTA.

Objective To explore the short- term efficacy of argon- helium knife cryotherapy followed by chemotherapy in treating advanced non- small cell lung cancer (NSCLC) and to investigate its effect on the long- term survival. Methods During the period from March 2005 to March 2008, a total of 61 patients withⅢb or Ⅳ stage NSCLC received argon- helium knife cryotherapy followed by chemotherapy (study group), and other 52 patients with Ⅲb or Ⅳ stage NSCLC were treated with chemotherapy only (control group). The clinical data were retrospectively analyzed. The pain was evaluated by numeric rating scale (NRS) and the quality of life (QOL) was assessed by functional assessment of cancer therapy- general (FACT- G) scale. The clinical effect was evaluated according to RECIST criteria for solid tumor, and the patient’s survival time was recorded. Results (1) Twenty- six patients had local pain before the treatment, and the pain was relieved in different degrees after cryotherapy. The QOL, including all respects of FACT - G, was significantly improved after cryotherapy in all 61 patients. (2) The remission rate of the study group and the control group was 34.4% and 15.4% respectively, the difference between the two groups was statistically significant (P <0.05). (3) The median survival time of the study group and the control group was 12.9 months and 9.5 months respectively, and the one- year survival rate of the study group and the control group was 53.6% and 35.4% respectively. The differences between the two groups were statistically significant (P < 0.01). Conclusion Argon- helium knife cryotherapy is a safe and effective local treatment for advanced NSCLC, which can quickly reduce the tumor load, relieve the pain and improve patient’s quality of life. Cryotherapy with subsequent chemotherapy is superior to simple chemotherapy in improving the patient’s survival rate.
