ObjectiveTo evaluate the diagnosis and operative modality for superior mesenteric artery compressing syndrome (SMACS).MethodsThe clinical data of 28 SMACS cases from January 2000 to December 2010 at this hospital was analyzed retrospectively.ResultsAll patients underwent barium meal examination and the diagnosis was established according to clinical symptom and barium study. The 28 cases of SMACS underwent surgical treatment. The operative modalities included lysis and downward movement of the Treitz ligament and extensive mobilization of the duodenum in 4 cases, gastrojejunal anastomosis in 2 cases, lysis of the Treitz ligament and Roux-en-Y duodenojejunostomy in 11 cases, Billroth Ⅱ gastrectomy in 7 cases, and anterior duodenojejunostomy in 4 cases.All patients were cured and discharged from hospital.ConclusionsThe diagnosis of SMACS should mainly on barium meal examination besides the typical clinical manifestations such as epigastric distending pain and vomiting. Lysis of the Treitz ligament and Roux-en-Y duodenojejunostomy is appropriate surgical procedure to deal with SMACS.

Objective To analyze the influence of personality factors on mental health and its mediating variables in the support officers and soldiers of a certain navy air force unit. Methods A total of 1311 participants from the support officers and soldiers of a certain navy air force unit were selected. Cattell's 16 Personality Factors Questionnaire (16PF), Automatic Thoughts Questionnaire (ATQ), Military General Information Questionnaire designed by authors, Zung's Self-rating Anxiety Scale (SAS), Zung's Self-rating Depression Scale (SDS) and Self-Rating Scale of Sleep (SRSS) were used to assess the personality factors, automatic thoughts, general events and mental health level respectively. And structural equation model was used to analyze the mediating effect of automatic thoughts and general events between the personality factors and mental health, and the significance of mediating effect was tested with Bootstrap method. Results The participants' average rough scores of SAS were (29.72±6.26), and 6.10% of the subjects (80/1311) were anxious (standard score of SAS≥50); while the average standard scores of SDS were (38.83±10.76), 11.21% (147/1311) of the subjects were depressive (standard score of SDS≥53); and the average scores of SRSS were (19.59±5.81). There is a significant correlation between the subjects' adaptation and anxiety type, mental health factors and ATQ total score, general evaluation total score and SDS, SAS, SRSS score (P<0.01), ATQ total score and general condition evaluation were obviously correlated with SDS, SAS, SRSS score (P<0.01). The Bootstrap test showed that the indirect effects of adaptive and anxious personality factors model, of which the 95%CI was -0.237-0.149), yet the regression weights of the direct path was not obvious (P>0.05), which indicated that general events and automatic thoughts totally mediated the relationship between the personality factors and mental health. While for indirect effects of mental health personality factors model, the 95%CI was (-0.307-0.217, P<0.01), and for the direct effects, the 95%CI was (-0.299-0.137, P<0.01). Conclusions It suggested that the mental health condition of the support officers and soldiers in a certain navy air force unit is still acceptable. Age, educational level, personality factors, general events and automatic thoughts could affect the mental health, and the personality factors may affect the mental health mediated by general events and automatic thoughts.

Objective To investigate the expression of thymosin ?4 in the foreskin,normal skin(except the foreskin),hypertrophic scar,and keloid,and its relationship to pathological scars.Methods In situ hybridization was applied to detect the expression of thymosin ?4 mRNA [With the probe of BM005698(EST) gene marked by digoxin] in four groups: foreskin,normal skin(except the foreskin),hypertrophic scar,and keloid.Both qualitative and semi-quantitative analysis were performed.Results Thymosin ?4 mRNA was expressed in all the four groups.The proportion of thymosin ?4 mRNA expression in the keloid was 2/10,which was 25% of that in the foreskin(8/10),and was 28.6% of that in the normal skin(7/10).The positive rate of thymosin ?4 mRNA expression in the dermis was higher than that in the epidermis;and was high in fibroblasts,intravascular endothelial cells,and macrophages.The mean OSD of the positive samples was 0.03?0.01 in the keloids,which was significantly lower than that in the normal skins(0.09?0.03) and hypertrophic scars(0.09?0.02)(P=0.000 for both).Compared with the normal skins,the expression increased by 231.5%(P

BACKGROUND: The sensorineural deafness occurs as a result of loss of inner ear hair cells in the cochlea or of their primary afferent the spiral ganglion neurons. Stem cells to restore hearing following inner ear cell death has become a focus in recent years.OBJECTIVE: To summarize research progress in stem cells differentiating into inner ear cells in vitro and in vivo and to review the achievement in stem cells replacing inner ear cells in treating sensorineural deafness.METHODS: With "inner ear, stem cells" as key words, a computer-based online search of Pubmed and CNKI was performed for articles published from January 2000 to August 2009. RESULTS AND CONCLUSION: A total of 170 articles were collected, and experimental studies and review articles on stem cells in sensorineural deafness were included, while repetitive articles were excluded. Finally, 32 articles were summarized and analyzed. Different types of stem cells have the capacity to differentiate into inner ear cells. They can differentiate into neural cell types. Stem cells can live and migrate, differentiating into cell types of the sites of injury. It provides a therapy strategy to restore hearing following sensorineural deafness by he capacity of stem cells differentiating into inner ear cells. However, it remains further investigation how to function following cell differentiation and how to form the appropriate neural pathways by stem cell transplantation in sensorineural deafness.

Objective To study the prothrombotic state ,the factors leading to this status,and its relation-ship with the risk of cardiovascular disease in impaired glueese tolerance (IGT) patients. Methods The patients were divided into 4 groups : ①20 control subjects, ②38 patients with IGT, ③20 patients with diabetes only,④31 with diabetes and coronary heart disease. The vWF, throbomodulin, protein C, protein S, plasminogen activator inhibi-tor 1 were measured by ELISA. Results ①)There was statistical difference between IGT and controls in vWF, activi-ty of PC and PAI-1 (P <0.01 ,respectively). ②There were different factors that probably lead to the prothrombotic status among patients with IGT and diabetes. ③There were statistical differences among IGT, diabetes and diabetes with coronary heart disease in vWF, activity of Protein C. There were statistical difference among IGT , diabetes and diabetes with Coronary heart disease in PAI-1 (P < 0.05,respectively). Conclusion ①There is the prothrombotic status in the patients with IGT, similarly to those with diabetes.② IGT has the similar prethrombotic status as diabe-tes, So IGT could be considered as the predromal stage of diabetes, because of some continuity in the progress. Mean-while,it still has some different specialties. ③The abnormality of the system of endothelium, anti-coagulation and fi-brolysis in the patients of IGT is similar to those with diabetes. These variations might have the similar risk of cardio-vascular disease with the diabetes.

Zusanli (ST 36), Shangjuxu (ST 37) and Xiajuxu (ST 39) are considered as the lower He-sea points, which are located along the leg lines of stomach meridian of foot Yangming from top to bottom. According to the corresponding zang-fu of lower He-sea points, the distribution order from top to bottom is stomach, large intestine and small intestine. This is significantly different from digestive physiology function sequence of stomach, small intestine and large intestine in modern medicine. However, it conforms to their anatomical position from top to bottom. In order to make a further confirmation that the indication specificity of Zusanli (ST 36), Shangjuxu (ST 37) and Xiajuxu (ST 39) is closely associated with anatomical location, a great number of ancient and modern literature is reviewed. It is found that the functions of the three acupoints are obviously related to stomach, large intestine and small intestine, respectively, indicating that the sequence of Zusanli (ST 36), Shangjuxu (ST 37) and Xiajuxu (ST 39) is related to anatomical position of corresponding zang-fu. This study conclusion could play an important role for exploring the significance of anatomical location in indication specificity of acupoints.

Objective To improve clinicians'understanding of nephrotic syndrome and myasthenia gravis as adverse events of penicillamine(D-PA).Methods Two patients were reported to develope adverse reaction after taking D-PA.The related literature were reviewed.Result The first case was a systemic sclero- sis(SSc)patient treated with D-PA who developed nephrotic syndrome and blepharoptosis.The second case was a rheumatoid arthritis patient treated with D-PA who developed dysphagia and drinking bucking.The clinical symptoms were improved after disconting D-PA.Their conditions were stable up to now.Conclusion D-PA is used widely for various of autoimmune diseases.Clinicians should pay attention to these special and rare adverse effects.

Objective To study what affect the incidence and recurrence of keloids. Methods A total of 476 cases were treated with surgery plus adjunctive radiation therapy, intraleisional injection of triamcinolone acetonaide acetate, or surgery plus postoperative topical medicine. The family history and other clinical data were analyzed, including patients' age, sex, the duration of keloids, initiating factors, location, size, treatment method, and recurrence of the keloids. Results Keloids occurred in both male and female with the rate of 1. 83 ∶ 1. 00, with a predominance in woman aged from 22 to 45 years old. Trauma and pierce were the most significant initiating factors. The recurrence was affected by patients family history, treatment methods and the location of keloids, and less affected by keloids size, patients' age and sex. Conclusions Both surgery following adjunctive radiation therapy and intraleisional Kenalog injection are useful methods, which are prior to surgery plus postoperative topical medicine. The keloids incidence has a preponderance in some patients with family history. Therefore,genetic research is important to identify the pathogenetic factors in the keloids.

Objective To study the anatomic landmarks of recurrent laryngeal nerve (RLN) in minimally invasive video-assisted thyroidectomy (MIVAT), and to evaluate the operative skills to avoid nerve injury. Methods 106 patients were enrolled in the study dated between August 2008 and August 2010, in which 8 patients were converted to the conventional thyroidectomy. Intraoperative anatomic landmarks for location of RLN were the gap between trachea and carotid artery (GTC), as well as the middle and posterior portion of tracheal wall. Results 98 RLN were at risk, and 97 (98.98 %) nerves were recognized by means of two landmarks. 1 nerve failed to locate which was non-recurrent laryngeal nerve. Temporary RLN paralysis happened to 1 nerve(1.02 %), and no permanent RLN paralysis appeared. Conclusion GTC combined with middle and posterior portion of tracheal wall are safe and effective anatomic landmarks to locate RLN in MIVAT.
