Objective: To study the effects of halothane and sevoflurane on myocardial function and energy metabolism. Method:The model of Langendorff perfused isolated rat heart was used to investigate the effects of halothane and sevoflurane on HR,LVEDP,LVDP,+dp/dt,-dp/dt and coronary flow(CF)before and after ischemia. and the myocardial ATP content were measured with HPLC before ischemia and 10 min after ischemia and at the end of reperfusion. Result: 1.5 MAC sevoflurane significantly increased CF in normal isolated rat hearts. Both halothane and sevoflurane differently depressed myocardial contratile function,increased normal myocardial energy storage. At 10th min of ischemia the decrease of myocardial ATP content was delayed by halothane and sevoflurane. At the end of reperfusion,the both anesthetics improved the recovery of myocardial function and matebolism,especially sevoflurane. Conclusion: Both anesthetics can protect myocardium from ischemic reperfusion injury through improving post-ischemic myocardial energy recovery.

Objective To investigate the expression of apoptosis-related genes Bcl-2 and Bax after cerebral ischemic reperfusion in hippocampal CA1 and CA3 areas and the effect of curcumin on the expression of Bcl-2 and Bax in gerbils.Methods Gerbils were randomly divided into sham group(SH),ischemia-reperfusion group(IR),curcumin group(CU) and solvent control group(SC).Forebrain ischemia was induced by occlusion of bilateral common carotid arteries.Observations were made in each group 1d,3d,5d and 7d after ischemia.Open field test was used to examine the behavioral change.The apoptotic neurons in hippocampal CA1 region was counted,and the expression of Bcl-2 and Bax in hippocampal CA1 and CA3 areas was detected by SABC immunocytochemical technique.Results The behavioral mark and the number of apoptotic neurons in hippocampal CA1 and CA3 areas were much smaller in CU group than in IR group(P
4.Early Peritoneal dialysis after repair of congenital heart disease in children

Objective The mortality rate of the children after surgical repair of congenital heart disease associated with acute renal injure has been reported to be 30%-79%.Peritoneal dialysis is the common treatment method for those patients,but the optimal time to initiate peritoneal dialysis is still controversial.The aim of this study was to investigate the clinic outcome of early peritoneal dialysis to the children following cardiac surgery.Methods We performed a retrospective chart of children following cardiac surgery from January 2008 to December 2009.The total number was 4561,62 of them were performed PD treatment.Data on the PD treatment children were collected from pre-operation to hospital discharge.Early peritoneal dialysis group was defintes as PD initiated at operation room or the interval between the end of operation to starting peritoneal dialysis less than 3hrs.the others were traditional PD group.Results The whole PD treatment rate was 1.36%,15 patients died.The mortality were 10.71% (3/28) in early PD and 35.29% (12/34) in conditional PD group respectively (P =0.036).2 patients were met RIFLE(risk,injury,failure,loss,and end-stage renal disease,RIFLE)classification as failure grade in early PD group while 10 patients in traditional PD group (P =0.001).After PD treatment 24hrs,the Vasoactive-Inotrope Scores and serum creatinine values were much lower in the early PD group children than those in the traditional PD group[(19.00 ±2.39) vs(37.00±5.22),P=0.002;(50.51 ±21.84) vs(130.13±76.09),P=0.001)].Conclusion This study shows that early PD can prevent the children after congenital heart surgery to develop AKI,and reduce the severity of the clinic status.Early intervention with PD is associated with low mortality.
5.Recurrent pulmonary infection, offline difficulties and airway abnormalities

Objective To discuss the relationship between pulmonary infection, offline difficulties and airway abnormalities and to discuss the type of airway abnormalities. Methods The clinical data of 43 patients of severe pneumonia with inspection of electronic fibro-bronchoscope in PICU from Feb 2008 to May 2010 were retrospectively analyzed. Results In the 43 cases,24 cases existel different degrees of airway abnormalities, most of airway cartilage softening,in 15 cases; laryngeal dysplasia in 3 cases; absence of the right bronchus and right lung in 1 case. Conclusion Many children may exist airway abnormalities,if they have such performance as the clinical manifestations of recurrent wheezing after birth, refractory respiratory infection or difficulties in clinical offline, the increase of PCO2 though the effective suction, paroxysmal cyanosis and excluded with heart disease. Those children should undergo inspection with electronic fibro-bronchoscope to confirm the diagnosis as early as possible.

Objective To investigate the life quality and influencing factorin stroke patient.MethodTotally 205 hospi-talized stroke patientin the neurology departmenwere investigated with SF-36 scale .The datwere performed the descriptive sta-tistic,t-tesand multiple linearegression analysi.ResultThe scoreof general healthy statu,physiological function ,physiolog-ical role ,somatipain and social function in the patientwith stroke were significantly lowethan thain the national norm (P<0 . 01) .The recurrence frequency ,age ,comorbidity ,education level and income were significanfactoraffecting the life quality in the stroke patien.Conclusion The level of general health statu,physiological function ,physiological role ,somatipain and social function are decreased significantly .Therefore iirecommended to perform the pertinenintervention in the clinical work .
7.Surgical treatment of infective endocarditis: a report of 25 cases

Objective To summarize and report the experiences of surgical treatment of infective endocarditis. Methods From March 1995 to March 2004, surgical operations were performed in 25 patients with infective endocarditis, 9 of who received emergency surgery and 16 received selective operation. Replacement of mitral valve was performed in 3, aortic valve replacement in 13, mitral and aortic valve replacement in 8, Bentall procedure in 1.Associated cardiac lesions included patent ductus arterioles in 1, ventricular septal defect in 2, rupture of Valsalvaps sinus in 1. Results There was one postoperative death, which gave an overall hospital mortality of 4%. Twenty-four patients were cured and improved. Conclusion Surgical operation should be the preferred mode of surgical correction for infective endocarditis. The timing of surgery and proper perioperative management are key factors for the successful treatment of infective endocarditis.
8.Mechanism of Tubular Epithelial Hypertrophy Induced by High Glucose

[Objective]To study the mechanism of tubular epithelial hypertrophy induced by high glucose.[Methods]Human tubular epithelium line HK-2 cells were cultured in DMEM/F12 medium containing 1 g/L glucose(normal control group),4.5 g/L glacose(high slucose group),and 1 g/L glucose+3.5 g/L mannitol(iso-osmia control group),respectively.The cells of every group after cultured for 24 h,48 h.and 96 h were collected.The mRNA levels of CTGF and p27~(kip) were detected by real-time PCR.The protein levels of CTGF and p27~(kip) were detected by Western blot.The cellular proliferative activities were observed by MTT.The total cellular protein contents were detected by Bradford method.The cell cycle process was analyzed by flow cytometry.The cells of every group after being cultured for 30 win,1 h,24 h,and 48 h were collected.The levels of phospho-ERK1/2 were detected by Western blot.Every step was repeated for 3 times.[Results]High slucose could significantly up-regulate the mRNA and protein levels of CTGF and p27~(kip),activate ERK1/2 signal conductive pathway in HK-2 cells,make more cells arrest in G1 phase,decrease cellular proliferative activities,increase total cellular protein content reflecting cellular hypertrophy.[Conclusion]CTGF is an important mediator of tubular epithelial hypertrophy induced by high slucose.The mechanism is that CTGF up-regulates the p27~(kip) expression through activating ERK1/2 pathway.Up-regulated p27~(kip) lead proliferative cell to be arrested in G1 phase and cause cellular hypertrophy.

10.Clinical features of invasive pneumococcus with resistance to antimicrobial agents in Pediatric Intensive Care Unit

Objective To study the clinical features of invasive pneumococcus disease (IPD) with resistance to antimicrobial agents in children,and to improve the diagnosis and treatment of this disease.Methods The clinical data from 21 IPD patients younger than 13 years old were collected from January 2008 through December 2010 in Pediatric Intensive Care Unit in Beijing Children's Hospital for retrospective analysis. Specimens of blood,pleural effusion,cerebrospinal fluid and soft tissue aspirated were collected from these children,and 23 strains of streptococcus pneumonia (SP) were cultured,isolated and confirmed,and the antibiotics susceptibility to penicillin and other antibiotics of these strains were assayed.Results Among the 21 IPD children,the ratio of male to female was 0.9∶1,and the age was 5 months to 13 years,with 61.9% of them under 2 years.Of them,12 patients (57.1% ) had purulent pleurisy,and 1 (4.8% )patient had an underlying disease diagnosed to be X - linked agammaglobulinemia (XLA).There was no seasonal difference in the occurrence rate of this disease. Eight (38.1%) patients were cured,11(52.4% ) were improved,while 2 (9.5% ) patients not improved without death.There was no statistically significant difference in the annual detection rate of invasive SP (x2 =3.711,P =0.156).The incidences of penicillin-intermediate susceptibility SP (PISP) and penicillin-resistant SP (PRSP) were 47.8% and 26.1% respectively.The rate of resistance to multiple antibiotics was 91.3%.Conclusions Children aged less than 5 years,especially younger than 2 years,are prone to IPD,and purulent pleurisy and septicemia are often seen in this disease. Some patients had the underlying diseases.The complications included hemophagocytic syndrome,acute respiratory distress syndrome,septic shock,bronchial pleural fistula and so on.The multidrug resistance rate was 91.3%.It is important to put great emphasis on the monitoring antibiotics resistance to invasive pneumococcal disease.
