1.Investigation and analysis on the perceptions of professional benefits in the clinical teachers of nursing

Objective The purpose of this study was to examine the status quo of the perceptions of professional benefits in the clinical teachers of nursing,analyze the influencing factors and put forward effective measures to improve it.Methods A convenience sample of 117 nurse preceptors in 4 teaching hospitals participated by completing a two-part questionnaire the Preceptor's Perception of Benefits and Rewards Scale and a demographic questionnaire.Results The total average score of benefits and rewards to the preceptor role was(63.64±9.93) points,low than the study done by Hyrkas(2007) in American.Nurse preceptors in different nursing unit felt different benefits and rewards to the preceptor role.Medicine unit and surgical unit were higher than Pediatrics unit.The longer time of being preceptor,the higher score of benefits and rewards to the preceptor role.Conclusions The score of benefits and rewards to the preceptor role is low.Hospitals should increase support for clinical teaching program,particularly pediatrics.Nurse managers should adjust promotion opportunities,workload,rewards of nurse preceptors to meet the needs of nurses in clinical teaching and to improve teaching effectiveness.
2.Sequential follow-up MR studies of hepatocellular carcinoma after radio-frequency ablation

Objective To describe the characteristic MR appearances in hepatocellular carcinoma after radiofrequency ablation (RFA) during follow-up studies. Methods MR images of 110 patients with hepatocellular carcinoma after RFA ,which were categorized into 3 MR examination intervals ( ≤48 h, 1-6 m, >6 m), were analyzed retrospectively. The sequential changes of MR images were assessed and compared using Chi-square test. Results All RFA areas of 110 patients typically exhibited hyperintensity on GRE-T1 WI and hypointensity on TSE-T2 WI with fat suppression within 48 h after ablation. Subsequently,the signal intensities of the RFA areas decreased on GRE-T1 WI which showed significant differences after 6 months( P <0. 015 ). On GRE-T1 WI , RFA lesions were hyperintense in 72, iso-or hypo-intense in 4 at 1-6 months and hyperintense in 60, iso-or hypo-intense in 17 after 6 months. On TSE-T2 WI, 65 RFA lesions were hypointense, 11 isointense at 1-6 months and 47 were hypointense, 30 isointense after 6 months. The increased intensity on TSE-T2 WI showed significant differences after 6 months( P < 0. 015 ).On contrast-enhanced MR images, RFA lesions showed peripheral rim of enhancement within 48 h and exhibited a tendency to show no enhancement ( n = 37 at 16 months and n = 63 after 6 months ) with significant differences between MR examination intervals( P < 0. 015 ). In 6 patients with residual tumor or local tumor progression, nodular lesions were found at the periphery of the RFA. These nodular lesions showed moderate to marked enhancement as well as hypointense signal on GRE-T1WI, and moderate hyperintense signal on TSE-T2WI. Conclusion MR imaging could depict the characteristics of coagulative necrosis of tumor, residual tumor or local tumor progression in patients with hepatocellular carcinoma after RFA therapy.
3.Modified free vascularized fibular grafting for the osteonecrosis of the femoral head

Objective To evaluate the short term results of repairing osteonecrosis of the femoral head by modified free vascularized fibular grafting through Smith-Peterson approach without cutting the rectus femoris.Methods Between January,2009 and September,2012,14 osteonecrotic hips of 14 patients who underwent free vascularized fibular grafting were retrospectively studied including 10 male and 4 female.The follow up period was 24 months.The preoperative and postoperative symptoms relief,recovery of hip function (Harris score),radiographic assessment (X-ray).Results All patients got postoperative ipsilateral hip pain relief.Average Harris hip score improved significantly (68.47 ± 5.33,86.80 ± 4.72 points in preoperation and postoperation,respectively; paired t test,t =18.724,P < 0.01).Postoperative X-ray results showed there were no progresses of osteonecrosis and no collapse in the operative side within 2 years follow-up.Conclusion The short term treatment results of femoral head avascular necrosis were satisfactory by using free vascularized fibular graft through Smith-Peterson approach without cutting off the rectus femoris and it may be a relatively practical option of surgical approach.
4.Construction and identification of ERβ419 gene silencing recombined vectors of beagle ERβisoforms

Objective To construct and identify retroviral-mediated short hairpin RNA ( shRNA ) expression vectors of ERβ419, and explore ERβ419 unknown biological function in beagles in future.Methods To screen out the most effective gene silencing sequence of beagle ERβ419 mRNA using qRT-PCR and Western Blot assays, imitate beagle estrogen target cells.Results qRT-PCR results showed, ERβ419-shRNA1 ( P <0.01 ) and ERβ419-shRNA3 ( P <0.01)differed significantly, Western Blot result as same as qRT-PCR,ERβ419-shRNA3 is the best choice.Conclusion Beagles ERβ419-shRNA3 retrain most effectively target gene repression. It is applied to explore ERβ419 unknown biological function in beagles reproductive system, and to prevent and treat beagles reproductive function diseases.
5.Efficacy analysis of sacral canal injection in patients with lumbar disc herniation associated with non-sciatica.

OBJECTIVETo observe the outcome after sacral canal injection in patients with disc herniation associated with without sciatica.
METHODSFrom December 2010 to June 2011, 65 patients with acute low back pain without sciatica due to lumbar disc herniation or bulging confirmed by CT or MRI were randomly divided into sacral canal injection group (experimental group) and lumbar oblique wrench group (control group): the experimental group had 35 cases, including 30 males and 5 females, with an average age of (43.90 +/- 1.14) years old ranging from 33 to 56 years old. The control group had 30 cases, including 27 males and 3 females,with an average age of (44.00 +/- 1.19) years old ranging from 34 to 57 years old. The course of morbidity was 1 to 3 days. All patients received sacral canal injection or lumbar oblique wrench method. The visual analog scale (VAS) scores before and at 30 min after treatment were compared between two groups.
RESULTSThe symptom of acute low back pain were relieved obviously. The average VAS scores before and after treatment in experimental group were decreased from 6.63 +/- 0.97 to 3.06 +/- 1.51,in control group were from 6.67 +/- 0.96 to 3.93 +/- 1.20 respectively. These two methods could improve the VAS score,but the effect of sacral canal injection group was better than that of lumbar oblique wrench group, there was statistically differences (P < 0.05).
CONCLUSIONIt is effective that the methods of sacral canal injection and lumbar oblique wrench applied to patients with acute low back pain without sciatica due to lumbar disc herniation or bulging confirmed, the former has better effect.

6.Allografted olfactory mucosa gliacytes repair Wistar rats' sciatic nerve long defect.

OBJECTIVETo investigate whether or not allografted olfactory mucosa gliacytes could repair peripheral nerve injure.
METHODSOlfactory mucosa gliacytes had been cultured in vitro for 2 weeks, then purified and condensed for later transplantation.Sixty adult female Wistar rats were randomized into 2 groups of 30 rats each, A (control) and B (test). Rats' left sciatic nerves were excised 25 mm long axons and retained epineurium lumen anastomosed to proximal ends. Culture mediums, and olfactory mucosa gliacytes were transplanted into epineurium lumen of A and B groups respectively. At 3 months postoperatively, the regenerations of injured sciatic nerves were evaluated by methods of macroscopy, photomicroscopy, transmission electron microscopy, retro-marked fluorescence red, the condensation of glial fibre acid protein (GFAP) and nerve growth factors (NF) assayed by immunofluorescence, and the concentration of myelin basic protein (MBP) and neurofilament protein (NF) assayed by enzyme linked immunosorbent assay.
RESULTSThe regenerations of injured sciatic nerves were superior in B group to in A group; the transportation distance of retro-marked fluorescence red were longer in B group than in A group (P < 0.01). The condensations of GFAP and NGF were more dense in B group than in A group. The concentrations of MBP and NF were more high in B group than in A group (P < 0.01). The function scores of injured limbs were superior in B group to in A group (P < 0.01). The quantifications of nerve fibers and myelin fibers of injured sciatic nerve were larger in B group than in A group (P < 0.01).
CONCLUSIONAllografted olfactory mucosa gliacytes could repair injured nerve defect.

Objective To study the different effects of valsartan and extended realse nifedipine tablets on lowering blood pressure of essential hypertension patients and their reversal effects on left ventricular hypertrophy. 8.Prevalence of social distance and restriction among college students with suicide attempts in Chongqing

Objective To explore the attitude towards 10 different populations with different characteristics on their social distance and acceptance among those people with or without suicidal attempts among college students in Chongqing.Methods 9808 college students were randomly selected from 11 universities in Chongqing and administered a self-constructed questionnaire"Suicide Attitude and Mental Health Status Questionnaire".A t-test was employed to analyze the social distance and degree of acceptance of college students with or without suicide attempts towards different groups as criminals,hypertensive,with suicide actions,homosexuals,depressive disorder,HIV infection,drug addiction,death of family members,hospitalization history in mental service and alcohol addiction.Results 169 college students had suicide attempts;the reporting rate of suicide attempts was 1.7%.Among college students.the scores higher than 50 on nine items related to social distance towards strangers with those 10 different characteristics and the top three were on those with HIV infection,drug addiction and hospitalization history in mental service.There were differences(P<0.05)in scores of social distance attitude towards strangers with drug addiction(with suicide attempts 83.38±21.82,without suicide attempts 78.55±21.55,t=2.88).hospitalization history in mental service(79.27±21.23,75.67±21.31,t=2.17),homosexuality(66.87±24.77,74.14±21.94,t=-4.25),alcohol addiction(66.72±21.80,61.00±22.80,t=3.23)and hypertension(56.65±20.40,53.36±21.05.t=2.01)between college students with or without suicide attempts.College students scored higher than 50 in 7 items of social rejective attitudes towards acquaintances,of those with hospitalization history in mental service,drug and alcohol addiction ranked the top three.It showed statistical significances in social rejection attitudes towards acquaintances with homosexuality(35.28±30.38,42.83±30.76,t=-3.14),severe depression(56.59±28.49,61.64±25.56,t=-2.53)and suicide behaviors(51.46±28.19,56.56±26.35,t=-2.48)between students with or without suicide attempts.Conclusion College students in Chongqing kept quite far social distance and restrictive behaviors in college students with or without suicide attempts.Targeted interventions on suicide attempters should be carried out accordingly.
9.Effects of Tangzhi Kangping Granules on the blood sugar and lipid in rats with disordered glucose and lipid metabolism

AIM To observe the effects of Tangzhi Kangping Granules (Astragali Radix,Ligustri lucidi Fructus,Schisandrae chinensis Fructus,etc.) on regulating blood sugar and lipid of rats with glucose and lipid metabolism disorders.METHODS Among the seventy rats for the trial,ten rats were randomly assigned to normal control group,the other 60 rats were injected with STZ citric acid-sodium citrate solution (60 mg/kg) combined with high-fat emulsion (10 mL/kg) for 4 weeks to be the models of glucose and lipid metabolism disorders.The model rats were divided into model control group,mefformin + fenofibrate group,Xiaoke Pills + Xuezhikang group,and high,medium and low dose Tangzhi Kangping Granules groups.After 4-week intragastric administration,the rats had their levels of fasting serum glucose (GLU),triglyceride (TG),total cholesterol (TC),high density lipoprotein cholesterol (HDL-C),low density lipids (LDL-C),insulin (INS),adiponectin (ADP),leptin (Leptin),interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) measured.RESULTS Compared with the model control group,the rats in high,medium and low dose Tangzhi Kangping Granules groups displayed significantly decreased serum contents of GLU,TG and TC,lowered levels of LDL-C,ADP,Leptin,IL-6,TNF-α,and yet significantly increased levels of HDL-C and INS.CONCLUSION Tangzhi Kangping Granules,a blood sugar andlipid regulator may adjust the levels of insulin,adiponectin and leptin,and therefore improves the body's inflammatory environment to alleviate insulin resistance.
10.Clinical application of the scapular free flap extended to the upper arm.

OBJECTIVETo apply the scapular free flap extended to the upper arm for resurfacing the face and neck, as well as the upper lip in one stage.
METHODSThe scapular free flap was designed with extended portion to the posterior and interior part of the upper arm. Then the free flap was transferred to resurface the face and neck with the routine portion and to resurface the upper lip with the extended portion.
RESULTS6 cases with extensive upper lip, facial and cervical burn scar were treated with the extended scapular free flaps. The flap size ranged from 22 cm x 11 cm to 40 cm x 9.5 cm (36.57 cm x 10.20 cm in average) for the routine portion and from 7 cm x 4 cm to 12 cm x 4 cm (10.32 cm x 3.67 cm in average) for the extended portion. All flaps survived completely.
CONCLUSIONSThere are direct communicating branches ("choke vessel") between the circumflex scapular artery (CSA) and the posterior humeral circumflex artery (PHCA). When the blood supply of PHCA is cut off, the CSA can provide blood supply through the communicating branches to the upper arm skin area previously nourished by PHCA. So the blood supply of the extended portion of the scapular free flap is not only from the branches of CSA, but also from the direct communicating branches between the CSA and PHCA. The extended scapular free flap has a reliable blood supply and can be applied to construct the facial and cervical scar contraction with the extended portion to resurface the upper lip. The satisfactory result can be expected.
