1.Effect of clozapine exposed in the pregnant day on the development and the expression of MAPK44/42 in brain of filial rats

Objective To set up the experimental animal model of the filial rats exposed clozapine in the pregnant clay and expect to know what is the effect on the offspring exposed the antipsychiatrie in gestation.Methods Female Wister rats were divided into both the control group (n=17)and elozapine group (n=2.5) which were intraperitoneally injected saline and clozapine respectively from pregnant day 6 to day 15,and their pup rats were examined development indexes such as the physical growth ,neural reflex,neural behavior and the expression of MAPK44/42 in brain in neonatal stage.Results it was shown that clozapine- exposed in pregnant day lead to the retardation of the weight of filial rats in earlier age such as postnatal day 4 (PND4) and PND7(F=12.56/7.51,P=0.001/0.008).As for neural reflex,the time of cliff avoidance of rats was significantly delayed in PND 3 and day 9 (F=4.969/4.348 ; P=0.03/0.041),and the ratio of positive response of air righting decreased compared with the control group in PND13 and PND15 (F=7.959/6.475,P=0.007/0.016).There was no difference between two groups on the behavior of the filial rats in the open field of filial rats.The expression of PhoMAPK44/42 in hippocampi of exposed-clozapine offspring in PND15 was significantly lower than the control' s(F=18.729/23.824,P=0.001) ,however,no difference was demonstrated on the expression of MAPK44/42 in hippocamaias well as the expression of both Pho-MAPK44/42 and MAPK44/42 in forehead cortex in two groups.Conclusion The resuits suggest the aberration of both physical and mental development of filial rats exposed clozapine in gestation.

3.Sodium hydroxybutyrate decreases hypoxic-ischemic insult-induced neuronal apoptosis in neonatal rat cortex

Objective To investigate the effects of sodium hydroxybutyrate (?-OH) on neuronal apoptosis in neonatal rat cortex induced by hypoxic-ischemic (HI) insult. Methods One-hundred and fifty 7-day old newborn SD rats were randomly assigned to one of S groups ( n = 30 each): sham operation group (A); HI + normal saline (NS) group (B) and three HI + ?-OH groups (C, D, E) . HI was induced by ligation of left common carotid artery followed by 2 h inhalation of hypoxic air (8% O2 92% N2). In sham operation group (A) left common carotid artery was exposed but not ligated and no hypoxic air was inhaled after operation. In group B (HI + NS) NS 0.2 ml?10 g-1 was injected intraperitoneally (IP) t.i.d immediately after HI until the animals were killed; whereas in HI +?-OH groups ?-OH 50 (C) or 100 (D) or 200 (E) mg?kg-1 was injected IP instead of NS. Six animals were killed at 1 h, 3 h, 1 d, 3 d and 7 d in each group and brains were removed. The number of apoptotic neurons in the left cortex was detected using TUNEL staining technique. Results The number of apoptotic neurons at 1 h-7 d after HI was significantly greater in group B, C, D and E than in group A ( sham operation) . The expression of apoptosis positive neurons reached the peak at 1 day after HI. The number of apoptotic neurons at 3 h-3 d was significantly greater in group E (?-OH 200 mg?kg-1) than in group C and D (?-OH 50 and 100 mg?kg-1) . There was no significant difference in the number of apoptotic neurons between group B (HI + NS) and E (HI +?-OH 200 mg?kg-1). Conclusion Sodium hydroxybutyrate 50 and 100 mg?kg-1 can attenuate neuronal apoptosis in neonatal rat cortex induced by hypoxic-ischemic insult.
4.Effects of Hippophae rhamnoides L. subsp. chinensis Rousi polysaccharide on alpha-glucosidase enzyme activity and level of blood glucose.

The enzyme-inhibitor model and the sugar tolerance mouse model were used to evaluate the relationship between the inhibition rate of enzyme activity and concentration of Hippophae rhamnoides L. subsp. chinensis Rousi polysaccharide (HRP). The inhibitory patterns of enzyme and dose-dependent effects of HRP's effect on blood glucose using acarbose tablets as control were also examined. The mechanism underlying hypoglycemic effects of HRP was discussed. The results showed: in the enzyme-inhibitor model, the inhibitory activity of different concentrations of HRP (9.80, 19.60, 39.20, 78.40, 156.80 and 312.50 mg x L(-1)) on alpha-glucosaminidase (AG) inhibitory activity were 6.62%, 18.02%, 33.26%, 48.23%, 62.11%, 76.31%, 90.12%, IC50 was 31.59 mg x L(-1). The inhibitory rate of 25.00 x 10(3) mg x L(-1) acarbose tablets was only 64.87%, and IC50 was 10.75 x 10(3) mg x L(-1). In the sugar tolerance mouse model, different doses of HRP (240, 480, 960 mg x kg(-1)) tended to decrease levels of blood glucose compared with control group (acarbose tablets 375 mg x kg(-1)) at 15, 30, 60 and 120 min. It's further confirmed that HRP is a kind of competitive inhibitor of AG activity. Its inhibition rate increases with the increase of concentration in normal mice, and it subsequently improves the sugar tolerance showing the effect of reducing blood sugar.
5.Deep fungal infectious in patients with systemic lupus erythematosus:a clinical analysis

Objective To investigate the infection sites,the offending species,diagnosis and prognosis of deep fungal infections (DFI) in patients with systemic lupus erythcmatosus (SLE).Methods Fifty-one patients with fungal infections in 1466 SLE patients admitted to Peking Union Medical College (PUMC) Hospital from 2000 to 2006 were reviewed retrospectively.Results Candida albicans was ranked the first pathogen,followed by ncoformans and Aspergillus species.The infection sites were lungs,cerebral meninges and blood in the order of prevalence.The overall mortality was 20%(10/51).Aspergillasis carried the highest mortality which couht be as high as 80%.Hypoproteinemia,multiple focus of fungal infections,Aspergillasis and fungemia might he the independent risk factor for mortality.Conclusion Candida albicans is the most frequent species of fungal infections in SLE patients.Iungs are the most prevalent location of infection.Earlier diagnosis is important.Special attention should be paid to aspergillasis.
6.Effect of qiang chang heji on peripheral red blood cell in rats with blood insufficient supply in right brain

BACKGROUND: It has been discovered in experimental researches that qiang chang heji acts on prevention and treatment of brain injuries induced by insufficient blood supply and improvement of blood rhelogical property.OBJECTIVE: To study on the effects of qiang chang heji on peripheral red blood cell in rats with insufficient blood supply to brain so as to probe into its mechauism on improving blood rhelogical property.DESIGN: A randomized controlled trial.SETTING: Central Laboratory of General Hospital of Chengdu Military Area Command of Chinese PLA.MATERIALS: The experiment was performed in Central Laboratory of General Hospital of Chengdu Military Area Command of Chinese PLA from July to September 2003. Totally 50 SD white rats of either sex were employed, weighted varied from 150 g to 170 g.METHODS: The common carotid artery(CA) on the right side was ligatured to induce insufficient blood supply in the brain. The control and qiang chang heji(QCHJ) group were designed for comparison. On the same day of CA ligature, in treatment group, QCHJ of various dosages were applied daily for gastric perfusion; and in the control, physiological saline was applied. Three or seven days later, the parameters of peripheral red blood cell and its rhelogical property were analyzed with equipment.the sham-operation group, mean cell hemoglobin (MCH), haematocrit(HCT) (%) and RBC aggregation index were in the tendency of increase and mean cell hemoglobin concentration (MCHC) (g/L) was increased obviously (375±7 vs 363±13) (P < 0.05) .In the treatment group of 10 g/kg QCHJ, HGB(139.2±12.4 vs 153.6±9.6), MCHC(355±7 vs 375±7) and haematocrit(HCT) (35.4 ±0.9 vs 42.8 ±4.5) were significantly lower the control compared with the sham-operation group, both rigidity index and aggregation index of RBC were in the tendency of further increase. In treatment of QCHJ 10 g/kg, MCHC(354±4 vs 361 ±4), RBC rigidity index (1.02±0.35 vs 4.54±3.04) and aggregation index (2.76±0.66 vs 4.67 ± 1.21 ) were significantly lower than the control( P < 0.05).CONCLUSION: Treatment with QCHJ can prevent changes in peripheral red blood cell in rats with insufficient blood supply in the brain and reduce MCHC and RBC rigidity index and aggregation index so as to improve RBC deformability and rhelogical property.
7.The role of IDO/TTS mediated tryptophan metabolic pathway in patients with immune thrombocytopenia

Objective To discuss the role of indoleamine 2, 3-dioxygenas e (IDO) and tryptophanyl-tRNA synthetase (TTS) mediated tryptophan catabolism in immune thrombocytopenia (ITP) patients treated with high doses of dexamethasone through the expressions of IDO and TTS in T cells , and the concentrations of plasma kynurenine and tryptophan. Methods 20 newly diagnosed or relapse ITP patients were treated with 40 mg/d × 4 d dose of dexamethasone. The heparin anticoagulant blood samples were obtained before treatment and the 5th day after treatment. 20 healthy subjects were selected as the control group. The IDO and TTS expressions in CD4+and CD8+ T cells were analyzed by flow cytometry. The concentrations of plasma kynurenine and tryptophan were detected by liquid-mass spectrometry system. Results Compared with healthy controls group, the plasma tryptophan and kynurenine concentration and the ratio of Kyn/Trp were significantly elevated in ITP patients (P <0.05); the IDO expressions of CD4+ and CD8+ T cells in ITP patients were significantly lower than those in healthy controls (P < 0.05), but the TTS expressions were significantly higher (P < 0.05). The concentration of tryptophan in effective group was significantly lower than before treatment (P < 0.05), in contrast, the kynurenine concentration and the ratio of Kyn/Trp were significantly higher than before (P < 0.05). The expression of IDO in effective group were significantly higher than that before treatment (P < 0.05), conversely, the expression of TTS in effective group were significantly decreased (P < 0.05). No significant difference can be found in ineffective group. Conclusion IDO/TTS-mediated tryptophan catabolism pathway could indicate the onset of ITP. The sensitivity of ITP patients with high dose of dexamethasone treatment can be observed through the level of IDO and TTS.
8.Double L-shape osteotomy combined bony Z-plasty for prominent malar complex

Objective To evaluate the effect of double L-shape osteotomy combined bony Z-plasty for prominent malar complex.Methods Thirty-two patients with prominent malar complex were treated with double L shape osteotomy combined bony Z plasty for prominent malar complex.Width of lower face was observed during follow-up phase.Questionnaires were used to assess the patents' level of satisfaction at 6 months after operation.Results All wounds got primary intention healing and no severe complication in perioperative period.Postoperative appearance of all 32 cases showed that the width of middle face was efficiently reduced.All patients expressed high levels of satisfaction 6 months after operation.Conclusions Double L-shape osteotomy combined bony Z-plasty for prominent malar complex is an effective and safe method for the treatment of prominent malar complex.
9.Clinical Observation of Ginger-partitioned Moxibustion plus Chinese Medication Enema for Antibiotics-induced Diarrhea

Objective To observe the efficacy of ginger-partitioned moxibustion plus Chinese medication enema in treating diarrhea caused by antibiotics. Method Forty patients with antibiotics-induced diarrhea were randomized into an observation group and a control group. Twenty cases in the control group were treated by Metronidazole and Live Combined Bifidobacterium and Lactobacillus tablets; twenty cases in the observation group were given ginger-partitioned moxibustion plus Chinese medication enema. The total effective rate and the change of abdominal pain index were observed after successive 7-day treatment. Result The total effective rate in the observation group (90.0%) was significantly higher than that in the control group (70.0%) (P<0.05); the diarrhea score was significantly lower in the observation group than in the control group (P<0.05). Conclusion Ginger-partitioned moxibustion plus Chinese medication enema can promote the recovery of antibiotics-induced diarrhea, mitigate abdominal symptoms and signs, and enhance the quality of life of patients.
10.Analysis of Three Treatment Regimens and Pharmaceutical Care in One Patient with Lipoprotein Glomeru-lopathy

Objective:To provide reference for the treatment of lipoprotein glomerulopathy and to investigate the participation of clinical pharmacists in the whole treatment course. Methods: Three different treatment regimens, including immunosuppressive thera-py, dual plasma filtration therapy and lipid-lowering combined with reducing urinary protein therapy was respectively adopted for one patient with lipoprotein glomerulopathy, and the efficacy was evaluated. Clinical pharmacists assisted physicians in deciding treatment regimen, performed pharmaceutical care, adjusted medication and analyzed the prognosis of the patient during the follow-up. Results:1. Immunosuppressive therapy was ineffective for the patient;2. The dual filtration acted quickly, while the expense was high and the disease was easy to relapse;3. The third therapy was relatively safer and more economical with long-term effect. Conclusion:Combi-nation therapy of lowering lipid and reducing urinary protein is the most suitable treatment regimen for the patient with lipoprotein glo-merulopathy. Clinical pharmacists play an important role in the whole course of treatment to assist physicians in obtaining the maximum benefit of patients.
