OBJECTIVETo analyze the causes of delayed union or nonunion of the ulna after intramedullary nailing in pediatric forearm fractures.

METHODSFrom February 2005 to February 2010,5 patients with forearm fractures who were treated with titanium elastic nailing (TEN) were identified to fulfill the criteria of having developed a delayed union or nonunion of the ulna. The causes of delayed union or nonunion were investigated according to mechanism of injury, fracture location, treatments methods and postoperative management. All patients were male and the age was 3 to 14 years old with an average of 9.4 years. All fractures were located on the mid-third part of forearm. Two cases had a re-fracture. Among them, 3 cases caused by high-energy injury and 2 cases by falling down. Open reduction were performed in 4 cases while the other one was treated with closed reduction. Four patients were immobilized in an above-elbow cast, postoperatively.

RESULTSAll patients were followed up from 7 to 19 months with an average of 11.4 months. There were 4 delayed union and 1 nonunion. Three patients healed after the removal of the nail and avoidance of weight-bearing. Two patients healed by replacing another fixation. No patients had soft-tissue irritation or nail-entry-site infections.. The clinical effect was evaluated according to Daruwalla and Price scores with 3 excellent and 2 good of the results.

CONCLUSIONSUsing titanium elastic nailing for the treatment of pediatric both-bone forearm fractures is a good method. However,strict indication selection should be followed to avoid delayed union or nonunion.

Objective To investigate the morphology characters of upper airway and its surrounding tissues in different age non-snoring males.Methods Total of 114 non-snoring males,with age-nlnge from 22 to 78 year old,were included in this study.They were divided into 3 groups:30 in younger group (22-29 years old),57 in middle-aged group(36-57 years old),and 31 in aged group(70-78 years old).All subjects had magnetic resonance imping sanning.Results (1)The nasopharynx in aged group was statistically different from that of the younger group and the midge aged group.The volume of nasopharynx in aged group(7.81±1.59)cm3 (χ-±s,same hereinafter) is greater than that in younger group(4.89±1.20)cm3 and middle aged groups(6.06±2.07)cm3.(2)The AP diameter/transverse dimeter of velophatynx,glossopharynx and laryngopharynx in the aged group tended to be smaller.The average value of AP diameter/transverse diameter of velopharynx in aged group is 0.46±0.14.while the value is 0.59±0.14 and 0.57±0.14 in midge and young groups respectively,compared with that of the aged group,there is statistical diffrence(P＜0.01).Further more,the min/max Section area of velophatynx,glossopharynx,hryngopharynx in aged group are smaller than that of young group and midlife groups.(3)The aged group had the thinnest thickness of posterior and lateral velopharyngeal walls,but the largest volume of soft palate and fatpads among all groups.Conclusions In non-snoring males,there is more fat deposition around the upper airway with aging.While the large naxopharynx and large transvdrse diameter of upper airway seem to balance the disadvantaged changes and to benefit the their sleep respiration.

OBJECTIVETo investigate the precise locations of the blood vessels and nerves surrounding the seminal vesicles (SV) in men and provide some anatomical evidence for SV-related minimally invasive surgery.

METHODSWe observed the courses and distribution of the blood vessels and nerves surrounding SVs and obtained the data for positioning the SV neuroplexes in 20 male pelvises.

RESULTSOne branch of the neuroplexes was distributed to the SVs bilaterally with the neurovascular bundles, (2.85 ± 0.18) cm from the median sulcus of the prostate (MSP), while another branch ran through the Denonvillier fascia behind the SV, (0.81 ± 0.06) cm from the MSP. The arterial SVs (ASV) originated from the inferior vesical artery and fell into 4 types, 55% going directly to the SVs as one branch, 15% running between the SV and the ampulla of the deferent duct as another branch, 25% downward as 2 branches to the SV and between the SV and the ampulla of the deferent duct respectively, and 5% as the other ASVs. The shortest distance from the ASV through the prostatic neuroplexus to the posterior SV was (1.08 ± 0.09) cm.

CONCLUSIONIn SV resection, neuroplexus injury can be reduced with a bilateral distance of < 2.85 cm and a posterior distance of < 0.81 cm from the MSP, and so can bleeding by vascular ligation between the SV and the ampulla of the deferent duct.

Objective To explore the effects of gossypol acetate on the morphological features and the gene expression in the femoral head of Sprague-Dawley rat in vivo after treated with dexamethasone .Methods Dexamethasone(Dex) was injected into the abdominal cavity of SD rats at an dose of 10 mg/kg ,twice a week ,and feed gossypol acetate 5 mg · kg -1 · d-1 .The controls re-ceived saline 2 mL injection .The treatment lasted for 12 and 20 weeks .The slices of the femoral head were made for HE and immu-nohistochemical study .The total mRNA was extracted for RT-PCR assessment .Results The cancellous bone trabecular became sparse ,trabecular bone area ratio decreased ,bone marrow fat tissue increased .These changes were fitted for pathological character of bone necrosis .The gossypol acetate could not affect the pathological changes .The proportion of the positive stained osteoblasts increased ,adipocytes decreased .PPARγ,C/EBPα,11β-HSD1 expression enhanced ,Runx2 down regulated in the treatment groups and GAA group .Conclusion Dex can induce evident pathological changes conform to the characters of femoral head necrosis .They may have closed correlation between 11β-HSD1 and the gene expression .But GAA could not affected the pathological changes and abnormality of the gene expression .

Objective To investigate the correlation between serum homocysteine (HCY) and chronic heart failure (CHF) hypercoagulable state in patients.Methods A total of 105 cases of patients with CHF was divided into three groups according to the New York Heart Association (NYHA) classification standard functions:heart functional grade Ⅱ group (42cases),cardiac function grade Ⅲ group (35 cases) and,NYHA class Ⅳ group (28cases).At the same time,40 healthy individuals were regard as the control group.HCY,fibrinogen (Fbg),D-dimer (DDI),HCY,N-terminal pro-brain natriuretic peptide (NT-proBNP) were detected by fasting venous blood samples which were collected within 24 hours after admission.Results Compared to the control group,the expression of Fbg,DDI,HCY and NT-proBNP increased,whereas,antithrombin Ⅲ (AT-Ⅲ) was reduced.Fbg,DDI,HCY,NT-proBNP,and AT-Ⅲ were found in all patient cases.Four groups were compared with each other,except for cardiac function Ⅱ group and the normal group had no significant difference between them (P ＞ 0.05),the difference between both other groups was significantly different (P ＜ 0.05),HCY had a positive correlation with Fbg,DDI,and NT-proBNP (r =0.268,0.295,and 0.404,P ＜ 0.05),and negative correlation with AT-Ⅲ (r =-0.240,P ＜ 0.05).Conclusions HCY might be a reliable indicator as a judge of CHF patients with hypercoagulable state,to detect HCY,FBG,DDI,and AT-Ⅲ in CHF patients.It benefits for judging thrombosis risk and determining the severity of the diseases.Anticoagulant therapy might be beneficial to reduce the long-term adverse events.

Objective To explore the influence of obstruction sleep apnea syndrome (OSAS) on plasma cystatin C (CC) levels in patients with primary hypertension.Methods A total of 244 cases of primary hypertension patients was chosen.The patients were divided into observation group (with OSAS) and control group (without OSAS) according to apnea hypopnea index (AHI).The observation group was then divided into three subgroups:mild OSAS group,moderate OSAS group,and severe OSAS group.The levels of CC were compared.Results First,the plasma CC levels in patients with primary hypertension had no statistical significance in the differences among different grades of hypertension (P ＞ 0.05).Second,CC levels of observation group were significantly higher than control group (P ＜ 0.05).Third,CC levels of the severe group were higher than the moderate group,and the plasma CC levels of the moderate group were also higher than the mild group and control group.Rank correlation analysis and comparison of CC levels and AHI showed that CC levels were positively correlated with AHI (r =0.585,P ＜ 0.01).However,there were no statistically significant differences between CC levels of the mild OSAS group and control group (P ＞ 0.05).Conclusions The patients with OSAS and primary hypertension had higher levels of CC,and aggravated with the progress of the degree of obstruction.CC may be involved in the progression of the disease,a high level of CC may aggravate the condition,it should be early prevention and treatment.

Objective To discuss the influence of the high quality nursing service demonstration project on the doctor-patient relationship.Methods Retrospective study was adopt to compare the nursing service,accompanying rate and patient's satisfaction rate before and after taking the high quality care service demonstration project.Results Nursing service had been improved,with accompanying rate falling from 100.0％to 70.0％ ( x2 =1 116.3,P ＜ 0.05),patients' satisfaction rate increasing from 92.0％ to 99.0％ ( x2 =180.3,P ＜ 0.05 ),the doctors' satisfaction rate increasing from 96.0％ to 100.0％ ( x2 =2.8,P ＜ 0.05 ),and nursing quality control deficiency rate decreasing from 25.0％ to 12.0％ ( x2 =5.6,P ＜ 0.05 ).Conclusions The nursing service demonstration project can ameliorate the doctor-patient relationship by carrying out humanized service.

In this paper, Fourier transform infrared spectroscopy fingerprint analysis of Marsdenia tenacissima samples was used to develop a reliable method of tracing the geographical origins. Forty-eight samples from four provinces of China were analyzed by FTIR. We analyzed and characterized the fingerprints in both the full spectrum peaks and characteristic peaks, then the principal component analysis and the cluster analysis were carried out. The results of fingerprint analysis, correlation analysis, principal component analysis and cluster analysis can identify the geographic origins correctly, which verified and supplemented each other; the identification results and the actual location showed a high degree of consistency, namely the lower the space distance, the greater the similarity of different samples. These results revealed the obvious superiority and practical value in comparison to the more tedious and time-consuming wet chemistry method normally used. Using appropriate metrology methods can trace the geographical source correctly. The M. tenacissima materials from the region of Maguan should be considered as genuine medicinal materials taking into account the good quality.

OBJECTIVETo approach the correlation of upper airway and surrounding tissues with body mass index (BMI) of non-apnea aged males.

METHODSThirty-one non-apnea aged from 70 years to 84 years males (all were inspected by polysomnography and apnea hyponea index < 10 times/hour) had magnetic resonance imaging scanned. In images of MRI, the upper airway was divided into four sections, including nasopharynx, velopharynx, glossopharynx and laryn-gopharynx. The sizes, forms and surrounding tissues were measured and correlation analysis of duality vaniable about upper airway and surrounding tissues with BMI were carried out.

RESULTS(1) The volume of nasopharynx had negative correlation with body mass(P < 0.05). (2) The min anterior-posterior diameter of velopharynx had positive correlation with body mass and BMI. The max value of anterior-posterior diameter/transverse diameter of velopharynx had positive correlation with body mass. The mi value of distance of fatpads had negative correlation with body mass and BMI. The min and average value of distance of ascending ramus had positive correlation with body mass and BMI. The mn value of distance of fatpads/distance of ascending ramus had negative correlation with body mass and BMI. The transverse diameter/lateral velopharyngeal wall of velopharynx had positive correlation with BMI. All of above results had statistic significance (P < 0.05). (3)The volume of soft palate, tongue and fatpads had no correlation with body mass and BMI (P > 0.05).

CONCLUSIONThe forms of sectional areas of velopharynx are subjected to body mass, and with the increasing of body mass they tend to round. In non-apnea aged males, the distance of fatpads and ascending rami have intimate relationship with body mass and BMI. The volume of soft palate, tongue, tatpads and velopharyngeal walls has little things with hody mass and BMI.
